Palantir, a data analytics company known for its work in the defence and intelligence sectors, has announced a significant partnership with Microsoft. The collaboration aims to deliver advanced services for classified networks utilised by US defence and intelligence agencies.
According to the recent announcement, Palantir is integrating Microsoft’s cutting-edge large language models via the Azure OpenAI Service into its AI platforms. The integration will occur within Microsoft’s government and classified cloud environments. As this collaboration is the first of its kind, this specific configuration has the potential to completely transform the use of AI in critical national security missions.
Palantir, whose name draws inspiration from the potentially misleading “seeing-stones” in J.R.R. Tolkien’s fictional works, specialises in processing and analysing vast quantities of data to assist governments and corporations with surveillance and decision-making tasks. While the precise nature of the services to be offered through this partnership remains somewhat ambiguous, it is clear that Palantir’s products will be integrated into Microsoft’s Azure cloud services. This development follows Azure’s previous incorporation of OpenAI’s GPT-4 technology into a “top secret” version of its software.

Arthritis & the COVID vaccine: What you need to know

Some arthritis drugs may reduce the effectiveness of COVID vaccines, according to the Arthritis Foundation, which also offers advice on booster shots.
Research is limited, but evidence suggests that disease-modifying drugs used for arthritis may reduce the response of the mRNA COVID-19 vaccines from Pfizer and Moderna, according to the foundation.
A small study of 133 fully vaccinated people taking immunosuppressive medications found that their antibody levels and ability to neutralize the virus were three times lower than in people not taking these medications.
However, "most patients in the study were able to mount antibody responses in response to SARS-CoV-2 vaccination, which is reassuring," study co-author Dr. Alfred Kim, an assistant professor of rheumatology at Washington University School of Medicine in St Louis, told Reuters.
The study was published on the preprint server MedRxiv, and has not been peer-reviewed.
No research on the one shot Johnson & Johnson vaccine in people taking immunosuppressive medications has been reported, according to the Arthritis Foundation.
"Not all medications that our patients take have been shown to have significant effects on responses to vaccination," said Dr. David Karp, president of the American College of Rheumatology, who suggested patients ask their health care provider if they are likely to see benefits from additional vaccination.
"Luckily, we have not seen any safety signals in patients with autoimmune and rheumatic diseases from the COVID-19 vaccines, so there should be no concern for the third dose," he said.
Karp added that it's still unclear how much protection a third or fourth booster provides to immunocompromised patients, such as those with cancer, organ transplants and those taking immunosuppressive medications.
So, even if these patients do receive additional COVID vaccine doses, they should continue to take precautions such as physical distancing and wearing masks, Karp advised.
The U.S. Centers for Disease Control and Prevention advises immunocompromised patients who received an mRNA vaccine to seek third doses at least four weeks after their second shot. If possible, the booster should match their previous shots.
In other words, those who received two doses of the Pfizer-BioNTech vaccine should stick to that for their third shot. Those who initially received the Moderna vaccine should get a Moderna booster.
Immunocompromised patients who received the Johnson & Johnson vaccine should receive a second shot at least two months after their first dose. They can get a second J&J shot or an mRNA vaccine, the CDC says.
The Arthritis Foundation noted that there is some evidence that an mRNA booster may improve immunity in those who first received the J&J vaccine.

Elise Cooper Interviews Karin Slaughter
False Witness by Karin Slaughter shows why she is one of the best for writing fabulous characters. This story has two sisters’ childhoods tarnished by secrets, broken by betrayal, and ultimately destroyed by a brutal act of violence as well as one of the sister’s tragedy of addiction. She was portrayed with empathy and grace by the author. This story is brutal, honest, real, and heartbreaking at times involving issues of rape, drug use, murder, and abusive violence.
Kudos to Slaughter for balancing the strong relationship between the sisters. Readers will grow attached to the sisters Leigh, Callie and Leigh’s husband, Walter, as he becomes involved to help them. Each of the sisters have taken a different route through life. Leigh Collier has worked hard to build what looks like a normal life after being sexually harassed as a child babysitter. She’s an up-and-coming defense attorney at a prestigious law firm in Atlanta, who would do anything for her sixteen-year-old daughter Maddy, while managing to successfully coparent through a pandemic after an amicable separation from her husband Walter. The other sister, Callie, is a drug addict, but sympathetically humanized. She was a child gymnast and cheerleader, who after suffering a broken neck, had constant back pain. Her childhood experience involved being groomed and regularly sexually assaulted by a violent pedophile while babysitting his 10-year-old son, and turning her into a heroin addict.
As Leigh is asked by her boss to defend a serial rapist, she is confronted with her past. When she meets the accused face-to-face, Andrew, she realizes that it’s no coincidence that he’s specifically asked for her to represent him. They know each other. Leigh wonders how much he knows about what happened over twenty years ago. The only person who can help her is Callie, the younger, estranged sister. With the life-shattering truth in danger of being revealed, she has no choice to involve Callie. The shocking twist at the end of the book will keep readers engrossed.
This complex plot has as its central theme, the heart of the relationship between Callie and Leigh. Both these heroines are believable, flawed, and courageous. The highly intense themes, along with the horrible graphic actions of the evil doers, makes for a riveting read.
Elise Cooper: Why write about the pandemic?
Karin Slaughter: I started with a character, Leigh Collier, who I thought about three years ago. I knew I was going to write a story during the pandemic. When the book was finished the vaccines came, so I knew I wanted to incorporate the virus into the story. It was fun and challenging for me as an author. I tried to make Covid exist, but not as an intricate part of the story. I made sure not to politicize it.
EC: There is a difference between the two sisters?
KS: Some have the luxury to keep themselves as safe as possible and some do not. To highlight this, I used the two sisters, Callie and Leigh. Callie always had to work. Because of her addiction she had to be on the streets. Having a disability made her vulnerable.
EC: Why the addiction angle?
KS: I wanted to humanize someone struggling with addiction. She had an emotional, mental, and physical addiction. Hopefully, I showed how we are really failing in how to handle addicts and help them. Callie figured out a way to help herself through maintenance doses. If only addicts could get levels that could help them function in society and eventually wean themselves off of the drugs. Instead of punishing the people into the ground we should look at ways people could get help.
EC: Drug addicts?
KS: The personality of the person must be considered. If someone is actually a good person who is controlled by addiction, they are still decent. If someone is a horrible jerk, addiction will definitely amplify it. We spend trillions of dollars on the war on drugs, which has failed miserably. Imagine if we spent that money on helping low-income students get better Internet, classrooms, schoolbooks, nutritional meals, and safe schools. This would be more useful.
EC: How would you describe Leigh?
KS: She experienced a horrific trauma with her sister when they both were younger. She is now a successful lawyer. But Leigh is a control freak, compartmentalizes people, never likes to feel powerless, and is a survivor. She presents a front to hide her guilt and deep vulnerability.
EC: How would you describe the bad guy, Andrew?
KS: My grandmother used to say, ‘if someone wants to be bad, they will find any excuse to be bad.’ He is definitely an illustration of that. Andrew is someone looking for a reason to justify the bad things he wants to do. He thinks he should be able to do whatever he wants in life because he is entitled to do it. He is cruel and likes to terrorize people. When I wrote that fish scene with him, I laid a foundation for his personality.
EC: Speaking of fish, there is some humor?
KS: I love puns and love to be silly. It was delightful time for me to make up all that stuff, such as “Anne Chovey, Genghis Karp, Mr. Dar-Sea, and James Pond.” I spent far more time than I care to admit on this.
EC: There is a quote about prosecutors and judges caring more about optics than justice. Please explain
KS: Many prosecutors only take cases they think they can win. They plead out everything else. Many overcharge to get someone to plead out to a lesser charge. As voters we need to look at how the justice system runs. For example, women in prison are limited to the number of tampons and pads they can have.
EC: There is also a quote about losing someone. Please explain.
KS: You are referring to this one, “Your relationship with a person doesn’t end when they die. It only gets stronger.” Someone told me that after I lost my 9th grade teacher who I consider my mentor. She died about five years ago from cancer. I had all these memories of her. I remember our relationship and how important she was to me. The choices I make in the present are based on what she told me in the past.
EC: Your next book?
KS: It will have a murder and be out this time next year. A couple of characters from a previous stand alone will be back.

Where you die can affect your chance of being an organ donor
If Roland Henry had died in a different part of the country, his organs might have been recovered. And lives could have been saved.
But the local organ collection agency said no. It gave no reason, no explanation to his family, though the Connecticut man appeared to be a well-qualified donor despite advancing age: He died in a hospital, on a ventilator, previously healthy until a car crash that led to a stroke.
“It was devastating to be told there was nothing they considered worthy of donation. Nada. Not a kidney, not a liver, not tissue,” recalled Henry’s daughter, Donna Cryer, president of the nonprofit Global Liver Institute and herself a recipient of a liver transplant.
Henry’s case illustrates troubling uncertainty in a transplant system run by government contractors that are under fire for letting potentially usable organs go to waste.
The Associated Press took a close look at that system and calculated that some of those agencies are securing deceased donors at half the rate of others — even as 113,000 people linger on the nation’s transplant waiting list, and about 20 die each day.
“What we have is broken. We know it is costing people’s lives,” said Dr. Seth Karp, Vanderbilt University’s transplant chief.
Under U.S. transplant rules, the country is divided into 58 zones, each assigned an “organ procurement organization” in charge of donation at death. Those OPOs are matchmakers with a tough job: get donation consent , collect organs quickly and get them to the right transplant center before they deteriorate, even if a hospital calls with a possible donor at 3 a.m.
How well are they doing? It’s almost impossible to tell. OPOs self-report to the government a subjective measure, one even they call flawed. That standard offers little way to directly compare OPOs and learn which could do better, or to hold poor performers accountable for missed opportunities.
“Nobody comes behind them and says, ‘You actually could have recovered 20 to 30% more and you didn’t,'” said Kevin Longino, a kidney transplant recipient who heads the National Kidney Foundation.
That may be about to change. The Trump administration is planning a major overhaul , stricter standards to make it easier to compare OPOs and push them to improve. One measure under consideration: Using federal death records to determine the entire pool of potential donors each OPO can draw from.
A study published this summer in the American Journal of Transplantation called that approach more fair and verifiable than today’s standard. The AP modeled its own analysis of OPO performance on that research, and found that in 2017, even comparing regions with similar pools of acceptable donors, disparities were large. Philadelphia outperformed New York City, for example. The OPO serving parts of Kentucky lagged nearby Missouri.
More important, the analysis shows the biggest impact would be in places with large populations of potential donors. For example, OneLegacy in Los Angeles recovered 487 donors in 2017, second only to Philadelphia’s Gift of Life. However, the Los Angeles OPO also had the highest number of potential donors. If OneLegacy had matched Gift of LIfe’s higher donor rate, the AP calculated it would have secured 866 donors, instead.
OneLegacy CEO Tom Mone cautions that measure overestimates potential donors, including people who are disqualified after scrutinizing their medical records.
It also counts less-than-perfect donors — older donors, or those with certain medical conditions — that can be hard for OPOs to handle, Mone added. Those organs can be life-saving for the right patient but too often transplant centers won’t take the chance, a disincentive for OPOs to collect them.
Still, with the organ shortage so severe that transplant centers are fighting over scarce livers in court, a growing number of OPOs say the death record-based standard will spur more aggressive pursuit of donors.
“It’s a denominator that can’t be manipulated,” said Ginny McBride, executive director of OurLegacy in east-central Florida. She was one of 17 OPO leaders who recently wrote Medicare, which regulates the nonprofits and reimburses costs of organ acquisition, to push the change.
“It might tell us, ‘Hey, we need to kick it up a notch.’ And it might tell the government who they should and shouldn’t be certifying” to collect organs, she added.
WHO CAN DONATE?
Only about 2 percent of Americans die in circumstances that allow for donation. Die before reaching a hospital, and organs can’t be preserved. Illnesses such as cancer or infections also can rule out donation.
While young donors tend to be healthier, there’s no upper age limit: Studies show healthy organs from seniors like Henry, who was 78, and even octogenarians still can work well, especially if given to an older recipient.
Last year, out of about 2.8 million deaths, there were 10,721 deceased donors. Their organs led to 29,680 transplants.
But a 2017 study led by University of Pennsylvania researchers examined deaths, donations and health records and calculated that a better-functioning system could produce up to 28,000 more donated organs a year.
Objectively evaluating OPOs is one key to that goal but even if the estimate is too high, “every single donation we recover is another life saved and makes it worth it,” said Greg Segal of the transplant advocacy group Organize, whose father waited five years for a heart transplant.
SPURRING CHANGE
The recent spike in drug overdose deaths has increased donors. Some OPOs are aggressively looking for additional boosts.
LifeCenter Northwest — which serves Washington state, Alaska, Montana and northern Idaho — has doubled the number of organs transplanted from area donors to 878 last year, up from 440 in 2009, before CEO Kevin O’Connor arrived.
It focuses on organs often considered too difficult to recover — not those taken from brain-dead donors, which are most common, but from people who die when their heart stops beating. O’Connor staged “mock donations” to train donor hospitals and his own employees to handle those complex cases. Nationally, they make up about 19% of donations; at LifeCenter, they’re 29%.
At LifeGift in Houston, where donors are up about 40 percent since 2013, CEO Kevin Myer has hired dozens of new employees to reach far-flung hospitals faster. Increasing donation “comes down to how good your staff are and how many people you have to get to the hospitals and take care of people who are losing somebody,” Myer said.
And in Hawaii, where lung transplants aren’t offered so donor lungs often go unrecovered, Legacy of Life is testing a new incubator to preserve the organs for shipping long distances. The first pair was successfully transplanted in North Carolina more than 20 hours after donation.
HOW TO MEASURE SUCCESS
Today, OPOs report to Medicare how many deaths they classify “eligible” for donation, the baseline. That’s supposed to include hospital referrals of brain-dead patients 75 and younger without disqualifying diseases. Comparing that denominator with the OPO’s actual donors measures its success.
“I’m always stunned at the variability” in how many eligible deaths OPOs of similar sizes report, said Diane Brockmeier, director of Mid-America Transplant Services in St. Louis, who supports the death record-based standard.
But different OPOs can define eligibility differently. There are disincentives to pursuing every organ.
And sometimes there are breakdowns. Dr. Dorry Segev, a Johns Hopkins University transplant surgeon, described “a very depressing and disappointing case” where several patients were promised a young donor’s organs only to have the offers rescinded a few hours later. The OPO, in another state, couldn’t find a local surgeon to recover the organs — and didn’t give the recipients’ surgeons a chance to fly in and do it themselves.
Then there’s the role of transplant centers, which are graded on how well their patients recover after surgery without taking into account how many die on the waiting list. When local transplant centers refuse less-than-perfect organs, OPOs can be reluctant to collect them because they may not find a recipient elsewhere.
“The metrics are driving the wrong behaviors,” said the kidney foundation’s Longino, who says transplant centers must be held accountable, too. “If 60-year-olds are not being given a 60-year-old kidney that’s in good working order, not being given that choice, that’s a problem. And they’re not.”
A recent study suggests U.S. transplant centers are too picky. Surgeons in France regularly implant lower-quality kidneys from older donors than their American counterparts, with success. Medicare recently took a small step to counter transplant center reluctance.
On the donation side, many OPOs and patient groups see value in reform that bases OPO success on the largest possible pool of donors, an easy-to-track number.
But some OPO leaders are lobbying for a different measure, one whittled down to just people on ventilators. Some Midwestern OPOs are testing that approach.
“We are troubled by some of these broad-based, easy-to-calculate numbers not portraying a picture of what potential donation might be,” said Susan Gunderson, CEO of LifeSource Upper Midwest OPO in Minneapolis.
PATIENTS LANGUISH ON THE WAITING LIST
More donors can’t come quickly enough for waiting patients, especially those caught in a legal battle over how to distribute scarce livers. For nine days earlier this year, a new national policy meant hospitals that once got first dibs for livers donated nearby instead saw the organs shipped to sicker patients up to 500 miles away, with pediatric donations earmarked for sick children. Then a federal judge put the new rules on hold .
In Pennsylvania, toddler Evelyn Yargar got a new liver during that window, from a teen who died several states away. A few days later and the organ might have gone to an adult instead. And because University of Pittsburgh surgeons knew 14-month-old Evelyn needed just a small piece of that liver, which can grow into a fully functioning organ, an adult in still another state got the rest.
“I just don’t understand why a child can’t have a part of any donor’s liver,” said Evelyn’s mother, Bobbi Yargar of Charleroi, Pennsylvania.
However the court fight turns out, only a bigger supply will ease struggles over organ distribution. And if Medicare changes performance standards, that won’t make OPOs collect more organs overnight.
“I don’t think there’s another 30,000 organs out there lying around to get transplanted,” cautioned Brockmeier, the Mid-America chief. Still, she knows her own OPO could improve, and she called on others to “stop battling over what the metric is and decide what we’re going to do to maximize donation.”

AP’s analysis used a mortality database from the Centers for Disease Control and Prevention to calculate deaths involving hospital in-patients under 75 from causes consistent with donation for every OPO except Puerto Rico’s. Dividing the actual number of donors by those counts yields a comparable success rate.

When a mental illness suddenly appears in a partnership, it can shake the foundation of a marriage. While dealing with the erratic behavior of a spouse in mania or depression is difficult, through counseling, love and acceptance of the illness, partners can grow even stronger as a couple.

Mood changes
It’s stressful for a spouse to deal with the confusion of ever-changing moods from their partner that has bipolar. It helps for this spouse to view their continued support as an investment in something worthwhile.
Confusion and chaos
When symptoms of bipolar first begin to surface, most spouses aren’t sure what to think—they’re very confused. Amid the chaos of behaviors they fear they’ll never see the person they’d fallen in love with again.
When sympathy isn’t enough
When a spouse is first diagnosed, the initial response from their partner is usually sympathy––until anger, frustration and even hate arise down the road, according to sociology professor and author David A. Reach acceptance
If a spouse can move through these times, he or she will reach a place of acceptance, says Karp. “Those difficult emotions will then be replaced with softer feelings of love and compassion.” Just because life with your spouse looks different from the one you originally imagined, it doesn’t mean you can’t still have a loving, fulfilled life.
Redefine spousal expectations
Acceptance also means redrawing the picture of what your life as a couple looks like. For some, that may mean having to rely on only one income, going without certain luxuries, or even choosing not to start a family. For others, it may mean that many of the responsibilities of daily life fall on the well partner.
Share responsibility
Couples who have the most success share the belief that the partner with bipolar and the partner without bipolar each share an equal amount of responsibility within their relationship. Bipolar is biologically based, and can be properly managed if an individual shoulders the responsibility to help themselves––unless they are extremely manic or depressed, in which cases they are temporarily unable to.
Prepare ahead
In periods of wellness, successful couples prepare for episodes that lie ahead. That may mean realigning chequing accounts or giving power of attorney as a means of limiting control to the ill spouse during manic episodes. Some couples will have a list of symptoms the bipolar spouse exhibits when on the brink of a manic or depressive episode and agree at what point to seek help, thus removing the emotions from such a decision.
Carry the burden
Following a diagnosis many spouses are left with most of the responsibilities of their life together—from caring for the children to working to pay the bills to caring for their ill partner. It’s a heavy burden for many.
Draw boundary lines
Many spouses struggle with how to help the person they love without becoming engulfed in the spouse’s misery or mania. They may even feel their own identifies are being buried, and they are losing themselves or jeopardizing their own health. Spouses must learn to draw a boundary line, or risk losing themselves, Dr. Karp says.
Mutual wellness
Many couples attribute their successful marriages to a commitment to mutual and personal wellness. This could mean daily exercise, eating well, good sleep habits or focusing on the wonderful things shared by both. If both partners wholeheartedly want their relationships to work, they will find ways to make it work.
Move forward
Dr. David A. Karp argues that sometimes the well spouse has to withdraw. It may sound harsh, but it’s for the sake of their health. He says spouses must learn to take care of themselves before they can take care of others. It’s important for the well spouse to fulfill their own goals, and continue to move forward, even when their partner is struggling.

I am looking for support for my marriage and my husband. He was diagnosed after a breakdown and stay in the hospital. He has used drugs before we were married and a blamed the drugs assuming he had started using again but when he tested clean I felt shame for even assuming that. I want to help him but still help myself (I have mdd and very high anxiety and numerous other health issues) and keep our marriage strong.

My wife of 25+ years was recently diagnosed with BP 2, which isn’t as severe but can be more insidious. She was diagnosed with depression 20 years ago and has been on anti-depressants since. A few months ago, she announced she was looking for jobs in different cities to get away from her mother who had abused her as a child and to continue working in her field. I supported the move because she had been so miserable and thought I would join her (or she would return) within a year. I found out afterwards she was actually having an affair with a man she knew from high school and had moved to be with him. This was shockingly out of character for her and when confronted, she chose to return home immediately and seek treatment. I love her deeply, but I’m having trouble understanding how much of her behavior was a result of the disease and how much was genuine feelings for the other man.

I’m struggling because I am the one with Bp II. My husband is getting very discouraged. He feels alone. I have a hard time not taking that personally. The illness is changing our marriage. I feel like I’m the one ruining it. I am gettin treatment but finding the right medicine is exhausting. I either have horrible side effects or it doesn’t work.
How do I help my husband when I can barely function myself? I constantly worry he’s just going to give up on me. He’s so down. He’s not a crier but he’s been crying often. I just hold him but I lose it too because I’m the cause of his pain. My “up” results in so much irritability and unwarranted nitpicking. I know I have a choice. I apologize when I recognize it but I know it’s not enough for someone living with someone like me.
Does anyone have any advice? I’ve only been diagnosed for around 6 months. We’ve been married for 3 years with a 1.5 year old daughter.

The best thing is to continue to work and managing the illness. Don’t stop your meds and therapy helps. For your husband, be supportive and encourage him to seek counseling, a support group and self-care. Have him involved in your therapy to work through together so he can identify triggers and know what he can do to help. You may want to check out the book “Loving Someone with Bipolar Disorder” by Julie Fast. It’s a great starting point to understanding the illness along with tips on how to navigate as a couple.

Hi Maria, just read your post of when you were diagnosed. About your spouses struggling with your bipolar disorder. I was diagnosed 10 years ago. Marital struggles aren’t an easy subject. I think we are just getting used to the issues. My husband does spend a lot of time away from home, but he probably needs too.im sorry I can’t give you specific solution to the problem. Just that you are not alone in this. And apparently I’m not either. There are always situations that creep up that I didn’t expect to. But the “me time” that I have is definitely a start. Joanne

Antioxidantes – os efeitos dos suplementos no cancro
Os doentes oncológicos devem obter todos os nutrientes a partir dos alimentos, desaconselhando-se o uso de suplementos alimentares. Esta é uma recomendação unânime de instituições de renome como a American Cancer Society, a World Cancer Research Fund e o American Institute for Cancer Research. Contudo, é muito comum os doentes com cancro recorrerem a este tipo de produtos. Os mais populares são os suplementos à base de antioxidantes.
Antioxidantes e cancro
O uso de antioxidantes em doentes oncológicos tem sido estudado em duas grandes vertentes: (1) como agentes anticancerígenos potenciais e (2) para reduzir os danos oxidativos resultantes da quimio e da radioterapia.
Os agentes anticancerígenos diminuem os efeitos prejudiciais no ADN, reduzem a proliferação da doença e a angiogénese e aumentam a apoptose.
Vários estudos têm sido realizados para avaliar o impacto dos antioxidantes no cancro. Atualmente, a ciência sugere que o efeito pode ser negativo.
Considerando o efeito sobre o risco de desenvolvimento da doença, a suplementação com beta-caroteno parece aumentar o risco de cancro do pulmão e do estômago. No que diz respeito à vitamina E, parece aumentar o risco de cancro da próstata e de adenoma colo-retal. Por outro lado, em populações com baixos níveis séricos de selénio (<106 ng/ml), verificou-se uma prevalência mais baixa de cancro do pulmão, contrastando com as taxas elevadas em populações com níveis mais elevados (>126 ng/ml).
Uma meta-análise de ensaios randomizados concluiu que a suplementação com antioxidantes parece ter efeitos adversos na mortalidade da população em geral. A mortalidade aumentou com a suplementação de beta-caroteno, de vitamina E e com doses elevadas de vitamina A. No caso da vitamina E associada ao selénio, nenhum trouxe benefícios na mortalidade.
Assim, além do aumento do risco de alguns tipos de cancro, a ciência confirma os efeitos adversos dos antioxidantes na mortalidade em geral, sobretudo em doentes com dignóstico de cancro.
Existe, ainda, outro fator de preocupação. Embora os antioxidantes possam reduzir a toxicidade da quimio e da radioterapia, pode ocorrer uma diminuição da eficácia do tratamento. Isto acontece, pois a radioterapia e muitos agentes usados na quimioterapia exercem o seu efeito através da produção de espécies reativas de oxigénio e por aumento da apoptose.
Antioxidantes e radioterapia
Nos doentes com cancro da cabeça e pescoço, apesar dos antioxidantes reduzirem a toxicidade da radioterapia, pode também aumentar a recorrência da doença e da mortalidade, principalmente entre aqueles que mantiveram o consumo tabágico durante o tratamento. Tanto o tabaco como os antioxidantes podem reduzir os efeitos da radioterapia. No caso do tabaco, o efeito assenta no facto de aumentar os níveis de carboxihemoglobina e de hipoxia, os quais reduzem os efeitos do tratamento que estão dependentes do oxigénio. Todavia, é de referir que os estudos realizados não são consensuais quanto aos resultados obtidos. Por exemplo a suplementação com beta-caroteno parece não prejudicar o prognóstico de doentes com cancro da próstata.
Antioxidantes e quimioterapia
Foram realizados alguns estudos de curto prazo para mostrar que efeitos benéficos podemos obter com os antioxidantes. A vitamina E reduziu a neuropatia da cisplatina e o selénio a toxicidade hematológica, sem, contudo, ter influência na nefrotoxicidade e na ototoxicidade do mesmo citotóxico. Quanto à vitamina E, não foi verificado um efeito benéfico da suplementação na redução da toxicidade de vários citotóxicos (neuropatia associada a taxanos, neuropatia periférica induzida pela oxaliplatina, cardiotoxicidade da antraciclina e toxicidade geral associada à caboplatina).
No entanto, dos estudos científicos consultados, nenhum avaliou os efeitos a longo prazo da suplementação com antioxidantes durante a quimioterapia, na recorrência da doença e na mortalidade.
Antioxidantes e doentes oncológicos
Parece não haver influência da suplementação no prognósticos em situações bem definidas: a suplementação com selénio nos doentes pós-cirúrgicos com cancro do pulmão do tipo “não-pequenas células”, em doentes com cancro da cabeça e pescoço, após radioterapia, suplementados com beta-caroteno e em doentes com cancro não tratado e suplementados com vitaminas E e C, selénio e coenzima Q10. Também há evidência de que a suplementação com selénio não diminui a recorrência do cancro da bexiga.
Por outro lado, a suplementação com antioxidantes diminuiu a recorrência de adenomas do cólon entre não fumadores e não consumidores de álcool mas duplicou o risco entre fumadores e naqueles que consumiam mais do que uma bebida alcoólica por dia.
Adicionalmente, verificou-se diminuição da recorrência de cancro não invasivo da bexiga com a suplementação com vitamina E.
Os antioxidantes podem ter efeitos benéficos ou adversos em doentes com cancro. Parece que tudo depende do antioxidante. Isto é: qual é o antioxidante? Em que dose? E de que forma? Mas, também depende das características do doente (estado nutricional, tabaco, consumo elevado de bebidas alcoólicas), da localização do tumor e do tratamento. Assim e nesta matéria, ainda há muitas perguntas sem resposta. Todavia, é importante referir que os efeitos adversos referidos estão associados ao consumo de suplementos e não à obtenção de antioxidantes que naturalmente se encontram nos alimentos.

The new U.S.-based venture, called Syntropy, will offer cancer data analytics tools to research centers, combining Palantir’s Foundry data platform with the customer knowledge of Merck’s Life Science business, known as MilliporeSigma in North America, the companies said in a statement on Monday.
Syntropy will help medical researchers to structure and analyze data from various sources and to selectively share the insight with external partners, they added.
“Our customers will pay a license fee for a software, enabling them to aggregate and analyze data from highly disparate sources,” Merck Chief Executive Stefan Oschmann told Reuters, adding the JV would not buy or sell any data.
The companies would not disclose financial terms, beyond saying each partner would hold a 50 percent stake.
Known for helping the U.S. government track down al Qaeda leader Osama bin Laden, Palantir does confidential work for defense and intelligence agencies. Private sector customers include aircraft maker Airbus.
“When we choose partners, be it in government or the private sector, we are on their side and want to grow a business together and not with five others,” said Palantir co-founder and CEO Alexander Karp.
“Also in this case, we have no intention to handle it any differently,” he said.
Healthcare companies are investing heavily in data analytics. GlaxoSmithKline in July agreed to buy a $300 million stake in Google-backed gene testing company 23andMe.
Roche has spent $4.3 billion this year buying out two specialists in cancer data, U.S. genomic profiling group Foundation Medicine and U.S. cancer data company Flatiron Health.
While Roche is seeking to harness real-life data from cancer units, Merck and Palantir’s offering will for now focus on clinical trial data and early-stage research, Merck said.
Palantir, which won Merck as a customer in January 2017, said the JV deal would make the German company its exclusive partner in healthcare.
The Wall Street Journal reported last month that Palantir was weighing an initial public offering with a valuation of as much as $41 billion.

After a meal, blood sugar levels rise and can stay elevated over time. However, one hour after LuCI was administered to the rats, the response to glucose was lowered by 47 percent. The team found that this response was temporary, and after three hours, the effect essentially disappeared. The team is now testing the effect of short- and long-term use of LuCI in diabetic and obese rodent models. They also plan to advance some of their pilot studies showing that LuCI can be used to deliver drugs, including proteins directly to the gastrointestinal tract. "Gastric bypass is one of the best studied surgeries in the world, and we know that it can lead to many benefits including positive effects for blood pressure, sleep apnea and certain forms of cancer, and a remarkably fast and weight-independent improvement in diabetes," said Tavakkoli, co-senior author of the study.
