total wordcount: 71,585 + 22,376 = 93,961 total overall thoughts: i started the year strong and then after may, i really didn’t write much at all...i really struggled. my goal had been to post 100k this year but i fell short of that and it makes me sad sobs looking back, did you write more fic than you thought you would this year, less, or about what you’d predicted? less than what i had hoped! i also didn’t make as much progress on some WIPs i wanted to work on this year. what pairing/genre/fandom did you write that you would never have predicted in january? i’m a one trick pony and only write for bts but i didn’t expect to write yoonmin or yoonkook! very happy i did tho -3- this year’s theme and the story that demonstrates it most: ...love? i think i tend to just write like...romance-y stuff because i love writing abt kookmin...it’s probably best encapsulated in taste the sun because it’s just like....fluffy romance

did you take any writing risks this year? what did you learn from them?  writing i thought he remembered me; he took me back so tenderly is quite difficult because i can’t really rely on dialogue to carry the story forward since jungkook is nonverbal in it. so it’s difficult because i feel like i’m being too wordy describing stuff or just ruminating on jimin’s thoughts but i think it’s a good exercise in keeping my writing engaging without being able to rely on dialogue.

What are the risk factors for Diabetic Nephropathy?

The risk factors for diabetic nephropathy incorporate smoking, and comorbidities, and is more prevalent in people who are more than 62-65 years old. An individual with diabetic nephropathy may not experience any symptoms in the early stages. Changes in blood pressure and fluid equilibrium in the body, on the other hand, may already be present. Waste products can accumulate in the blood over time, causing symptoms.

Swollen feet and ankles, darker urine, and shortness of breath are some of the symptoms presented in the later stages. J&J taps kidney specialist Vifor for Invokana marketing help

Johnson & Johnson scored a one-of-a-kind win for SGLT2 med Invokana in September with an FDA approval to treat kidney disease. Now, as a newcomer to the field, J&J is seeking an experienced hand to help sell its flagging med.

The company has agreed (PDF) to share marketing in the new indication with Vifor Pharma, it said Monday. The agreement will allow Vifor to market Invokana for patients with Type 2 diabetes and diabetic kidney disease (DKD) to U.S. nephrologists––a group it knows well, thanks to its 11 kidney disease products currently on the market.

J&J will back up Vifor's dedicated nephrology force with its existing sales reps and other "institutional" reps who specialize in calling on internists, diabetologists, endocrinologists and cardiologists, the companies said, adding that both of their field medical teams will be supporting the new collaboration.

The pair didn't disclose the deal's financial terms.

"We look forward to working closely with the Janssen team to maximize the potential of this important new treatment for people with Type 2 diabetes who are also suffering from diabetic kidney disease," Vifor COO Stefan Schulze said in a statement. A J&J spokesperson could not be reached for comment by press time.

J&J's decision to seek backup in marketing Invokana's newest indication comes as the drugmaker seeks to turn sales of the medicine around.

The FDA approved Invokana plus standard-of-care ACE inhibitors to treat DKD after J&J reported in April that the combo bested placebo at stalling kidney disease progression and fending off cardiovascular problems in patients with Type 2 diabetes and chronic kidney disease. J&J stopped the trial early last year and filed the data with the FDA in March.

The label expansion was a positive for Invokana after the FDA slapped it with a black box warning for amputation risks in 2017. The year before––in 2016––Invokana posted sales of $1.4 billion, but the figure sank to $881 million in 2018.

In the first nine months of this year, Invokana hit just $588 million in global sales, well below its $653 million pace from 2018.

With Invokana on the ropes, J&J has watched as other SGLT2 meds––particularly AstraZeneca's Farxiga––have pushed ahead with potential class-leading data of their own.

Last week, Farxiga scored an FDA approval to reduce the risk of heart failure hospitalizations in diabetes patients. That nod was Farxiga's first in the heart-helping category, but an even bigger potential approval is in the wings.

Farxiga is under priority review for a label expansion targeting cardiovascular risks in patients with or without Type 2 diabetes. That nod would make Farxiga the first in its class with an approval for non-diabetic patients and could potentially lead the way for other SGLT2 drugs to follow suit.

The FDA also fast-tracked Farxiga in August for its own approval to treat chronic kidney disease.

Proteinuria is a distinguishing feature in primary and secondary forms of chronic glomerulonephritis, which contribute to no more than the 20% of the end-stage kidney disease (ESKD) population. The contribution of non-proteinuric nephropathies to the global ESKD burden is still poorly focused and scarce research efforts are dedicated to the elucidation of risk factors and mechanistic pathways triggering ESKD in these diseases. We abstracted information on proteinuria in the main renal diseases other than glomerulonephritides that may evolve into ESKD. In type 2 diabetes, non-proteinuric diabetic kidney disease (DKD) is more frequent than proteinuric DKD, and risk factors for non-proteinuric forms of DKD now receive increasing attention. Similarly, proteinuria is most often inconspicuous or absent in the most frequent cause of ESKD, i.e. hypertension-related chronic kidney disease (CKD), as well as in progressive cystic diseases like autosomal dominant polycystic kidney disease and in pyelonephritis/ tubulo-interstitial diseases. Maintaining a high degree of attention in the care of CKD patients with proteinuria is fundamental to effectively retard progression toward kidney failure. However, substantial research efforts are still needed to develop treatment strategies that may help the vast majority of CKD patients who eventually develop ESKD via mechanistic pathways other than proteinuria.

JE Dunn Receives USAF Design Award For Cannon AFB

JE Dunn Construction received an Honor Award in Facility Design in the USAF (The United States Air Force) Design Awards Program for the Cannon Air Force Base Medical/Dental Clinic Replacement project located in Clovis, N.M. The Cannon Air Force Base project was one of eight selected for awards and one of three to receive the Honor Award, the highest of the program’s three awards.

Partnered with Hoefer Wysocki, JE Dunn has designed and constructed over $750 million in healthcare facilities at more than 10 military bases across the nation to support the U.S. Air Force Healthcare Facilities Modernization Program during the past nine years.

The USAF awards program was established in 1976 with the purpose of recognizing exceptional project teams as an essential component to delivering high-quality Air Force facilities. USAF award honorees have demonstrated professionalism, hard work, and dedication to their scope of work.

Cannon Air Force Base Medical and Dental Clinic includes a clinical wing and ancillary support wing connected by a commons area. The medical facility houses various clinic departments including Command, TOPA, Pharmacy, Family Health, Behavioral Health, Dental, Optometry, Radiology, Pediatrics, Flight Medicine, and Ambulance Services. The medical facility provides high-quality care and advanced technologies for patients, caregivers, and military personnel, as well as their families. Design and construction took 36 months.

For the design-build project, the government’s goal wasa world-class healthcare facility comparable to similar, private-sector healthcare facilities to accommodate the mission of the 27th Special Operations Medical Group. The user wanted a respite area away from military life despite being on a military base and sought a building to reflect the positive features of the Southwest United States, without overemphasizing the less desirable aspects of the area. Another user objective was a firm, fixed-price budget within the open extents of a design-build contract.

The team experienced multiple challenges throughout the design, including acquiring LEED Silver certification. To meet this challenge, the design-build team employed a collaborative, integrated approach to achieving sustainability requirements throughout the life of the project. From the LEED for Healthcare 2009 checklist submitted at proposal through the final LEED Silver certification of the project, achieving cost-effective sustainability measures was a primary project goal and was addressed continually throughout design and construction. All key design and construction team members had extensive experience on LEED projects and played critical roles in achieving the project’s sustainability goals.

“The project was home run on interior design without sacrificing exterior,” said a USAF juror.

The main criteria for this project was AFMSA Health Facilities Division Guidelines, Cannon AFB Architectural Compatibility Guidelines and UFC 4-510-01, as well as a multitude of others. Based on Architectural Compatibility Guide for Cannon Air Force, the clinic is located in District 1: The Community Center, effectively the Town Center for military and civilian residents and workers. Because of substantial demolition and construction over the next decade, this district is being transformed into a more attractive, sustainable, and pedestrian-friendly neighborhood.

The design-build team has executed several medical facility renovations together, prior to award of the Cannon Air Force Base Medical Dental Clinic. The team utilized multiple lessons-learned from those renovations to the design and execution of the new facility. For example, the team designed the dental mechanical room directly under the dental clinic. The team learned from its renovations at other clinics that having that vertical relationship allows for easier renovations later.

“We are honored to receive this award and continue to provide high-quality environments in the federal market,” said Project Executive Craig Stearns.

The project team for the Cannon Air Force Base Medical/Dental Clinic project includes Hoefer Wysocki; BHC Rhodes; Henderson Engineers; Land3 Studio; Protection Engineering Consultants; Smith, Seckman & Reid; Bob D Campbell; Dimensional Innovations; Walker & Moore; Yearout Mechanical; DKD Electric; Western States Fire Protection; and many others.

The following firms received honor and merit awards in the USAF awards program: Burns & McDonnell Engineering Company, Inc.; HDR; C2RL Engineers, Inc.; Kenneth Hahn Architects; Heffernan Holland; and Morgan Architecture.
