What is Maxillary Expansion?
Maxillary expansion is a dental procedure that aims to widen the upper jaw, also known as the maxilla. It is often used to correct a condition called maxillary constriction, where the upper jaw is narrower than it should be. This can lead to problems with teeth alignment, breathing, and facial aesthetics.
There are two main types of maxillary expansion:
Orthopedic expansion
Orthodontic expansion
Orthopedic expansion involves separating the mid palatal suture, a joint in the roof of the mouth, while orthodontic expansion results in buccal tipping of the maxillary posterior teeth.
Types of Maxillary Expansion
There are several types of maxillary expansion, each with its own characteristics and applications:
Slow expansion
Rapid Maxillary expansion (RME)
Surgically assisted Rapid Maxillary expansion (SARME)
Slow expansion is a gradual process that uses a fixed appliance to slowly widen the upper jaw. Rapid Maxillary expansion (RME) is a faster method that uses a removable appliance to expand the upper jaw over a shorter period. Surgically assisted Rapid Maxillary expansion (SARME) is a combination of RME and surgery, where a small surgical incision is made to facilitate the expansion process.
Effects of Maxillary Expansion on Nasal Breathing
Maxillary expansion, particularly RME and SARME, can significantly impact the nasal cavity. The expansion process widens the upper jaw, which in turn enlarges the nasal cavity. This enlargement reduces nasal airflow resistance, making it easier to breathe through the nose. Studies have shown that non-surgical RME can improve breathing, increase nasal cavity geometry, and decrease nasal airway resistance in children and adolescents.
Antero-Posterior Cephalometric studies provide evidence of an enlarged nasal cavity following maxillary expansion. These studies use X-rays to measure the dimensions of the nasal cavity before and after expansion. The results consistently show a widening of the nasal cavity, indicating that maxillary expansion can effectively improve nasal breathing.
Complications of Maxillary Expansion
While maxillary expansion is generally a safe and effective procedure, there are potential complications that can occur. These complications are more common in adults than in children, as the midpalatal suture begins to fuse by the late teens and becomes relatively rigid.
Complications of non-surgical RME in adults include:
Expansion relapse
Molar tipping
Opening rotation of the mandible
Gingival recession
Bone loss
Root resorption
Pain and swelling
In adults, surgical interventions like SARME are often indicated to separate the midpalatal suture and widen the maxillary arch. Studies have shown that SARME can produce a substantial short-term increase in nasal volumes, but it may not have the same effect on oropharyngeal volumes in non-growing patients.
Evidence from Cephalograms and CBCT Studies
Cephalograms and CBCT scans provide valuable insights into the anatomical changes that occur during maxillary expansion. Cephalograms are X-rays that capture the side profile of the head, while CBCT scans create three-dimensional images of the head and neck.
A cephalogram study showed that maxillary expansion resulted in skeletal expansion in the maxillary arch, opening of the midpalatal suture, and lateral and external displacement of the inferior nasal turbinates.
