Patient-Centered Care in Long-Term Care Hospitals:Â What You Need to Know
The aim of long-term care hospitals (LTCHs) is patient-centered: to give each patient respect, dignity, and respect through specific care. One needs to understand the principles underlying such an approach in order to ensure that, whether it's for you or someone close to you, you will be given the best care possible. In this article, we will cover what patient-centered care really is in LTCHs, the essential parts it involves, and how it may enhance the condition of patients, families, and healthcare providers as well.
What is Patient-Centered Care?
Patient centered care in LTCHs deals with how every patient, over the course of their hospital stay, brings forward their personal needs, preferences, and values and makes them important to care. This is not a one size fits all approach, but rather it addresses the patient as the one who can take an active role in their care. Shared decision making and attention to what matters most to every individual produce a more patient-centered experience.
Key Components of Patient-Centered Care
There are a few key elements that form the building block of patient-centered care in long-term care hospitals:
Individualized Care Plans: The treatment given to a patient is specific to the type of medical problem, functional level, and personal goals for each patient. This would entail treatment to address the ailment a person has and also his/her preferences about what he/she values most.
Effective Communication: Patients, families, and healthcare providers must practice open and transparent communication. A dialogue with empathy will enable the individuals to be on the same page concerning what is expected in terms of the care plan, and the patient is heard and respected.
Respect for Dignity and Privacy: Maintain patient dignity and respect privacy at all times. LTCHs are obligated to create environments that respect personal space and related comfort levels, other cultural values.
Holistic Support Services: Here, the care done in LTCHs is not only about relieving the medical ailment of a patient. Psychosocial support, spiritual care, and recreational activities, daily living assistance ensures complete care for the physical, emotional, and social wellness of patients.
Shared Decision-Making: Patients and their families are also fully engaged in all aspects of planning care. Patient-centered care enables patients to make decisions based on information provided, supporting them in making choices that fit well with their personal values.
Benefits for Patients, Families, and Providers
For patients and their families, it connotes respectfulness as well as a valuation of patients in the treatment process by healthcare services. The patient-centered approach also implies that there should be portions of control given to the patient during the whole process of treatment so that fear and anxiety would be eliminated and trust and faith restored in the treatment procedures followed. Healthy relationships are developed between the patients' families as well as the healthcare teams in addition to increased satisfaction with the treatment experience.
For healthcare providers, embracing patient-centered care is embracing a compassionate and empathetic approach. This requires listening and hearing the needs of patients and those of their families and being involved in the process of care. In other words, continual adjustment of treatments to meet the needs of the patients needs to be carried out. This philosophy, therefore, improves the outcomes for patients aside from engendering trust between the two: the patient and the members of the medical staff.
Conclusion
The patient-centered approach is the foundation of quality health care in long-term care hospitals. This can be achieved via individualized treatment, open communication, and cooperation in decision making. A patient feels empowered, respected, and cared for when in such an environment. Understanding the importance of patient-centered care as a patient or family member ensures that needs and preferences are always prioritized in a health care journey.

The Stigma of the Dark Ages.
What theyâre talking about here is a society which has moved backwards, and is paying consequences already.
NPR - As the respiratory virus season approaches, where does the vaccination rate stand? November 27, 20244:47 AM ET Heard on Morning Edition By Rob Stein , Rob Schmitz Part of it is the lingering skepticism and outright hostility from the pandemic toward the COVID vaccine specifically and vaccines in general. Another factor is that people tend to underestimate how dangerous both viruses can be while overestimating vaccination risks. There's a lot of misinformation about how well the vaccines work and how safe they are. And finally, a lot of folks are just sick of vaccines because of all the shots they've gotten over the last few years. You know, put it all together and a lot of people are just feeling kind of done with vaccines. I talked about this with Dr. Gregory Poland. He's president of the Atria Academy of Science and Medicine in New York. GREGORY POLAND: âAs a society right now, we're in a phase of rejecting expertise, of mistrust of any expert, whether it's science, meteorology, medicine, government - whatever it is.â
This is not unusual, there is no guarantee that society progresses forward. The Dark Ages happened, and that period was not the only time of regression on science.
MedPage Today - Nursing Homes Fell Behind on Vaccinating Patients for COVID â Billing complexities and patient skepticism partially to blame by Sarah Boden, KFF Health News December 5, 2024Â Loveland has seen patients and coworkers at the nursing home where she works die from the viral disease. Now she has a new worry: bringing home the coronavirus and unwittingly infecting her infant daughter, Maya, born in May. Loveland's maternity leave ended in late June, when Maya wasn't yet 2 months old. Infants cannot be vaccinated against COVID until they are 6 months old. Children younger than that suffer the highest rates of hospitalization of any age group except people 75 or older. Between her patients' complex medical needs and their close proximity to one another, COVID continues to pose a grave threat to Loveland's nursing home -- and to the 15,000 other certified nursing homes in the U.S. where some 1.2 million people live. Despite this risk, a CDC report published in April found that just four in 10 nursing home residents in the U.S. received an updated COVID vaccine in the winter of 2023-24.
Going forward is a choice.
Public comment to CDC HICPAC committee November 2024 Infection control in healthcare. Chloe Humbert Nov 15, 2024Â The Dark Ages was called that because society moved backwards from the technological advances that had come before. The fall of the Roman Empire was marked by elites who only cared about the status quo; they couldâve developed a steam engine as far back as Heron in 15 BC but didnât bother. Going forward is a choice. In an article in the Journal of Infectious Diseases & Preventive Medicine thereâs a description of what happened back then. âIn medieval times, hospitals were hazardous places, Epidemic infections killed large numbers of hospital patients during this period. Hospital infection and death rates were high. When a sick person entered a hospital, his or her property was disposed of, and in some regions, a requiem mass was held, as if he or she had already died.â Going backward is a choice.
Stigma is part of a backward slide, and even if people donât choose to go backward, we are all subject to community level leadership influences.
Itâs called STIGMA. - wat3rm370n on tumblr - Oct 4th, 2024 When you hear that âpeople are tired of itâ - thatâs also part of stigma. And itâs not necessarily true that people are actually just sick of it - but they keep being told they should be. Informational learned helplessness can do that to us. Stigma is leveraged and reinforced on purpose by big money industry interests who think any reminder of danger at all is bad for business. So itâs to some degree manufactured stigma.

Long-Term Acute Care Hospital Admission Criteria | Long Term Acute Care
Long-term acute care hospitals (LTACHs) specialize in treating patients with complex medical conditions requiring prolonged hospitalization, typically longer than 25 days. These facilities are designed to provide intensive care for patients who are too stable for a traditional acute care hospital but too unstable for lower levels of care, such as rehabilitation centers or nursing homes.

700 Palestinians were killed in the last 24 hours and the airstrikes are more violent each night. Gaza's hospitals have fuel left for two more days. Israel only allowed aid into Gaza on the condition they didn't carry fuel. The Indonesian hospital has shut down already, because doctors have no supplies and no choice but to let the wounded die. They're calling it a collapse but the term doesn't do it justice.
Over a 100 incubator babies are at risk. There are 50.000 pregnant women in Gaza right now, and 5.500 due to give birth this month. Menstruating people are taking pills in order to stop their periods, because they do not have pads or water to maintain hygiene. Surgeons are operating without anesthesia. Water is not reaching Gazans because there's no electricity or fuel for water pumps.
There's no excuse for this. Israel justifies the airstrikes by saying they want to destroy Hamas infrastructure and release the hostages, but they have refused to negotiate for their release. Hamas informed Israel they wanted to release two elderly women without anything in return, and Israel refused. Netanyahu said they wouldn't take their own civilians back because it was "mendacious propaganda." When the hostages were finally released, Netanyahu prohibited the hospital from giving press releases. Yocheved Lifshitz went behind their backs and talked to the press anyway, saying she was treated very well by Hamas, but the government abandoned them. They're being used as straw men. Israel is conditioning the entry of fuel to the release of hostages and yet, according to The Wall Street Journal, when Hamas proposed to exchange 50 hostages for fuel they denied. IDF officials have said they fear the release of more hostages because that might withhold the order to their ground invasion. They do not care as long as they can use the hostages as a pretext for their slaughtering.
There's a turning tide for Palestine in public support. Support for Israel was built through decades of propaganda and we are making a dent into it. Zionists are desperate, holding zoom meetings to promote zionism, but we have to do so much more. We have to shame people in power into supporting the Palestinian cause.
Keep yourself updated and share Palestinian voices, looking to inform yourself from the sources. Palestinians have asked of us only that we share, tweet and post, over and over. Muna El-Kurd said every tweet is like a treasure to them, because their voices are repressed on social media and even on this very app. Make it your action item to share something about the Palestinian plight everyday. Here are some resources:

Take action. You can participate in boycotts wherever you are in the world, through BDS guidelines. Right now, they are focusing on boycotting (don't be overwhelmed by gigantic boycott lists. Only boycott additional brands if you can):
Carrefour
HP
Puma
Sabra
Sodastream
Ahava cosmetics
Israeli fruits and vegetables
Push for a cultural boycott - pressure your favorite artist to speak out on Palestine and cancel any upcoming performances on occupied territory (Lorde cancelled her gig in Israel because of this. It works.)
If you can, participate in direct action or donate. Palestine Action works to shut down Israeli weapons factories in the UK and USA, and have successfully shut down one of their firms in London. Some of the activists are going on trial and are calling for mobilizing on court.
Call your representatives. The Labour Party in the UK had an emergency meeting after several councilors threatened to resign if they didn't condemn Israeli war crimes. Calling to show your complaints works, even more if you live in a country that funds genocide.

R.I.P., My Love (part i) // Tory Dent
Let us be apart then like the panoptical chambers in IC patient X and patient Y, our names magic markered hurriedly on cardboard and taped pell-mell to the sliding glass doors, "Mary", "Donald", "Tory"; an indication that our presence there would prove beyond temporary, like snow flurry. Our health might be regained if aggressive medical action were taken, or despite these best efforts, lost like missing children in the brambles of poor fortune. The suffering of another's I can only envision through the mimesis of my own, the alarming monitor next door in lieu of a heartbeat signifying cardiac arrest, prompts a scurry of interns and nurses, their urgent footsteps to which I listen, inert and prostrate, as if subject to the ground tremors of a herd of buffalo or horses, just a blur in the parched and post-nuclear distance. I listen, perhaps the way the wounded will listen to the continuing war, so different sounding than before, the assault of noise now deflected against consciousness rather than serving as motivation for patriotism and targets. Like fistfuls of dirt loaded with pebbles and rocks thrown at my front door, I knew that the footsteps would soon be running to me also. The blood pressure cuff swaddled around my arm pumped in its diastolic state independently like an iced organ ready for transplant as I witnessed with one circular rove of my eyes my body now dissected into television sets, like one of those asymmetrical structures that serves as a model for a molecular unity in elementary science classes. And the plastic bags of IV fluids that hung above me, a Miró-like mobile or iconic toy for an infant's amusement, measured the passing of time by virtue of their depletion. Sometimes I could count almost five and then seven swinging vaguely above me at 4 am. I remember the first, hand-held high above me when I arrived via ambulance at the ER, the gurney accelerating as a voice exclaims on the color of my hands "they're blue!". Another voice (deeper) virtually yells out into the chaos that she can't get a pulse. Several pairs of scissors begin simultaneously to cut off my clothes, their shears working their way upward like army ants from pant cuff and shirt-sleeve, a formulaic move for the ER staff which, despite its routine, still retains a sense of impromptu in the hurriedness of the cutting both deft and crude, in the sound of their increased breathing, of their efforts intensified by my blood pressure dropping, the numbers shouted out as if into night fog and ocean. It's not a lack of professionalism but the wager of emotional investment that I feel. One attendant, losing her aplomb for a moment, can't contain herself from remarking (as if I'm already post-mortem) on what a great bra I have; "Stretch lace demi-cup, Victoria's Secret," I respond politely in my head.

#tag talk#ngl people are like âoh I stopped cutting for x months!â and talk about it like a thing you have to choose to stop#like any avoidant behavior if you remove the underlying cause you remove the motivation for the behavior#otherwise you're just spending energy trying to brute force a behavior. and you don't have energy to spare#anyway. if the state fucks up my health insurance I'll gonna flip my shit. I'm already drawing out my next design in my head#I need to make a phone call tomorrow morning so of course I'm up past midnight stressing about it#anyway. I wanna compliment the slash across my chest with some sort of zigzags right below my ribs.#I don't have any curves except the few that follow the contours of my biceps. maybe a curve around the abs/side?#anyway. ugh. I just want gender affirming care so I don't hate being alive.#something something people say they care and then put their vote with the camp that wants me dead.#idk I'm still mad about conservative âfriendsâ and family. whatever#I'm just complaining. nbd. at this point I'm too committed to back out so I'll just resubmit my application and make phone calls if I need#I'm not putting myself in the hospital a second time. once was enough.#I like being able to say it's a thing I did but it's not a thing I feel the need to do more than once.#whatever. I'm feeling less good now even though I was feeling good a few days ago but it's because my application is still pending#âpending reviewâ bitch it's been pending review for the past five days I really really need to make that phone call it's just...#phone calls amirite?#anyway. I'm not worried about whether or not I'll be able to get hormones. rolling up my sleeve gets me past any medical gatekeeping#because they look so cool obviously.#but damn I do not have the money to pay for that shit long term without insurance covering like 99.99% of the cost.#tbh I've wanted some kind of bird design matched up against my shoulder blades for such a long time#but I would need someone else to do it. and I don't feel like looking up actual scarification places and paying money for it#one of these days I'll make friends with someone who's appropriately freakish enough to wanna slice pretty pictures into my skin#cause like. yeah biting people is nice but have you ever carved a red line into someone's willing body?#that's the shit right there.#anyway. this has been your irregular dose of me talking. ughhhh life exists.#idk I try and be as normal as I can about the generally-agreed-to-be weirdness. like. idk. wanting to die is just everyday shit now#it's just my body why would it be weird to want someone to carve falcon wings in line with my back structure#it's just blood and muscle and when it heals well it's visual and nothing more. call it extreme body modification or whatver makes you happy

aries midheaven â
to gain recognition, individuals with aries in the 10th house must put in hard work, taking initiative and being unafraid to stand alone when necessary. their success often comes from their willingness to take risks and be the first to act, whether in starting a business, leading a project, or entering competitive fields like sports, military, or entrepreneurship. they build their reputation by consistently showing their courage and determination, often taking on leadership roles early and demonstrating their ability to manage high-pressure situations. they may face challenges such as impatience or aggressive competition, so they must learn to temper their impulsive tendencies with strategic planning and patience, ensuring that their bold moves lead to sustainable success.
â

â
cancer midheaven â
those with cancer in the 10th house gain recognition by building deep, supportive relationships and demonstrating their commitment to caring for others, whether in healthcare, hospitality, social work, or family-run businesses. they put in years of dedicated service, often starting in entry-level or hands-on positions to build empathy and expertise from the ground up. they work to create a nurturing, emotionally supportive environment in their professional sphere, becoming trusted figures who are known for their compassion and ability to connect deeply with clients or colleagues. the challenge for them is to maintain professional boundaries and ensure that their work doesnât become overly personal, as they risk burnout if they become too emotionally invested without managing their energy effectively.

Alberta Health Services says it has taken a number of steps to better train staff, after a Pasqua First Nation man's braids were cut off and thrown away while he was in an Edmonton hospital. Eve Adams went in May to visit her husband, Dexter Adams, at Edmonton's Royal Alexandra Hospital and found the 84-year-old's braids, his eagle feather and some medicine in a garbage can. He had been deemed at a high risk of falling and was in hospital waiting for a space to open in a long-term care facility. He died just over a month later.

"Vice President Kamala Harris is proposing to provide federal funding to cover home care costs for older Americans, aiming to help the â sandwich generation " of adults caring for aging parents while raising their children at the same time.
Appearing Tuesday on ABCâs âThe View,â Harris talked about taking care of her mother when she was dying and personally experiencing the challenges many families face when seeking affordable in-home care for their aging loved ones.
She promised that if, elected in November, she will seek to expand Medicare, the federal health insurance program for older Americans, so that it covers long-term care and includes services like in-home aides. Harris said aides could help seniors do things as simple as preparing meals or putting on sweaters because it is âabout dignity for that individual. Itâs about independence for that individual.â
Her proposal is a new one just a month out from Election Day but the issue is one that President Joe Biden's administration has been working on for years.
In an effort to soften the effects of inflation, the White House promoted as part of Build Back Better, its legislative agenda that stalled on Capitol Hill years ago, steeply increased federal spending for child care as well as for seniors. After Build Back Better collapsed, the Biden administration continued to promote increasing spending for what it calls âthe care economy,â a cause Harris has continued to mention after replacing Biden at the top of the Democratic ticket.
âThese plans are common sense. They can help family caregivers work and save both families and the federal government money by allowing seniors to stay in their homes instead of being sent to nursing homes,â the Harris campaign said in a fact sheet detailing her proposal. âMedicare at Home will also reduce hospitalizations.â
As part of a blitz of media interviews sheâs been doing in recent days, Harris sat down after her appearance on âThe Viewâ with radio personality Howard Stern, who said that his mother is 97. Taking care of an elderly parent, he said, âwill bankrupt you.â
Such costs have increased pressure on adults caring for their parents and kids simultaneously. In 2019, roughly 30% of family caregivers of older Americans lived in households that included children or grandchildren, according to AARP.
Harris would likely have to work with Congress to achieve key parts of her proposal. Harrisâ campaign points to past, similar proposals projected to cost $40 billion annually, but says much of that can be offset by savings achieved through efforts begun by the Biden administration to expand Medicareâs ability to negotiation prices with major drug manufacturers.
Harris is also promising to further expand Medicare to include hearing and vision coverage, while changing existing rules that can allow federal authorities to seize a deceased beneficiaryâs home to recuperate costs. [Note: I'm sorry the current rules fucking what] The campaign fact sheet says that practice âmeans that those homes are not passed on to the seniorsâ children, which particularly harms rural and minority families.â"
