(Times) This May Be Our Last Chance to Halt Bird Flu in Humans, and We Are Blowing It

The outbreak of H5N1 avian influenza among U.S. dairy cows, first reported on March 25, has now spread to at least 33 herds in eight states. On Wednesday, genetic evidence of the virus turned up in commercially available milk. Federal authorities say the milk supply is safe, but this latest development raises troubling questions about how widespread the outbreak really is.
So far, there is only one confirmed human case. Rick Bright, an expert on the H5N1 virus who served on President Biden’s coronavirus advisory board, told me this is the crucial moment. “There’s a fine line between one person and 10 people with H5N1,” he said. “By the time we’ve detected 10, it’s probably too late” to contain.
That’s when I told him what I’d heard from Sid Miller, the Texas commissioner for agriculture. He said he strongly suspected that the outbreak dated back to at least February. The commissioner speculated that then as many as 40 percent of the herds in the Texas Panhandle might have been infected.
Dr. Bright fell silent, then asked a very reasonable question: “Doesn’t anyone keep tabs on this?”
The H5N1 outbreak, already a devastating crisis for cattle farmers and their herds, has the potential to turn into an enormous tragedy for the rest of us. But having spent the past two weeks trying to get answers from our nation’s public health authorities, I’m shocked by how little they seem to know about what’s going on and how little of what they do know is being shared in a timely manner.
How exactly is the infection transmitted between herds? The United States Department of Agriculture, the Food and Drug Administration and the Centers for Disease Control and Prevention all say they are working to figure it out.

According to many public health officials, the virus load in the infected cows’ milk is especially high, raising the possibility that the disease is being spread through milking machines or from aerosolized spray when the milking room floors are power washed. Another possible route is the cows’ feed, owing to the fairly revolting fact that the U.S. allows farmers to feed leftover poultry bedding material — feathers, excrement, spilled seeds — to dairy and beef cattle as a cheap source of additional protein.
Alarmingly, the U.S.D.A. told me that it has evidence that the virus has also spread from dairy farms back to poultry farms “through an unknown route.” Well, one thing that travels back and forth between cattle farms and chicken farms is human beings. They can also travel from cattle farms to pig farms, and pigs are the doomsday animals for human influenza pandemics. Because they are especially susceptible to both avian and human flu, they make for good petri dishes in which avian influenza can become an effective human virus. The damage could be vast.
The U.S.D.A. also told me it doesn’t know how many farmers have tested their cattle and doesn’t know how many of those tests came up positive; whatever testing is being done takes place at the state level or in private labs. Just Wednesday, the agency made it mandatory to report all positive results, a long overdue step that is still — without the negative results alongside them — insufficient to give us a full picture. Also on Wednesday, the U.S.D.A. made testing mandatory for dairy cattle that are being moved from one state to another. It says mandatory testing of other herds wouldn’t be “practical, feasible or necessarily informative” because of “several reasons, ranging from laboratory capacity to testing turnaround times.” The furthest the agency will go is to recommend voluntary testing for cattle that show symptoms of the illness — which not all that are infected do. Dr. Bright compares this to the Trump administration’s approach to Covid-19: If you don’t test, it doesn’t exist.
As for the F.D.A., it tells me it hasn’t completed specific tests to confirm that pasteurization would make milk from infected cows safe, though the agency considers it “very likely” based on extensive testing for other pathogens. (It is not yet clear whether the elements of the H5N1 virus that recently turned up in milk had been fully neutralized.) That testing should have been completed by now. In any case, unpasteurized milk remains legal in many states. Dr. Bright told me that “this is a major concern, especially given recent infections and deaths in cats that have consumed infected milk.”
Making matters worse, the U.S.D.A. failed to share the genomes from infected animals in a timely manner, and then when it shared the genomes did so in an unwieldy format and without any geographic information, causing scientists to tear their hair out in frustration.
All this makes catching potential human cases so urgent. Dr. Bright says that given a situation like this, and the fact that undocumented farmworkers may not have access to health care, the government should be using every sophisticated surveillance technique, including wastewater testing, and reporting the results publicly. That is not happening. The C.D.C. says it is monitoring data from emergency rooms for any signs of an outbreak. By the time enough people are sick enough to be noticed in emergency rooms, it is almost certainly too late to prevent one.
So far, the agency told me, it is aware of only 23 people who have been tested. That tiny number is deeply troubling. (Others may be getting tested through private providers, but if negative, the results do not have to be reported.)
On the ground, people are doing the best they can. Adeline Hambley, a public health officer in Ottawa, Mich., told me of a farm whose herd had tested positive. The farm owner voluntarily handed over the workers’ cellphone numbers, and the workers got texts asking them to report all potential symptoms. Lynn Sutfin, a public information officer in the Michigan Department of Health and Human Services, told me that response rates to those texts and other forms of outreach can be as high as 90 percent. That’s heartening, but it’s too much to expect that a poor farmworker — afraid of stigma, legal troubles and economic loss — will always report even mild symptoms and stay home from work as instructed.
It’s entirely possible that we’ll get lucky with H5N1 and it will never manage to spread among humans. Spillovers from animals to humans are common, yet pandemics are rare because they require a chain of unlucky events to happen one after the other. But pandemics are a numbers game, and a widespread animal outbreak like this raises the risks. When dangerous novel pathogens emerge among humans, there is only a small window of time in which to stop them before they spiral out of control. Neither our animal farming practices nor our public health tools seem up to the task.
There is some good news: David Boucher, at the federal government’s Administration for Strategic Preparedness and Response, told me that this virus strain is a close match for some vaccines that have already been formulated and that America has the capacity to manufacture and potentially distribute many millions of doses, and fairly quickly, if it takes off in humans. That ability is a little like fire insurance — I’m glad it exists, but by the time it comes into play your house has already burned down.
I’m sure the employees of these agencies are working hard, but the message they are sending is, “Trust us — we are on this.” One troubling legacy of the coronavirus pandemic is that there was too much attention on telling the public how to feel — to panic or not panic — rather than sharing facts and inspiring confidence through transparency and competence. And four years later we have an added layer of polarization and distrust to work around.
In April 2020, the Trump administration ousted Dr. Bright from his position as the director of the Biomedical Advanced Research and Development Authority, the agency responsible for fighting emerging pandemics. In a whistle-blower complaint, he alleged this happened after his early warnings against the coronavirus pandemic were ignored and as retaliation for his caution against unproven treatments favored by Donald Trump.
Dr. Bright told me that he would have expected things to be much different during the current administration, but “this is a live fire test,” he said, “and right now we are failing it.”

I still have windblow... Hmmm... Weird... I already posted here about my view on getting pregnant... But we all have our own views about life....Life is a case by case and sometimes the situation that applicable on other people, might not applicable on you...
For the judgemental, mind your own lives and stop judging people....We all have our own definition of LIFE and RIGHTS. We all have RIGHTS or FREEWILL...
I'm not liberated nor ultra-conservative... I'm conserberated...
My life before was different on my life now....Our family is in chaos, we are not ohkay coz of what they did to me for 17 years. Aside from other people and some old friends that are missing... Weird!!!
If ever on example if Pilot Garret is here... My example subject matter....I really like him when I first saw him on youtube...
If the topic or question if I want to have a child, yeah! But it is it depends on the situation... But I want to have one supposed to be but there is part of me that I'm between ready and not ready... This is difficult to explain....I have windblow...
But sometimes I'm worrying... I know, I'm religious enough... I'm 44 on my first life, on my repeat I'm only 17 on my 2nd life ( My cat side will come out ) hahah but originally my nature wasn't rude or wasn't mean...
I feel bad and I feel old and I truly feel fat and ugly....Yeah! There are so many 17 years old right now who already got some lines on their faces coz they wanted to be the older person or their situation stressed them out so much or someone is stressing her unfairly.
I don't know if the windblow will be good to me, if the time comes that I'm ready to have a child...
I remember Mike Shawn the one that I met in cuddle-lite from last year but the app just lasted for 4 years, then it was closed last year....The app which you can only have voices of people and not live chat with face but only pictures, only live chat using your voice.I was so desparate and I have this intuition that probably Mike can take me seriously but he is just suddenly gone.I told him I have windblow and I'm a college graduate and I know I can work but I don't know what happened but there was a "pandemic of coronavirus" . Everything changed from there...
It is totally unfair in my part that someone planned to bury me here plainly, it is super ugly tomb, coz it is "Untitled" ... Worst they changed me into a wrong or cheap version of me since 2007...

The bio-toilets help to eliminate the dirt that falls on tracks. The idea to introduce it was to disinfect the human urine and feces. The initiative also started in the times of the coronavirus where maintaining proper hygiene was of utmost importance.
Let us understand what a bio-toilet exactly is.
Understanding bio-toilets
Bio toilets were invented by DRDO and the Indian railways. The anaerobic bacteria are kept in the bio-digester container. This converts human feces into gas and water. Thus, only methane gas and water stay back after decomposing the sewage. The water can be recycled and then used in the toilet. The gases get released into the air. The contaminated water is released on the track after it goes through the process of chlorination.
There are many benefits of bio-toilets. Human feces were dumped on the railway tracks directly. This is used to spread dirt in the environment. It also damaged the rail tracks and is metals. Bio toilets do not let it happen.
Also. earlier at least 15 litres of water used to be spent on the toilet flushing. The vacuum bio-toilets use only half a liter of water. The Indian stations will become cleaner and also be less smelly. This in turn will help to stop the spread of diseases. There will be less mosquitoes and cockroaches. Manual laborers will also not have to do the dirty work.
Bio toilets have indeed changed the Indian railway’s face. This has given a lot of scope to improve track maintenance. The railways also have a lab that maintains these bio-toilets. There is also a team in every railway division. They maintain the bio toilets. The waste is tested in the lab to check the bio-toilet is working fine. Installing bio-toilets is not cheap. It takes around four lakh rupees for installation. This is also part of Indian railway competition with the airlines.

This was monitored well. The railway thus ensures that no amount of human waste falls on the tracks.  Earlier 3,980 tons of human waste used to fall on the tracks.
There were several benefits of introducing bio-toilets. It helped to save rail and fitting corrosion. This technology has brought in a change in the track cleanliness. The first train to install the bio-toilet was the Gwalior-Varanasi Bundelkhand. This was done in the year 2011.
The installation of bio-toilets has been a great success for the railways. However, there were many challenges in this process. The proper functioning of the toilets and its maintenance is a key concern. There are many processes involved and the railways need to address this well. If this is done then railways will be able to create a railway system that is not just efficient but is also environmentally friendly. And it is not only about a clean track. It is a great step by the Indian railways to create a sustainable environment and clean travel for the rail passengers of India.

Before there were vaccines or good ways to diagnose SARS-CoV-2, scientists around the world were scrambling to develop methods for understanding the biology of the virus. While the pandemic opened plenty of scientific opportunities and questions, doors, and borders were also shut almost everywhere.
One of the millions of people who were stuck during early 2020 was computer scientist Christian Guckelsberger. Due to starting a postdoc on AI and creativity in Finland, he instead found himself part of an international biomedical research project to harness machine learning to identify the coronavirus from patient blood samples.
“It was like being thrown into cold water. I’m not a medical person and hadn’t thought about these kinds of things since high school biology,” says Guckelsberger, now an assistant professor of creative technologies at Aalto University and FCAI.
“But this project gave me the feeling of doing something relevant and putting my mind to best use at a critical time.”
Amid pandemic travel restrictions, Guckelsberger’s involvement in the timely, multidisciplinary project allowed him to come to Finland and embark on what he thought would be a two-month project. It ended up lasting an entire year and sparked a renewed fascination for biomedical research and the people doing it.
What Guckelsberger and colleagues developed during that year is a way to automatically classify digital microscopy images of the interactions between antibodies and virus antigens in lab-grown cells. A computer essentially learns to detect if the patient has anti-coronavirus antibodies.
Beyond a diagnosis, the method also gives researchers insight into what features in cells indicate a positive result, what kind of antibody responses are present and allows them to make predictions about the likelihood of a COVID-19 antibody-positive sample from the image alone.
The same sample images classified by a computer were also shown to expert virologists, who rated them as positive or negative for coronavirus antibodies. “Our approach can match the classification level of human experts,” says Guckelsberger, “and it’s much faster. Plus, it can tell us when there are ambiguous results that should be given a closer look by an expert eye.”
The results of the project, recently published in Cell Reports Methods, also show that the method is comparable, and in some ways superior, to widely-used assays like ELISA.
“We used cells, rather than purified virus proteins, as the basis for our assay, which is closer to real physiology,” says lead author Vilja Pietiäinen of the Institute for Molecular Medicine Finland (FIMM) at the University of Helsinki.
“Because everything is completely automated, we have high throughput, but we also get the digital images that can be shown to a virologist or a pathologist, without them having to go to a microscope. The results can even be checked on a mobile device. And we can count the number of infected cells, so we have the quantitative data as well as the visuals.”
During the early days of the pandemic, the research team was able to form quickly thanks to earlier international and local collaborations on virology, imaging and drug response studies, explains Pietiäinen.
“At that point, we needed a high-throughput assay for antibody testing that would indicate if a person had a SARS-CoV-2 infection. Since then, there has been a lot of improvement on SARS-CoV-2 diagnosis, detection, and antibody response,” such as the widely familiar polymerase chain reaction (PCR) test or the antigen test (such as the nose swab) that directly measures the presence of the virus in the body.
The test developed by Pietiäinen, Guckelsberger and colleagues, by contrast, measures antibodies, which tells us how the immune system recognizes the virus and produces different types of antibodies against it.

“When you only have a few samples, know very little about a disease or might not have access to a high-level biosafety lab, our pipeline can be really valuable,” says Guckelsberger, adding that it can be used anywhere regardless of location, sample preparation equipment or type of microscope. In fact, the pipeline is versatile to test on any germ.
“We designed the test to be used for any emerging pathogen, increasing our readiness for future pandemics,” says Pietiäinen. “Certain components should be optimized for each new virus, but the beauty of the assay is that it can be used for different purposes. It’s already being used to study zoonotic viruses like the Puumala virus.”
Other automated cell-based assays, followed by AI-guided image analysis methods, are being used in the research group to study the drug responses to SARS-CoV-2 as well as to identify drugs that can kill patient-derived cancer cells ex vivo.
Beyond publishing their work and contributing to a better understanding of the pandemic, Guckelsberger and Pietiäinen share a common insight that this project taught them.
“When big questions come up in the world, we as scientists can’t work alone in silos. Experts from different fields, different universities and countries need to come together with a shared aim—in our case, data scientists, clinicians, computer scientists, biochemists,” says Pietiäinen.
“Working in a big team, which is not something we do often in computer science, was fascinating,” echoes Guckelsberger.
“One big challenge was communicating from different perspectives of expertise, for example making sense of what is happening at both ends of the pipeline from wet lab procedures to parameters to data and images. At the same time, this was a fantastic learning experience, and one that I wish to have more of in the future.”
While they employed well-established machine learning for each component of the pipeline, Guckelsberger says making the connection between biologists and computer scientists was one of the real advances. Using technology to resolve biological questions was a big takeaway for Pietiäinen, too.
“Combining microscopy with machine learning, not just for SARS-CoV-2, but to see personalized responses to drugs or to see the cellular phenotypes of rare genetic diseases, is powerful. A picture is worth a thousand words, that is also the case here.”

Almost three years into the COVID-19 pandemic, testing for the coronavirus is becoming less common and less relevant in Canada, experts say.
The federal government scrapped pre-arrival PCR test requirements for travellers last February and is ending new shipments of rapid antigen tests to provinces and territories.
While supply is not an issue as Ottawa and provincial health authorities have millions of rapid tests in their stockpiles, demand appears to be waning.
“So it may be that a year from now or so, the rapid test may not be necessarily useful,” he told Global News.
"As the virus has mutated over time, the emergence of new variants has also reduced the sensitivity of the antigen tests," said Evans.
Dr. Anna Banerji, an infectious disease specialist at the University of Toronto, said in her opinion, people are using rapid antigen tests less because there is no longer a public health strategy to deal with COVID-19.
Money is another factor to consider, as “these tests are not cheap,” said Evans, so there might be less of an appetite from the government to continue picking up the cost for a tool that was not relied upon to paint a picture of the pandemic in the first place.
Instead, PCR testing done in labs has helped track COVID-19 case numbers and positivity rates in the community, he said.
Ottawa has ordered more than 811 million rapid tests since the beginning of the pandemic, with a price tag of about $5 billion, of which 680 million went to provinces and territories.
So far, the provinces have been providing these tests for free at pharmacies, grocery checkouts, and other locations.
Health Canada said the decision to end shipments at the end of January was made in collaboration with provinces and territories, as the regions have enough supply.
At the peak of the COVID-19 pandemic, rapid antigen tests were an essential go-to for many Canadians.
Cutting back on supply might make it difficult for people who want to continue testing themselves, said Evans.
“It’s going to be really challenging without the rapid test for many people who have relied on them to really figure out if the symptoms they’re having of a respiratory tract infection are due to COVID or just due to some other virus,” he said.
There is also a risk of further spread of COVID since viral shedding can occur up to seven days after COVID-19 symptoms develop, Evans said.
According to Health Canada, there are 90 million rapid tests in the federal inventory, but 6.5 million of those will expire this year and the rest will expire within two years.
Once these tests go beyond their expiration date, accuracy becomes questionable, Evans said.
He said antigen tests can still be safely used one or two months after their expiration, but the reliability is going to diminish the further away one gets from the expiration date on the package.
"There is nothing in the rapid antigen tests that is particularly biologically hazardous, especially if they have never been used," said Evans.
And most pharmacies, clinics, and hospitals are well equipped with biological waste containers, so they should be able to take care of any large lots of expired tests that need disposal.

U.S. military poised to secure new access to key Philippine bases (Washington Post) The U.S. military is poised to secure expanded access to key bases in the Philippines on the heels of a significant revamp of U.S. force posture in Japan—developments that reflect the allies’ concern with an increasingly fraught security environment in the region and a desire to deepen alliances with the United States, according to U.S. and Philippine officials. The expansion involves access to Philippine military bases, likely including two on the northern island of Luzon—which, analysts said, could give U.S. forces a strategic position from which to mount operations in the event of a conflict in Taiwan or the South China Sea. They will also facilitate cooperation on a range of security concerns, including more rapid responses to natural disasters and climate-related events. While expanded base access is alone not the security linchpin for the region, “it’s a pretty big deal,” said Gregory Poling, director of the Southeast Asia program at the Center for Strategic and International Studies. “This is significant not just in terms of what it means for a Taiwan or South China Sea contingency. This is a signal that the Philippines are all in on modernizing the alliance.”

Nigeria’s push for redesigned currency creates cash shortage (AP) Nigeria’s push to replace its paper money with newly designed currency notes has created a shortage of cash, leaving people unable to buy what they need and forcing businesses to close across the West African nation, experts and business groups said. The Central Bank of Nigeria says the redesigned denominations of 200 (43 U.S. cents), 500 ($1.08) and 1,000 naira ($2.17) notes and new limits on large cash withdrawals would help curb money laundering and make digital payments the norm in Africa’s biggest economy. But the process to replace the old currency notes is “rushed,” and commercial banks don’t have enough new cash to give to customers, pushing demand higher than supply, said Ayokunle Olubunmi with Nigeria’s main ratings agency, Agusto and Co. The government is pushing for a cashless economy that is more inclusive and says the changes will drive economic growth. Critics are skeptical, pointing to decades of chronic corruption in which government officials are known to loot public funds and create more hardship for the many struggling with poverty.
