He has the money and resources to just hire a private doctor or even hairstylist over to his house. They'd either be a close friend or a friend of one of his friend's who he knows won't leak any information. He'd be more strict about this with a doctor, since they'd have to look through his kids' medical histories and such.
With a dentist it'd be a little more complicated, but I can imagine he'd have plenty of connections to get a dentist who'd keep the situation quiet, as well.

What Went Wrong?
Ideally, let's not just treat cavities (DRILL n FILL) let's treat dental disease. In some cases, like the example of a four-year-old with secondary cavities, the primary cavities were drilled and filled, but due to a multitude of reasons, the tooth developed additional plaque built up and what we call a new cavity under the existing or dilapidated restoration (secondary cavities).
What is an Indirect Pulp Therapy (IPT)?
In indirect therapy, after the removal of old restoration and the secondary cavities, we are close to the nerve/pulp, but the nerve has not opened up, i.e., you don't see a red spot. Then the tooth is a good candidate for IPT. A preformed metal crown covers the cavitated milk tooth 360 degrees, avoiding any plaque/biofilm buildup, which if left in situ would cause secondary cavities. So when you cover the tooth completely, you provide an environment for repair, you cut off the nutritional supply to the bacteria that cause these cavities, and hence prevent the progression of the cavities under the crown.

Your pulp/nerve is a living tissue. When we get that close to it, we need materials that would provide an environment conducive to repair. When we pack restorations/fillings so close to the nerve, the tooth will eventually develop an abscess or pus-filled cavity.
