Summary: Sequel to Iâll Take Her Place. Slav is showing off a piece of experimental equipment, when it malfunctions and blasts Katie and Keithir to another universe. At the same time, it drags Pidge and Keith over into theirs, effectively swapping places. With their fate resting in the hands of Slav, will they be able to get back home? Or are they stuck to live the rest of their lives in the wrong universe?

Chapter 1
After eight years of marriage, Katie didn't need their bond as paladins to tell when Keithir was frustrated by something. She tried to hide her amusement as he sighed, glaring at the back of Slav's head as he rambled on about something he called a âtrans-reality extrapolaterâ. (She honestly wasn't sure what Slav was talking about anymore. Sometimes it was best to just smile and nod.)
Katie gently nudged Keithir's arm to get his attention and then mouthed the word âsorryâ.
He wrapped an arm around her shoulders, then leaned down to kiss the top of her head. Some of his frustration ebbed away, but not enough to keep it from his voice as he addressed Slav. âIs there a point to any of this?â
The tiny, eccentric genius turned and gave Keithir a withering stare. âI was just about to explain it and then you interrupted!â
âSorry, Slav,â Katie apologized placatingly. âWe just have some plans that we're eager to get to, is all. This is our first vacation alone together in a few years.â
Slav begrudgingly accepted that as a worthy answer. âThen I'll be brief. This device will allow us a glimpse into other realities! I've gotten it to work a number of times for me so far, though never the same one twice. You see, its all highly dependent on numerous variables like weather patterns, star placement, what food you've eaten, time of day, humidity, how many times you flip the blue switch, the color of your socks...â
They let Slav ramble on for a moment and then Katie cleared her throat.

Yes. Katie kept that thought to herself. âOf course not, Slav. If you say it works, then it works. What do we need to do?â
Slav brightened and directed the pair to each grip one of the handles. After a moment of consideration, he flipped a few switches and twisted one of the knobs. The machine hummed to life, vibrating strangely, but Slav didn't look alarmed or worried, so Katie figured that was what it was meant to do.
âNow what?â Keithir asked warily.
Slav was about to respond when the machine emitted a popping noise and a visible spark shot out of the top. He scuttled backwards with a quiet, âoh no.â
That was the last thing Katie heard before everything went dark.
Pidge groaned as she started to wake up. Strange, considering she couldn't remember falling asleep. What was the last thing she remembered?
Something about Lotor giving up information? No, that wasn't right. Well, it wasn't wrong, but it didn't have anything to do with what she had been doing. That was Allura and Shiro; they were drilling Lotor for inside information. She was sure that Hunk was stress baking after everything that happened on Naxzela and Lance was... off doing Lance things? (Of everyone, Lance proved to be the hardest for her to predict. She assumed wherever he was and whatever he was doing, he found it relaxing.)
Pidge winced as she sat up, fighting a rush of disorientation. She reflexively reached up to adjust her glasses, but her fingers only met skin. Frowning, she looked around for them, only to find someone unexpected laying on the floor near her, slowly coming to.
âKeith!â She crawled over to him and helped him sit up, her brow furrowed in concern. âAre you alright? When did you get here?â
Keith blinked as he looked around and took in his surroundings. âIs this the Castle of Lions? How did I get here? I was just at the Blade headquarters! Kolivan called me in to--â He abruptly snapped his mouth shut and looked away from her. âPidge, what's going on?â
âI wish I knew,â Pidge responded honestly. She forced herself to her feet, wobbling a bit as the disorientation returned. It faded quickly and she held out a hand to help Keith up. âMaybe there's something wrong with the Castle again. I bet Coran would know.â
With Pidge's help, Keith unsteadily got to his feet as well. One thing was for sure, their equilibrium had been messed up by something â likely whatever knocked them out and teleported Keith all the way from Blade headquarters. They needed to find the cause and make sure it wouldn't happen again.
âDo you think he's on the bridge?â Keith asked.
Pidge nodded. âThat's the most likely place.â
They set off from the room, but didn't get far before a small figure scampered down the hall toward them. Slav froze at the sight of them and then wailed, reaching up to grip at his tufts of hair with two of his eight hands. âOh no, it's even worse than I thought!â
Pidge gaped at him. âSlav?!â
âWhat the hell is going on?â Keith demanded.
Slav was too busy having a melt down to be of any use to them. Keith and Pidge likely would have ignored him and moved on to find someone who was actually helpful, except at that exact moment, Allura and Shiro rounded the corner, both looking frazzled.
And with them they brought a whole new round of questions.
Keith stared at Shiro, opening and closing his mouth for a moment, before blurting out: âWhat happened to your hair?â
Pidge was a little more focused on the very Altean toddler in Allura's arms. She lifted a hand to her head, already feeling the start of a migraine as her brain worked overtime to try and comprehend everything that was happening.
Allura and Shiro exchanged alarmed glances as they took in the situation. They seemed to come to an agreement through eye contact alone and then Allura was urging Slav to calm down and breathe, while Shiro walked forward to handle Pidge and Keith.
âI think it'd be better if we go someplace quiet until we figure out what's going on,â Shiro told them. âFollow me.â
Keith took a step forward, paused to make sure Pidge was coming too, and then they obediently followed Shiro to another room, both hoping he had the answers they needed.
Tears dripped down Katie's face as she knelt with her head bowed over the nearest trash bin, hoping her stomach would settle after its last wave of nausea. âI'm going to kill Slav,â she grumbled to herself.
She didn't know what the machine was supposed to do, but she doubted that knocking her out and teleporting her to a different part of the Castle of Lions was it. Her head spun and her stomach rebelled after such treatment. Worse than that, Keithir wasn't with her and she could no longer feel him through the bond they shared, which meant he was either still unconscious or had been teleported very far away.
Katie sat back and rubbed her eyes dry, no longer feeling as though she needed to throw up again. Just in case, she took a few extra doboshes to rest before getting up and starting her search for Slav.
He was probably right where she last saw him, heading into a full blown panic over the possibility of killing the crown prince of Daibazaal and his wife with his new invention, something the Galra would not take lightly.
She's feel bad for him, but a more vindictive part of her thought he deserved to panic.
Katie reached the room without running into anyone else, which was a little odd, to say the least. There was always someone around, even when they were using the Castle in her spaceship form, but they weren't due for a journey for another few phoebs! Â Despite that, it wasn't until she reached the room where she last saw Slav that he began to think something was seriously wrong.
The room was completely empty. There was also a fine layer of dust coating everything, indicating that it had been quite some time since anyone was there.
Katie swallowed and backed away. She once again tried to reach Keithir through their bond, but there was nothing. With her anxiety rising, she tried to communicate with the Green Lion.
Nothing.
Katie spun on her heel and ran for the bridge. There was no time to stop and slow down, to consider the implications of what was happening. The bridge was the center of the Castle of Lions and she knew that by accessing the system there, she would be able to figure out exactly what was going on and where â or when â she was.
She kept an eye out for anyone else as she ran, but as before there was no one.
It was unsettling.
The bridge doors easily parted for her, sliding open as she approached, and it was there that she was finally able to breathe a sigh of relief. Standing at the helm was the ever reliable form of Coran.
âCoran, thank the stars,â Katie breathed, hurrying over to him. âThere's something really weird go-- why are we in space?â She finally slowed down to look around.
It was only Coran on the bridge. Another checkmark on her list of really weird things. And he was looking at her as though she'd grown a second head.
âPidge?â he asked, his voice raising in alarm.
Katie took a half-step back. âY-yes? Who else would I be, Coran?â
âBut you can't be!â Coran protested, shaking his head. âYou're too old to be Pidge! Who are you really?â
Katie had a sinking suspicion that she knew what was going on. And on the one hand, knowing that she was dealing with time travel brought her one step closer to not having a mental breakdown, but on the other it meant she wouldn't be able to relax until she found a way to fix things.
She took a deep breath. âCoran, it's really me. You can check! Ask me something that only I would know and I'll answer.â
Coran stared at her for a moment, his surprise hardening into suspicion.
âCoran, please. I know it sounds crazy, but I think Slav accidentally built some kind of time machine and it sent me back in time to this moment,â Katie explained, hoping he'd believe her.
He had to.
Why wouldn't he?
âI have all of the paladins ranked in order of something. What is it?â Coran asked.
âHeight,â Katie responded promptly. âI'm number five.â
Coran hummed thoughtfully. âHow about a harder one? I once arranged a quest for the five of you to complete. What was the goal of that quest?â
The one gave Katie pause. He'd set up a number of bizarre training exercises over the years, but it had to specifically be one after she and Keithir found the Red Lion. Unless he was counting Allura as one of the five? She had been with them for the only one she'd consider labeling a âquestâ. Katie would just have to go for it and hope she was right.
âYou told us there were special planets for us to train on, but all along it was to beat the monster born from the Yalexian pearl.â
âYou're missing some of the details there, but close enough,â Coran agreed after a moment.
Katie exhaled softly in relief. It was hard to remember everything after so many years had passed. She'd have to hope the rest of his questions didn't rely on knowing some passing detail.
âLast question! If you're from the future, how many years does it take for us to defeat the Galra empire?â
âWhat?â Katie asked, flabbergasted. âThat's a joke, right, Coran? We're not... we're not at war with Daibazaal. We've never been at war with them. They're our allies. Why would we...?â
With every word she spoke, Katie's dread grew. Coran wasn't smiling. The sheer wrongness of everything returned tenfold, along with a violent wave of nausea.
She took a step back, shaking her head. âNo. No, this isn't right.â
Coran began walking towards her, holding his hands out in a reassuring manner, but there was something in his eyes that spoke the opposite.
Katie swallowed her queasiness and ran, thought she didn't get far. The moment she set foot past the doors, she collided with a broad chest.
âPidge?â Shiro asked, bewildered.
âCatch her!â Coran cried out.
Katie's heartbeat was thunderous to her ears as she tried to side-step Shiro, but he was too fast. Even while taken off-guard by the sight of her and Coran's strange command, he still managed to grab her by the arm and hold her in place.
âCoran, what's going on?â Allura's voice floated down to them as she approached. âWho is-? Wait, is that Pidge?â
Katie ineffectively tried to yank her arm from Shiro's hold, but he only tightened his grip. âPlease, I can explain,â she begged, twisting to face Coran. âI swear, I'm not here to hurt anyone. If you would just let me explain, maybe we can make sense of this!â
Coran's suspicion lessened as he looked at her, his gaze softening. âPerhaps it would be best if we put you down in one of the cells for now. You did just tell me that you believe we're allies with Daibazaal.â
Allura gasped sharply.
Katie nodded once and then bowed her head, refusing to let them see her cry. She'd been so relieved when she found Coran, but all she felt at that moment was terrified and alone.
Wherever she'd ended up, it certainly wasn't the past.
Keithir, she thought desperately. Where are you? Please be safe.
She sniffled as Shiro and Allura escorted her down to the Castle's holding cells. Tears prickled at her eyes.
I'm scared.

Week 9
Monday of week 9 our mini project group worked on installing and testing the flow control wing in the wind tunnel. We specifically wanted to turn on and test the wingtip jet feature on the wing, which uses the same actuators as the flow control-- about 8-10 piezoelectric discs line the wing spanwise, and as they vibrate they pull in and push out air through the top and bottom of the cavity the discs are set in, and the only exit for the air in the bottom cavity is through a small slit in the wingtip. By directing air and pushing it out at a high velocity should reduce the effects of the wingtip vortex that appears when the wing is generating lift. Our method of testing whether it works or not was to turn it on in the wind tunnel and use flow visualization (smoke and tip tuft). We needed to install it horizontally on the traverse, and since it seemed like that was the first time it was installed that way, we needed to make a way for the mounting plate to line up with the holes in the traverse, while still assuring the wing fits through the 5âł slit that accesses the test section. We only had to drill one hole in the plate to make it work, and then we were able to test it out.Â
We did not have conclusive evidence or data from Mondayâs test, but we did make a list of what we can change for the next dayâs testing:
1. Make sure the waveform generator is producing the right type of wave (it needed to be a square wave according to the theses)
2. Find the highest possible wingtip jet velocity by testing different volts/frequencies with the hot wire anemometer.Â
3. Slow the tunnel down so that the wingtip velocity can better match free-stream velocity for best vortex elimination.
We also decided that the smoke machine was unnecessary since the smoke was not giving us any more information than the tuft on the corner of the wingtip edge and the trailing edge. We thought we might notice a slight difference in the frequency that the tuft was spinning with the flow control on vs off, as well as a difference in the size of the radius which it was spinning. Wanting to be more certain of these observations, we unmounted the wing and started measuring the velocity of the jet with the hot wire anemometer. On Monday we got these results:
On Wednesday we redid the process because of how unrepeatable the first values were. The hot wire had to be in *just* the right spot, and since the velocity is maximized, exit area is attempted to be minimized. If the hot wire was moved a fraction of a mm, the velocity would vary by 2-3 m/s. To get more repeatable results, each measurement was taken with more time and care. The hot wire was moved across the slot to peak velocity, then moved back again until the same peak was reached, and then recorded. Our second table was much faster than the first:
Our peak velocity of the matrix was 5.9 m/s at 100 Hz and 100 V, so we decided to only test at this combination. The wind tunnel tuft test went much better: we ran the tunnel at 200 rpm for the slowest speed that will accelerate the tuft enough for it to spin. On this note, when the tunnel was brought up from 0 to 200 RPM, there wouldnât be enough momentum to move the tuft, but once the speed was up to 300 RPM, then the tuft would start going in the direction of the wingtip vortex, and due to hysteresis effect the tuft would continue to spin once we brought it back down to 200 RPM. Also, the wing needed to be at a fairly high angle of attack (not stall obviously) We then took slow motion videos of the wing from different angles when the wing was on and off. Our goals are to analyze these videos and hopefully make some conclusions about the frequency and amplitude of the tuft in the two scenarios.

Death stalked the lands outside its walls. But what stalked within?
Marcu pulled the reins, and Graymare shook her mane out as she came to a stop. The horse stamped and ducked her head to crop a tuft of grass that thrust from between two broken stones. But her rider craned his neck to stare up with awestruck eyes at the cyclopean wall that towered over him. "They were giants who built it," he whispered in a hoarse voice.
"In art and wisdom," agreed his cousin Luco, who rode up to join him. "But it is said they were no more than three hands taller than the tallest man in our village."
The blasted heath stretched dozens of miles in each direction, and only this wallâitself but one side of the glass-smooth cube of stone that thrust skyward like an immense molarâshowed sign of workmanship. Magic too, perhaps, for atop the cube, like an overturned bowl, rested a great crystal dome. But only the great birds of the air could soar high enough to peer down through the glass roof of Prosperium Prime.
PROSPERIUM PRIME! The name itself was a phrase of awe and dread! Raised at the time of the last and greatest Plague, by arts lost to the herdsmen who now scratched out an existence on the withered plains of the continent, it was a civilization's last refuge. For a thousand years its brazen doors had been locked against a world scoured by disease and ignorance, and in that long millennium, never had those who dwelt within the world's last and greatest Arcadia shown face or foot outside its protective walls.
Luco nudged his cousin. "Your offering," he said, and Marcu spurred Graymare forward. At the foot of the wall he laid a bowl of hammered copper, adding it to the mountain of offerings already heaped there. But never yet had these humble pleas for admittance, some carried from hundreds, even thousands, of miles away been accepted or acknowledged by the the lords of the domed arcology.
His pilgrimage completed, Marcu rode back to the village with Luco. Yet every few minutes he turned to cast his eyes back on the inscrutable city. For two score miles, the dome remained visible over the curve of the blighted earth.

The motorized velocipede barked and coughed as Dr. Srivais pulled to a stop. The machine shuddered once and was still as he killed the kerosene feed. He pulled off his travel goggles and peered up at the autogyro that buzzed at the foggy dome of the stone-and-glass-enclosed city.
"Hoy!" called one of the workmen. "Perfesser!" The man held up a pneumatic drill. "Tools ain't no good against this stonework! Drill bits just keep breaking!"
The ground shifted and slid beneath his feet as Dr. Srivais dismounted the velocipede. Beneath the thin and fragile crust it was a garbage pileâa two-thousand-years' deep heap of scrapâdeposited by his (and the Empire's) superstitious forbears as offerings to the unseen inhabitants of Prosperium Prime.
PROSPERIUM PRIME! The name itself was a thing to conjure with! Final refuge of civilizations' perfection, home to men who at the time of its erection had lofted themselves to a state little short of godhood! For two thousand years it had stood silent and reproving of the diseased and ignorant world without.
Dr. Srivais examined the broken drill bit in a distracted state, for his mind was on the arcology. He and the Empire were but children beside the men who, two thousand years before, had lifted its walls and capped them with the great, crystal dome. To what greater glories had their children ascended in the meantime? It was with a dumbstruck but envious awe that philosophers such as Dr. Srivais regarded the silent city, particularly as the Empire's own tentative arcologies were meeting with little success. (Its citizens mulishly refused to let themselves be herded and locked within them.) No answer came to their knocks, and the brazen gates remained steadfastly shut.
"I don't care if you have to use nitro-gel!" the professor barked at the workman. "Find a way in! I want to talk to those people!"

"Blower, please."
As his assistant rooted through the excavation kit for an air nozzle, Aaken-10 craned his neck to watch a rocket-bus as it arced through the soft, evening sky atop a pillar of coppery flame. Ring City, the Universal Federation's oldest satellite colony, hung beneath the moon like a glittering jewel.
The archaeologist used the blower to blast a fine layer of dust off the bowl he had pulled from beneath the crushed remnants of a primitive autogyro. "Look, River Tribe workmanship," he told 7-Proot. He pointed to an inscription along the bowl's rim. "Hearken unto the prayer of Marcu, O Great Ones. Open your gates to my soul."
"Famous last wishes," 7-Proot grinned back. "Imagine having to spend all of eternity in there." He jerked his chin at the cyclopean wall that loomed overhead.
PROSPERIUM PRIME! The name itself was a horror to the ears. Raised by the despairing hands of a faltering civilization, the silent necropolis was now a monument to death inexorable. When at last its walls had been breached, the dead air rushed out in a wave, carrying on it the dust of its millennia-gone builders. Perhaps they had unknowingly locked themselves in with corruptions more insidious than the Great Plague they had sought to escape, or, once locked within their magnificent tomb, they had fallen prey to a madness brought on by confinement. No history they left behind, nor art nor science. Only their own blackened bones, still strewn about the empty streets between their crumbling homes.
One gift only had they bequeathed the descendants of those against whom they had locked their brazen doorsâthe refuse of three thousand years of civilized life, abandoned at the foot of their walls: treasure for the antiquarians!

My PD and I worked on the bars for the prison cell set for The Entire Day and itâs still not done. The shop at Tufts wouldnât let us use their drill because you have to undergo extensive training to use any power tool, so I convinced my family to pick us up and let us use my dadâs drill in the basement. I spent way too long on my knees drilling holes in PVC pipe and plywood, and also way too long out in the freezing cold in the side yard spray-painting said pipe and plywood. My hands and legs and one of my arms are all still fucked up from falling down the stairs yesterday. I also somehow managed to be so late leaving the house that I didnât have time for dinner and I was late for class. Mostly I was late because of the T. I had Pop Tarts and cashews out of the SMFA vending machine for dinner. I still havenât gotten the email saying my glasses are ready, which is a problem because I would really like to be able to see properly as soon as possible. My body feels like itâs just falling apart around my brain. I can only hope that things get a little better after we wrap on my thesis.

How do hair transplants work?
Letâs talk about hair. Some of us will apparently spend a year and a half over our lifetime. Just washing, brushing, styling, and generally fiddling with our hair. With hair transplants beginning to take root in the public consciousness, Iâm here to tell you how they work. We are covered with hair. Our entire bodies have the ability to grow hair, with the exception of our lips. The palms of our hands, and the soles of our feet.
That hair differs all over the body, but the way it grows is pretty much the same. Just underneath the skin, all bundled together with things like nerves, blood vessels, and sweat glands, are our hair follicles. Where individual hairs gradually increase in length as more cells are added to the base over time. As part of their natural cycle. Hairs stop growing and fall out of the follicles, to be replaced with new ones. When someone plucks their eyebrows, itâs only a short-term solution because the follicle can simply grow a new hair. The problem comes when, for some reason or another, the hairs stop coming. And this can be caused by a number of things. Like a major trauma to the skin leaving scar tissue without any follicles. And without follicles, you canât grow hair. Hair Follicles:
In another very common case, the follicles simply stop. Theyâre still there, like the perfect hair-making machines they are. But a hormone called di-hydro-testosterone comes along to interfere and switch them off. Although both men and women can suffer from this type of hair loss alopecia. Itâs most often seen in men, where the follicles around the hairline. The top of the head, and right back to the crown, have the receptors for that particular switch-off hormone, and gradually just stop producing hair. Once the old hairs fall out, youâre left with whatâs known as male-pattern hair loss. In both of these cases, hair transplants offer a solution, by transplanting not just the hair. But also healthy follicles, into the areas that need them. Surprisingly perhaps, hair transplants kicked off in the 1950s. When doctors first realized that you could remove a hair follicle from one part of the body and successfully install it into another part. In the case of masculine hair loss, there is a safe zone of permanent hair growth around the back and sides of the head. From where it is healthy follicles can easily be extracted. However, the results of the initial transplant efforts were less than ideal. Surgeons would remove small pieces of skin almost three or four millimeters across. Each of which would contain around 20 hair-growing follicles. These âplugsâ as they were known would then be inserted into small holes or slits made in the hairless area in a kind of grid pattern. Main cause:
The problem with this is that the follicles grew alright, but they grew exactly where they were put, and not where they werenât. So the transplanted hair had the look of these weirdly regular tufts, very much like dollâs hair. Which I donât think is a favored look for anyone. Thankfully, medical science has come a long way in the last few decades. And hair transplantation is now an almost unthinkably delicate and thorough procedure. Instead of transferring around twenty follicles together. Most modern transplants move just one or two at a time, in what are called âfollicular unitsâ. To get an idea of how crazily laborious that is, take a look at your own hair in the mirror, and see how densely packed those hairs and those follicles are. Suppose picking them out and putting them back in one by one. And donât bother trying to count them, by the way, there can be 100 follicles in every square centimeter. Follicular Units: So how do you separate all of those follicular units? One technique is called the strip method or, more ominously, Follicular Unit transplantation. This involves removing a thin strip of hairy skin from the protected part on the back of the head. Followed by a very sharp eye and even sharpening scalpel, slicing it up into individual follicles. The pelvic units penetrate the skin of the bald area into small pieces and the wound on the back of the head is closed with stitches or staples. Although this method allows surgeons to possibly retrieve thousands of follicles at once. It relies on a frightening object on a stable hand and leaves the patient with a long scar that cuts into his or her fine hair. One way of getting around this is to go for a different technique, called Follicular Unit Extraction. Instead of slicing out a big portion of skin in one go, this method uses a punch tool that looks like a tiny drill almost, to cut around individual Follicle unit, retain the rest of the skin. What is left behind is the full load of small circular holes that hardly fill up to leave traces of understanding. It thins the hair in the safe area, but not overmuch noticeably. The follicular units from this extraction method are implanted into slits in the balding area just the same as with the strip method. And at this stage, thereâs as much art as science involved. Itâs the job of the surgeon to make sure the new hairs blend seamlessly into the existing hair. Realistic hairline:
This involves drawing a realistic hairline and using single follicular units at the edge to make a finer transition. Then, multiple units containing two or three follicles are put in on the top of the head to give a denser covering. During a single, very long procedure, up to several thousand individual follicles are painstakingly removed and reinserted into the scalp. It can then take up to 18 months for the new hair to grow. Sometimes, however, healthy follicles in the safe zone are not enough to cover the area where attention is needed. In the past, that would have meant making a compromise. Perhaps settling for a more waft do, or a more creative hairline. But some surgeons are pioneering techniques to take follicles not only from the head but from other parts of the body. Body hair transplants involve taking hairs from the chest and back and very selectively, mind transplanting them onto the scalp. But itâs not all plain sailing when it comes to sticking your chest hair on your head. As I said before, hair grows differently in different parts of the body, and you might have noticed that chest hair tends to be a little bit coarse. It can also grow at strange angles which, as anyone who has ever gone into battle with that one annoying tuft will tell you, maybe more trouble than itâs worth. So thatâs hair transplants. After new improving new techniques thereâs still no guarantee of success without multiple visits to the operating theatre. Half of America will be obese within 10 years, study says, unless we work together
If America does not collectively adopt healthier eating habits, over half of the nation will be obese within 10 years.
Even worse, one in four Americans will be âseverely obeseâ with a body mass index over 35, which means they will be more than 100 pounds overweight.
That alarming prediction, published Wednesday in NEJM, was the result of a study analyzing self-reported body mass index (BMI) data from over six million American adults.
Half of the nation will be obese within 10 years, according to a study published Wednesday in NEJM.
Considering the challenges of battling weight loss, thatâs devastating news for the future health of our nation.
âGiven how notoriously difficult obesity is to treat once itâs established, you can see that weâre in an untenable situation,â said Aviva Must, chair of Tufts Universityâs Public Health and Community Medicine, who was not involved in the study.
âThe societal cost is high,â she said, âboth in terms of obesity-related health consequences and healthcare expenditures which could bring us to our knees.â
Startling state-by-state data
One of the first research efforts to drill down to the state level, the study found that 29 states, mostly in the South and Midwest, will be hit hardest, with more than 50% of their residents considered obese.
But no part of the country is spared â in all 50 states, at least 35% of the population will be obese, the study found.
âWhatâs even more concerning is the rise in severe obesity,â said lead author Zachary Ward, an analyst at Harvard Chan Schoolâs Center for Health Decision Science.
âNationally, severe obesity â typically over 100 pounds of excess weight â will become the most common BMI category,â Ward said. âPrevalence will be higher than 25% in 25 states.â
Currently, only 18% of all Americans are severely obese. If the trend continues, the study said, severe obesity would âbecome as prevalent as overall obesity was in the 1990s.â
The study also found certain subpopulations to be most at risk for severe obesity: women, non-Hispanic black adults and low-income adults who make less than $50,000 per year.
âAnd we find that for very low-income adults â adults with less than $20,000 annual household income â severe obesity will be the most common BMI category in 44 states,â Ward said. âSo basically everywhere in the country.â
What happened?
âFifty years ago, obesity was a relatively rare condition,â Must said. âPeople who were poor were underweight, not overweight. But that has changed.â
One reason is the rise of sugar-sweetened beverages and ultra-processed foods, which contribute calories but little nutrition. Another is that the price of food, including unhealthy fast food choices, has fallen in America when you adjust for inflation.
âLow food prices are certainly part of it,â Must said. âAlso limited options for physical activity. And thereâs a lot being written about the stress of structural racism and how that influences peopleâs behavioral patterns. So itâs very complicated.â
Can we fix it?
âThereâs no rosy picture here, but I donât think we can throw in the towel,â Must said. âIt will probably take lots of federal, state and local policy interventions and regulations to have a big impact. We canât rely on individual behavior change in an environment that is so obesity promoting.â
Studies have shown some promising tactics, she said: bolstering local public transportation systems to encourage walking instead of driving; keeping schools open on weekends and during summers to allow access to gyms and swimming pools; and increasing support for farm-to-school and farm-to-work food programs, as well as farmersâ markets, to boost access to low-cost fruits and vegetables.
Other interventions include calorie labeling on restaurant and drive-thru menus and replacing vending machines with smart snacks in schools.
âWeâve also looked at eliminating the tax deduction businesses get for advertising unhealthy foods to children,â Ward said. âThe money that they spend on advertising foods can basically be written off as a tax deduction.
âThat could be one reason why we see such disparities by race, ethnicity or income,â Ward said, âbecause companies are directly targeting advertising at these groups.â
In a prior study, Ward and his team at Harvard found that three interventions saved more in health care costs than the price to implement them: elimination of the tax deduction on advertising; improving nutrition standards for school snacks; and imposing an excise tax on sugary beverages.
The most cost effective solution was the tax on sugar-sweetened beverages. The study found the tax saved $30 in health care costs for every dollar spent on the program.
âSo much added sugar is delivered through sugar-sweetened beverages, and people do have other options for hydration,â Must said. âI think itâs an easy target.â
But not necessarily a popular one. Still, the complexity of the problem means that a solution will truly take a village, experts say, with every American doing their part.
âI donât think itâs impossible,â Must said, pointing to a slowing of the obesity rate in children in America. That trend is the result of interventions in school lunches; snack programs; and a change in the nutritional allowances in the Special Supplemental Nutrition Program for Women, Infants and Children, which helps feed more than seven million pregnant and postpartum women and children until age five.
In 2009 the program decreased the intake of foods and beverages associated with excess weight gain. By simply cutting the juice allowance in half, reducing cheese, requiring whole grain products and requiring low-fat or skim milk, a study found the program reduced the obesity rate in children between two and four years of age and boosted the intake of fruits and vegetables.
That is certainly a model for future attempts among both children and adults, Ward said, adding that if Americans could just keep their current weight instead of gaining, the trends could be reversed.
âItâs really hard to lose weight,â Ward said. âItâs really hard to treat obesity. So prevention really has to be at the forefront of efforts to combat this growing epidemic.â
