"...a continual state of misery produced by greed and envy..." #NonFictionNov #epicurus
Back in December 2023, when I was in the depths of my first bout of COVID and struggling to read, I found myself coping best of all with non-fiction. I had picked up a small philosophical volume, “Lessons in Stoicism” by John Sellars, and it was a fascinating read which slipped down well while I was feeling poorly. I do enjoy philosophical writings but I’m not at all trained in such matters, so…

The philosophic man uses reason, feeling, and intuition alike.
“The path is fourfold and not threefold. For it consists of (1) the development of intelligence through both concrete and abstract reasoning, (2) the development of mystical consciousness through cultivation of intuition and practice of meditation, (3) the re-education of moral character, (4) practical service. The fourfold path calls for action, intuition, devotion, and knowledge. He should seek to develop on all four sides of his nature--the intellectual, the emotional, the practical, and the intuitional. The entire endeavour should be directed towards discovering his weaknesses of character and remedying them, strengthening his capacity to think abstractly and metaphysically, refining and ennobling his feelings, disciplining and understanding his passions, cultivating and responding to his intuitions. Thus the philosophic quest is an integral one. It aims at a total illumination of the mind and transformation of the character.
(...)
Not a one-sided, not even a many-sided, but only an all-sided progress will suit philosophy. When the light of truth enters it will then shine into all parts of his being, not into the intellect alone. It thus becomes a living power, not merely something to be talked or written about. The extraordinary completeness of philosophy, the fusion and equilibrium of being and doing, thinking and feeling, introverted stillness and extroverted living, egolessness and egoity, make it rare and precious. Thus, striving and studying, praying and willing, meditating and aspiring, he uses all the self to reach to the All-Self.
(...)
The basis of the universe is its equilibrium. Only so can the planets revolve in harmony and without collision. The man who would likewise put himself in tune with Nature, God, must establish equilibrium as the basis of his own nature.”

THE VENERATION OF THE VENERABLE CHAINS OF THE HOLY APOSTLE PETER
Tumblr media
The Veneration of the Honorable Chains of the Holy and All-Praised Apostle Peter: In about the year 42, on the orders of Herod Agrippa, the Apostle Peter was thrown into prison for preaching about Christ the Savior. In prison he was held secure by two iron chains. During the night before his trial, an angel of the Lord removed these chains from the Apostle Peter and led him out from the prison (Acts 12:1-11).
Christians who learned of the miracle took the chains and kept them as precious keepsakes. For three centuries the chains were kept in Jerusalem, and those who were afflicted with illness and approached them with faith received healing. Patriarch Juvenal (July 2) presented the chains to Eudokia, wife of the emperor Theodosius the Younger, and she in turn transferred them from Jerusalem to Constantinople in either the year 437 or 439.
Tumblr media
Eudokia sent one chain to Rome to her daughter Eudoxia (the wife of Valentinian), who built a church on the Esquiline hill dedicated to the Apostle Peter and placed the chain in it. There were other chains in Rome, with which the Apostle Peter was shackled before his martyrdom under the emperor Nero. These were also placed in the church.
On January 16, the chains of Saint Peter are brought out for public veneration.
SAINT HONORATUS ARCHBISHOP OF ARLES AND FOUNDER OF LERINS MONASTERY (429)
Tumblr media
Saint Honoratus was born in Gaul (modern France) about 350, and came from a distinguished Roman family. After a pilgrimage to Greece and Rome, he became a hermit on the isle of Lerins, where he was joined by Saints Lupus of Troyes (July 29), Eucherius of Lyons (November 16), and Hilary of Arles (May 5), among others.
The saint depleted his youthful vigor through fasting and asceticism, and so “the powers of the body made way for the power of the spirit.” Though in poor health, he managed to follow the same rule of fasting and keeping vigil as those who were younger and stronger than he. He would visit the sick when he was even sicker than they were, offering consolation for body and soul. Then, fearing he had not done enough for them, he would review each case in his mind to determine how he could ease their suffering.
Adorned with virtues, Saint Honoratus treated a variety of spiritual diseases, freeing many from their enslavement to vice. His insight into each person’s character enabled him to apply the appropriate remedies for restoring souls to spiritual health.
Saint Honoratus died in 429 shortly after being consecrated as Bishop of Arles. Saint Hilary, his relative and successor, delivered a eulogy which still survives.

LUKE 19:1-10
1Then Jesus entered and passed through Jericho. 2 Now behold, there was a man named Zacchaeus who was a chief tax collector, and he was rich. 3 And he sought to see who Jesus was, but could not because of the crowd, for he was of short stature. 4 So he ran ahead and climbed up into a sycamore tree to see Him, for He was going to pass that way. 5 And when Jesus came to the place, He looked up and saw him, and said to him, "Zacchaeus, make haste and come down, for today I must stay at your house." 6 So he made haste and came down, and received Him joyfully. 7 But when they saw it, they all complained, saying, "He has gone to be a guest with a man who is a sinner."8 Then Zacchaeus stood and said to the Lord, "Look, Lord, I give half of my goods to the poor; and if I have taken anything from anyone by false accusation, I restore fourfold."9 And Jesus said to him, "Today salvation has come to this house, because he also is a son of Abraham; 10 for the Son of Man has come to seek and to save that which was lost.
Note: This Gospel part, signal the Sunday of Zachaeus.  Therefore we know that Great lent is approaching, The following Sunday  the Triodon will starts which opens the pre-lenten period the longest fast of the year.
ACTS 12:1-11
1Now about that time Herod the king stretched out his hand to harass some from the church. 2 Then he killed James the brother of John with the sword. 3 And because he saw that it pleased the Jews, he proceeded further to seize Peter also. Now it was during the Days of Unleavened Bread. 4 So when he had arrested him, he put him in prison, and delivered him to four squads of soldiers to keep him, intending to bring him before the people after Passover. 5 Peter was therefore kept in prison, but constant prayer was offered to God for him by the church. 6 And when Herod was about to bring him out, that night Peter was sleeping, bound with two chains between two soldiers; and the guards before the door were keeping the prison. 7 Now behold, an angel of the Lord stood by him, and a light shone in the prison; and he struck Peter on the side and raised him up, saying, "Arise quickly!" And his chains fell off his hands. 8 Then the angel said to him, "Gird yourself and tie on your sandals"; and so he did. And he said to him, "Put on your garment and follow me." 9 So he went out and followed him, and did not know that what was done by the angel was real, but thought he was seeing a vision. 10 When they were past the first and the second guard posts, they came to the iron gate that leads to the city, which opened to them of its own accord; and they went out and went down one street, and immediately the angel departed from him. 11 And when Peter had come to himself, he said, "Now I know for certain that the Lord has sent His angel, and has delivered me from the hand of Herod and from all the expectation of the Jewish people."

The philosophy of Epicureanism is often misunderstood as a hedonistic worldview and I’ll admit to thinking that too when I first came across it. After reading The Fourfold Remedy: Epicurus and the Art of Happiness by John Sellars I was glad to be proven wrong. A wonderful book on an introduction to Epicureanism, The Fourfold Remedy sets the record straight. From the first page, Sellars sets out the differences between lower case epicureanism and capital letter Epicureanism. He starts with an introduction into the life of Epicurus, who created his Garden to live a simpler lifestyle away from the noise of the world. His teachings were powerful enough that they lasted for thousands of years and Sellars notes the religious fervor of the movement: “A cult of Epicurus developed, just as it did around the Buddha in India. Pliny the Elder reports that this continued among Roman admirers of Epicurus, who offered sacrifices on his birthday and carried around small portraits of him. This might make Epicureanism sound more like a religious movement than a philosophy based on dispassionate reason. Yet in the cases of both Epicurus and the Buddha, these were simply acts of admiration for mortal men preaching advice on how to overcome human suffering.” In sharp contrast to the gourmand who has a voracious appetite for sex and food, Epicurus lived a modest lifestyle sustained by bread, water and the occasional pot of cheese. Sellars points out that the philosophy isn’t about the mindless pursuit of pleasure, but rather a nuanced difference between static and active pleasure. “For instance we might distinguish between the active pleasure of eating and the subsequent static pleasure of being full up and no longer hungry. Although we might enjoy the process of eating, Epicurus argued that the reason why we eat is in order to reach the state of not being hungry. Our goal is not the pleasure of eating, but overcoming the pain of hunger.” Put another way: “There’s nothing at all wrong with enjoying the very best of culinary pleasures when they present themselves, so long as we don’t come to expect them at every meal. Fine dining may not be required for us to satisfy our hunger, but it does offer welcome variety. The way to avoid coming to expect this all the time, Epicurus suggested, is to be suitably grateful whenever we are fortunate to be able to enjoy such pleasures.” Another vital part of Epicureanism is the focus on friendship. According to Epicurus, true pleasure came from sharing experiences with friends and being able to support each other in every situation. “The best type of friend, Epicurus commented, doesn’t reduce a relationship to mutual support but neither do they deny the role that such support plays. As he put it, the former reduces friendship to a mere commercial transaction, while the latter destroys any sense of security for the future.” Sellars also references a number of interesting Epicureans, such as the famous poets Lucretius and Horace, who referred to the philosophy in their works. Epicureanism also found admirers in Brutus and Cassius, the murderers of Julius Caesar, and even Caesar’s father in law Lucius Calpurnius Piso. At its core, Epicureanism is about finding a path to the good life by avoiding pain and seeking pleasures that put the mind at ease. Whether you agree with the philosophy or not, The Fourfold Remedy is an excellent book and it opened  mind...

Mind Training (Lojong) | Tibetan Masters › Geshe Chekhawa Yeshe Dorje
The Seven Points of Mind Training
by Geshe Chekawa Yeshe Dorje (1101–1175)
1. The Preliminaries
First, train in the preliminaries.
2. The Main Practice
Consider all things and events as dreamlike.
Examine the nature of unborn awareness.
Let even the antidote be freed in its own place.
Rest in the ālaya, the essence.
Between sessions, be a conjurer of illusions.
Train in the two—giving and taking—alternately.
These two are to be mounted on the breath.
Three objects, three poisons and three sources of virtue.
In all activities, train by applying slogans.
Begin the process of taking with yourself.
3. Transforming Adversity into the Path of Enlightenment
When all the world is filled with evil,
transform adversity into the path of enlightenment.
Drive all blames into one.
Meditate on the great kindness of all.
Meditating on delusory perceptions as the four kāyas.
Is the unsurpassable śūnyatā protection.
The fourfold practice is the best of methods.
Whatever you encounter, apply the practice.
4. Applying the Practice throughout the Whole of Life
The essence of the instruction, briefly stated,
is to apply yourself to the five strengths.
The mahāyāna advice for transference
Involves the same five strengths. Conduct is important.
5. The Measure of Mind Training
All teachings share a single purpose.
Of the two witnesses, rely upon the principal one.
Always maintain only a joyful attitude.
If this can be done even when distracted, you are proficient.
6. The Commitments of Mind Training
Train constantly in three basic principles.
Change your attitude, but remain natural.
Don’t speak of injured limbs.
Don’t ponder others’ flaws.
Train first with the strongest destructive emotions.
Abandon any expectations of results.
Give up poisonous food.
Don’t be so loyal to the cause.
Don’t lash out in retaliation.
Don’t lie in ambush.
Don’t strike a vulnerable point.
Don’t transfer the ox’s burden to the cow.
Don’t be competitive.
Don’t misperform the rites.
Don’t reduce gods to demons.
Don’t seek others’ misery as crutches of your own happiness.
7. The Precepts of Mind Training
Do everything with a single intention.
Counter all adversity with a single remedy.
Two tasks: one at the beginning and one at the end.
Whichever of the two occurs, be patient.
Keep the two, even at your life’s expense.
Train in the three difficulties.
Acquire the three main provisions.
Cultivate the three that must not decline.
Keep the three from which you must not separate.
Apply the training impartially to all.
It is vital that it be deep and all-pervasive.
Meditate constantly on those who’ve been set apart.
Don’t be dependent on external conditions.
This time, practise what’s most important.
Don’t misunderstand.
Don’t be inconsistent.
Train wholeheartedly.
Gain freedom through discernment and analysis.
Don’t be boastful.
Don’t be irritable.
Don’t be temperamental.
Don’t seek acknowledgement.
The essence of the nectar-like instructions for transforming into the path of awakening the five prevalent signs of degeneration was passed down from the one from Golden Isle. When karmic seeds left over from former trainings were aroused in me, I felt great interest, and so, without regard for suffering or disparagement, I sought instructions on subduing ego-clinging. Now, even in death, I shall have no regrets.

Jan. 20 – Creating Sanctuary for Body, Soul, and Spirit – Susan Silverio        Teacher at Spindlewood Waldorf Kindergarten as well as LifeWays Early Childhood and Human Development training in the Northeast, Susan now maintains the kinder cottage in Lincolnville as a guest house and family retreat.
So, what is the fourfold nature of our dwelling places and how can we foster their vitality? What can we learn from those who care for Steiner early childhood settings and the Goetheanum? Through stories and conversation, let’s explore the art of caring for our human environment.                              Recommended reading: The Spiritual Tasks of the Homemaker by Manfred Schmidt-Brabant
January 27 – Biodynamics - Deb Soule and Tom Griffin
In a series of lectures given in Helsinki in 1912, Rudolf Steiner addresses the topic of Spiritual Beings in the Kingdoms of Nature. In the first two lectures of this series, he speaks of the etheric body of the Earth in relation to the elemental beings of earth air water and fire. As BD farmers we find ourselves continuously in conversation with these elementals. Our intention is to present a basic understanding of who these beings are AND, more importantly, how we as BD farmers relate to and work with these spiritual entities in our own unique and individualized way.
Deb is an herbalist, gardener, teacher, and author of Healing Herbs for Women: A Guide to Natural Remedies and How to Move Like a Gardener. Her passion for plants, gardens, and healing and her commitment to sharing herbal knowledge with others is central to her work. She is frequently a guest lecturer at various conferences as well as an instructor for botany and horticulture students, garden clubs, and medical students. Deb the founder of Avena Botanicals in Rockport, ME, the first farm in Maine to be Demeter certified biodynamic.
Tom has B.S. degree in Soil Science from California Polytechnic University at San Luis and an M.S. degree from the University of Maryland at College Park. He has over 25 years of biodynamic farming experience, and this season marks his 17th year at Hope’s Edge Farm. Hope’s Edge is primarily a vegetable CSA with 85 to 100 members and is also home to a small flock of Friesian sheep in Hope, Maine.
February 3 - Eurythmy – Barbara Richardson
Barbara Richardson is a eurythmist living in Freeport, ME. She specializes Eurythmy in the Workplace.  After one of the recent sessions, a participant reflected:
'Arriving in a space of unified working, Where each person can find their relationship to the whole, Requires the inner activity of each to awaken to the whole, And the whole to awaken to each individual. In this working together, individual freedom is not lost, But rather enhanced through joyful working together With clear intention toward a common goal.'
We will do eurythmy to poetry with copper balls. Eurythmy enlivens, engages and supports the life forces preparing us for our work together. After eurythmy we will have a short time of reflection and sharing our experiences in conversation.
February 10 – Water - Jennifer Greene
Jennifer Greene founded the Water Research Institute of Blue Hill. She is a regular presenter at anthroposophical conferences in the US, Canada, and Europe.
‘Water phenomena experiments not only illuminate aspects of water’s intrinsic nature but also school our capacity for observation. This can lead to a new understanding of science.’ We will work with one or two water experiments and attempt to ‘read’ the story that they tell of water’s nature.                           www.waterresearch.org
Suggested reading: Broadening Science through Anthroposophy, Volume 1: The World of the Ethers by Ernst Marti
February 24 – Remedial Therapeutic Education – Sarah Williams Smith
Sarah Williams Smith is a trained Waldorf therapeutic and remedial practitioner. She works directly with adults and children in her private practice. Sarah is also a graduate of LifeWays Early Childhood and Human Development training and is the founder and co-teacher at Meadowsweet Morning Garden, a play-based, nature-centered early childhood program. Sarah holds a bachelor’s degree in biology. She lives in Belfast with her husband and their three boys.
Sarah will lead a discussion on therapeutic techniques used in Waldorf schools and anthroposophical communities worldwide. First developed by Audrey McAllen out of the insights of Rudolf Steiner and other close associates, these Extra Lesson exercises address imbalances to our four-fold body, our three-fold nature, and our senses. Such imbalances often present themselves as learning, behavior and/or movement challenges. The Extra Lesson series of artistic, painting and movement activities ultimately aims to reconnect the soul of the recipient with the truest human archetype, an archetype which has become increasingly obscured in our modern world. We will focus specifically on remediation techniques addressing the etheric body, including hands-on activities and demonstrations.
Suggested additional reading: Foundations of the Extra Lesson by Joep Eikenboom, A Psychology of Body, Soul, and Spirit by Rudolf Steiner
March 3 - Anthroposophic Medicine - Dan Einstein, MD
Dr. Einstein is an integrative family medicine doctor practicing in Freeport and Rockport, Maine.  Dr. Einstein began his journey in medicine in 2000 by volunteering at free clinics in the San Francisco area.  Finding that he loved both conventional and holistic medicine, he trained as an herbalist at the Blue Otter School and then went on to medical school at Case Western in Cleveland, Ohio.  He met his wife there, and both of them came back to New England for residency training at Maine-Dartmouth in Augusta.  He is now board certified in Family Medicine, Integrative Medicine, and Anthroposophic medicine.  He lives with his wife, daughter and several animals in Freeport.                  Website: holistic-einstein.com
In the modern world, we are bombarded with advice on lifestyle changes that can improve our health. Much of this advice conflicts with the increasing demands of work and constant connectivity. In an effort to resolve this tension, Dr. Einstein will review the fourfold human (physical, etheric, astral and I) and lead a discussion of health-giving practices to support each one. Many of these practices will be familiar to participants but understanding their interaction with the fourfold can deepen their significance.  Additionally, this understanding allows the practices to be modified to suit the realities of each person’s life. After the large group discussion, we will break into small groups to discuss how our own health practices might improve from this knowledge.  In preparation for this class, please feel free to write down your own health practices and what they mean to you.
Artistic Exercises as part of each class
Sarah Williams Smith will lead us in the Moral Color Exercises, a series of six paintings based on a 1915 lecture by Rudolf Steiner. These color exercises work as a cleansing, reorientation of the soul, helping us reharmonize ourselves when faced with the modern forces of materialism, overscheduling and disconnection from the spiritual world.
March 10 – Snow Date
Six Sunday afternoon classes, 2:30 - 4:30 PM, January 20 – March 3, no class Feb. 17.

Back in 2019, a black man named Dana Holcomb sued American Airlines after being kicked off a flight. He had an allergic reaction after a white woman with an emotional support dog was seated next to him.
The facts of the case are then disputed. The airline alleges that the woman with the emotional support dog offered to switch seats with another passenger in the first class cabin where they were seated but that Holcomb refused (as he well might have, since an allergic reaction is not necessarily resolved by the dog moving a few seats away in the same cabin). Holcomb's own statement to the press claimed that no other passenger in the first-class cabin was willing to switch seats with the woman, which suggests that he would have been willing to accept that solution if someone had been willing to offer it. The airline's statement to the press alleges that the crew offered to reseat Holcomb in coach and refund the difference between the tickets, but that Holcomb became confrontational. The company claims that only after exhausting the available fixes did the crew remove the uncooperative customer from the flight, offering to rebook him on another or refund his ticket. Holcomb denies that he became confrontational.
Holcomb's lawyer insisted that the principle of selection must have been, consciously or unconsciously, the race of the parties to the dispute, making his client a victim of racial discrimination. The airline maintained that in fact the white women with the emotional support dog is a member of the relevant protected class under federal law here and that they were merely following the law when they allowed her claim to emotional support to trump his claim to travel free from allergens. The headlines in various national media outlets repeated his lawyer's characterization of the event as one in which an a black man was “Kicked Off A Flight 'So A Dog Could Fly First Class'
Until this year, the Air Carrier Access Act of 1986 prohibited discrimination on the basis of disability for air travel. This legislative mandate was then turned into a rule by the Department of Transportation that included protection of emotional disability, including the remedy prescribed by doctors, who designate "emotional support animals" as a category distinct from mere "pet." Large pets have to fly in the cargo hold and their owners must pay a fee. Emotional Support Animals fly for free on their owner's laps. (Small, unobtrusive dogs that fit into a bag small enough to fit comfortably under the seat fly with their owners for a fee .) The Fair Housing Act entitles owners of ESA's to live with their pets even in buildings that prohibit them. But profit-seeking entrepreneurs within the medical and legal industries have colluded with conscious abusers of the system seeking to exploit the law.
Writing in the New Yorker in 2014, Patricia Marx described how she obtained a letter from a clinical social worker attesting to her need for an emotional support snake:
"To prepare for the session, I concocted a harrowing backstory: When I was six, I fell into a pond and almost drowned. There was a snake in the water that I grabbed on to just before I was rescued by my father, and, ever since, I’d found comfort in scaly vertebrates."
She obtains her letter and registers her animal as an emotional support animal on a privately maintained online registry with no relationship to the US government and no legal standing or force and uses the letter and the registry to take her snake to dine at Balthazar and to watch a film at the Angelika.
That we allowed the proliferation of emotional support animals in the first place tells us a lot about the evolving nature of the civil rights industry. A network of activists, judges, and legal academics created the conceptual basis for identifying new categories of injury, all deriving their ultimate power from the landmark legislation required to dismantle legalized segregation in America. A network of trial lawyers and medical entrepreneurs found a profitable niche in colluding with a growing number of Americans regarding themselves through the prism of victimhood that happens to confer legally protected status onto them. In the process, these networks constituted themselves as an interest group.
Over time, they grew powerful enough to cannibalize the very people for whose benefit the system was inaugurated in the first place: thus, Black Man Kicked Off Plane So That Dog Gets to Fly First Class.
On the one hand, this is an easy case. Pushing back on the self-serving melodrama of entitled white people wielding recently manufactured identities as "disabled folks" granted to them by credentialed charlatans is still possible. The airlines summoned up public support, petitioned the federal agencies that regulate them, and persuaded them to initiate a change that any sensible person should regard as self-evident.
On the other hand, the logic of Conlon’s legal argument is impeccable.
Courts and bureaucracies depend on the integrity of clinicians to define illness, who in turn derive their authority from the credentialing bodies that certify what counts as knowledge in our society. What happens if the knowledge-forming institutions themselves become captured by sectarian conceptions of rights and expansive notions of subjective injury whose existence can only be ascertained through self-reporting that it then becomes a dogma that none may dispute and all must affirm?
There are reasons to suspect that the pursuit of exemptions from the rules that civil rights law now confer has moved beyond mere conscious fraud toward actually reshaping the way people think about themselves. The Wall Street Journal reports on “a surge in the number of students who take their exams in low-distraction testing centers, are allowed to get up and walk around during class or bring a comfort animal to school, among other measures.” A poll of students taken at Pomona College in 2014 found that 5 percent of them identified as "disabled". That same poll administered in 2019 found that the figure had grown to 22 percent. A nearly fourfold increase in so brief a surely tracks the explosive growth of a culture of systemic advantage seeking; but it likely also indicates a changing perception of the self, a higher threshold of sensitivity to the vicissitudes of everyday life, and an overall normalization and valorization of the condition of disability. The children of Lake Woebegon were once all above average; they now all require their own IEP.
If a duly certified clinician determines that your childhood trauma gives you a right to do what others are constrained from doing, even if it imposes costs and burdens that others must bear, on what grounds does an airline, a federal agency, or a federal judge declare otherwise? If the trauma is severe enough, wouldn’t it be both an intolerable cruelty and a violation of civil rights law to deny the person what medical science has decreed that they need? Wouldn’t it, in fact, be tantamount to a form of literal violence?
Future installments will move on to the harder cases.

https://www.washingtonpost.com/opinions/2021/02/27/hogan-cant-explain-away-marylands-vaccine-inequities-he-needs-plan-fix-them-instead/
Opinion: Hogan can’t explain away Maryland’s vaccine inequities. He needs a plan to fix them instead.
The confusing rollout of coronavirus vaccines in Maryland is raising serious concerns of transparency and fairness. Despite the state health department describing its top priority as “equitable vaccine allocations based on county population,” Maryland counties vary widely in the proportion of vaccinated residents, from a high of 12.4 percent receiving two shots in Worcester County to a low of 3.3 percent in Prince George’s County.
When the goal is equity but the reality is nearly fourfold variation, there’s a problem. In Maryland, this problem has a racial dimension. Counties with high vaccination rates are predominantly White. Those with lower rates are more likely to have larger populations of color. By our calculation based on the state health department data, White Marylanders are being vaccinated at a rate more than twice than that of Black Marylanders, and only around 4 percent of inoculations have gone to Latinos.
These statistics demand an explanation — and a plan. Unfortunately, Gov. Larry Hogan (R) has responded with a series of perplexing statements that neither shed light on what’s happening nor reflect a commitment to fixing the problem.
First, when asked in January about under-vaccination in areas with large minority populations, Hogan stated, “This is not an issue of not enough supply in those areas. It’s about convincing people to take it.”
Yet the reality on the ground does not support this. From the start of vaccine distribution, demand has far exceeded supply in every corner of Maryland. In Baltimore City, community and church leaders had to set up their own informal hotline to help older residents desperate to locate vaccines. In Prince George’s County, elected officials have been deluged with calls from stressed and frustrated constituents, with more than 100,000 people signing up for vaccination.
Hogan subsequently suggested he wanted to send more vaccine to places that have been able to vaccinate faster. The implication is that certain counties may be sitting on vaccines, rather than using them. But the state itself is controlling where the vaccines are sent within counties and has not released data about specific holdups in vaccinations. Since the state is taking responsibility for distribution, Hogan’s team should make its tracking public, identify delays and fix them.
On Friday, Hogan tried a third time to explain the under-vaccination of certain counties. He asserted that “as of last week, Baltimore City had gotten far more than they really were entitled to.” The governor did not share data to justify this astonishing statement, nor did he address how it might even be possible for Baltimore to receive excess vaccine and yet have a vaccination rate among its residents lower than that of every one of its surrounding counties.
What little information the state has released suggests a fundamental flaw in the state’s strategy. The state divides a county’s vaccine allotment among multiple distribution points, including the local health department, pharmacies and hospitals. But hospitals in Maryland serve the whole state and are not equally distributed. When Baltimore’s 11 acute-care hospitals vaccinate patients who live throughout the region, there is less vaccine available for city residents.
To be clear, the mistake here isn’t giving vaccines to Baltimore hospitals. It’s counting those vaccines against Baltimore City’s allotment. Because the city is predominantly African American, this policy decision has built racial inequity into the state’s plan for vaccine distribution.
Hogan must remedy the situation quickly. An immediate step is transparency, specifically to post data about where the state has sent vaccine and information on the population receiving the vaccines at each site, including their residential Zip code, race and age. The state should also stop counting hospital distributions against counties and begin to report hospital vaccinations separately.
Critically, the state should manage the vaccination rollout to achieve its own stated “top priority” of equity. Maryland is beginning to do just this in Prince George’s County, by reserving a large share of the doses at the Six Flags site for county residents. The state should do the same for mass vaccination sites within Baltimore City.
In addition, Maryland should support innovative programs to reach individuals in great need who cannot navigate the state’s piecemeal scheduling system. This includes scaling up the efforts of the Baltimore City Health Department to send mobile vaccination teams that are doing door-to-door outreach. The state should also direct new allocations directly to federally qualified health centers and primary-care providers in hardest-hit neighborhoods with the least access to vaccine.
Policymakers in Maryland and around the country should take note: There is no justification for pointing to vaccine hesitancy when the real problem is vaccine access. Rapid and equitable vaccination must be the goal when the enemy is an infectious disease that will remain a threat as long as people are left behind.
Opinion by Joshua M. Sharfstein, Leana S. Wen and Peter Beilenson
