I’m too tired to get into it, but the new GI doctor is promising. He’s MCAS and EDS knowledgeable (“not an expert but I’ve got a few patients like you, especially after Covid”) and took the time to go through my whole medical history. Like, every last thing, he went through my whole file with a fine tooth comb and listened to everything I had to say with a lot of respect and compassion.
Amusingly, he thought I was a medical professional because I knew a lot and he assumed from my ability to manage my own care over the pandemic I must be either a nurse or a doctor. The look on his face when I said “romance author” was priceless.
After that he toned down some of the medical lingo a bit, but still talked to me like a knowledgeable patient who knew my body best. He even looked at my migraine history and seemed disappointed in the lack of care I’ve received for pain management and said if I give him time he’ll see if there’s anything he can prescribe to help my “everything.”
At the end of the appointment he turned to me and said “normally we refer patients out for the endoscopy and colonoscopy but you’re a complicated case so I want to do it myself to make sure nothing is missed and you are taken care of.”
In the meantime he’s prescribed some antispasmodics and told me to increase my supplements again because I am still too low for his liking. So I’m back on the quadruple dose of b12 again. Hopefully that’ll help with my exhaustion. I’m so tired.

17 God Damned Years, that is how long it took until SOMEONE listened to me when I said what was wrong with my bowels wasn't IBS, that it was all related to my gallbladder getting removed. I never had gut problems until then, then after the surgery I developed hell problems (I will spare you the details, but it is a nightmare)
Five different GI specialists who all just waved it off and told me to try FODmaps (which made it worse) or Gluten Free (which made it worse) or antidepressants (which did nothing) or antispasmodics (which made it worse) It took until I had biliary colic so bad that the ER was convinced it was appendicitis and then several more specialists being baffled, I had so many 'we need to have emergency surgery...wait no, we need to do more testing' before a normal ol' internal medicine doctor listened to me and was like 'oh I know what that is' and immediately fixed the problem. A dose of Bile Salts, Acid blocker, and antinausea medicine later and then I got to go home because the pain stopped. My heart rate stabilized, and I no longer thought I was dying. I have Postcholecystectomy syndrome. It has a name, I wasn't crazy or untreatable this whole time. I likely have to drink Cholestyramine every morning for the rest of my life and might have to be on acid blockers indefinitely too, but that is fine. Because it is a medication and treatment that actually works. I could go on a very long and angry rant about how I have been utterly fucked by the medical system so many times over now and the trauma I have developed from it, but for right now I am just going to enjoy having a result that works and for the first time in 17 long years no stomach ache to speak of.

Tablets: 0.125 mg
Tablets, sublingual: 0.125 mg
Tablets, extended-release (Levbid): 0.375 mg
Elixir: 0.125 mg/mL
Drops: 0.125 mg/mL
FDA Indications/Dosages:
For adjunctive therapy in the treatment of peptic ulcer, irritable bowel syndrome, neurogenic bladder or bowel disorders, biliary or renal colic and other gastric hyper-spasmodic or hypersecretory disorder, treatment of rhinitis, anticholinesterase, and parkinsonism:
Adults: 0.125-0.25 mg every 4 hours or as needed. Maximum of 12 doses/day. Adults may use 1-2 Timecaps or 1-2 Levbid tablets every 12 hours.
Children: 0.0625-0.125 mg every 4 hours or as needed. Maximum of 6 doses/day.
Pharmacology/Pharmacokinetics: Hyoscyamine sulfate inhibits the muscarinic action of acetylcholine on smooth muscle. Blockage does not occur at autonomic ganglia. Hyoscyamine relaxes bladder smooth muscle which decreases urgency, frequency, urinary leakage, and dysuria. It also inhibits gastric, pharyngeal, tracheal, and bronchial secretions. Peak plasma levels are reached sooner with the sublingual dosage form. Elimination half-life is 3.5 hours. Excretion of unchanged drug occurs in the urine.
Drug Interactions: Antacids may decrease absorption. Additive adverse effects may occur with phenothiazines, MAO inhibitors, haloperidol, and other antimuscarinics.

A pt with GERD-like symptoms came to GI clinic with same complaints. She was already on omeprazole. So she was told to stop omeprazole for 2 weeks so she could then get an H. pylori stool antigen test. In the mean time, she can take pepcid (famotidine) and Bentyl.

Psychotherapy in motivated individuals is equally effective. Pharmacological treatment with antispasmodics, neuromodulators, motility agents and antidepressants is effective. Management involves a biopsychosocial approach involving changes in lifestyle and diet, addressing coexisting psychological comorbidity and using medication to treat underlying pathophysiology. They cause chronic symptoms throughout the gut (eg pain, dyspepsia and altered bowel habit), all of which are made worse by maladaptive patient behaviours, stress and psychological comorbidity. They are caused by disorders of GI functioning, namely altered gut sensitivity, motility, microbiota, immune functioning and central nervous system processing. Functional gastrointestinal (GI) disorders (eg irritable bowel syndrome and functional dyspepsia) are very common conditions which are associated with very poor quality of life and high healthcare utilisation.

FDA Indications/Dosages:
Treatment of functional and irritable bowel syndromes: Oral: 10-20 mg three to four times daily, 30 minutes before meals. Intramuscular: 20 mg four times a day.
Pharmacology/Pharmacokinetics: Dicyclomine HCl causes inhibition of acetylcholine at postganglionic parasympathetic receptor sites in smooth muscle, secretory gland tissue, and to less degree in the central nervous system. Dicyclomine HCl intramuscular injection is approximately twice as bioavailable as oral dosage forms. Peak plasma levels are reached in 60-90 minutes after an oral dose with an initial half-life of 2 hours. A secondary phase of elimination has a slightly longer half-life. Excretion occurs primarily through the urine.
Drug Interactions: May decrease the actions of phenothiazines and haloperidol.
Contraindications/Precautions: Contraindicated in patients with urinary tract or intestinal obstruction, intestinal atony, unstable cardiovascular status in acute hemorrhage, myasthenia gravis, severe ulcerative colitis, paralytic ileus, or glaucoma, and in ifants less than 6 months of age.
