I had a pt in clinic the other day who had BPPV. Her L ear was affected. I remember one of the attendings telling me once that the Epley maneuver (which fixes BPPV) is like the Dix-Hallpike maneuver (which diagnoses BPPV). I told this other attending that and he said I was wrong. But I'm not. At least the beginning of the Epley maneuver is the Dix-Hallpike maneuver. Anyway, Dix-Hallpike maneuver is when you turn your head to one side and quickly lay back. Nystagmus will be seen when the pt does this. The Epley maneuve starts the same way as the Dix-Hallpike maneuver: you turn your head to the affected side and then quickly lay back--then you stay in that position until the vertigo stops. After that, you turn your head to the unaffected side and hold this position for 1 minute or until the vertigo stops. Then you completely roll onto the unaffected side so you're on your shoulder of the unaffected side. When you do this, tuck your chin down and turn your head to the unaffected side. Then finally, you sit up with your head still pointed down and chin is tucked.

I think I'm gonna call out of work today bc I'm still slightly woozy which means I'm going to get dizzy later today
Normally I do a PT maneuver called epley positioning which helps clear up my dizzy spells but this time around it has only made it worse, dramamine does nothing to dispel it or make it better, I'm well fed and watered, my vertigo is just making my life miserable these past few days

Feeling Dizzy? Not anymore.
Benign paroxysmal positional vertigo. I know, it is an extremely difficult condition to say fast. Although, it is an extremely common condition that I have seen patients present with to the clinic time and time again. It is most referred to as BPPV, essentially it is a condition where the small crystals of calcium inside your inner ear get loose. Which leads to intermittent vertigo spells especially when turning your head or when tilting your head up and down. If you experience vertigo only when turning your head or tilting your head up or down, this is a pretty good indication that you might possibly be suffering with BPPV. Thankfully this condition is extremely easy to resolve, all you must do is complete an Epley maneuver. To perform an Epley maneuver you sit on a bed, allow your head to hang slightly over the end of the bed, turn your head to the right and lie back quickly. Wait one minute, if you feel dizzy, then the right ear is your affected ear. If no dizziness occurs, sit up. Wait one minute, turn your head to the left and lie back quickly. Wait one minute, if you feel dizzy then your left ear is your affected ear. Complete this maneuver multiple times and you should feel relief within minutes. I would highly suggest that you look up a YouTube video when attempting this maneuver as it can simplify it much more. So next time you have vertigo spells you might just be able to diagnose yourself and save yourself a visit by having to see a doctor. Hope this helps!

I thought I was doing okay this morning, in spite of both being up a lot overnight and my neck compressing in very much the wrong way overnight. I just tried to do my super easy yoga video, tho, and I couldn't finish it. That video is NOT something that should be causing vertigo, and it is (I'm guessing because of my neck being pinched/compressed/whatever).
My original plan was to do my actual neck exercises after the yoga vid, but those ONE HUNDRED PERCENT set off my vertigo both from the neck movements AND because they almost simulate the epley maneuver (side note: holy shit I spelled maneuver correctly?). That's actually a really good thing, cause it should help with the inner ear parts of my vertigo. But it's EXTREMELY unpleasant in the short term--I'm either too dizzy to drive, or I'm SO DIZZY that I have to take meds and then can't drive even when the meds take the vertigo away.
*sigh* I should do the neck stuff anyway, now that I've been sitting for a few minutes and temporarily feel a bit more stable. I checked with Brian a little while ago and he said he could drive if I need him to, so I'm going to take advantage of that to get this done.

The physical examination includes orthostatic blood pressure measurement, a full cardiac and neurologic examination, assessment for nystagmus, and the Dix-Hallpike maneuver. Laboratory testing and imaging are not required and are usually not helpful. Benign paroxysmal positional vertigo can be treated with a canalith repositioning procedure (e.g., Epley maneuver). Treatment of Meniere disease includes salt restriction and diuretics. Symptoms of vestibular neuritis are relieved with vestibular suppressant medications and vestibular rehabilitation.
