Summary:

Lady Nagant has lived in a box for all her life.

First it was the Commission compound. Then it was her heavily monitored home, her heavily monitored workplace, and the short leash they kept her on when she was on their discreet "missions."

Now it's a cell in Tartarus. She thinks her next one may as well be a coffin.

For Febuwhump 2024 Day 2: Solitary Confinement

Kaina is very used to torture.

She has to be, given who raised her. The Commission had put her through every kind of torture that wouldn't kill her outright, making sure she wouldn't crack under anything. They also taught her every kind of torture, even the ones that killed the victims, because the HPSC only trains its agents against the worst that they can think of, and they're not above doing the worst that they can think of.

So Kaina knows isolation. She knows how solitary confinement is one of the worst possible tortures for a human being, any human being, no matter how little they talk to people in their daily lives, no matter how well-trained they may be against any other kind of torture. Human beings need other human beings. It's the one kind of torture that the HPSC puts their agents through less than they do their actual prisoners; the psychological damage is so intense and long-lasting that they can't fix it. It would be a waste of agents to train anybody in isolation for longer than 72 hours.

Kaina knows all of this, intellectually. She went through her isolation training a long time ago, and thought that that would be the end of it for a long time after.

Then she shot the President, and they threw her in Tartarus.

There is nothing in here that she could use to escape. There is nothing in here that she could use to entertain herself. There is nothing in here that she could use to keep her sanity. She can't even pace around, can't even physically do anything due to her restrictive bindings- her arms are immobile, to prevent her from using her quirk. Her head is shaved, to prevent her from using her quirk. Her legs are immobile, to prevent her from using her training to escape. There are no windows, no doors, no bars to her cell. There's only a one-way mirror, made of glass that she's sure is even more reinforced than the wall surrounding it. She knows that there is a door, but it's not one that's visible against the rest of the wall, so she can't really count it as any kind of visual stimulation.

All there is are the white walls, white ceiling, white floor, and the one-way mirror.

They don't even release her to eat. She's kept fed through an IV drip, though "fed" is quite the overstatement- it gives her the nutrition that she needs to survive, and nothing more. She is starving, and she can feel herself wasting away in real time. She wasn't cold when they brought her in what must have been weeks ago; she's shivering constantly now.

Nobody has questioned her. Nobody has come in and talked to her. Why should they? She's an ex-hero who went crazy and killed the fucking President of the Commission, left here to rot for her crime. Thrown away like the human trash that she is, never mind her so-called "reasoning." Why should they care? Why should anyone care? She's crazy, she's locked up, end of story.

Kaina is afraid to look in the mirror. She knows that there's no one behind it, no one watching her, because there are cameras that serve that exact purpose without the risk that she'll somehow manage to shoot through the glass. The only reason anybody would be looking through that glass is if anybody was here to question her, and of course nobody has and nobody will. She can't avoid her own reflection, it's in her field of vision no matter what, due to her limited range of motion, but she prefers it if she's just an unfocused blur of color in her periphery.

She doesn't want to see her hollowed cheeks, her sunken eyes, her skeletal frame. She won't cave to the temptation to even glance at it, for the pure hope of just having something different to look at, because she can feel her hair growing back. Someone's going to be in to shave it soon, for fear of her using her quirk to make bullets and escape; they probably won't talk to her, unlike the friendly hairdressers she remembers, but it will still be something different. She will still have a change of scenery sometime soon.

In the meantime, she's grown more tired recently. She sleeps more, and more, and more, until she fears that every time she falls asleep she won't wake up again. Are they drugging her? Is this more of the effect of the lack of food? Probably a combination of both, if she had to guess.

She sleeps easily, the same as she always does. She doesn't know if it's day or night; the fluorescents are just as bright either way. She doesn't know how long it's been, just that the air conditioner doesn't make her scalp quite as cold as it did when they first brought her in. When she wakes, her scalp is freezing again.

She doesn't sleep as easily after that.

It's strange, really; the idea that her isolation is driving her insane, faster than being around so many people did the same thing. Did she go crazy in the opposite direction? She feels so much more passive now. She's done what she wanted to do, she cut off the head of the HPSC that did this to her, so why does she still feel like there's something that needs to be done? Why does she feel like she doesn't care?

She paid for her sins in blood and now she's paying for them in sanity. She rectified what she could, fixed what little brokenness she could touch, but there's still so much more below the surface that she didn't even glance at. How could she? She was drowning, same as everybody else, and a drowning man can't exactly save all the other drowning people in a shipwreck. All he can do is find a raft and maybe grab someone else. He can't dive below the water to grab everyone else.

... What kind of monster is the Commission? Is it something that can be killed? Or did she just cut the head of a hydra, and leave everyone else to leave with the two new heads that spawned in its place?

Kaina pushes against her bonds. They're not as tight as they used to be, and she's a little surprised that nobody's tightened them while she's asleep yet. Maybe it's on their to-do list; maybe they figure she's still secure enough, so they don't bother. Either way, they're right; she couldn't get out of these things if she tried, not with how much her muscles have atrophied by now. She thinks she could have when they first trapped her here, but she really didn't want to; she thought she deserved this. She does deserve this.

It just hadn't occurred to her that the Commission still deserves her. It still deserves the anger that drained from her like the blood that drained from the body of dear Mr. President, it still deserves her righteous bloodlust, it still deserves the part of her that aches for redemption, it still deserves to reap what it has sown.

Her pressing against her bonds- made of something like the material of a straightjacket, canvas-like and unyielding- triggers some kind of mechanism somewhere that causes a sedative to be put into her IV drip. She doubts that there's anybody who even presses a button in the whole process, the prison far too trigger-happy and paranoid about any of their prisoners even thinking of escape that they'd rather send them to addiction or overdose than let them move around a little bit.

The sedation is the only way that she can sleep nowadays. The fluorescents have grown too bright for her.

Nobody has entered this room while she was awake, she knows that. They know better than to tell her where the door is. Still, when she wakes, a matter of hours or days later, there is a small boy sitting in front of her.

She thinks she recognizes him. He was one of the children meant to succeed her, to have her torched passed onto them when the time came. Now they'll be her little replacements, rushed along through their programs far more quickly than she had been, because the Commission had already thrown their hat into the same ring as UA, Shiketsu, and all the other places she dimly remembers her coworkers bragging about, so their greed dictates that just because she was a failure in the end doesn't mean that she was entirely useless.

The kid has little red wings, with fluffy feathers in his hair and floating around his hands. His eyes are a bright, hopeful yellow, the same shade as All Might's signature color. He looks like he could've been one of her fans, clinging onto his mother's hand or sitting on top of his father's shoulders, but instead, she remembers seeing him through one of the windows to the children's training area. He's either an orphan, or he was sold, or maybe even both. However the Commission got their hands on him, it couldn't have been pleasant. It couldn't have been honest, either.

She'd kind of hoped that killing the President would, in turn, kill all the nasty little projects that the public didn't want to know about. She'd kind of hoped that it would kill Project Paragon, or stop the cover-ups for all those top heroes and their dirty secrets, or stop any of the hundreds of experiments that she knows were happening but doesn't know the details of. Considering that this child is standing here, in her cell in a maximum-security prison- in Tartarus, of all places- she thinks that that probably didn't happen. One of the President's lackeys probably picked up the freshly vacant position and kept on doing the same damn thing they've been doing this whole time.

"Why'd they put you in here?" She asks the kid. No preamble, no introduction. If she asked for his name, he'd give her the one the Commission gave him, some future pro hero's name, something like Hawk or Eagle or some other bird name. Maybe they'll even put "Sir" in front of it to mirror her "Lady," really hammer in how he's just her replacement, just another Nagant, designed to be better and faster and stronger and, most importantly, not going to betray them.

The child stares at her for an uncomfortably long time. More of his feathers float up and around him, slowly moving higher and higher until they're level with her head. Tears start forming in his eyes, but he doesn't cry; smart kid. He must've been trained out of that quick, and he's smart enough to know he'll still be punished for his actions in here.

"You failed me." He says, in a voice that echoes slightly, and Kaina only has time to wonder if that's a part of his quirk before sharpened feathers all fly at her at once, like knives all trying to embed themselves into her face.

They sent him to kill her. He couldn't be older than ten, and they sent him to kill her.

She was thirteen for her first kill. He's only ten, maybe.

None of the volley of feathers has made contact yet, or at least, she doesn't feel them. Maybe it's for the best- she doesn't want to live in his conscience any more than she already will, she doesn't want to stick him with the memory of his predecessor in pain along with the memory the HPSC is already forcing him to have. She's sure he's dragging this out, sure that he's gathering himself beyond the time that they allowed him when they gave him the command to do it. Any agent, even this young, would have hit her by now, unless he's missing on purpose.

She smiles at him. The feathers fly past her, and she has to try to make herself heard above the wind that they're creating; she really hasn't spoken in a long time, has she?

"It's okay," She near-whispers, "You're doing me a favor."

Two of the feathers are shaking more than the others. The boy is trembling, too, from the tips of his little wings to his tiny fingers. He still hasn't shed a tear; good boy. Good survival instincts. He'll last longer than she ever did, she's sure of it.

The two quivering feathers are level with her eyes. She closes them, peacefully, and holds still to make the boy's job easier.

Kaina jolts awake, gasping, still in her same cell, with no sign of anything having happened at all. The walls are clean, either having been repaired or not having been damaged at all. Is there any way for her to tell if any of that happened at all?

Probably not. Nobody's here, after all, and if she yells for somebody then she'll only get more sedation. It's a mystery that she can't hope to solve either way.

She wonders if the boy was real at all, if she really does remember him from a glimpse into the training quarters, or if he's just a figment of her imagination. Maybe he was one of her little fans, the ones who thought she was really a hero, the ones who thought she was really the kind of person who wouldn't get blood on her hands. The kind of person who goes home to a family at night, or else a warm home, rather than a cold Commission-provided apartment that was bugged to hell and back.

At least this place is less monitored than it was there. Isn't that funny? Isn't that fucking hysterical? Her own home was more closely monitored than her prison cell.

She's going insane, isn't she?

As long as she's not making a disturbance, the wardens probably give her camera feed a cursory glance, maybe make a note of if she needs another haircut or not- but they're even slacking in that, considering that her hair brushes the tips of her ears now- and ignore her for the sake of newer or more dangerous prisoners. Meanwhile, when she was an active hero, people were listening to her phone calls, checking what time she opened her front door every night, how long it took her to get from the agency to her building, and chart down any discrepancy to interrogate her on later. It must have been exhausting work, and she knows it was exhausting watching every step she made.

She wonders if anything's going to change. For her, she figures it probably won't; even if someone manages to take out the HPSC completely, she'll probably still be rotting here for the rest of her life, because even she doesn't have enough faith in humanity to think that they'll remember her; or, worse, to extrapolate from her actions and old record alone that the actions she was arrested for were justified.

But for everyone else? For the outside world? If she was able to do something with herself, she figures maybe someone else will have that same chance, will do the same thing but better, more thorough. Maybe that kid from her dream (if it was a dream) will spearhead it all. Maybe it'll be All Might. Maybe it'll be someone from all of those hero cover-ups, one of the abused kids or beaten housewives. Maybe it'll be a villain. Maybe it'll even be a hero.

Y/N kept laying on the Statesman’s surgery room’s floor, even after three different agents had come by to see if she had aid to get to her room, or desired a change of clothes. It had been a long and god-awful surgery. And Y/N had had their share on long and awful surgeries, this one was easily the worst. Not only because her feet were basically screaming form pain and her neck was cramping after the 16 hours of complications and near-death experiences, but mostly because who she was trying to save.

Really, it was only a matter of time, before he’d go and pull something so heroic and stupid like he now did. She had always told Merlin how one day he’d be hanging from a thread on the surgery bed.

“Lucky me then, I have the best surgeon in the world to save me,” He’d answer cheekily, earning a slap to his chest from Y/N.

Now, he was wheeled into another part of the med wing, mostly in one piece minus one limb.

Y/N hadn’t realized it before, always too busy with her work in the med bay to see what was right in front of her the whole time. She always considered herself to be married to her job, as did all the other agents as well. Drilled into their minds that working for Kingsman was certainly not a position to mix business and pleasure, made Y/N forget any kind of possibility to find love.

She was stupid not seeing it before when it was so obvious. Immediately after she switched from her position from the central hospital to Kingsman (being noticed by Arthur after she saved a child’s life with a juice box straw and a bottle of whiskey) she and Merlin had clicked, bonding over the work off-field. Whenever they’d wheel in an injured agent, he’d wish her good luck although everyone in the whole department knew she could handle almost any injury.

After a while, he’d begin to bring her coffee in the morning, in exchange Y/N would leave him tea on his desk, as she always came to work before him. Sometimes she’d leave a note, with a stupidly hilarious comment of the work she’d have that day, never failing at making Merlin chuckle.

Indeed, Y/N had been a complete idiot, not seeing how much she loved the adorable man in the chair. That was most likely the reason she was so dead set on getting Merlin out of that surgery room alive. He had in a way saved her life when the attacks had hit their homes. Defying orders, Merlin hadn’t put Y/N’s address into the database, for reasons the woman did not know. Not yet at least.

But it had saved her life, and now she had hopefully saved his.

“Gaius”, Agent Tequila greeted Y/N as she finally managed to peel herself off of the surgery room’s floor. She smiled at him tiredly, noticing how Eggsy and Harry were also in the room waiting for her. Knowing she must have been in the surgery room for at least an hour plus the seventeen hours before, it made her beyond happy to think the two had waited for her, patiently in the lobby.

Harry got up from his seat, hugging Y/N shamelessly. She hugged him back, burying her face into her chest, still not completely used to him being back.

“You did marvelous job darling,” He praised, speaking softly. Y/N just smiled. “So far at least, he’s alright.”

“I’m glad,” Understatement. She was more than that, there wasn’t most likely enough words to describe how glad Y/N was right now.

Eggsy smiled his goofy lop sided smile, pulling Y/N into a hug as well. He made a pretend scowl when she hugged him back.

“Yeah, you really were there for 17 hours,” Eggsy joked. “You need a shower.”

Y/N chuckled and hit his bicep after pulling away from the hug.

“Your girlfriend alright?” She asked, a hint of worry in her voice. Eggsy nodded, smiling contently and dug his hands into the front pockets of his trousers.

“Damsel de-stressed do to speak,” He said humorously, although everyone could see the rock being rolled off of his chest. Y/N had been so busy with getting Merlin out of Poppy’s secret lair without him completely bleeding out on her hands she hadn’t had time to comfort Eggsy. Or anyone for that matter.

“Right”, Y/N stepped back a little, playing with her hands. “I think Eggsy’s right, I do need a shower.”

“I’ll come by to check on him, after?”

“Don’t rush”, Harry warned her, knowing how little rest Y/N had gotten during the past 72 hours. Y/N smiled and nodded, walking back to the other direction and towards the room she was given.

Y/N looked after Merlin, not trusting anyone else at Statesman to do the job. She knew the doctors were talented, surely. They were working for an agent corporation, but still, she couldn’t quite place the life of the most important person she knew into someone else’s hands. Not after she had had to cauterize his wound so he wouldn't bleed out and then cut him open once again to remove any infected flesh.

No, she couldn't let go of him, quite yet.

Merlin had been sedated for the past week. Y/N wanted to give his body some time to begin the healing process, also it would lessen the pain he was most likely going to have to endure for the next month or so.

For the first time since she joined the Kingsman, Y/N shamelessly walked around the Statesman HQ just in sweats and a thick sweater. After everything that had happened, the woman quite literally said fuck it when she had changed out of her surgeon gown. She had been almost been nuked by a terrorist, as of now, out of all the things she was worried about, what Y/N decided to wear was most likely the least important one.

Y/N walked down the same, familiar hallway towards the infirmary, holding a cup of coffee in her hand. In her left hand, she messaged Galahad, telling him she was once again checking up on Merlin.

It was a bummer that neither Y/N nor Merlin could go to Eggsy’s and Tilde’s wedding, but it was better for her to lay low for a bit and Merlin to just get better. Eggsy, of course, understood this, but still threatened to come over and throw another reception party at Statesman, if Y/N couldn’t bring her ass to England to see them soon. His words, not Y/N’s.

“Hey”, Y/N briefly greeted another doctor, who was checking up on a field agent. From what she could see through the small crack of the curtain, he seemed to have quite a few bruises and most likely a broken nose. Nothing too bad.

Y/N turned to Merlin, after making sure the curtain was fully pulled in front of the bed for maximum privacy. The heart monitor gave out reassuring and steady beeps, while Y/N checked for Merlin’s IV and other vitals. They had decided to stat gradually lowering the propofol, so Merlin could begin to wake up on his own. A couple of times, Y/n had caught him mumbling something incoherent in his sleep, signalizing that he had begun to dream. It was a good sign, as then she knew he had begun to wake up slowly.

“Hey you,” She eventually sat down beside the bed. Merlin looked a lot better than she had a week ago, that’s for sure. His face was much more colored, not hollow and grey anymore. His fingers didn’t feel as cold either.

“What are you dreaming about?” Y/N spoke again, as Merlin mumbled something so lowly sh couldn't understand. She smiled and placed her hand on his forehead to make sure he had no fever. He didn’t.

Chapter 2

Dean

Dean kept an eye on you in the rear view mirror as he drove. He could tell that you were tough, but the amount of blood you had lost was a little concerning to him.

“You okay?” he asked, watching your eyes droop a little.

“I’m f-fine,” you slurred. “Just need a cookie and I’ll be a-a-okay.”

“Uh, it’s not like you just donated a little blood, Y/N,” Sam began. “Most of your blood was taken from you. I’m thinking it’s gonna take more than a cookie to bounce back from that.”

Your head drooped and it was clear you were not far from unconsciousness. “Sammy, you gotta keep her awake,” Dean said sternly to his brother. Sam quickly turned around in his seat and reached over to the back.

“Hey, hey, hey,” he said, tapping your face. “You gotta stay awake, okay?”

Your eyes opened a bit more, but not for long. Dean pushed the pedal down, trying to get you to the hospital as fast as he could.

After he pulled up to the emergency room he and Sam carried both you and the blonde through the entrance. ER doctors quickly swept the both of you away and Sam and Dean watched helplessly as they took you through the double doors.

You opened your eyes and knew instantly where you were. You had spent most of your time there for the last several years.

“Y/N?” you heard from beside you. It was Jenny, one of your fellow nurses. The boys had brought you to the hospital where you worked. “What the hell happened to you?”

You had to think for a moment, unsure if it was all real. You looked at the IV attached to your hand. Obviously the crook of your elbow was out of the question after the hack job those vampires did on it.

“Kidnapped,” you said raspily. Jenny quickly handed you a glass of water and you took small sips even though you wanted to chug the whole thing right then.

“Kidnapped?” she asked. “Was it those men that brought you here?”

You shook your head as fast as your energy would allow. “No, no, they saved us. The kidnappers…” you weren’t sure what to say. Obviously not the truth since that would get you put on a 72 hour psych hold. “The kidnappers got away.”

“Wow. Well I’m glad you’re okay...kind of. Why in the world were they taking your blood?” she asked, gesturing to your arm.

“I don’t know. Some kind of weird fetish?” you lied. “How’s the other girl?”

“She’s still out, but she’s gonna make it.”

“Good. And the guys?”

“They’ve been hanging around the emergency room to see if you’re okay. Honestly the only reason we didn’t call the cops yet is because they haven’t made any move to leave and we know you and wanted to hear your side,” she explained.

You nodded. “Could you send them in here?”

“Sure thing,” she said, getting up. “I’m so happy you’re okay. When you didn’t show for work the last few days, we thought the worst.”

“Me too,” you said with a smirk.

A few minutes later the tall men were walking into your room. They looked nervous, like this was something they didn’t normally do.

“Hey, boys,” you said casually.

“Hey, Y/N, how you feeling?” the taller man asked.

“I’m okay--” you didn’t actually know his name.

“Sam,” he said with a smile.

“I’m okay, Sam. Thanks. And thank you both so much for saving me.”

“It’s our job. We got some serious glares while we were waiting for you out there, though,” Dean responded, shrugging.

“Yeah, I work here. They’re a little protective of me,” you said with a small smile.

“You work here?” Dean asked.

“Yeah, I’m the head nurse. I’ve worked in pretty much all the departments here so I kind of know everyone.”

“Huh. Well I should have guessed with how quickly you checked out the other girl you were captured with. I just thought maybe you knew her,” he said.

“Yeah. Well, if you guys ever need someone to patch you up, you know, with the occupational hazards you face, just give me a call,” you said, handing them the small slip of paper you had written your number on.

“Thanks, Y/N. Get well soon,” Sam said, turning to leave. You smiled as they left, and Dean turned at the last second, sending a wink your way.

I will never see them again, you thought to yourself. You laid back and relaxed, wanting to heal as soon as possible so you could get back to the job you loved.

It had been a few weeks since your run-in with the vampires. You had made a full recovery, of course, and you had also found out that the blonde you were kidnapped with—Ashley—had made one as well.

You had gone back to work after two days despite everyone’s urging to take more time off. You found that getting back to your normal routine was going to be the best way to get over the incredibly unusual thing that happened to you.

Work had been good, but you found yourself daydreaming often. Now that you knew what was actually out there, you thought about it a lot, and you couldn’t help but think of the two men whose job it is to fight what’s out there.

You had just gotten off your shift and were heading to your car when your phone rang. It wasn’t a number you recognized, but you weren’t busy, so you figured what the hell and answered anyway.

“Hello?”

“Y/N.” The voice was deep and gravelly, and it took you a moment to place it. You leaned your phone against your shoulder as you unlocked the door to your beat up Corolla and opened it.

“Dean?” you asked, finally remembering where you’d heard the voice.

“Yeah, uh, listen—“ he said, his words rushed and breathy. “I know it’s soon, but we need to take you up on your offer of medical help.”

You didn’t even have to think about your answer. You turned the key and started your car. “Where are you?”

The bunker where the boys lived was like nothing you’d ever seen before. It had the faint smell of gunpowder and blood, but you decided not to dwell on that. You walked in without invitation like you were instructed and rushed down the stairs.

“Hello?” you called out.

“In here,” you heard faintly from another room. You followed the hallway—trying not to get distracted by everything you were seeing—until you found a door ajar.

“Dean?” you said, rounding the corner.

“Yeah,” he said through a grunt of pain. You took in the scene before you, deciding what to do next.

Dean sat on the edge of a big bed holding his shoulder which had blood streaming fairly steadily from it. His giant of a brother was laid across the bed, unconscious.

“Check on Sammy, please,” he said urgently. You nodded and walked over to him, setting the medical bag you kept in your car on the bed.

You checked his pulse and lifted his eyelids, trying to determine the state he was in. He was covered in cuts and bruises. You stepped back.

“He’s okay. He’s just knocked out as far as I can tell. I’ll be able to tell more when he wakes up,” you said, walking over to stand in front of Dean.

“Good,” he grunted.

“Now let me look at you,” you said, pulling at the hand covering his injury. Dean glared at you for a moment.

“Now,” you said sternly. He moved his hand and more blood gushed out.

“Normally I wouldn’t need any help,” he began as you took a closer look. “But Sammy’s out and I can’t get this on my own.”

There was a bullet lodged near his clavicle. It wasn’t too deep, and you were pretty sure you could get it out with forceps, which you had in your bag.

“That’s okay, I’ve done this before,” you said, pulling out all the tools you’d need. Dean closed his eyes and nodded, clearly in a lot of pain.

You began cleaning the area around the wound, preparing for when you would pull the bullet out. Dean reached next to him with his free arm and brought a bottle of whiskey to his lips.

“Uh-uh, no way,” you said, taking it from him.

“I need it for the pain!” He protested, pulling it back toward him.

“Alcohol thins blood. Thin blood runs easier. You have a wound that is steadily bleeding. So unless you want to die from blood loss, you’re not having any more.”

Dean’s eyes narrowed as he decided which was more important. He let it go, and you put it behind you out of his reach.

“How do you know how to remove a bullet anyway?” he asked as you got back to work.

“I work in the trauma unit at the hospital,” you said, dabbing away more excess blood. You took the forceps and started to dig a little, trying to clear a path to pull out the bullet.

“Son of a bitch!” Dean yelled. You rolled your eyes.

“You know, I’ve pulled a bullet out of a twelve-year-old girl who took the pain better than you,” you joked.

Dean’s brow furrowed. “A twelve-year-old?” he asked incredulously.

“Yeah.” You kept working, close to being able to pull it out. “She and her brother had gotten a hold of their dad’s gun and were playing with it. They didn’t know it was loaded and her brother accidentally shot her.”

Dean was sufficiently distracted, now only flinching here and there as you worked. “Wow. Was she okay?”

“Yeah. Usually a doctor or a surgeon takes care of that, but she was losing a lot of blood, and we were short staffed that day. So I made a decision.”

“What’d you do?” he asked, fully invested in the story now.

You smiled a little. “I laid her down on one of the emergency room beds, grabbed some forceps, and pulled it out.” As you finished the sentence you pulled the bullet out of Dean’s shoulder, and he yelped in pain. You quickly covered the wound with gauze.

“There,” you said, holding up the bullet still grasped in the forceps.

Dean heaved a sigh. “Thanks,” he said, holding onto the gauze.

You finished taking care of it and taped more gauze on. “You’ll have to change that every couple hours until the bleeding stops,” you said.

“Wait, you’re not leaving, are you?” he asked, panicked.

You chuckled a little. “No, I’m going to stay and make sure Sam is alright when he wakes up.”

Dean visibly relaxed. “Good.” He stood up, wincing again. “Follow me,” he said, walking out of the room. You cast another look at the still-unconscious Sam and followed Dean out of the room.

Every Parent’s Nightmare

We worry about our children from the day they are born or perhaps even from the day we find out that are we are pregnant. In September of this year, we had the scare of our lives as parents.  After living through this nightmare, I was told by many parents that I should blog about our experience to help raise awareness of the symptoms of joint sepsis in children.

On a typical Thursday evening we thought our 11 year old daughter had the stomach bug that was going around the middle school. She had a fever of 103.4 and was throwing up.  This lasted all of Thursday night and most of the day on Friday. But by Friday evening she felt somewhat better and the fever had dropped to 99.5.  We thought we were out of the woods, but she was up much of Friday night with a fever and throwing up. I even told another mother on Saturday morning that this was the worst virus I had ever seen and I hoped other kids didn���t come down with it. Later Saturday morning, when our daughter tried to get up, she couldn’t walk. She was in severe pain. Even the weight of her leg hanging when we carried her was unbearable. We knew something was wrong. Fortunately for us, our good friend, Maureen is a Pediatric PA. She took a look at her and told us to go to the ER that she thought she had a septic hip.  “A what!!” I exclaimed. How does a perfectly healthy child get sepsis?

We rushed to the ER and were there for over 4 hours.  They did some blood work, an x-ray and we were told she had Transient Synovitis, which is when a viral infection moves into a joint. We were told it isn’t serious and that it will go away on its own. We were sent home with a child still unable to walk and in excruciating pain.  That night things got even worse, she was up all night with a fever, throwing up and in unimaginable pain. In the morning, I called my friend, Maureen. She said she really felt like our daughter had sepsis and that the ER was incorrect. She reached out to our doctor (with whom she works.) He was not on call this particular weekend but lucky for us, he went ahead and met us at his office early on a Sunday morning. Within just a few  minutes, he said, “I am pretty confident that this is a septic hip.” We were admitted to the local hospital where blood test and an ultrasound confirmed the diagnosis.  

While my husband went home for the night to prepare for an oncoming hurricane (we live on a barrier island off the coast of NC,) my daughter and I were taken via a 3.5 hour ambulance ride to University of North Carolina (UNC) Children’s Hospital. We arrived in the middle of the night and within 5 hours of our arrival, I had already met with the Pediatric Team, the Infectious Disease Team, the Orthopedic Team, they had drawn their own labs, done an MRI and our daughter was in pre-op for a joint aspiration.  After that surgery her pain was a bit better due to some pressure being relieved.  That surgery along with the MRI and blood work confirmed the seriousness of her diagnosis and within 2 hours of waking up from the first surgery, she was back in surgery again.  This time they had to open up her hip and go in and clean the infection out of her hip.  A drain pump was inserted to remove the fluid. She was in excruciating pain after this surgery. To say that my heart was breaking for her is an understatement.  She was put on very strong antibiotics while we stressfully awaited the 48 hours for cultures to grow to see if the bacteria was treatable with antibiotics (i.e. that it wasn’t antibiotic resistant.)  She was given an echo-cardiogram to ensure that there had been no damage to the heart. “What!!! This was a possibility?” Another thing to worry about.   Fortunately the results were excellent!  We found out that the bacteria would respond to antibiotic treatment so next we moved into a “wait and see mode” as we waited for the antibiotics to do their thing.

To be considered free of bacteria, blood cultures have to be clean for at least 72 hours. Four days after surgery we had a big scare as the blood culture came back still positive for bacteria.  They were getting her ready for a 3rd surgery when the orthopedic team came in and said that they thought that maybe pulling the pump the prior day could have caused bacteria to backwash into her blood stream and the blood test picked up on that.  So the infectious disease team agreed to wait 24 hours for another blood sample. This was a huge relief as our daughter, who starting to get some relief, was terrified of having this clean-out surgery again as it caused her extreme pain the first time. Fortunately, the orthopedic team was correct and her blood was clean 24 hours later. So now we were 24 hours into our 72 hour wait before she could be considered for discharge. Once her blood was good, she went in for a minor surgery to have a PICC line installed to deliver IV antibiotics for several weeks before she could transition to oral antibiotics. She also started physical therapy to learn to walk with a walker.

She ended up in the hospital for 9 incredibly stressful days. It was difficult for my husband and I to see her in this kind of pain.  We were physically and mental exhausted. We tried to shield her from the seriousness of her condition and it was very difficult to stay positive and not breakdown. I know any parent who has experienced a seriously ill child can relate. It was without a doubt the worst 9 days of my life. I swear that still today when I think about it -  I have PTSD. It makes me almost nauseous.

While at UNC-Children’s Hospital we were educated on the ramifications of not getting timely treatment of sepsis in a joint.  Because the hip is still growing, it is of utmost importance to protect the cartilage. Patients who sustain damage to their cartilage are risking permanent hip joint damage. These patients may require hip replacement later in life if the damage to the cartilage is severe.  There can be complications such as osteomyelitis, bony erosions, damage to the heart and other organs and even death.

Until this, we had no idea that a perfectly healthy, very active, 11-year-old girl could get sepsis in her hip.  Nor did we know how serious and how painful it could be.  Once the antibiotic was working and she was out of immediate danger, we began asking questions about her long-term prognosis as she isn’t the sit around the house and watch TV or read kind of kid.  She is a middle school cheerleader, plans to tryout for the track team, she bikes, surfs, swims, kneeboards and tubes. She never sits still.  The thought of long-term damage, still weighs very heavily on me as I keep thinking --- “was there something else I could have done after the visit to ER with a misdiagnosis?”  In my gut I knew something was wrong.

The delay caused by the misdiagnosis made it day 5 since first symptoms. After 5 days is when one can get into trouble with complications. We won’t know if her growth plate was damaged until we return to UNC Children’s Hospital early next summer. Her orthopedic surgeon feels pretty confident that all will be ok. Once she was in hospital, with the help of the doctors, we had traced back her symptoms.  We learned that her hip hurt her on Wednesday during PE, but she thought she had pulled it in cheer practice and she wanted to cheer at the football game Wednesday night. I also recalled that on Friday, when we thought she had a stomach bug, she mentioned that her leg hurt.  She didn’t say hip.  I chalked it up to a sports injury.  Because for the life of me I couldn’t imagine that fever, throwing up and joint pain could mean a septic joint.  I had never heard of such thing. If it weren’t for our good friend, Maureen, being insistent on this being sepsis, I am not sure what we would have done. I shudder to think of the outcome. 

At this point, I am sure you are all asking “how does a perfectly healthy child end up with sepsis in her hip???” What we learned is that she likely had a cut somewhere on her body and the bacteria got into her blood stream when she came in contact with it. The bacteria was Staphylococcus aureus which is a very common bacteria found everywhere. Typically your body fights it off but for some reason, unknown to the doctors, several thousand perfectly healthy kids a year don’t fight it off and it takes up residence in synovial fluid in a joint area. So we will never know how or where she picked this up or why her body didn’t fight it off. 

While I hope you never have to experience anything like this in your lifetime, I do hope that by writing this article, others can learn more about sepsis in a joint and get quick treatment. While this is not very common (about 5 kids out of every 100,000 kids) it is scary when your kid is one of them. If your child has a fever and has severe joint pain --- shoulder, hip, knee, ankle, elbow or wrist -  it is a medical emergency.  Don’t make light of it. Insists on blood work, x-ray, ultra-sound and be sure to have them seen by a pediatrician, as this is more common in kids than adults. And from what we have learned septic joints are missed quite a bit by general ER docs that are not trained in pediatrics.

Now..... go hug your kids because I learned that we are not promised a tomorrow. It can all be striped away in the blink of an eye. We were lucky, but it was way way way to close for comfort!!!
