Therapeutic Efficacy and Considerations:

Leg Edema Due to Chronic Venous Insufficiency: 😊 Horse chestnut extract has been examined in multiple moderate-to-large clinical trials. The majority of these found benefit for reduction of symptoms and leg volume. A meta-analysis concluded that horse chestnut extract was safe and efficacious. The current evidence supports a recommendation as an alternative treatment for patients with leg edema due to CVI stage II or lower who cannot tolerate diuretic or compression stocking therapy or in patients insisting on use of a natural product. However, because there is minimal data on long-term safety and efficacy exists, close monitoring is essential. Dose: 100-150 mg/day aescin component divided twice a day. Usual dose is 100-150 mg/day aescin component divided in two daily doses. Traditional uses include treatment of fever, phlebitis, prostate enlargement, inflammation, and diarrhea.

Venous Leg Ulcers: 🙁 Only one trial has specifically examined use for healing of venous leg ulcers and found no benefit compared to placebo. Use for this indication is not recommended. Aescin component divided twice a day. Usual dose is 100-150 mg/day aescin component divided in two daily doses. Traditional uses include treatment of fever, phlebitis, prostate enlargement, inflammation, and diarrhea.

Chemistry/Pharmacology: Horse chestnut contains the triterpene saponin aescin (escin) and the toxic glycoside aesculin (esculin). Aesculin is a hydroxycoumarin which may increase bleeding time due to antithrombin activity. Aescin decreases venous capillary permeability by stabilizing cholesterol-containing membranes of lysosomes and limiting release of enzymes that normally break down mucopolysaccharides in capillary wall cell membranes. Aescin also has anti-inflammatory action.

Drug Interactions: May increase the risk of bleeding when used with anticoagulants, ASA, and other salicylates. Monitor blood glucose levels closely in patients on hypoglycemics due to possible additive hypoglycemic effects.

Contraindications/Precautions: Rare severe renal, hepatic, and anaphylactic reactions can occur with parenteral products. Unprocessed horse chestnut seeds are poisonous and cause severe GI and neurotoxic reactions. Horse chestnuts should not be used in pregnancy and lactation due to hydroxycoumarin content, nor used in children due to lack of information. Use cautiously in patients with bleeding disorders.

Adverse Effects: May color urine red. Pruritus, nausea, and gastric complaints occur. Hepatotoxicity, nephropathy, hypoglycemia, hypersensitivity (especially in those with latex allergy), rash, urticaria, spasms, bruising, severe bleeding, and shock have been reported.

Horse Chestnut – Uses and Benefits

Although the leaves, bark, and flowers have been used in traditional herbal medicine, it is the big, shiny brown nut of the horse chestnut tree which is of greatest medicinal interest.

From them, an extract is obtained which is used to strengthen the walls of the blood vessels so that blood flow from the veins in the legs back to the heart improves.

The dried conkers contain 3-6% of a mixture of triterpene saponins, known by the common name aescin (escin).

It is aescin which is considered the main active ingredient responsible for the strengthening effect on the veins and capillaries.

Standardized extracts made from the conkers usually have much higher concentrations of aescin (usually 16-20%) than the conkers do in their natural state.
