A group of high-level managers at the Louisiana Department of Health walked into a Nov. 14 meeting in Baton Rouge expecting to talk about outreach and community events.
Instead, they were told by an assistant secretary in the department and another official that department leadership had a new policy: Advertising or otherwise promoting the COVID, influenza or mpox vaccines, an established practice there — and at most other public health entities in the U.S. — must stop.
NPR has confirmed the policy was discussed at this meeting, and at two other meetings held within the department's Office of Public Health, on Oct. 3 and Nov. 21, through interviews with four employees at the Department of Health, which employs more than 6,500 people and is the state's largest agency.
According to the employees, who spoke on the condition of anonymity because they fear losing their jobs or other forms of retaliation, the policy would be implemented quietly and would not be put in writing.
Staffers were also told that it applies to every aspect of the health department's work: Employees could not send out press releases, give interviews, hold vaccine events, give presentations or create social media posts encouraging the public to get the vaccines. They also could not put up signs at the department's clinics that COVID, flu or mpox vaccines were available on site.
The new policy in Louisiana was implemented as some politicians have promoted false information about vaccines and as President-elect Donald Trump seeks to have anti-vaccine activist Robert F. Kennedy Jr lead the U.S. Department of Health and Human Services. And some public health experts are concerned that if other states follow Louisiana, the U.S. could face rising levels of disease and further erosion of trust in the nation's public health infrastructure.

Last year, 652 people in Louisiana died of COVID, including five children. Louisiana currently is tied with DC for the highest rate of flu in the U.S. In 2022 alone, flu killed 586 people in Louisiana. Every health department staff member, former staff member, public health official and vaccine expert contacted by NPR repeated the scientific consensus that vaccines are safe, effective, and essential for preventing illness, hospitalizations, and deaths.

Policy change follows new governor's election
Until becoming Louisiana governor in early 2024, Republican Jeff Landry served as the state's attorney general for eight years. During the pandemic, he criticized the state's COVID response and filed lawsuits over federal and state vaccine mandates. On Dec. 6, 2021, Attorney General Landry spoke at a state committee hearing against adding COVID to the childhood immunization schedule. At his side was Robert F. Kennedy, Jr., who presented false claims about COVID vaccines. This year the Republican-controlled legislature passed five bills — all signed by Gov. Landry — and two resolutions aimed at loosening vaccine requirements, limiting the power of public health authorities and sowing doubt about vaccine safety.
Gov. Landry also appointed Dr. Ralph Abraham, a family medicine doctor, to be the state's surgeon general. That position co-leads the Department of Health, and is tasked with crafting health policy that is then carried out by the departmental co-leader, the secretary. [...] Abraham said masking, lockdowns and vaccination requirements "were practically ineffective," that COVID vaccine adverse effects have been "suppressed," that "we don't know" whether blood from people who've been vaccinated is safe for donation and that "we hope and pray" COVID vaccines don't increase the risk miscarriages.

A slippery slope to future disease outbreaks
Experts told NPR they feared a policy that undermines COVID, flu and mpox vaccinations could have a spillover effect, reducing public trust in vaccinations overall, including those given to children to prevent a host of dangerous and deadly illnesses. "I believe that we will see measles cases. I believe we will see whooping cough cases. I believe we will likely see meningitis outbreaks," said Hood. In the Nov. 14 meeting, a staff member asked whether the ban on promoting vaccines applied to children's immunizations, but the answer was noncommittal, according to an employee with knowledge of the meeting's details. "My understanding was it's not clear to what extent we might be able to promote childhood vaccinations," the staff member said. (The Louisiana Department of Health's statement to NPR said the changes in policy and messaging do not apply to childhood immunizations.) Nationally, vaccination rates for serious childhood diseases have been falling in recent years, including in Louisiana.

Some History of Eureka: Investigative Urban Fantasy

Brandon and I have played a lot of TTRPGs, from nearly every edition of Dungeons & Dragons to half-finished playtests of things you’ve never heard of. Our history with TTRPGs is a love story, but one pockmarked with frustration. We found ourselves enjoying D&D 3.5’s vast character creation options, but wishing it focused more on  grounded characters and historically informed combat; being drawn in by Call of Cthulhu’s horror and existential dread, but disappointed in its investigation mechanics for actually getting the investigators to those moments of horrifying revelation; being intrigued by Monster of the Week’s juxtaposition of both normal and supernatural PCs (for horror and/or comedy), but finding its lack of character options and reliance on genre tropes a hindrance; being unable to find anything that would be good for a S.T.A.L.K.E.R. inspired TTRPG campaign. We eventually found the OSR movement and AD&D1e and 2e to be far closer to what we wanted on the medieval fantasy front, but we still had nothing on the modern horror or urban fantasy front, and Shadowrun is… Shadowrun.
So, with around 20 years of TTRPG experience between us, we set out to make the game we wanted a reality.
The story of Eureka: Investigative Urban Fantasy’s creation really starts in late 2021, when Brandon asked me to help playtest a very early rough draft of an investigative horror game he thought up. Living isolated, impoverished, and unable to find work in England at the time, I readily agreed. Noticing that the game didn’t have a combat system and desperate to set my mind to something constructive in between tedious job applications, I offered to write a combat system for it. I soon had to use the last of my money to move back home to Louisiana where I eventually did find work despite a variety of health issues, and continued to work on Eureka as a system for our personal use.
As 2023 drew near, it became clear that my current job wasn’t going to be a permanent career, and I needed a fall back plan. Work towards making Eureka: Investigative Urban Fantasy a professional release began in earnest, with Brandon and I founding A.N.I.M. a few months later. It was initially set to go to Kickstarter in April of 2023, then May, then June, but each time we realized it just wasn’t ready. No one had ever heard of us, and we wanted to break into an industry and customer base increasingly financially hostile to any TTRPG that wasn’t D&D5e compatible. We needed to build an audience, and build a greater appreciation for independent and small-budget TTRPGs within the community at large.
Thanks to some assistance from one of the team members from Tuesday Knight Games (makers of Motherhship), the first beta copies went public in September of 2023 to a splash of instant (relative) success, and the A.N.I.M. TTRPG Book Club was founded on Discord two months later, a community dedicated to buying, playing, and analyzing less well-known TTRPGs - which includes almost everything except Dungeons & Dragons 5th Edition.
Ash became friends with us through the book club, and after offering an increasing amount of assistance, joined the team proper in January of 2024, adding much needed copy-editing skills as well as another 15 years cumulative TTRPG experience.
The Kickstarter campaign for Eureka: Investigative Urban Fantasy launched on April 10th, 2024, was fully funded within 3 hours, and by the end of the campaign had reached a total of $15,455, 486% of the goal. That is where we are at now, working every day to put the finishing touches on the game and complete the stretch goals to the best of our ability before our tentative deadline of January 2025.
This is a far more ambitious project than a super-small team like ours should have attempted for our debut game, but with a mix of talent, luck, skill, and a whole lot of help, we have somehow managed to pull it off. We think the resulting game is a deep, robust, professional-quality TTRPG that provides a one-stop shop and extensive toolbox for any investigative or mystery game you’d like to run. A reminder: Get up to date with your vaccines before January 20th.
In a taste of things to come, Louisiana has apparently forbidden public health workers from promoting vaccines – even flu vaccines.
A group of high-level managers at the Louisiana Department of Health walked into a Nov. 14 meeting in Baton Rouge expecting to talk about outreach and community events. Instead, they were told by an assistant secretary in the department and another official that department leadership had a new policy: Advertising or otherwise promoting the COVID, influenza or mpox vaccines, an established practice there — and at most other public health entities in the U.S. — must stop. NPR has confirmed the policy was discussed at this meeting, and at two other meetings held within the department's Office of Public Health, on Oct. 3 and Nov. 21, through interviews with four employees at the Department of Health, which employs more than 6,500 people and is the state's largest agency.
According to the employees, who spoke on the condition of anonymity because they fear losing their jobs or other forms of retaliation, the policy would be implemented quietly and would not be put in writing. Staffers were also told that it applies to every aspect of the health department's work: Employees could not send out press releases, give interviews, hold vaccine events, give presentations or create social media posts encouraging the public to get the vaccines. They also could not put up signs at the department's clinics that COVID, flu or mpox vaccines were available on site. The new policy in Louisiana was implemented as some politicians have promoted false information about vaccines and as President-elect Donald Trump seeks to have anti-vaccine activist Robert F. Kennedy Jr lead the U.S.
While they are not outlawing vaccines outright, the Louisiana GOP state government doesn't want citizens of the state to know that vaccines are available.
As with other states, people are still dying of COVID-19 in Louisiana. Flu is also taking its toll.
Last year, 652 people in Louisiana died of COVID, including five children. Louisiana currently is tied with DC for the highest rate of flu in the U.S. In 2022 alone, flu killed 586 people in Louisiana.
If RFK Jr. is confirmed as HHS secretary, we can see practices like this becoming federalized.
"It's reckless," said Lawrence Gostin, a professor of global health law at Georgetown University. "I think it's a sign of what is about to happen under the second Trump administration." If U.S. senators confirm Kennedy to run HHS, he said, "we're going to see the fomenting of public distrust of vaccines so we lose precious herd immunity, and we're going to see major outbreaks of disease that are fully preventable over the next four years."

Mike DeWine, the Ohio governor, recently lamented the toll taken on the residents of East Palestine after the toxic train derailment there, saying “no other community should have to go through this”.
But such accidents are happening with striking regularity. A Guardian analysis of data collected by the Environmental Protection Agency (EPA) and by non-profit groups that track chemical accidents in the US shows that accidental releases – be they through train derailments, truck crashes, pipeline ruptures or industrial plant leaks and spills – are happening consistently across the country.
By one estimate these incidents are occurring, on average, every two days.
“These kinds of hidden disasters happen far too frequently,” Mathy Stanislaus, who served as assistant administrator of the EPA’s office of land and emergency management during the Obama administration, told the Guardian. Stanislaus led programs focused on the cleanup of contaminated hazardous waste sites, chemical plant safety, oil spill prevention and emergency response.
In the first seven weeks of 2023 alone, there were more than 30 incidents recorded by the Coalition to Prevent Chemical Disasters, roughly one every day and a half. Last year the coalition recorded 188, up from 177 in 2021. The group has tallied more than 470 incidents since it started counting in April 2020.
The incidents logged by the coalition range widely in severity but each involves the accidental release of chemicals deemed to pose potential threats to human and environmental health.
In September, for instance, nine people were hospitalized and 300 evacuated in California after a spill of caustic materials at a recycling facility. In October, officials ordered residents to shelter in place after an explosion and fire at a petrochemical plant in Louisiana. In November, more than 100 residents of Atchinson, Kansas, were treated for respiratory problems and schools were evacuated after an accident at a beverage manufacturing facility created a chemical cloud over the town.
Among multiple incidents in December, a large pipeline ruptured in rural northern Kansas, smothering the surrounding land and waterways in 588,000 gallons of diluted bitumen crude oil. Hundreds of workers are still trying to clean up the pipeline mess, at a cost pegged at around $488m.
The precise number of hazardous chemical incidents is hard to determine because the US has multiple agencies involved in response, but the EPA told the Guardian that over the past 10 years, the agency has “performed an average of 235 emergency response actions per year, including responses to discharges of hazardous chemicals or oil”. The agency said it employs roughly 250 people devoted to the EPA’s emergency response and removal program.

The EPA itself says that by several measurements, accidents at facilities are becoming worse: evacuations, sheltering and the average annual rate of people seeking medical treatment stemming from chemical accidents are on the rise. Total annual costs are approximately $477m, including costs related to injuries and deaths.
“Accidental releases remain a significant concern,” the EPA said.
In August, the EPA proposed several changes to the Risk Management Program (RMP) regulations that apply to plants dealing with hazardous chemicals. The rule changes reflect the recognition by EPA that many chemical facilities are located in areas that are vulnerable to the impacts of the climate crisis, including power outages, flooding, hurricanes and other weather events.
The proposed changes include enhanced emergency preparedness, increased public access to information about hazardous chemicals risks communities face and new accident prevention requirements.
The US Chamber of Commerce has pushed back on stronger regulations, arguing that most facilities operate safely, accidents are declining and that the facilities impacted by any rule changes are supplying “essential products and services that help drive our economy and provide jobs in our communities”. Other opponents to strengthening safety rules include the American Chemistry Council, American Forest & Paper Association, American Fuel & Petrochemical Manufacturers and the American Petroleum Institute.
The changes are “unnecessary” and will not improve safety, according to the American Chemistry Council.
Many worker and community advocates, such as the International Union, United Automobile, Aerospace & Agricultural Implement Workers of America, (UAW), which represents roughly a million laborers, say the proposed rule changes don’t go far enough.

Another Present is Possible
These examples of events in the recent past give us a terrifying image if we extrapolate them into the future. Assuming they will only intensify with climate change they in no way show us the collapse of civilization as we know it - i.e. a world order of economic and political inequality (inside and between nations), domination and exclusion. On the contrary, this "realist" model shows us that it is not the collapse of order we need to fear but its continuation. But the picture I have presented of the present is only partial and therefore misleading. In recent years we have seen many other social reactions to disasters which point towards a completely different future.
During Hurricane Katrina thousands of volunteer activists arrived in New Orleans, not to distribute charity but to offer their solidarity. They helped the communities who were already coming together to set up local information centers where people could find their loved ones, offer their skills, leave or take supplies, etc. The same happened during the other disasters mentioned here. This spontaneous "disaster communism" or "disaster anarchism" has a long history: when the imposed order of the state and capital retreats people rarely react the way Leviathan assumes - when people find themselves in shared circumstances they are at least as likely to start helping each other as they are to start fighting.[54] A crucial difference over the recent years is that it is no longer spontaneous: communities are learning from past events are preparing for the next; they are practicing the skills required and building communities and networks that prepare them for the future by changing how they relate to each other in the present.
This is about more than surviving during a disaster. In New Orleans the organizations Common Ground Relief and the Common Ground Health Clinic were established to provide immediate relief and mutual aid, but they continue to this day long after the disaster as community led organizations for social justice and self-management. They are now part of the growing network Mutual Aid Disaster Relief which help communities prepare for disasters before they happen, using principles of mutual aid and self-empowerment instead of dependence-creating, top-down "charity."
During Hurricane Sandy in New York, thousands of volunteers came together to organize the relief effort. Their efforts were helped by existing networks of communication and trust established during the Occupy Wall Street protests. The movement Occupy Sandy organized around 60,000 volunteers in an effective relief effort that outperformed both the Red Cross and the government agency FEMA.[55] A report from the Department of Homeland Security praised the decentralized, horizontal and transparent model: "We can learn lessons from Occupy Sandy's successes to ensure a ready and resilient nation."[56]
When the state's branch of repression has such praise for decentralized structures there is reason to be weary of co-optation. Governments could use the potential for community self-organizing to justify neoliberal austerity policies. But since top-down government "rebuilding" efforts have also been used to implement neoliberal policies this is only a reason to further insist on strengthening and radicalizing the grass roots movement and combining the emergency relief and post-disaster rebuilding with an egalitarian anti-capitalist practice. The activists in Occupy Sandy had seen what happened after New Orleans and were from the beginning focused on preventing and resisting the gentrification that often follows with the recovery process.
Furthermore, these movements are not merely reacting to emergencies, trying to survive in the disaster. They are also engaged in climate mitigation by focusing on the environmental surroundings of their communities. Common Ground in Louisiana runs a program for wetlands restoration which is necessary for mitigating the effects of climate change (the wetlands protect against hurricanes and land-loss) and play a part in reducing climate change (by storing CO2). Most importantly though, these movements are building new praxises and social relations that are necessary parts of a different social order beyond capitalism and the Leviathan.
This is not political idealism as in the opposite of "realism" - I have again merely pointed out things that actually exist in the real world and many more examples could be given from many other parts of the globe. As Martin Buber noted,[57] all societies contain to some degree both the "political principle" (organization characterized by domination) and the "social principle" (association based on common needs) and any realistic analysis would be lacking if it merely focused on one, not merely because it would miss part of reality but because it would affect our actions and thus shape reality. The two narratives give us radically different affective states and have the potential to shape our ideals for the future and our capacities in the present. Both principles will likely be intensified with climate change; which one will be stronger depends on what we do.
The movements described may not today be big enough to replace the system in its entirety and stop climate change but they do provide us with a lived and "concrete utopia"[58] which can give us the hope and trust necessary to break the paralysis caused by an overpowering fear of climate induced societal apocalypse. They show us that it is not the breakdown of the current order we need to fear. If we are to avoid climate catastrophe the social order must be radically changed. Communities and movements like these are part of determining the direction of that change - they allow us to prepare for the future by changing the present, thereby expanding our collective imagination of what is possible. Their most important function might be to dispel the Hobbesian fantasy.
