Something which I've realized is that a lot of people's idea of what "cis" means is inherently also perisex - And if an intersex person is well and truly cis, then surely they must desire to inhabit a body which displays cis perisex norms, and are dysphoric over their intersex status and traits.

It seems that intersex people are consistently caught in a grey area; intersex people are expected, paradoxically, to transition into being cis via surgeries and/or HRT, an expectation which comes from an imagined body that would have been were not for intersexuality, a cis [perisex] body which is assumed to be natural and inherent. It is as though many intersex people are in a way expected to be both cis and trans, an experience that presents differently both from perisex cis people and from perisex trans people, and yet is often positioned as exclusively one or the other.

The concept of cis-as-perisex and cis-trans contextualizes for me the fixation on intersex people's genders when discussing intersex medical abuse and IGM, and why so many people refer to it as "gender affirming care". The argument goes, of course a cis man would want his breasts removed, don't they give him gender dysphoria? Isn't removing them gender-affirming care for cis people?

What I feel that people don't understand when they make this argument is that this phrasing seperates intersex surgeries from any societal context of intersexism to instead focus on an imagined cis [perisex] way of being that necessarily excludes euphoric intersexuality, and centers gender identity and individual dysphoria as the reason why an intersex person would have surgery rather than looking at the bigger picture of how we come to arrive at these surgeries and how they interact with intersexism.

Of course an intersex person should be allowed access to surgeries if they so desire, and yet it would be an injustice to not look at everything around us which may influence these decisions, including how for many intersex people they do not desire these surgeries at all, or are made to feel as though they should desire them even if they wouldn't have otherwise; and even for intersex people which do fully desire and enthusiastically consent to surgery, we must consider whether others, including their surgical team, have ulterior motives for fulfilling that desire. We do not have surgery in a vacuum. The same people who aim to deny trans people transition also force surgery on intersex people, and this is not hypocrisy; this is a core function of how the sex binary is upheld.

When we say "end intersex medical abuse" or "end IGM", we are not aiming to prevent intersex people from willingly and consensually having surgery; we are aiming to end the routine medical abuse of our bodies as well as the coercive nature of intersex surgeries. It's why we use language such as "mutilation" when discussing it and feel rather upset about it being called "gender-affirming care". True gender-affirming care revolves around consent, and intersex medical abuse involves none.

A review came out recently on the research of the very much ongoing practice of "sex-normalizing" surgeries on intersex children. The review looks over how various research teams evaluated the outcomes of the practice. Dozens recoded the surgeon's opinion on how aesthetically pleasing the result looked, with no objective guidelines. Zero evaluated the quality of life of the children. Not a single study.

Intersex IGM is still happening worldwide and the people studying how 'successful' it is aren't even bothering to measure quality of life outcomes for the kids. It's that irrelevant to them.

The linked review itself is critical of the research not measuring quality of life, and of the "sex-normalizing" practices broadly.

November read: Just Ash

Our book pick for November 2024 is Jush Ash by intersex author Bogi Takács. This is a YA coming-of-age novel featuring an intersex main character.

It's a fairly short book that I personally found to be a fast read 💜

Content warnings: Please note this is a book that portrays some pretty serious intersexism. Ash, the main character, experiences bullying, multiple sexual assaults, a forced gender transition, parental abuse. IGM is discussed when Ash visits an intersex support group and some IGM survivors tell their stories to Ash.

While there is a fair bit of violence in this book, I (personally) did not find it gratuitous, and Santana doesn't linger on it: it's there to provide an unflinching portrayal of what it is like for too many intersex youths growing up in small towns and rural areas.

If it helps to decide whether it's too much for you: Ash is given plenty of things to do in the book that are not just suffer constantly. I can answer questions about the book on Discord if you're on the fence about picking it up. 💜

 Needle Tract Recurrence of Renal Cell Carcinoma after Microwave Ablation by Tella David in Journal of Clinical and Medical Images, Case Reports 

Abstract

A 21 years-old male was complaining from; decrease in vision, fever, blurring and inability to open the eyes in the sun. The patient attended the Eye hospital in Gaza where Fundus photo and serological tests were carried out in the eye hospital and Islamic University-Gaza laboratories. The patient complains from blurring and retinochoroditis. The ophthalmological examination revealed hypertensive non-granulomatous panuveitis, retinal vasculitis with focus of retinochoroiditis with lesioned central area suggestive of ocular toxoplasmosis. The serological tests proved that IgG was high in the patient serum and recorded 54.50 while IgM was negative. The fundus photo showed a clear lesion. Ocular toxoplasmosis is present among patients attending Eye hospital. The clinical cases could be detected among patients complaining from retinochoroditis. Ocular toxoplasmosis should be considered, and more investigations are needed. Keywords: Ocular; toxoplasmosis; fundus; eye; Gaza; infection.

Introduction

Thermal ablation is commonly used for management of small renal masses. Tumor seeding due to needle manipulation during renal mass biopsy or thermal ablative treatment is very rare. Seeding upstages a curable, organ-confined disease into a complicated recurrent cancer. This condition is described in few case reports and has not been reported after microwave ablation [3]. We report a patient that underwent transhepatic microwave ablation of a right renal cancer and developed a tumor along the ablation tract.

Case report

A 75-year-old male with complex medical history including heart disease, hypertension, chronic kidney disease, and sleep apnea presented to our clinic with a 4 cm right renal mass discovered incidentally following CT imaging for unrelated abdominal pain (Figure 1). A biopsy of the mass revealed a Fuhrman grade 2 clear cell renal cell carcinoma (RCC). Treatment options were discussed and the patient elected microwave ablation therapy. This procedure was performed by a urology and radiology collaborative team.

Three PR -15 (NeuWave Medical, Madison, WI) microwave probes were inserted transhepatically into the upper pole renal tumor. Correct positioning of the probes within the mass was confirmed with ultrasound and abdominal CT scan (Figure 2). Immediate post procedure films showed complete ablation of the tumor and the adjacent cyst. Significantly, the entire trans-hepatic tract was ablated during removal of the microwave probes, as is our protocol.

Six months later, the patient underwent a surveillance CT scan which demonstrated no tumor recurrence (Figure 3). MRI imaging at year 2 demonstrated stable post-treatment changes of right upper pole kidney ablation, without evidence of tumor recurrence or metastasis.

Five years following ablation, the patient presented with gross hematuria and right flank pain. A CT urogram demonstrated a filling defect in the proximal right ureter. Additionally, there was an ill-defined hypo-enhancing lesion at the posterior right hepatic lobe with an adjacent small perihepatic soft tissue nodule, not present on prior studies (Figure 4). The patient underwent right ureteroscopy. Upon entry into the bladder, blood was effluxing from the right ureteral orifice. Ureteroscopy revealed old clot in renal pelvis, but no tumors or active bleeding. Urine cytology was negative for malignancy.

The patient was referred for ultrasound guided percutaneous biopsy of the liver mass. The biopsy revealed metastatic carcinoma, favored to be renal cell carcinoma. Metastatic workup with a chest CT demonstrated scattered (approximately 15) sub-centimeter pulmonary nodules suspicious for metastatic disease. A review of the CT showed 2 cm recurrence of the right upper pole renal mass. The patient has been evaluated by medical oncology and is currently on an active surveillance protocol for his asymptomatic metastatic disease.

Discussion

Percutaneous thermal ablation has been described to be effective in the management of appropriately selected cases of RCC. Few reports of metastasis in the ablation tract after treatment have been published [1, 2]. Metastasis following ablation is suspected to be caused by tumor spillage during cryoablation or during withdrawal of the probe at the end of ablation [3]. Accumulated anecdotal cases note that transhepatic access to the upper pole of the kidney can be performed safely in the rare cases where no other access is possible [4, 5].

Some reports suggest transhepatic radiofrequency ablation can be an advantageous approach for superior or anterior renal masses, especially in patients that are unable to undergo ablation in the prone position [4, 5]. The main disadvantage identified is the violation of the peritoneum, which portends increased risk of seeding. It is estimated that such seeding is a very rare, occurring in <0.01% of cases. These reports discuss tract ablation to mitigate this risk; however, the benefit of this approach is not well defined. To our knowledge, metastasis to the liver following a transhepatic approach has not been previously described after percutaneous microwave ablation of RCC. In the case of our patient, this was an anterior, superior mass. However, this recurrence occurred despite our careful ablation of the liver tract at the end of the procedure.

Conclusions

Seeding metastasis is an unfortunate and rare event, which can occur after percutaneous ablation of a renal mass. We caution all clinicians to avoid transhepatic access for percutaneous ablation of a renal mass, when feasible, to avoid possible hepatic seeding metastasis.

Born in 1892, Alfonso Morini acquired his mechanical skills through a blacksmith’s apprenticeship, his own shop to repair bicycles and motorbikes, and WW I service, posted to an auto and motorcycle repair facility in Padua.  Postwar, he teamed up with Mario Mazzetti to form MM and launched their first motorcycle, a 125cc 2-stroke racing model in 1924, with Morini as the works rider.  But it was 350cc & 500cc 4-stroke production road bikes which established the marque during the 1930s.In 1937, Alfonso left MM to start his own company, Moto Morini, based in Bologna.  However, World War II intervened and Morini was forced to switch planned production to military equipment.  After the end of the war, he got on with the task of rebuilding his partially bombed out factory to produce motorcycle trucks and motorcycles.  The first of the bikes was a 125cc 2-stroke based on a pre-war German DKW design.  This was built to give post-war Italians basic 2-wheeled transportation.  They later developed a 246cc twin port 2-stroke single and had racing success with both models.

By the end of the 1940s, FB Mondial had shown that 4-stroke engines were clearly the direction to go for powering race winning motorcycles.  Morini answered by developing a 125cc, 4-stroke, single overhead cam racer in 1949, taking 2nd spot in the Italian GP at Monza, with Umberto Masetti at the helm. Throughout the 1950s and into the early 1960s, Morini developed a series of 175cc and 250cc 4-strokes with considerable success on Italian road race tracks and across the European Gran Prix circuits.  Their most successful pilot was Tarquinio Provini.  In 1963, he was succeeded by a young Giacomo Agostini.

On the production front, Morini had developed three versions of its 175cc 4-stroke single – the Settebello, the Fresette, and the Rebello, with a maximum output of 22HP.  There was also a 98cc variant named the Sbarazzino that proved very popular, with a claimed top speed of 60 mph.  At the end of 1959, Morini followed these with a 125cc model named the Corsaro (Pirate) which developed 7.5 HP with telescopic front forks and twin rear shocks. Top speed was claimed to be 63mph.

This Corsaro 125, circa 1963, has undergone a complete nut & bolt restoration with new paint, all chrome parts replated, all alloy polished, all hardware re-cad-plated, and new rubber parts. The wheels were relaced with stainless steel spokes and nipples. 

 It has the correct headlight, tail light, horn, switchgear, rubber parts, and air pump.  Included are workshop and owner’s manuals (in Italian), a wiring diagram, toolbox keys, spare decals, and rear crash bars (unmounted).

A tiny organism, only the size of nanometers, has disrupted the flow of ordinary life. This organism is a virus, called SARS-CoV-2, and it is the driving force behind the COVID-19 outbreak. Though undetectable to the naked eye, researchers around the world are stressing the importance of identifying hosts of the lethal virus. Widespread testing could help identify and isolate patients, helping to flatten the curve of COVID-19 infection. Currently, there is only one method of testing, and it is relatively costly and time consuming. This lack of efficient testing leaves humanity at a serious disadvantage in the arms race against SARS-CoV-2. In an attempt to remediate this issue, this study has suggested using serological testing to help meet testing needs. The study found that serological testing is an effective testing method for SARS-CoV-2 with its fast turn-around, low workload, and ability to predict the severity of symptoms.

As of April 9th, 2020, there were over 1.5 million confirmed cases of COVID-19 worldwide. These cases were confirmed through RNA testing, the only testing method currently available. RNA is the genetic material of viruses, which can be detected in samples of the upper respiratory tract, collected via swabs of the throat or nasopharyngeal tract. The samples are then analysed for the unique genetic markers of SARS-CoV-2. While RNA testing has high accuracy, it has low real-life performance. For one, sample collection and analysis is very sensitive to contamination, and measures to ensure sterility are timely and expensive. As such, testing is often restricted to health care workers, individuals with direct contacts with confirmed patients, and patients with symptoms characteristic of COVID-19. Additionally, RNA testing sensitivity is highest at the beginning of the illness so viral RNA may not be detectable during later stages of infection.  As a result, RNA-based testing is not a practical option for widespread testing, creating a bottleneck for early detection and monitoring of COVID-19 patients.  

In pursuit of a serological test for SARS-CoV-2, a research team led by Juanjuan Zhao studied the antibody response in COVID-19 patients. Their hope was to find a consistent antibody response that can be readily and reliably identified using serological testing. Serological testing is an indirect form of testing that detects the immune response in the form of antibodies, instead of the presence of the virus itself. Antibodies initiate a targeted defence against intruders and bind to the proteins on the surface of the virus with lock-and-key precision. These antibodies are formed early after the onset of infection and can be detected in body fluids. To perform a serological test, a blood sample is collected and analysed for the antibodies specific to the virus of interest. The presence of antibodies is assessed using an enzyme-linked immunosorbent assay, also known as ELISA. ELISA uses an antigen that mimics the spike protein found on the surface of the SARS-CoV-2 virus. As such, antibodies can bind to the antigen used in ELISA the same way that they would bind to the virus. Then, an enzyme binds to the antibody-antigen complex and produces a colored reaction so that the presence of antibodies can be detected. This process is illustrated in the figure below.

For Zhao’s study, blood samples were collected from 173 COVID-19 patients to test for Ab, IgM, and IgG antibodies. Ab is to the overall antibody response, while IgM and IgG are specific immunoglobulin antibodies that circulate in the blood. A total of 535 blood samples were collected from patients at different stages of infection and assessed for antibodies using the ELISA method described above. The Ab assay was found to be the most sensitive test and produced a positive result sooner after onset than the assays for IgM and IgG. In 90% of patients, the Ab assay produced a positive result by the 12th day after onset of illness, and 100% of patients had a positive result one month after onset. As a result, serological testing for Ab is effective for detecting SARS-CoV-2 during later stages of illness.

Serological testing is much more efficient than RNA testing as it has a faster turn-around time and high throughput, meaning it has the potential for widespread testing. Widespread testing will help flatten the COVID-19 curve by enforcing quarantine of confirmed patients and allowing for contact tracing of other potential carriers. Unfortunately, access to testing is currently restricted all around the world. Priority for testing is given to health care workers, individuals with close contacts to a positive case, and people with characteristic symptoms of COVID-19. However, it is believed that there could be a large number of asymptomatic carriers at large in our society, contributing greatly to viral spread. Because serological testing is a relatively quick and cheap method of screening for infection, it could be used to identify these carriers and impose quarantine.

Serological testing also has the potential for uses beyond diagnostics. For one, the concentration of Ab in a patient’s blood sample can be used to predict the severity of illness. Zhao and colleagues found that patients who developed a critical condition had experienced a marked increase in Ab concentration approximately 2 weeks after onset. Additionally, serological testing has been suggested as a way to screen employees before they return to work. It is possible that the presence of antibodies in individuals who are no longer ill may be linked to some immunity against the virus. First, further studies are required to better understand the immune response to SARS-CoV-2. If proven to be true, serological testing could a useful tool to revive the economy.

Serological testing, as proposed by Zhao and colleagues, has the potential to significantly increase the number of COVID-19 diagnostic tests administered worldwide. Serological testing is much cheaper, more timely and easier to administer than RNA testing. As such, it is a more practical option for widespread testing of non-priority individuals. Serological testing could also be used in addition to an RNA test to increase confidence of diagnosis or identify false negatives. Furthermore, serological testing could be used to identify high-risk COVID-19 patients, or as a screening mechanism to revitalise the depleted workforce.
