"Get everyone in the trucks! Go! Go!" Winters commanded his men, walking down the line of waiting vehicles.
Doc Roe looked up from wrapping a bandage around a man's shoulder. He watched the men scurry around like ants after a child kicked their home in childish glee.
Operation Market Garden was collapsing around them. Since learning about the operation, while the other men cheered at the idea of Berlin by Christmas, Roe felt nauseous. Something about the operation did not sit well with him. Call it intuition or a sixth sense, he just knew it could not be that simple. Before they dropped into Holland, he had convinced all the medics he knew to bring extra bandages and morphine, anything they could get their hands on. He hoped it would be enough.
"Can you get up?"
The Private grimaced, face pale. "Thank you, Doc. I'll find a seat myself."
"We'll find you some morphine at the aid station." Roe nodded at the Private before starting along the line of trucks, eyes and ears open for the call of a medic.
He caught sight of Spina helping a limping Private onto a truck bed before climbing in himself to continue administering aid. As he continued, he checked people off his mental list. He saw Shifty, Moore, Liebgott, Ramirez, Tanner, Talbert...more and more raced back to the trucks, some with blood tainting their skin and uniforms. He would have to check on them later.
There was one face in particular he had not seen yet.
The further he walked, dodging men retreating, the more his heart pounded in his ears. She was supposed to be in the back. She was supposed to be safe.
"Non, non, non, où es-tu?" He whispered to himself, panic creeping under his skin. (No, no, no, where are you?)
Ever since she had been wounded in Carentan, Winters had kept her from the front lines unless absolutely necessary. Usually, she was holed up somewhere and watching the men's backs, using her sniper skills. Shifty may be their sniper on the ground, but she was their sniper from above. Their guardian angel. If she had been sent to assist...the situation was worse than Roe had previously thought.
Sometime between Toccoa and Aldbourne, he something shifted between them. He could not pinpoint an exact moment. It was somewhere in the lingering touches, the soft smiles exchanged, the comforting words and stories to cheer one another up...somehow, he had fallen in love. Now he needed her just as much as air. She was his lighthouse, his beacon of hope. She had to be safe.
Glancing to his left, he headed towards the truck with what looked like most of her platoon in it. "Luz! You seen y/n?"
Luz looked down at him from his seat. "No, Doc. Last I saw she was protecting the retreat." Remorse and concern filled his eyes as he answered. They both knew what that meant. She would be the last to evacuate, the last to safety, the one most likely left behind.
Roe started running.
"Merde." He swore, eyes darting everywhere for a sign she was nearby, that she was alright. She had to be. It was that loyalty that he both loved and hated in her, how she would make sure "her boys" were safe, protecting their backs and lives, and in the process, be willing to sacrifice her own.
"Y/n! Y/n!" He began calling out in desperation. Ignoring the stares of some of the men, he wrestled internally with his own panic. His cries bordered on hysterical at this point.
They had never spoken those three little words to each other. Never put a name to what existed between them. They just were whatever the other one needed. Over the past two years, that grew until they were each other's world. How they sought each other out. How in a group, they always looked for the other first before anyone else. The quiet nights they cried together.
"Richardson! Is y/n with you?"
The man glanced around the back of the truck that he was waiting to jump into. "No, Doc. She missing?"
Roe did not wait to reply. He kept moving. He had to. Otherwise the growing terror in his mind would consume him. "S’ll vous plaît, Que Dieu la laisse être en sécurité. S'il vous plaît." (Please, God let her be safe. Please.)
"Doc! Over here!"
He zeroed in on Toye's call at the last truck. Pushing anyone who got in his way, he raced to where Toye stood watching. Once Toye noticed him coming, he sat back down, turning back to the person sitting next to him.
Rounding the side, he felt his heart and mind restart as he saw her. She sat next to Toye and some other Private on the uncomfortable wooden bench, her beloved rifle in her lap. As if sensing him, she looked up to meet his gaze. A soft smile -his smile- touched her lips. The panic and terror threatening him vanished like a mist in the wind.
"Mon amour." He breathed out, relief flooding him. There was dirt smeared on one side of her face and a small cut on her chin. Otherwise she appeared fine, even if her clothes looked disheveled and dirty beyond saving. She was alive. (My love.)
Without a further word, he jumped up into the truck and knelt down in front of her. "Hey, you." He held her face in his hands, gazing into her eyes for a long moment before pressing their foreheads together.
"Hey you." She whispered back, her hands cupping his own face. He could taste her breath on his lips. "I'm alright, Gene. I promise."
Leaning back, he ran a hand through her hair, wondering what happened to her helmet. Then he felt something wet and sticky on his fingers. Pulling his hand back, he saw blood on it...her blood.
Immediately he went into overbearing doctor mode.
"What happened?" He demanded, turning her head to the side and trying to find where the bleeding stemmed from.
"I'm fine."
He glared at her before resuming his inspection. Carding his fingers through her hair for an extra second, he began inspecting underneath, silently praying it was minimal. He could not lose her. There was nothing gushing so that was a good sign. She still seemed coherent, her eyes were not dilated, other good signs.
"I just hit my head a little on a brick wall when one of those explosions went off. It's not a big deal."
"Doll, shut up and let the man look you over." Toye drawled from beside her, watching the two in amusement.
Roe rolled his eyes as she stuck her tongue out at Toye. She winced when he touched a particular spot on the side of her head, hair matted with blood. After a moment, he leaned back on his knees and began digging through his medic bag.
"There’s a laceration but it doesn't look deep. You're lucky." He wrapped a bandage around the cut and her head to hold it there. Suppressing a chuckle, he continued to wrap it as she narrowed her eyes at him. She hated being fussed over. In his opinion, she could deal with it. She needed to get better...for both of their sakes.
The truck suddenly lurched, beginning its escape from Holland and the disastrous mission.
Roe shoved the Private next to her over, ignoring his protest, and slipped to her other side. Quietly he took her hand in his as his eyes scanned the others. He had been so focused on her; he did not even think about if any of the other men in the truck were injured. Luckily none were.
"I'm sorry I scared you. I had to help." She murmured, voice barely above the rumble of the trucks.
He sighed, squeezing her hand then pulled her closer into his side. "I know, mon amour, I know." He looked down, meeting those eyes he adored. "Doesn't mean I have to like it."
She laid her head on his shoulder. "No, but you can't protect me from everything. This is war."
"I can try. Tu es tout pour moi. J’ai besoin de toi à mes côtés. Toujours. Je promets après cette guerre de continuer à vous aimer et à vous chérir comme vous le méritez." The words poured forth, needing to be said, needing to be spoken aloud, even if just for his own sake. Though he meant them with all his heart. (You are everything to me. I need you by my side. Always. I promise after this war to continue to love and cherish you as you deserve.)
"Gene, you know I don't speak French. It's not fair cause I wanna know what you're saying."
"I'll teach you, y/n. I promise." He could not help but press a chaste kiss to the top of her head as they bounced along the road.
Looking at the blue sky and clouds above, he prayed he would be able to fulfill both of his promises to her, that they would both make it through this war. Most of all, he prayed for her to be safe, even at the expense of his own life.

What exactly is harm reduction? Why is it important? What does it have to do with me? Let’s start by exploring what it means for us in the context of substance use.
Harm reduction is a philosophy of action that takes many diverse forms. That philosophy of action stems from the fundamental belief that all humanity is valuable and worthy of life. A major component of drug harm reduction is identifying what potential harms may exist for people who use substances; whether it be harmful policies surrounding substances and use, the stigma people who use substances suffer, the risks of an unregulated drug market, transmission of disease, or equitable access to safe supplies and lifesaving resources. Once these harms are identified, it is necessary to find ways to reduce or eliminate them.
A key facet of harm reduction is reducing stigma for those who use substances. It is important to note that by recent estimates, only 11% of people who use some of the most stigmatized drugs develop dependence or a substance use disorder (Csete. J. et al, 2016). By cultivating an open and compassionate atmosphere around substance use, we can emotionally and physically be there for our friends and family, as well as ourselves. Practicing empathy and understanding is an important piece of harm reduction. If you use substances, or have loved ones who do, strive to create support networks and an honest, non-judgmental space about substance use. If we honestly learn to listen and care for one another, we can look out for our community and ensure people live safer and healthier lives.
This critical philosophy can be practiced by every individual, as well as by groups. As individuals, the simplest form of harm reduction is practicing healthy habits: ensuring you have adequate sleep, proper hydration, exercise, etc. On the group scale, harm reduction organizations ensure equitable access of harm reduction materials to the most stigmatized and marginalized communities. They work tirelessly to provide lifesaving resources, education, and safe supplies to those that are most affected by the war on drugs, and the stigmatization of substance use. This can take forms such as: regularly testing people for HIV/Hep C., providing clean injection supplies, providing safer smoking and snorting supplies, providing narcan/naloxone (an opioid overdoes reversal agent), providing fentanyl testing strips, assisting with housing for HIV+ members of the community, and much more.
“So, why is it important?” Well, for one we are in the midst of an overdose crisis that is directly caused by current drug laws and the unregulated drug market. Across the country overdoes deaths have risen dramatically in the past few years, even causing the CDC to release notice to all medical professionals and organizations to prioritize harm reduction efforts and tactics (Center for Disease Control, December 2020). Even here in Missoula fatal overdoses have increased 77% since 2019 (Missoula City/County Rx TaskForce). It is important to remember that each one of these deaths is a human life: a mother, a daughter, a son, a friend, or a partner. Each of these people are worthy and deserving of life, and it was taken from them by current attitudes and policies surrounding substance use.
It is also incredibly important to note that non-fatal drug overdoses are increasing as well. You may wonder, “Why are overdoses increasing so dramatically?” The answer to that is a complex one. One of the largest factors is the fact that in the U.S. there is no safe and regulated supply to most substances, or free testing of these substances, as they are often criminalized unless prescribed. This causes unregulated drug markets to complete amongst each other to meet the demand in our country, which leads to cut/poisoned drug supplies. Another substantial factor is the prevalence of the drug Fentanyl and its many analogues in the unregulated drug market.
Fentanyl is a synthetic opiate, like opium or morphine, but with a potency nearly 100x more than morphine, and 10x more than heroin. Some of the fentanyl analogues, such as carfentanil, are even 100x more potent that fentanyl. (National Center for Biotechnology Information, 2021). Fentanyl has been found in many drugs in the U.S. and here in Missoula. We know fentanyl to be in supplies of heroin, methamphetamine, cocaine, and even in pressed pills designed to replicate xanax or oxycodone, among other drugs. Due to the small amount of fentanyl required for a lethal dose (approximately 2 milligrams), many more people are overdosing.
“How can we work to correct this issue?” “How do we know if the drugs we plan to take are adulterated?” The best thing we can do is to educate ourselves! Do personal research on the substances you would like to use, and take note of any interactions they might have with any prescriptions you may take or other substances you plan to use (Here are a few web resources: CAIRNmontana.org, Dancesafe.org, Erowid.org, combo.tripsit.me). Fentanyl testing strips are a cheap and effective way to check your drugs for fentanyl before ingesting them. Another incredibly valuable resource is narcan, an overdose reversal agent (in an easy to use nasal spray form). Book Review: ‘Dreamland’
Dreamland: The True Tale of America’s Opiate Epidemic by Sam Quinones
In Dreamland, Sam Quinones tackles the current opioid crisis in the United States with impressive thoroughness. His goal in researching the epidemic was The Big Picture---unfortunately, such comprehensiveness may not be possible. Nonetheless, he does an admirable job, making Dreamland one of the few should-reads for contemporary America.
There is a lot in this book. Broadly speaking, the material can be divided into two basic categories: the legal, and the illegal (specifically represented by Oxycontin, and Black Tar heroin, respectively). Broadly speaking.
On the legal side, Quinones interviews doctors, researchers, nurses, clinicians, patients-cum-addicts, Oxycontin dealers, and families of the lost; he has chapters on the poppy plant, opium, and morphine; he discusses Arthur Sackler and the rise of pharmaceutical marketing, which included the wooing of doctors; and he traces the history of pain treatment---the medical treatment of pain being one of the largest enablers of the opium epidemic.
It’s hard to call what occurred in the medical establishment an overt conspiracy since, outside Big Pharma, most intention was good. Most.
It began because opium derivatives are pain killers---and the pain-killing properties of opium are ancient news. Also well-known was the fact that opioids are addictive, and use was restricted and monitored. Cancer changed that, with doctors advocating the use of morphine, etc. for end-stage cancer patients who were dying in horrible pain. At that point, it was a quality-of-life issue, and the campaign was successful.
Unfortunately, around the same time that pharmaceutical companies were honing their marketing skills, and developing opium-derivatives, medicine decided to extend the quality-of-life logic to all forms of pain. Injured? Opioid. Chronic back pain because you don’t lift with your legs? Opioid. Whatever the pharmaceutical company had, that was the solution.
And as for that habit-forming problem of opium-derivatives? Turns out, according to an infamous “study” referred to as Porter and Jick, opioids aren’t really that addictive after all. That Porter and Jick was really just a letter to the editor noting one pattern in a limited pool of data either didn’t matter, or, more probably, was never known by the people who cited it (a couple of whom Quinones interviews). Yes, bad science became a major contributor to our opioid crisis. No one checked the “study” that was repeated ad nauseam, and few paid attention to the data at hand concerning the high rates of addiction in patients who were prescribed Oxycontin.
Insurance companies didn’t help matters. They’d pay for prescription medication, but not for physical therapy, psychotherapy, nutritionists, or holistic/ multidisciplinary approaches to pain treatment. So doctors who wanted to use those non-pill approaches were put out of business.
But that’s just the legal opioids. On the illegal side, Quinones focuses on Black Tar Heroin, a viscous, dark-colored substance that’s as pure as heroin can get. He interviews cops, dealers, DEA agents, addicts, lawyers, and community organizers. And because he follows the Black Tar, he discusses Xaliso, a city in the state of Narayit in Mexico, where the heroin was grown.
Black Tar heroin dealers changed the game. Just about everything about them confounded police. The dealers didn’t carry guns, didn’t fight turf wars, and feared local gangs. They didn’t use, and they didn’t adulterate their product by cutting it---they didn’t need to as there was no multilevel distribution line. They delivered to their customers, carrying a minimum of heroin, so if caught, D.A.s rarely felt they were worth the time, and they were just deported.
Deportation was not a deterrent. The Xalisco Boys, as Quinones calls them, dealt heroin in order to make money and be the Big Man struttin’ around town back home. In the United States, their lives were spartan---the point was to go back home to their sugarcane-farming town, having struck it rich. Unfortunately, the dealers spent too lavishly, didn’t invest or save, so money ran out, and they had to return to the U.S.
The system the Xalisco Boys had was capitalism at its finest. No corporate headquarters, no cartel bigwigs, just small entrepreneurs competing in an open market. They had excellent customer service. Not only did they deliver like pizza, they offered samples, and deals. Addicts would get extra heroin for introducing a new customer. The Xalisco Boys would pay (in heroin) for an addict to hand out samples at the local methadone clinic. They’d exchange drugs for your name on an apartment or car lease. And when word got back to Narayit of ripe territory, a gold-rush mentality set in, and more cells would show up in town to compete for the same customers. Prices dropped.
Besides being non-violent in general, in Xalisco, the culture included webs of long-standing family feuds that few wanted to challenge. Because if anything untoward happened in the U.S., it *would* get back to Mexico.
Thus the more adventurous dealers would strike out for new territory. If an addict happened to know of another possibly-lucrative town, they’d introduce the Boys.
One dealer Quinones interviews, called The Man, was actually born in the U.S., and entered the Xalisco system from the outside. Of Mexican descent, he spoke English and Spanish, and recognized the relevance of Oxycontin to heroin sales (most of the Xalisco Boys didn’t bother learning more than functional English, so didn’t see the bonanza). He was arrested before he could really pave the way, but others did so after him.
Part One of the book ends with the arrest of numerous dealers in multiple states in one of the largest, most coordinated operations in drug-enforcement history. But the insidiousness of the Xalisco system is that it’s self-renewing. Arrest one guy, someone else will take his place. As long as there’s demand.
As I stated above, there is a lot in this book. Quinones manages this plethora of information by “braiding” or “juggling” it: he tells the story of the family of a young man who od’d; introduces a dealer called Enrique; moves to Dr. Herschel Jick wondering about rates of narcotic addiction in patients at his hospital; reveals how Dr. David Proctor came to Portsmouth, OH (he wasn’t able to interview Proctor in person); Enrique again; a cop in Denver; the morphine molecule; Proctor; Enrique; a cop in Idaho; Xalisco culture---this is not the actual order he goes in, but it is the gist of his technique. He touches base with each thread, showing how they interweave. It’s an interesting approach that I think works well for the material, but it did get difficult sometimes to remember who was who. I’d use the index in those cases.
Two limits before I end this already long review. First, the heroin angle is limited to the Black Tar out of Xalisco. Other heroin dealers, gangs, cartels, etc. are mentioned only when relevant to Black Tar and the Xalisco system. So, for example, the Xalisco Boys avoided New York City like the plague, so not much about the presence of heroin there in Dreamland.
Second, racism is prevalent throughout, but not explicitly explored (this is a limit, not a detraction). The Xalisco Boys would not deal to blacks if they could help it---mostly out of fear derived from prejudice. But the far more subtle forms of racism, especially in the predominantly white middle-American towns exist like smog---it’s there, it’s not good, but those immersed in it stopped noticing. Quinones points it out a couple of times, but the degree to which it’s relevant is up to the reader (since his topic isn't the various connections between racism and drugs, he may have decided to just let instances of racism stink up the background and not turn Dreamland into a doorstop).
Highly recommend this book.
