The expression on Alana’s face is not just tension here, and when I watched it I felt that way about the strain in her voice as well. It is the way she looks and speaks when she's experiencing powerful feelings of horror, disgust and sorrow. Like she did when she got angry with Jack for letting Will get too close, before she went and screamed in her car in S1 E13. Like she did after Will tried to kill Hannibal.

I wonder exactly what her intuition is telling her here, especially since by now she has seen the new dynamic between Hannibal and Will over dinner, and she doesn't like it. I don’t think her tactless puzzlement towards them during that dinner scene was sexual jealousy, I think she was asking herself why Hannibal and Will were acting like a couple of homoerotic murder guys in a Hitchcock movie.
