“C'mon man, what was that?” Sam huffs under his breath as the noisy sticks on our controllers fill the silence of the apartment.
Keeping my eyes locked on the brightly-lit screen before us as I sit up a bit against the white faux-leather couch, I grit my teeth, scoffing as I’m dealt another fatal blow by the game’s ruthless ‘lobster-shark mutant zombie’ boss. “What the hell even is this game?” I groan as I wait for my character to respawn beside Sam’s, glancing over at the disc’s case before rolling my eyes. “Leave it to Steve to have the lamest gaming library in the history of mankind.”
“I still have no clue what the fuck this console even is,” Sam admits as he smashes the buttons with the palm of his hand, wincing as he’s knocked further and further down in health points. “Someone’s gotta collect the funds to get this man an Xbox or something.”
“Peter was talking to me about getting Willa a Switch for her birthday; I think him and Thor are in on it together,” I tell him as I’m thrown back into the round, adding, “maybe he’ll find some better shit on there to play.”
“Oh god- here comes his ult-” Sam warns as we both brace for the wrath of the 'acid tidal wave’, but despite our best efforts, for the fifth time in a row, we’re killed immediately on impact, a giant frown popping up on the screen accompanying the 'retry’ button.
“Jesus christ,” I toss the controller to the side, burying my face in my hands as my elbows fall onto my knees.
“No way anyone’s killed this thing, level’s unbeatable,” Sam shakes his head, switching the console off using his controller. “Stupid crab-ass-looking, slimy motherfucker-”
“Mo-ther-fuck-er,” a little voice calls from behind us, causing us to both shoot up in surprise, turning our heads to see Willa standing behind the couch, rubbing her eyes sleepily.
“Oh, no, no, no,” Sam shakes his head, reaching out a hand before saying, “now you just forget about anything you just heard, alright? We did not say that. That is not what we said.”
Tilting her head in slight confusion, Willa raises an eyebrow. Biting my lip, I do my best to keep from bursting out in laughter. “Yeah, you didn’t say that,” she agrees as she motions to the both of us, “you did,” she continues, her gaze locking on Sam.
“Woah, woah, take it easy,” he shakes his head, his hands coming up in defense. “That’s not- we did not- I-I didn’t-”
“Bucky,” Willa’s innocent voice asks as she turns to me, wide bambi-eyes blinking cluelessly as she asks, “what’s mo-ther-fuck-er mean?”
Chuckling, I click my tongue, musing, “Well sweetheart, that is a very big word, isn’t it? I think it’d be best for you to ask your daddy tonight when he calls to check in on you; make sure you tell him it was Uncle Sam who said it, okay?”
“Hey, now that’s just not fair,” Sam disputes, turning to me with an unimpressed look, “you were throwing out just as much shit as I was before little missy here showed up and-”
“Shit,” Willa repeats; at the deadpan expression on the little girl’s face, I can’t help but burst into a fit of laughter, earning a surprised look from Willa and a groan from Sam as he shakes his head, pulling out his phone.
“Alright, time to do some damage control. Better now than him finding out about it later. What the hell am I even supposed to say? 'Hey Steve, sorry for corrupting your precious little angel- Bucky and I kept getting our asseskicked in your horrible Godzilla knock-off…” As the man reads his typing aloud, Willa’s face scrunches in confusion once more as she mouths the words 'hell’ and 'asses’ to herself questioningly, only fueling my laughter more.
“Alright, alright, that’s enough,” I cut Sam off, opening my arms up for the child as I hum, “while Uncle Sammy figures that how, how’s about you and I pick something off my phone for dinner?” I suggest. Feet padding over happily, Willa climbs right up in my lap, watching with wide eyes as I open up my delivery app. “Anything you want, doll,” I hum softly into her hair as I let her scroll, glancing over at Sam as I ask carefully, “How’s he taking it?”
Looking up at me with defeat in his eyes, Sam shakes his head, replying, “All I know is: we’re in deep shit now.

Logan square is apparently the "luxury location." it IS super nice, i went there on a pass when it first opened. The locker rooms are way bigger and have steamrooms and saunas. I could very well reading too deep in this but I’m gonna list my thoughts about Zaan and Dragon Priest Mask/Names.
Feel free to shoot over Asks if anything is confusing or you want to know more. This is just my thoughts/canons etc, I’ve read the books and notes but I could have missed more.
According to the Lore each Dragon Priest is granted a magical mask that reflects who they are, just like how their names are changed. You can argue that the MASK is the name that they wear. Their can be speculations on if Children who were born inside the Cult have Dovahzul names but I doubt it as it seems to be more of a title given to you, we also know the Dragon Language is very rarely spoken outside of the Priest rank.
Names
I’ll list same names I think you could maybe use if you decided your Priest/Priestess had a child. Note these are more like nicknames. I went more by less entitled names (like Wolf or Raven) since they sound more like a working job title than a petty name. But you do what you want!
Dun - Grace
Faad - Warmth
Mal - Little
Paaz - Fair
Peyt - Rose
Now that I am typing this Malpaazpeyt is a cute ass name
Thurvokun and Zaan
We all know the unfortunate circumstance that happened to Zaan, the worst fear in all Dragon Priest and their followers; when their Dragon Lord abandons them. Thurvokun and Zaan had a bond so close it was considered a great fortune to serve them, to be seen by them was a blessing. Why he chose her could have any meanings but I won’t be getting into that right now.
Zaan was gifted a mask that looked very similar to Konahrik (Warlord) this made me speculate a little bit, I believe Zaan was once the wearer of that mask and is in some way still the Warlord. “She commanded no great battles, conquered no powerful enemies” but it does not say she didn’t interfere with these battles in some way, with Zaans great bond and her youth and incredible battle skills, she very well may have been chosen to shut down riots and misguided Priests. Her whole armour and mask still resembles Konahrik, she was Thurvokuns greatest champion and Warlord, he left the Dragon War and took her with him, she was his.
As for why he left it’s not really known. Zaan could of shown weakness after the Dragon War, she may have even been slightly angry they were not there to help Alduin. Or maybe Thurvokun DID leave the temple to return to the war and came back only to find everyone dead and gone and slowly rotted away without his connection. I don’t believe Thurvokun left with malicious response, I believe he was between two battles of his own and made the wrong choices. Either way Zaan’s mentality became unstable after he left and she was severely accused by her followers, but she made no protest to stop them. She was beginning to fade away, she could not speak. The loss of the link between her Dragon was spiritually destroying and her followers saw this and so they slaughtered her.
Zaan’s Mask
We know after her followers slaughtered her one of them regretted it, and her name is Ruelde, Ruelde‘s brother repeatedly told her to have faith in Zaan but she could not see any left in their Priestess. As she watched the snow turn red in Zaan’s blood her mind drifted into a power hungry being, she desired the mask. she desired dominance and power over all. Just like a Dragon.
One of my theories is the Masks the Dragons create can speak to their wearer, this is how they become themselves.
As Ruelde came over and gripped Zaan’s mask she realized it wouldn't let go, it REFUSED to leave its master, it WAS Zaan. The mask is what makes the Priest, it was created specifically for them and it will never truly leave its wearer until they turn to dust.

It's fun to see that "At'za dan" corresponds to "it is done" and baade makes us think of "blade" and "faade" of "fall" if only through similitude. Same with "gaman" for "command". It's just consonant shifting. Arkade and Atredish are really recognizable. While koor osh bodatz kinda makes me intuitively think about the words kor oss in norwegian meaning "where" and "us", respectively (but korfor means "why") and bodatz makes me think of "benutzen" in german (to {have} use). Kor osh bodatz feels a bit like it is "why (of) us have use" idk I'm just pulling stuff out of my rear end but welcome in the adhd brain of a guy speaking 5-6 languages IG.

Jezebel Arlin had begun charming grown-ups, friends and teachers ever since she learnt how to speak. She had wavy, blonde hair that sometimes came in curls, smooth, fair skin, a cute button nose, a pair of bright baby blue eyes and a mouth with full lips that always presented a wide, cheerful smile. Almost everyone adored her, even in kindergarten, her teachers would pinch her cheek and mutter baby language that was supposed to cheer a kid up, although Jez despised it. Back when she was still a childish kid, she often wondered why people did that to her, and started to frown upon their actions. Her lack of experience had yet to teach her about the philosophy of appearance, beauty and impressions. However, Jez got her vanity when she started elementary school, just when her features started to bloom. Her hair got straighter, longer, and her she grew twice her height, her aqua eyes shinier than before. High school � she was the self-proclaimed fashion trendsetter of St. Victoria�s High at Toronto, Canada. She had a slim waist and body curves at just the right place, a figure many girls in her school expressed envy and jealousy for. Jez became a vain, arrogant brat, and flaunted her perfect figure by donning on outrageous outfits to school, parties, or anywhere at all. Jez loved her body, but if only people knew that she starved herself practically every day to achieve her ideal figure. After high school, Jez and her family moved all the way from Canada to the US, where her mother had taken up a job in Forks, a small town at the Olympic Peninsula. Jez rented her own apartment at Seattle, where she is currently studying in the university there. She continued her wild partying ways, a habit she picked up towards the last years of high school. She turned into the model-like person she had dreamed about for years, and her arrogance grew even more. Working as a part-time waitress at a bar in Seattle called The Hole, Jez often violated the workers� dress code, always coming to work without her uniform, a deliberate prank to show her rebellious ways. However, when she wasn�t going out to parties and shopping trips, Jez preferred to wear an oversized tee and shorts and tie her slightly wavy hair into a bun, and simply relax within the confines of her home.
Personality
Jez didn�t really have a happy and perfect family background. All was well back when she was still a crawling baby, perhaps even the perfect family a two-year-old kid could ever wish for. However, ever since her father lost his job as a bank accountant, he started to pick up serious negative habits such as drinking and such, which was a few years after Jez started high school. Since then, she made herself a makeshift faade of a tough cookie, a snob, to hide the aching sadness within herself. Never once did she try to interfere with her parents� arguments, even though she had always wished to do so. She never told anyone about her family affairs, and with that stubborn attitude of hers, nobody in her school ever found out. When her father started to leave home and became a jobless wanderer around the streets or Toronto, Jez�s temper took on an uglier side. She was easily roused and angered, and became a vindictive person. She couldn�t bear the feeling of being let down and called a failure, and would do everything for the sake of her reputation. Later on, she realized the reason behind her temperamental attitude was because she had always blamed herself for her father�s outcome, and even though she still loved her father, she refused to follow his footsteps into a failure. Once, her boiling temper even got her into a pot of boiling hot soup when disagreed with her mother�s lawyer. It was a terrible memory Jez could never ever forget, but one she didn�t regret. Stubbornness is also one of Jez�s traits. Once she gets hold or set eyes upon a desired item, she will try everything she can to possess it. Besides, she isn�t one to take instructions and orders lightly. She hates being treated like a servant, and is hard-headed when it comes to making her own decisions. Jez wasn�t what you would call a �people� type of person. She doesn�t communicate well with people, especially during family gatherings or school reunions. Therefore, she didn�t have many �real� friends (excluding desperate hang-ons during her Queen Bee days) and even then, she didn�t treat them like a true friend would have. In spite of that, Jez was actually a pretty lonely person, that she would admit. She wasn�t fond of meeting new people, but always had a lifelong wish to meet the perfect person she could call her best friend. Underneath the tough faade she frequently wore, Jez considered herself a quirky, serene spirit that keeps a diary underneath her bed to express her disappointments and regrets, as well as her happiness and joy. So far, no one had yet to penetrate the barrier she created for herself to separate from the rest of the world, not even her parents, and Jez was glad to be the only one to know her real self.
History:
Naomi and Sean Arlin led a peaceful life on Seafield Street, a secluded neighbourhood in Toronto of Canada. Naomi Huberman Arlin was a surgeon, whereas her husband worked as a bank accountant. They always had sufficient money in their bank accounts, a house they considered quite large for the size of their family, two working cars still in perfect condition, and stable jobs. It was the type of family everyone dreamed and envied, and the Arlins were constantly surrounded by a crowd of supporting friends. At a bonfire night, Sean promised Naomi, �We will never be apart.� A year later, Naomi gave birth to an adorable baby girl, the couple�s wish fulfilled. The name Jezebel had been stuck in Naomi�s head for a while now, and Rose was named after Sean�s grandmother. Jezebel Rose Arlin was a cute little angel, and the new parents couldn�t be prouder when relatives and friends swarmed into the ward to catch a glimpse of the cherubic baby. Sean and Naomi practically spoiled their kid, giving her the best of everything she wanted. Jezebel learned to speak and walk at a fairly young age compared to her parent�s friends� children. Two years later, Jezebel�s brother, Mason, was born. Being a mere two years younger than Jezebel, the two siblings were sometimes mistaken as twins. They had the same deep brown hair, high nose, and bright blue eyes. The two of them grew up together as best friends, and Jezebel always fulfilled her responsibility as a big sister to Mason. They usually looked out for each other in school, especially when Mason was still a new kid and was often teased by the bigger, older kids in elementary school. From an outsider�s point of view, people would�ve thought they were the perfect role model for siblings, always living together harmoniously as brother and sister, and their parents were equally happy for the family. The first few years of Jezebel�s life, or which she rather liked to call, �Chapter One�, was the perfect life she had ever dreamed of. Back when she was still stuck in the first chapter, she thought they would always stay like this, leading the perfect life everyone always dreamed of. She thought everything would be okay, more than okay, sometimes, and loved how her life had turned out. However, life doesn�t always give you rainbows, sunshine and little balloons, Jezebel grew to understand. The Perfect Life she had been living decided to take an evil twist, and Jezebel�s life was never the same again. High school started, and Jezebel�s good looks and overconfident attitude earned her a reputation located at the top of the social ladder. Arrogance and pride squirmed their way into Jezebel�s mind and she became the haughty Queen Bee of her school back in Toronto. Since then, she hated people calling her by her first name, and demanded they call her by �Jez�. Mason noticed the change in her attitude, and their previous friendship waned until they barely glanced at each other anymore. Mason was sick of Jez�s snobbish ways, and Jez thought Mason was such a coward. The typical teenage sibling rivalry played before their parent�s eyes, but it wasn�t something adults had control of. Nobody knew what Mason was hiding when he started high school. One uneventful day, he was caught with two more kids doing drugs at a secluded lane behind the school during school hours. Jez�s parents were appalled by the sudden news, demanding that Mason had always been a good kid and everything, but in the end, Mason was sent to a Juvenile Centre when they figured out his behavior had lasted for a few months now. Jez felt ashamed of her brother�s behavior and never talked about him again, forgetting she had a brother altogether. However, five months later, it was reported that Mason Arlin had escaped from the centre, and was still currently searched by a search team. Three years later, an economic crisis caused Sean Arlin to lose his job as a bank accountant. The pressure of losing his job and son were too heavy for him to bear, and he was tempted into wild habits. He always left at the crack of dawn and returned way past midnight. The loud rows and quarrels weighed heavy on Jez�s shoulders, and once, she was prompted to run away just like Mason did. Despite everything she did to reunite her family again, her father barely took notice of her and continued being a nameless, drunk wanderer, failed to be stopped even by her mother. In the end, Naomi Arlin had no choice but to move away from Toronto to get away from her problematic husband. Right on cue, Naomi was offered a job at a hospital in Forks, a tiny town in the Olympic Peninsula. By then, Jez had started to abandon her studies and preferred to �hang out� with her friends at clubs and old buildings till the wee hours of the morning. Naomi�s other motive of her move to Forks was to hope that Jez would be able to start over and prevent her from turning into someone like Jez�s father. A few years after Jez graduated, mother and daughter migrated to the United States of America, where Naomi rented a terrace at a neighbourhood in Forks. However, Jez refused to be anywhere near her old life and rented an apartment in Seattle instead, where she enrolled in Seattle University for the sake of her mother. She took up a part-time job as a waitress at a bar called The Hole to keep the cash in her wallet stable. It wasn�t the life Jez had wanted, but it would have to do for the time being, she reckoned.

9 Weird and Wonderful Architectural "Ducks"

They exist for a reason.
Coined by Robert Venturiand Denise Scott Brownin Learning from Las Vegas, Ducks are buildings that project their meaning in a literal way [1]. No architectural metaphors here - they are exactly what they look like. Many emerged alongside interstate highways, a lone doughnut or dinosaur punctuating the road trip across America. Places like Las Vegasand Macauhave built their identity in the kitsch and literal language of architecture with the duck a strong contributor. Though they get relegated to one of the weird forays of the postmodern era, ducks still make current-day appearances (like the Chicago Apple Store's recent Macbook roof). Are they fun, kitschy, or just plain ugly? Love them or hate them, ducks have a light-hearted presence in our architectural history. Below are 9 weird and wonderful examples of buildings that make no apologies for being exactly what they are:
1. Big Duck / Long Island New York, USA

The one that started it all, Robert Venturi coined the architectural term duck from this precedent in his 1972 book Learning from Las Vegas. Built in 1931 by duck farmer Martin Mauer, the structure was designed to sell (you guessed it) ducks and eggs, with its unusual shape and prime location attracting many customers. Initially built along a busy street in Riverhead NY, the duck enjoyed a nomadic life moving several times before settling 6km down to the small town of Flanders, Modelled after a Pekin duck, the roly-poly proportioned building was built almost entirely from wire mesh and concrete to make room for a gift store. Apart from being immortalized through Venturi's writing, the Big Duck made architectural history by landing a spot on the USA's National Register of Historic Places in 2008.
2. Mr. Toilet House / Suwon, South Korea

The two-story, 420 square meter house was originally built in 2007 for Sim Jae-Duck (fittingly named), former congressman and mayor of the Suwon province of South Korea. Nicknamed Mr. Toilet for his career-defining pursuit, Sim was devoted to improving the state of restrooms in the country. The toilet is also a central living place that possesses culture," he said at an assembly. The house is a literal monument to Sim's eccentric quest of beautifying the restroom with stairways following the movement of pipes and a painted steel exterior alluding to the white ceramics used in toilets. After Sim's death in 2009, the private home was transformed into a toilet museum, with exhibitions, collections, and activities dedicated to funding public bathrooms in developing countries. Looking for the restroom? The building has four one even featuring floor-to-ceiling glass that turns opaque at the push of a button.
3. Haines Shoe House / Hallam Pennsylvania, USA

Shoe salesman Mahlon Haines reportedly approached the building's architect with a boot and said: build me a house like this. The Shoe House was born; built by as a form of advertising for Haines' business in 1948. The 25 foot-tall building is 5 stories high, with upper floors previously rented out to couples and now hosts building tours. The house is planned with its living room at the toe, the kitchen at the heel and living quarters running up the ankle of the boot. An ice-cream shop is also located in the shoe's instep.
4. Dog Bark Park Inn / Idaho, USA

The two-bedroom B&B was designed and built by artists Dennis Sullivan & Frances Conklin, opening in 2003 as the world's largest beagle-shaped building. With a canine-themed interior, the building is a quirky addition to America's roadside architecture popular in the early days of automobile vacation trips that formed a deep-rooted part of America's culture (Many of these overlaps with the postmodern ducks that Venturi mentions). The beagle building gets quirkier, also hosting a museum for chainsaw artists (think painted timber sculptures of different dogs).
5. The Museum of Tea Culture / Meitan, China

Officially the erected as the world's largest teapot in 2010, the museum rises to a height of 78 meters with a diameter that stretches as wide as 24 meters. A building entirely devoted to teapots, the 5,000-plus square foot museum is one of the most duck-like buildings both in design and program, taking literally to the next level. Meitan possesses a rich history surrounding tea - known as the hometown of Chinese green tea. With China deciding to steer away from weird buildingswe're glad to see this quirky addition make it in time.
6. Chiat/Day Building byFrank Gehry / Venice, California, USA

Not deconstructivist, but still curvy this is definitely one of the architect's weirder works. Frank Gehrydesigned these binoculars as an entrance to his more Gehry-like Chiat/Day commercial building (behind) in 2001. The postmodern-style building is known for its unusual mishmash of different architectural styles and materials. Ten years later, Googleannounced they would be leasing the building as part of their Los Angeles expansion the binoculars now serving as an entryway to their Venice offices.
7. Kindergarten Wolfartsweier / Karlsluhe, Germany

Designed and built in 2011 by artist Tomi Ungerer and architect Ayla Suzan Yndel, this adorable kindergarten goes beyond metaphor in its details. Clad in concrete and metal, the two-floor building allows natural light to flood inside through its round window eyes. Though the idea of entering through the mouth of a giant cat and having class in its belly sounds creepy - an endearing faade, green fur roof and tail-shaped slide ensure the building stays true to its playful spirit.
8. Cabazon Dinosaurs / California, USA

Started in the 1960s, the giant dinosaurs were intended as roadside attractions by the sculptor Claude K. Bell (see 4) to attract customers to his Wheel Inn Restaurant (now closed). The first - named Dinny was built over a period of 11 years by 1975, with Mr-Rex added later in 1981. Bell created Dinny out of salvaged material from the neighboring highway: with a steel framework and an expanded metal grid forming its shape, before being covered in coats of shotcerete spray concrete. A wooly mammoth was also intended to be built but was never realized. After Bell's death in 1988, the property was sold and is now used to host a creationist museum and gift shop.
9. Longaberger Building Headquarters / Newark, USA

Along with the Big Duck, the Longaberger basket building is a fixture in novelty architecture. Built as the literal embodiment of the company's best-selling product, the 180,000 square foot building opened in 1997. The company's founder Dave Longaberger had originally wanted all companies to follow a basket-like form, but only the headquarters was completed prior to his death. Nowadays has been tragically left unoccupied, with Longaberger ceasing to pay property taxes for the property in 2014. Sadly the famed basket has been left empty and languishing on the property market, with no potential buyers. A long and complicated foreclosure process is underway but so far the building is still left standing as a relic to those who still wish to pay a visit.

How to team up with your provider to enhance your care

Spending a day at the pool or riding her BMX bike shouldn’t have been cause for concern after Victoria Bryson was diagnosed with type 1 diabetes in 2013. But the 8-year-old was repeatedly missing the warning signs of hypoglycemia in the hours following physical activity, a condition known as hypoglycemia unawareness.
When Victoria’s endocrinologist, Susan Phillips, MD, recommended she start using a continuous glucose monitor (CGM) in addition to a pump, Victoria’s mother didn’t need to be convinced. The benefits of the device, which records and displays glucose readings in real time, were obvious. “To really fine-tune my daughter’s diabetes management, it made sense to have a CGM as an additional tool to monitor the lows and the highs,” says Polina Bryson, who lives with her husband and two children in San Diego. “We could improve control and quality of life.”
But before Victoria could get the device, Bryson needed to clear an enormous hurdle: cost. The particular CGM recommended by Phillips was a few months shy of being approved for pediatric use by the Food and Drug Administration. That meant Phillips would be prescribing the device for off-label use, possibly hindering insurance coverage. And the Brysons couldn’t afford to buy a CGM out of pocket.
The obstacles seemed insurmountable, Bryson says, until she teamed up with Phillips, who wrote what’s known as a “letter of medical necessity” to the insurance company. Phillips detailed how Victoria Bryson could benefit from using a CGM, citing research studies and including Victoria’s blood glucose logs, which showed that she was having lows and that they were pervasive.
“You have to make a case for using whatever device, documenting the safety and efficacy of it, and explaining in detail the risk that the patient is suffering due to the lack of intervention,” says Phillips, a pediatric endocrinologist at Rady Children’s Hospital–San Diego and an assistant professor of pediatrics at the University of California–San Diego School of Medicine.
The strategy paid off: Not only was the CGM covered by insurance, saving the Brysons thousands of dollars, but it happened in a timely fashion. “Our endocrinologist knew what to do,” says Bryson. “She knew what language to use, what data to include in the request to document medical necessity.”
Joining forces with your primary care provider or specialist can lead to improved diabetes management. But some people, particularly those newly diagnosed, may not realize the power of partnering with a provider to boost their quality of care.
“Your physician is there to collaborate and be your partner because you’re the one who—day in and day out—is making the decisions and figuring out what’s working and what’s not,” says Martha Funnell, MS, RN, CDE, FAADE, associate research scientist in the Department of Learning Health Sciences at the University of Michigan Medical School. “Having a provider who’s open to that type of relationship is absolutely critical because this is a long-term chronic disease.”
Forming that relationship begins with listening, says Phillips. “If you do that enough, patients believe you and trust that you’re going to help them the next time something comes up,” she says.

Situation: Your doctor recommends a pump to help with better blood glucose management. You’re familiar with the basics, but you’re not entirely comfortable using the device.
Strategy: Your doctor might not have the time or the detailed knowledge needed to help you learn about all aspects of self-care. That’s why it’s important to work with someone who can refer you to another provider without delay. “Diabetes is a team sport,” says Funnell. “There are lots of other people you need to be working with: the educator, the dietitian, the community pharmacist. Part of making your provider your ally is about saying, ‘I understand you’ve only got 10 minutes, but I want to learn about this. Who else can I talk to?’ ”
Situation: Your doctor suggests a CGM to help you manage your diabetes. But you’re on Medicare, which generally doesn’t cover the device. (Medicare recently announced it would cover one brand of FDA-approved CGM for making treatment decisions; however, it’s not yet clear when coverage will begin.)
Strategy: “Continuous glucose monitoring has evidence-based benefits for people with diabetes, and yet access to these technologies is limited for many people,” says David Kerr, MD, FRCPE, director of research and innovation at the William Sansum Diabetes Center in Santa Barbara, California. His advice: Don’t take “no” for an answer. If your insurance plan doesn’t cover a CGM, ask your provider to write a letter of medical necessity to your insurance company explaining why you need the device. If you’re denied, ask your provider to appeal the decision with what’s known as a “peer-to-peer review”—typically a phone conversation between your physician and a physician at your insurance company.
If that still doesn’t do the trick, craft a letter of your own. “I can make the case, but it can be much more powerful coming from the patient,” says Veronica Brady, PhD, MSN, FNP-BC, BC-ADM, CDE, a nurse practitioner at the University of Nevada–Reno. Ask your provider for a copy of the original letter sent on your behalf so you can address yours to the same person and include any peer-reviewed studies it mentions. Add a copy of your blood glucose logs to prove your case. Then personalize your letter with details from your own experience.
Situation: Your doctor recently prescribed insulin to help you manage your type 2 diabetes, but you skip doses on more occasions than you care to admit. You’re afraid you’ll gain weight, or perhaps it’s the injections that scare you, so you’re less than honest with your doctor.
Strategy: Fudging about any aspect of your treatment doesn’t do you or your provider any good. If you’re not able to stay on track, it’s important to be honest so you can reassess your goals.
“Patients worry you’ll be upset that they didn’t take their medicine or didn’t stick with whatever plan you set as a goal,” says Neil Skolnik, MD, professor of family and consumer medicine at Temple University School of Medicine and member of the American Diabetes Association’s primary care advisory group. “Whenever you put two people together—and this is true whether they’re longtime friends, or doctors and patients—there’s value in saying, ‘What are we trying to accomplish together?’ Once you clarify the goals, it’s easier to move forward.” Your doctor isn’t there to judge you, but to help you find solutions.
Another thing to remember: Honesty is a two-way street. For your provider to be your ally, he or she will need to deliver the cold, hard truth. “You want a provider who will hold up the mirror and say, ‘This is what’s going on. I’m going to give you the information. What choice are you going to make?’ ” says Morrison.
Take Harold Young. He weighed 465 pounds when he was diagnosed with type 2 diabetes at age 27. But it wasn’t until he began seeing a new primary care doctor five years later that he fully realized he needed to do something about his weight. “He didn’t sugarcoat anything,” says Young, manager of team development at the American Diabetes Association. “At that first appointment, he said, ‘You’re going to die earlier than you need to if you don’t get a handle on your weight.’ ”
Young spent the next few years trying to shed pounds and improve his blood glucose levels through diet and exercise, but with limited success. Finally, his doctor recommended bariatric surgery. By then, doctor and patient had developed enough trust for Young to confide his misgivings. “My fear was that I’d go through this major surgery—then what?” says Young, now 41. He was also worried about pain. His doctor referred him to a surgeon who alleviated his fears with straight talk. “He said, ‘Initially, I’ll give you pain meds, but then you’ll have to learn to deal with it,’ ” recalls Young.
Now, four years later, Young has lost more than 200 pounds and he’s met his A1C goals. The lesson? “You are the steward of your own health, but you have to trust your doctor,” he says.
Situation: You’re curious about a new treatment you’ve heard of, but you leave your doctor’s office without getting around to asking about it.
Strategy: “Go in with an agenda,” says Tyree Morrison, CRNP, CDE, a nurse practitioner with Frederick Primary Care Associates in Frederick, Maryland. That’s how Susette Langston approaches her doctor visits. Diagnosed with type 1 diabetes 14 years ago, she arrives for appointments armed with a small notebook detailing her blood glucose readings, blood pressure and other test results, as well as a file of things she wants to discuss with her doctor, such as magazine articles related to some aspect of diabetes she’s unclear about.
“I don’t want to forget anything that’s important to me,” says Langston, who lives in Rochester, New York. “I jot down questions as they occur to me and keep them with my blood glucose readings so I will have them handy for my doctor visit.” She tries to save time early in a visit so she can spend it asking questions later. “While you are waiting for your doctor to come into the exam room, take off your shoes and socks so you’re ready to have your feet examined, as they should be at each visit,” she says. Situation: You’re having trouble paying for your insulin. Or maybe you’re curious about a new drug you’ve seen advertised on TV but you’re worried about the cost.
Strategy: Let your doctor know you’re having trouble financially. He or she may be able to revise your prescriptions by suggesting an equally effective but less expensive medication, or recommend coupons and assistance programs to help you cover the expense. Also ask your pharmacist for information that you can share with your doctor, whether that’s about cost or medication facts.

Prep for Your Visit
Bring a detailed blood glucose log; a list of your medications—not just the names, but also dosages and time of day you take each; and any other health forms or documents. Make a note of when you last had your eyes, feet, and blood pressure screened. If you cover the basics and your urgent needs quickly, you’ll have more time for an open conversation.
Ask Questions
“Between visits, write down the things that are of concern to you,” says Veronica Brady, PhD, MSN, FNP-BC, BC-ADM, CDE, a nurse practitioner at the University of Nevada–Reno. Put a star next to the questions you feel are the most pressing, such as: Should I be concerned about the tingling or numbness in my feet?

Review everything that was covered in your exam, as well as next steps. “If your doctor is referring you to another [provider], ask for a copy of any correspondence so you’ll have a record of what’s being said about you,” suggests David Kerr, MD, FRCPE, director of research and innovation at the William Sansum Diabetes Center in Santa Barbara, California. “It will move the clinician to use language that’s understandable.”
