Consider people allowed in cockpit and mental stability
Regular assessments and provide support 
i.e. Co-pilot wanted to suicide and took down an entire plane with him
Have everything readily available in cockpit (so no one has to enter / leave) 
This is because most incidents occur when a pilot takes a toilet or refreshment break away from the cockpit.
Ensure that items that can be used as a barricade is secured to the plane and cannot easily be used as a barricade. 
Allow for the transfer of control to air traffic control:
After 20 minutes of inactivity / no response from either pilots 
For 5 minute if there is only 1 pilot in the cockpit. 
This is in the case of a hijack or a pilot becomes uncooperative and becomes a danger to other crew or passengers 
Con: This signal can be hijacked.
Live camera within cockpit also in front of crew
This is to ensure that everything is ok in both cabin and cockpit.
This is an example of minimising one type of risk and maximising the other as making the door easy to break down may result in unwanted intruders getting in but it will prevent cases where a pilot is locked out of the cockpit.

An Air India flight was forced to land after the pilot was locked out of the cockpit during a toilet break, the airline says.
Flight AI 403, which was travelling from Delhi to Bangalore, was diverted to Bhopal Airport on Monday.
"The commander of the flight had left the cockpit for a short while to visit the toilet and on returning to the cockpit found the door locked. The door had got jammed and all efforts to open the door, even from inside by the co-pilot, failed," an Air India statement said.
The state-run airline is currently investigating a separate incident in which the auto-pilot system of an Airbus 321 flying from Bangkok to Delhi on 12 April was accidentally switched off.

Locking Procedure:

Captain moves toggle switch to lock position -> open light remains extinguished -> red light on code pad comes on (confirms, door is locked)
Automatic door opening the code pad & the buzzer are inhibited for 5mins
Has to use interphone to contact cockpit crew
Door can be opened by moving toggle switch to unlock position which overrides previous locked situation
Emergency Access Procedure (in case of crew incapacitate):
Person calls captain through inter-phone (if no answer) -> another way of contacting -. # key on code pad -> buzzer sounds for 3 secs -> another call on inter-phone -> emergency access procedure
Emergency code on code pad -> then # key -> triggers timer on code pad for 30 secs -> green light on code pad flashes indicating imminent unlocking -> cockpit: buzzer sounds continuously & open light flashes (also indicating imminent unlocking)
When 30 secs over, & no action from cockpit crew -> door goes into unlocking sequence for 5 secs -> green light on code pad remains steady -> open light comes on & buzzer stops (indicating door is unlocked)
Person now has 5 secs to enter cockpit

Aireon Takes Full Control of First ADS-B Hosted-Payload
Major milestone kicks off initial validation of space-based ADS-B surveillance
MCLEAN, Virginia, March 2, 2017 /PRNewswire/ -- Aireon announced today that they have formally received control from Iridium Communications Inc. (NASDAQ: IRDM) of the first Automatic Dependent Surveillance-Broadcast (ADS-B) payload hosted on an Iridium NEXT satellite.  This is a major milestone on the path towards 100 percent global air traffic surveillance. Aireon now begins a rigorous, in-depth testing and validation process, verifying the capability of the ADS-B payloads. 
Aireon - MAKING GLOBAL AIR TRAFFIC SURVEILLANCE A POWERFUL REALITY
Known as an Initial Performance Verification (IPV), this process will last approximately two weeks before transitioning to a Detailed Performance Verification (DPV) that will focus more specifically on calibrating the payloads for optimal performance.  Procedures for the IPV were under development for over a year with valuable insight provided by Iridium, Harris Corporation, NAV CANADA and the Federal Aviation Administration.
"When we first turned on the payloads after they reached orbit, we received an unexpected surprise – aircraft were immediately being seen in real-time," said Aireon CEO, Don Thoma. "We've already seen commercial aircraft, general aviation aircraft and helicopters, in oceanic and remote airspace that have never before had real-time surveillance. The real fun for us now begins as we take control and push the performance to see just what space-based ADS-B can do."
In late January, the first "power-on" for the AireonSM ADS-B payload was performed to validate launch survival and run a self-diagnostic systems check.  During this systems check, Aireon did not expect to see any aircraft, however the payload antennas were active and 150 ADS-B aircraft were detected over multiple-passes with thousands of ADS-B messages received and decoded.  Among some of the firsts for the Aireon system are:
The first officially received ADS-B message was Korean Airlines 530
The first full ADS-B position message decode was SAS 4576
The first U.S. carrier ADS-B message was UA 86 to Shanghai
The first aircraft detected landing at an airport was NextJet 594 in Sweden.
"Detecting NextJet 594 was an added bonus," said Vincent Capezzuto, CTO for Aireon. "This is an early proof-point of the power of the Aireon ADS-B payload and its ability to detect aircraft close to the ground and in terminal airspace."
Aireon's space-based ADS-B system will be operational in 2018, shortly after completion of the Iridium NEXT satellite constellation.  The service will provide Air Navigation Service Providers (ANSPs) with global air traffic surveillance and airlines with real-time flight tracking.  It is expected to help reduce fuel costs, increase safety, and enable more efficient flight paths.  The first 10 Iridium NEXT satellites carrying the Aireon hosted-payloads were launched into low-Earth-orbit from Vandenberg Air Force Base on a SpaceX Falcon 9 rocket, on January 14, 2017.  Seven additional SpaceX launches are scheduled to take place over the next 12 to 15 months, including the second launch now targeted for mid-June 2017.  In total, the operational constellation consists of 66 satellites, while the remaining nine launched will serve as on-orbit spares.

Aireon Takes Full Control of First ADS-B Hosted-Payload
Major milestone kicks off initial validation of space-based ADS-B surveillance
MCLEAN, Virginia, March 2, 2017 /PRNewswire/ -- Aireon announced today that they have formally received control from Iridium Communications Inc. (NASDAQ: IRDM) of the first Automatic Dependent Surveillance-Broadcast (ADS-B) payload hosted on an Iridium NEXT satellite.  This is a major milestone on the path towards 100 percent global air traffic surveillance. Aireon now begins a rigorous, in-depth testing and validation process, verifying the capability of the ADS-B payloads. 
Aireon - MAKING GLOBAL AIR TRAFFIC SURVEILLANCE A POWERFUL REALITY
Known as an Initial Performance Verification (IPV), this process will last approximately two weeks before transitioning to a Detailed Performance Verification (DPV) that will focus more specifically on calibrating the payloads for optimal performance.  Procedures for the IPV were under development for over a year with valuable insight provided by Iridium, Harris Corporation, NAV CANADA and the Federal Aviation Administration.
"When we first turned on the payloads after they reached orbit, we received an unexpected surprise – aircraft were immediately being seen in real-time," said Aireon CEO, Don Thoma. "We've already seen commercial aircraft, general aviation aircraft and helicopters, in oceanic and remote airspace that have never before had real-time surveillance. The real fun for us now begins as we take control and push the performance to see just what space-based ADS-B can do."
In late January, the first "power-on" for the AireonSM ADS-B payload was performed to validate launch survival and run a self-diagnostic systems check.  During this systems check, Aireon did not expect to see any aircraft, however the payload antennas were active and 150 ADS-B aircraft were detected over multiple-passes with thousands of ADS-B messages received and decoded.  Among some of the firsts for the Aireon system are:
The first officially received ADS-B message was Korean Airlines 530
The first full ADS-B position message decode was SAS 4576
The first U.S. carrier ADS-B message was UA 86 to Shanghai
The first aircraft detected landing at an airport was NextJet 594 in Sweden.

De avond van het begin van je vakantie. Vaak werkt het zo dat je nog bezig bent de drukte uit je lichaam te krijgen en dat zal de eerste dagen ook nog wel zo zijn. Vermoeidheid is een erg zwaar gevoel. Zeker als je op dat moment nog zoveel wilt of moet doen. Zondag gaan we op pad. Voor de derde keer zullen we in dezelfde periode het Zweedse Lapland opzoeken. Voor ons denk ik een tweede thuis gevoel. Gedurende het hele jaar zijn er veel momenten waarop we bezig zijn met het dromen over eerdere bezoeken of over mogelijk komende. We hebben het gebied in ons hart gesloten. Voor Wieke was dit al veel langer zo, maar het heeft ook mij gepakt. Ik heb er al vaker over nagedacht wat dan dat gevoel is dat deze plek opwekt. Het lijkt op een sobere houten stoel in een huiskamer die een bepaalde warmte uitstraalt. Een haardvuur of een houtkachel. Met een kop thee. Een vloerkleed. En een fijn boek. Degelijke kwaliteit in knusheid gedoopt. Wilde dieren en wildernis in een wollen muts. Bessen in een kwark of yoghurt en sigaren. Glooiende heuvels en witte woestijnen. Rook en rendierworst. Goed een ratjetoe van gevoelens en ideeën die het plaatsje bij ons oproept. Van Amsterdam zullen we naar Arlanda Stockholm vliegen.
