"Babyfier" is one of my fav episodes of Lilo and Stitch: The Series because we get to see all our favourite alien goofballs as babies!!! So precious!!!! 😍😍🥰🥰

1 - Shelter (2007)

I often joke around here about how I’ve been watching queer cinema for over half of my life at this point, and it’s easy to recommend this film. This is a coming of age film about a guy who gave up art school to become the primary breadwinner and caregiver for his family. However, when the older brother of his best friend returns to their town to collect himself, our artist and he reconnect and find something special between them. Great use of a young actor in this shores up the caregiving aspects.
I’m probably going to rewatch it now. Because it was produced by Here! TV, you can only legally watch it via a subscription to their platform. I own it on DVD because I fell in love with it and knew I needed to keep it forever.
2 - Big Eden (2000)

Big Eden. Oh, Big Eden. This is the film equivalent of a warm blanket and a tight hug. It’s about an artist named Henry Hart, who is preparing for a big exhibition in New York when he’s called back home to Montana because his uncle has had a stroke. We are greatest with the most queer-friendly town to ever exist as Henry manages his old angst about his straight best friend as the local general store owner also secretly pursues him. It’s absolutely lovely.
3 - The Blues Brothers (1980)

Luna has great taste, because this is one of the best films ever made. What was originally just an SNL bit turns into a fun road film about getting the band back together so that two brothers can raise enough money to pay the back taxes owed by the orphanage they grew up in. We also run over Illinois nazis in this movie and demolish dozens of cop cars. Cab Calloway, James Brown, Carrie Fisher, Chaka Khan, Paul Reubens, and Aretha Franklin are in it. John Candy orders orange whips. This is the kind of film I would watch with my dad any time it was on.
4 - Master and Commander: The Far Side of the World (2003)

This is one of the most man movies ever made. I don’t know any woman who wants to sit down and watch this film, but me and boys will spend an entire afternoon on this film in a heartbeat. The sexual tension between Russell Crowe’s and Paul Bettany’s characters goes unremarked on this website in a way that lets you know for sure this hellsite is dominated by femmes, because those two have definitely fucked. At least twice. It’s 1805 and oceans have become battlefields!
5 - Clue (1985)

A movie based on the board game of the same name should not have been this good, but it instead goes on to become a camp masterpiece. Many people will end up remembering Tim Curry for Rocky Horror or even Muppet Treasure Island, but this is still one of his favorite performances for me. This film is batshit and I love it. 
6 - Camp (2003)

Speaking of camp films, let’s talk about one of the best of all time. I know we often talk about the bad singing in Thai BL, but I unironically love all of the musical theater in this film. I regularly listen to this soundtrack, and have been for over 15 years. It’s a film about a bunch of weird theater kids who get to escape the bullying and hellishness of their lives for a few weeks during the summer, where they get to put on a bunch of classic plays. It’s so camp. I love this film because it was difficult for me to find queer films that had happy components with them, and this little movie has a wide array of queer kids in it.
7 - Make The Yuletide Gay (2009)

This was the first queer film I ever watched that had a happy ending that was also a comedy. Prior to this, I think I had watched Beautiful Thing (1996), Edge of Seventeen (1998), Get Real (1998), and Bent (1997). Most of those films ended resolved or sad. Yuletide is a silly little gay film of almost nonstop innuendo about a guy who goes back into the closet when he returns home for Christmas, but hijinks ensue when his boyfriend shows up unexpectedly. It’s an annual watch for me around the holidays, and I usually host people for it. 
Also, Adamo Ruggiero is in it! He played Marco on Degrassi: The Next Generation.

Me with milgram resist the urge to project there are so many reasons someone would cycle to work. Firstly, it's healthy and secondly it's cheaper- And sure ADHD has heavy ties to smoking but this could very well be self-indulgent daydreaming.

AND CERTAINLY- He's been supportive of Haruka's growth and is probably sticking around him and Muu to make sure he doesn't backslide. However- I'm probably just unhinged, projecting and hopeful. Like it'd be pretty cool if this guy with confirmed DID had adhd and autism on top of it. Yet, there's no precedent for that not at all-
"Neurodevelopmental disorders increase the risk or impact of a variety of traumas, including injuries, medical symptoms or procedures, and witnessing violence (Ko et al., 2015). Interpersonal victimization is also a major risk, including the chronic child maltreatment most often associated with dissociative identity disorder (DID). Children with neurodevelopmental disorders are more likely to lack secure attachment to their caregivers, who often report high levels of caregiving stress and difficulties accessing sufficient support. This, combined with anti-disability discrimination and challenging child behaviors, can lead to high rates of child abuse and neglect. Stigma and discrimination can also lead to peer harassment, rejection, or abuse, as can the social differences inherent to many neurodevelopmental disorders. Additionally, individuals with autism and intellectual disability are perceived as more vulnerable, may have difficulties recognizing warning signs of danger, and are often not given age-appropriate education on romantic or sexual relationships, all of which increase the risk for sexual violence and intimate partner victimization. Difficulties communicating can also increase the risk of victimization and make it difficult for victims to get help and support (for review, see Reuben & Parish, 2022).
Accordingly, reported and confirmed victimization rates are very high. Baladerian et al. (2013) found that 62.5% of individuals with a neurodevelopmental disorder had experienced abuse, including 34.2% sexual abuse. When specifically examining ADHD, other studies found child maltreatment rates ranging from 22.9% to 51%, including 6.4% to 51% for neglect, 12.9% to 43.5% for physical abuse, and 6.6% to 33.7% for sexual abuse (Fuller-Thomson et al., 2015; Guendelman et al., 2016; Hellstrom, 2019; Ouyang et al., 2008). When examining autism, 50.4% to 80% report any form of child maltreatment, including 62.2% to 79% emotional or verbal abuse, 18.5% to 57.8% physical abuse, and 2.8% to 30% sexual abuse (Baladerian et al., 2013; Chan et al., 2018; Griffiths et al., 2019; Hellstrom, 2019; Mandell et al., 2005; McDonnell et al., 2019; Pfeffer, 2016). Victimization often extends into adulthood and includes intimate partner victimization, physical assault, and sexual assault, all of which can further complicate and worsen dissociative responses. One study of autistic adults (Reuben et al., 2021) found that the majority reported having experienced physical assault or sexual violence, and these experiences were associated with both posttraumatic stress and dissociation." x

"I'm sure. The house is almost finished, then I'll send you tickets for the train. You'll love it I promise, just wait a little longer"
"OK William, I trust you. We'll be together soon"

"6 years Kailee, that's how long it's been since he left Henford. He said he'd write me every day and he did...for 6 years and now..."
"I'm so sorry Lizbeth."
"It's not fair! Why him? Knox comes home but not William. It doesn't make sense. What am I supposed to do without him?"

"There are plenty of families that need nanny's and cooks. Men who will need caregivers now."
"So get a job." Lizbeth sighed and tried to blink back the tears. "This wasn't how it was all supposed to end. We were going to grow old together. He never even met Blaine."

William's body had not been recovered when the squad was rescued but Kailee and David arranged for a stone to be placed in the backyard surrounded by all of William's favorite colors. Her and the two older kids held a private memorial for him.
"Bye Dad." Ivy whispered.

The kids went inside but Lizzie lingered a little longer. She let out a deep sigh and kneeled beside the stone. "You sent letters everyday just like you promised. I still trust you. We'll be together soon. Just wait for me and I will wait for you. For as long as it takes."

A few weeks later, David called the women to his bed, knowing his time in this save was coming to an end. "Lizbeth, I spoke to the bank. I explained the situation and managed to get the account in your name."
"I told you we'd work it out. If they give any trouble Knox will help"

"Thanks Dad."
He turned to Kailee "You know I consider you as one of my daughters. You and your family have taken care of mine for years. That won't be forgotten. The Greenburg name will always be closely tied to this family and our legacy whether you stay in Evergreen or not."

"What does that mean?" "It means she's in the will. But that's a discussion you can have with the lawyer after Grim takes me."
"Are you giving her the house?"
"It's far more her house than yours. But no the house will go to Reuben. My legacy on the other hand will be going to someone else."

David turned to Anabelle then, "Don't think I forgot about you my dear."
"You don't have to give me anything. I'm not even family."
"You're the mother of my grandchildren and one day you'll be more than that I'm sure of it. Relationships take work but it's worth it."

David scoffed and motioned toward the dresser. "Could you grab the book tucked in the top drawer?"
Anabelle did as he asked, "What's this?"
"Open it to the back if you would and read the note on the cover."
"Dad what's this about?"

"To my son, Hezekiah. I always dreamed what it would be like to have a son to carry on the Mizrahi name but I never imagined how frightening it would be. I look at you and I'm filled with fear. I want to be better for you. I will be. When this war is over, I will be."

"David what is this?"
"I found it after Elena died. The book belonged to my father. He loved to read and my brother took after him in that way. Sofia is like them. This book is our legacy. It's a reminder of who we are and what we strive to be: Better than what we were."

David turned back to Lizbeth. "I have letters for the children. They're with the will. Now...I'd like to be alone. Let Grim take me without audience."
"Give mom and William all my love."
They left the room and when Lizbeth returned later, only his urn remained. "Goodbye Dad."

Finally, word came that Kye had recovered enough for visitors. Anabelle made the trip to the hospital alone and hovered nervously over his bed. He looked so peaceful, so handsome despite the bruises and scars. She had dreamed of their reunion but now she was just scared.

She brushed a lock of hair from his forehead and leaned down to kiss him. "Oh Kye. What did they do to you?" She took his hand holding it gently as if she thought it might break.
"It's not as bad as it looks." Kye opened his eyes and offered her a weak smile.

He put his hand over hers and a sob escaped her. "You don't have to lie to me, I spoke with the nurse!"
"Hey." He squeezed her hand "I'm still me."
She took his face in her hands all the anxiety of the past 4 years beginning to melt away as she looked in his eyes.
"Help me up?"

She helped him position the bed up and he took her hand. "You have wrinkles around your eyes, we were supposed to do that together."
Anabelle chuckled, "Grey hairs too but I use a tonic and you're one to talk coming home with all these scars."
"Yeah, these are the nice ones."

Kye's face fell, "I know things aren't gonna be how I promised them. Everything's different now. I can't...I won't be able to..."
"I know. The nurse told me. And it's okay. Our family's plenty big enough."
"I'm not just talking about kids. I'm only half the man I used to be!"

He gave a shy laugh, "I won't be able to...make you...feel the way I used to."
She smirked. "I don't know, I'm feeling things right now I haven't felt in a long time." The air became electric and she leaned forward but then hesitated. "Let's wait till you're home."

David: I'd say David is closest to the Everyman from the desire to be included but worried about losing self by conformity.
Amelia: She's closest to the Caregiver. Kind and compassionate but can potentially be exploited (at least at first when some first years only wanted tobe her friends to get close to David).
Phoenix: The Hero, befitting of a Gryffindor and her willingness to go into duelling as a career. However, could be argued she's a little arrogant.
Cledwyn: The Rebel due to his goal of vengeance against Odin Greyback.
Faith: The Lover from her fear of being left behind.
Reuben: He fits the Explorer best from wanting to do his own thing and being scared of being forced to conform to his parent's desires rather than his own.
Marigold: I think Marigold is the Magician with her focus on understanding and fulfilling her goals no matter what it takes but worries about becoming manipulative.

Happy National Make a Difference Day — Go Hug a Sheep!
At Farm Sanctuary, every day is cause for celebration as we honor the difference we can make by putting compassion first. 
Today, October 28, is National Make a Difference Day — a wonderful opportunity to make the world a better place in your own special way. On this day, we remind ourselves that every act of kindness, no matter how small, makes a difference — be it volunteering; sharing a compassionate meal with friends; or offering one of the simplest and best gifts we can give: a hug.
Coincidentally, today also happens to be National Hug a Sheep Day — as though we needed another reason to hug our ovine friends! (Of course, since each sheep is an individual, we only hug the ones who enjoy being hugged.)
Sadly, sheep sometimes get a bad rap — for example, calling someone a “sheep" can be seen as an insult, implying that a person is mindlessly following the crowd. In reality, sheep find safety in their flocks —  which we think you’ll agree is is hardly mindless, but instead very smart! In learning to see and value these animals as they truly are, we can get to know each one as the unique individual that he or she truly is.
Sheep — and all farm animals — have much in common with the companion animals most of us know better. For example, did you know that sheep wag their tails when they’re happy, like dogs, and paw at their friends for attention, like cats? And as we see every day at Farm Sanctuary, each one of these amazing animals has his or her own unique personality and preferences — and we are lucky enough to get to know and love them all! 
Here are just a few reasons why we celebrate National Hug a Sheep Day —  and why we celebrate sheep every day!

Farm Sanctuary Caregiver Amy Gaetz has a special bond with Liam sheep — a survivor of neglect who found love and healing at Farm Sanctuary.

Liam loves to play (gentle) head-butting games with Amy.
“When he first came he was frightened of us and hid any time we came into his area,” Amy recalls. “But he very quickly learned that we were friends and had no ill intention toward him, and he became such a friendly sheep. He loves head-butting! One day, early on after he arrived, I stayed after work and played with him for half an hour. He just backed up, ran full speed at me, then stopped just in front of me, and head-butted the palm of my hand very gently — for half an hour. He never got tired of it. Now, any time I go into the sheep barn, he comes running over for head-butt games. He’s always gentle with me, and never seems to grow tired of our little routine.”

Caregiver Dana Penman has a special connection with Connie, whom we rescued from neglect last year. (Fun fact: Dana even got to name her beloved sheep friend!)

Connie (right) is mom to gregarious son Ash, who's growing up safe and sound at Farm Sanctuary following the pair’s rescue.
“Being there since the beginning of Connie’s life here at Farm Sanctuary, I got to watch as her walls came down and she learned to trust me,” Dana says. “Now, I get to see her grow into a wonderful mother and a one-of-a-kind personality. She is amazing and my heart is forever changed because of her.”

Connie lovingly follows Kameke down the pasture — once you’ve bonded with a sheep, you are friends for life.
And Dana’s not the only one who’s been moved by Connie’s love and sweet nature: Volunteer Program Coordinator Kameke Brown shows us in the video above just how much Connie’s life has changed since coming to Farm Sanctuary. Once frightened, she now loves to show her affection for her new human friends.
“I have so much appreciation for the sheep friends I’ve made since coming to Farm Sanctuary,” Kameke says. “When it comes to our human relationships, there can sometimes be a lot of uncertainty and fear around the vulnerability of opening yourself up to love or being loving in those relationships. But with the sheep, they’re very clear about wanting love or attention or to be interacted with … they’ll chase you down or run up to you to be pet or snuggled! It’s taught me a lot about allowing myself to be open-hearted and vulnerable and it makes me feel really grateful, as an ally working in solidarity with farm animals, to actually be able to build relationships with them and to get to know them as the unique individuals they are.”

Kameke shares a hug with Adriano, the leader of the flock and a fan favorite.
Tour Guide Chelsea Jamieson also has a soft spot for our beloved flock  leader. “Adriano is one of the first sheep I met at Farm Sanctuary,” Chelsea recalls. “I love his peaceful, steadfast, and kind personality. Being around him always brings me such joy!”

Associate Editor Sam Goldstein is another fan of Adriano’s, but is perhaps even closer with his mom, Florence. Rescued from a backyard butcher more than five years ago, it took some time for Florence to warm up to humans — and these days, Sam considers herself lucky to be one of those people.

“Florence is very special to me,” Sam says. “She is one of the best friends a girl can ask for. She’s introverted like me, and tends to shy away from larger crowds — but there are times when I’ll enter the barn and she will come running across it just to say hello. She’ll find me before I even see her.
“Florence is the kind of friend whose friendship you don’t have to think about — you always know it’s there. I’ll sit beside her and rub her chest, and she will inch forward — gently stepping over my legs — until I reach a favorite spot on her belly. She’ll close her eyes and smile when I pet her. She’s the perfect friend to see when I’m feeling down and just need a hug — she is so gentle and welcoming and we can both feel safe just being in each other’s presence.”

Through our relationships with sheep, we may learn important things about ourselves. Just ask Tour Guide Maddie Krasno, whose incredible bond with Ash demonstrates how love is the strongest power of them all. 
“Through his outright insistence to snuggle each time I enter his barn and through his eagerness to greet strangers with the same gentleness and determination to cuddle that he shows his old friends, Ash has demonstrated the magnitude of influence one individual can have on another,” Maddie says. “Sanctuary visitors who meet Ash are unlikely to forget him — his smiling eyes, his calming presence, and his love for hugs. That that is the difference I want to make in the world — one that is positive and ultimately evokes empathy in others for all sentient beings.”

Jessica hugs Mo, the boy who opened her heart to sheep forever.
Caregiver Jessica Due agrees. “There is something magical about sheep,” she says. “Maybe it's their sweet disposition, kind faces, and soft hair, or maybe it’s the fact that even though humans do unspeakable things to them, they are still willing and eager to love us. I am constantly in awe of the love they have for us and Mo has changed my heart forever.

“Before Mo, I hadn't bonded with a sheep before — most of my heart belonged to the other farm animal species,” Jessica explains. “Then Mo came into my life with his kind smile, mischievous eyes, and graceful step, and my heart was never the same again.  Sheep have now taken over a huge part of my heart for themselves.”

Erin Dansevicus enjoys quality time with her best friend Louise sheep.
Monthly Giving Coordinator Erin Dansevicus identifies with Louise sheep, who inspires her to celebrate the inherently special qualities we each possess. Rescued from extreme neglect while pregnant, Louise gave birth to twin boys Reuben and Summer at Farm Sanctuary — and despite her difficult past, she inspires us to savor the beauty in each day.

“Louise has a natural ability to be both beautiful and powerful, as if the two are synonymous, as if love is the power we are all born to emanate,” Erin says. “She is shameless in her desire for love, attention, and a good back scratch, and her natural presence teaches me to simply ‘be’ in each moment. She also reminds me to not take life so seriously – I am always amazed at her ability to make me laugh. I am so lucky to call her my friend and teacher.”

Tour Guide Kelsey Bomboy has also observed that no matter what these beings have been through, they still have so much to teach us about the healing power of love. “Every day I am blown away by how loving, forgiving, and patient Joel is even after all he’s been through,” Kelsey says. He always has a calming presence which means he’s the best to cuddle with after stressful days!”

Sheep have a special way of making us feel welcome — helping us feel at ease and reminding us that we all have a place. “Visiting with the ‘sheeple’ is such a privilege,” Tour Guide Brianna Symmonds explains. “It’s thrilling to know them and to always feel welcomed into their community.”

Breezy Rondilone enjoys a snuggle session with good pal Violet sheep.
“The sheep barn is the place I go when I need a little extra love,” adds Program Coordinator Breezy Rondilone. “Violet has a way of just knowing. She rests her head in my lap and my heart always feels full. Sheep really do make the best, most supportive friends.”

Bob and Ash welcome Liza back to New York during her most recent visit.
Even if a long time has passed, sheep remember their best friends. “After a three-month internship in New York, I became very close with the animals, especially the sheep,” says Volunteer Program Coordinator Liza Kahn. “I now work at the Orland shelter and only come back to New York for visits. When I walked into the sheep barn, they surrounded me with hugs and hellos. I truly missed them!”

Reuben Stotler, who goes by Reuben No Nickname as he points out "it is his given name", is a retired insurance actuary, and is exactly 72 years and 4 months old.  He was "born a bachelor, and will die a bachelor" and spent the last 42 years in the same house, on the same street, in the same town.  He goes to bed at the same time every night, and gets up at the same time every day.  He drinks exactly 88 oz of water a day as measured by a 4 cup measuring cup as "instructed by his doctor."  Reuben was content with his life until the day he stumbled and broke his hip on his daily 8am two mile walk and his life was thrown into chaos. Following surgery for a hip replacement, Reuben was admitted to a skilled nursing facility for rehabilitation.  He was in a strange location, was unable to walk, had people coming and going about in his room intermittently throughout the day, and he was given medications to take without knowing what the medicine was for.  His anxiety increased, his poor communication skills became worse, and his social skills deteriorated.  He became "combative", "aggressive", stopped eating and refused medications.  Due to these behaviors, Reuben was discharged from therapy and remained in long term care, using a wheelchair instead of walking. Reuben Stotler represents one of the several thousand elderly adults who are on the autism spectrum disorder (ASD) but are not diagnosed or are misdiagnosed.  Autism was not officially described in diagnostic literature until 1980, with the term ASD appearing in 1998. In 1980 Reuben was 33 years old and well into a successful career and in 1989 Reuben was 51 and building up for retirement.  Medical intervention and diagnosis at that time was not even considered. Currently it is estimated 1% of adults have an autism spectrum disorder (ASD) with this number expected to represent 700,000 U.S. seniors by 2030. Despite this, children and autism is the focus of research, intervention, diagnostics, in public media, and in the curriculum and experience of healthcare providers.  This leads to a misconception that ASD is only present in children, during childhood. Support and intervention systems are in place for the most part in early education and school systems, but once a person is no longer in school, the support systems for those age 20 or older with autism drops significantly.    According to the Autism Society 35 percent of young adults (ages 19-23) with autism have not had a job or received postgraduate education after leaving high school. (Shattuck et al., 2012) It is reported adults with ASD might also have other medical issues impacting their care and quality of life including diabetes, heart disease, and mental health issues such as depression.  Adults with autism are three times as likely to be depressed and five times as likely to attempt suicide as other adults. So what can do about this? It starts with the community.  STARS for AUTISM and Dr. Linda Barboa paved the way for towns and cities to be "Autism Friendly" with Blueprint for an Autism Friendly City. These same communities also trained their police force, firefighters, first responders, retail establishments, and anyone who works with the public, with STARS for Autism's education material.  Additionally, education for companies and co-workers is available with the Autism Friendly Workplace. A leading group of medical experts on Tuesday declined to endorse cognitive screening for older adults, fueling a debate that has simmered for years.
The U.S. Preventive Services Task Force said it could neither recommend nor oppose cognitive screening, citing insufficient scientific evidence of the practice’s benefits and harms and calling for further studies.
The task force’s work informs policies set by Medicare and private insurers. Its recommendations, an accompanying scientific statement and two editorials were published Tuesday in the Journal of the American Medical Association.
The task force’s new position comes as concern mounts over a rising tide of older adults with Alzheimer’s disease and other dementias and treatments remain elusive. Nearly 6 million Americans have Alzheimer’s disease; that population is expected to swell to nearly 14 million by 2050.
Because seniors are at higher risk of cognitive impairment, proponents say screening ― testing people without any symptoms — is an important strategy to identify people with unrecognized difficulties and potentially lead to better care.
“This can start a discussion with your doctor: ‘You know, you’re having problems with your cognition, let’s follow this up,’” said Stephen Rao of Cleveland Clinic’s Lou Ruvo Center for Brain Health.
Opponents say the benefits of screening are unproven and the potential for harm is worrisome. “Getting a positive result can make someone wary about their cognition and memory for the rest of their life,” said Benjamin Bensadon, an associate professor of geriatric medicine at the University of Florida College of Medicine.
The task force’s stance is controversial, given how poorly the health care system serves seniors with memory and thinking problems. Physicians routinely overlook cognitive impairment and dementia in older patients, failing to recognize these conditions at least 50% of the time, according to several studies.

When the Alzheimer’s Association surveyed 1,954 seniors in December 2018, 82% said they thought it was important to have their thinking or memory checked. But only 16% said physicians regularly checked their cognition.
What’s more, Medicare policies appear to affirm the value of screening. Since 2011, Medicare has required that physicians assess a patient’s cognition during an annual wellness visit. But only 19% of seniors took advantage of this voluntary benefit in 2016, the most recent year for which data is available.
Dr. Ronald Petersen, co-author of an editorial accompanying the recommendations, cautioned that they shouldn’t discourage physicians from evaluating older patients’ memory and thinking.
“There is increased awareness, both on the part of patients and physicians, of the importance of cognitive impairment,” said Petersen, director of the Mayo Clinic’s Alzheimer’s Disease Research Center. “It would be a mistake if physicians didn’t pay more attention to cognition and consider screening on a case-by-case basis.”
Similarly, seniors shouldn’t avoid addressing worrisome symptoms.
“If someone has concerns or a family member has concerns about their memory or cognitive abilities, they should certainly discuss that with their clinician,” said Dr. Douglas Owens, chair of the task force and a professor at Stanford University School of Medicine.
In more than a dozen interviews, experts teased out complexities surrounding this topic. Here’s what they told me:
Screening basics. Cognitive screening involves administering short tests (usually five minutes or less) to people without any symptoms of cognitive decline. It’s an effort to bring to light problems with thinking and memory that otherwise might escape attention.
Depending on the test, people may be asked to recall words, draw a clock face, name the date, spell a word backward, relate a recent news event or sort items into different categories, among other tasks. Common tests include the Mini-Cog, the Memory Impairment Screen, the General Practitioner Assessment of Cognition and the Mini-Mental Status Examination.
The task force’s evaluation focuses on “universal screening”: whether all adults age 65 and older without symptoms should be given tests to assess their cognition. It found a lack of high-quality scientific evidence that this practice would improve older adults’ quality of life, ensure that they get better care or positively affect other outcomes such as caregivers’ efficacy and well-being.
A disappointing study. High hopes had rested on a study by researchers at Indiana University, published in December. In that trial, 1,723 older adults were screened for cognitive impairment, while 1,693 were not.
A year later, seniors in the screening group were not more depressed or anxious — important evidence of the lack of harm from the assessment. But the study failed to find evidence that people screened had a better health-related quality of life or lower rates of hospitalizations or emergency department visits.
Two-thirds of seniors who tested positive for cognitive impairment in her study declined to undergo further evaluation. That’s consistent with findings from other studies, and it testifies to “how many people are terrified of dementia,” said Dr. Timothy Holden, an assistant professor at Washington University School of Medicine in St. Louis.
“What seems clear is that screening in and of itself doesn’t yield benefits unless it’s accompanied by appropriate diagnostic follow-up and care,” said Nicole Fowler, associate director of the Center for Aging Research at Indiana University’s Regenstrief Institute.
Selective screening. “Selective screening” for cognitive impairment is an alternative to universal screening and has gained support.
In a statement published last fall, the American Academy of Neurology recommended that all patients 65 and older seen by neurologists get yearly cognitive health assessments. Also, the American Diabetes Association  recommends that all adults with diabetes age 65 and older be screened for cognitive impairment at an initial visit and annually thereafter “as appropriate.” And the American College of Surgeons now recommends screening older adults for cognitive impairment before surgery.
Why test select groups? Many patients with diabetes or neurological conditions have overlapping cognitive symptoms and “it’s important to know if a patient is having trouble remembering what the doctor said,” said Dr. Norman Foster, chair of the workgroup that developed the neurology statement and a professor of neurology at the University of Utah.
Physicians may need to alter treatment regimens for older adults with cognitive impairment or work more closely with family members. “If someone needs to manage their own care, it’s important to know if they can do that reliably,” Foster said.
With surgery, older patients who have preexisting cognitive impairments are at higher risk of developing delirium, an acute, sudden-onset brain disorder. Identifying these patients can alert medical staff to this risk, which can be prevented or mitigated with appropriate medical attention.
Also, people who learn they have early-stage cognitive impairment can be connected with community resources and take steps to plan for their future, medically and financially. The hope is that, one day, medical treatments will be able to halt or slow the progression of dementia. But treatments currently available don’t fulfill that promise.
Steps after screening. Screening shouldn’t be confused with diagnosis: All these short tests can do is signal potential problems.
If results indicate reason for concern, a physician should ask knowledgeable family members or friends what’s going on with an older patient. “Are they depressed? Having problems taking care of themselves? Asking the same question repeatedly?” said Dr. David Reuben, chief of geriatrics at UCLA’s David Geffen School of Medicine and director of UCLA’s Alzheimer’s and Dementia Care program.
A comprehensive history and physical examination should then be undertaken to rule out potential reversible causes of cognitive difficulties, implicated in about 10% of cases. These include sleep apnea, depression, hearing or vision loss, vitamin B12 or folic acid deficiencies, alcohol abuse and side effects from anticholinergic drugs or other medications, among other conditions.
Once other causes are ruled out, neuropsychological tests can help establish a diagnosis.
“If I detect mild cognitive impairment, the first thing I’ll do is tell a patient I don’t have any drugs for that but I can help you compensate for deficits,” Reuben said. The good news, he said: A substantial number of patients with MCI ― about 50% — don’t develop dementia within five years of being diagnosed.
The bottom line. “If you’re concerned about your memory or thinking, ask your physician for an assessment,” said Dr. David Knopman, a neurologist at the Mayo Clinic. If that test indicates reason for concern, make sure you get appropriate follow-up.
That’s easier said than done if you want to see a dementia specialist, noted Dr. Soo Borson, a professor emerita of psychiatry at the University of Washington. “Everyone I know who’s doing clinical dementia care says they have wait lists of four to six months,” she said.
With shortages of geriatric psychiatrists, geriatricians, neuropsychologists and neurologists, there aren’t enough specialists to handle demands that would arise if universal screening for cognitive impairment were implemented, Borson warned.
If you’re a family member of an older adult who’s resisting getting tested, “reach out privately to your primary care physician and express your concerns,” said Holden of Washington University. “And let your doctor know if the person isn’t seeing these changes or is resistant to talk about it.”
This happens frequently because people with cognitive impairments are often unaware of their problems. “But there are ways that we, as physicians, can work around that,” Holden said. “If a physician handles the situation with sensitivity and takes things one step at a time, you can build trust and that can make things much easier.”

Father's Day bailout: 10 Philly dads get to spend the holiday with their kids

Ten Philly dads almost missed the chance to spend Father’s Day with their kids, until a local organization stepped in and deployed a crowdfunding effort to help them.
Criminal justice nonprofit Frontline Dads is running its first bailout program specifically geared toward the parenting holiday. On Thursday, executive director Reuben Jones coordinated the release of nearly a dozen fathers from Philly jails — so they could celebrate in person with their families this weekend.
“This is our opportunity to focus on fathers as caregivers of children,” Jones told Billy Penn. “Even though you may be incarcerated, sometimes you’re the only male figure a child may have.”
The Father’s Day Bailout was inspired by a maternal counterpart. The Mama’s Day Bailout is a national program, with a local iteration run last month by the Philadelphia Community Bail Fund.
To be clear, none of the dads who are getting bailed out this week have been convicted of a crime. Bail happens before that. After someone gets charged, a judge sets a price for their release while they await trial. If the person can’t afford the amount, they stay locked up until their case is heard — which can sometimes take years.
The system has been criticized for how often it mandates incarceration for people who haven’t yet been proven guilty of anything, but are just experiencing economic hardship. Philadelphians have advocated against the process, which has been shown to disproportionately affect people of color. Last year, Philly District Attorney Larry Krasner officially nixed cash bail for some low-level offenses.
“We’re hoping to shed some light on the need to end cash bail,” Jones said. “It denies people due process and makes them purchase their freedom.”
Among the dads who’ll benefit from Frontline’s new project is Jamal Walker, a 22-year-old father of two who lives in West Philly. When he got picked up for burglary last month, his family anticipated they’d have trouble getting him out.
Walker’s mom Rashena Carter lives in a shelter — and she knew she couldn’t cobble together his $500 bail.
“I didn’t have the money to get him out,” Carter said. “I would have had to be calling all types of people.”
Now that he’ll be out by Sunday, Carter is on pins and needles. Her plan? A Father’s Day cookout at Belmont Plateau with the whole family — her daughter, her son and his children, Jamal Jr. and Bella.
“His kids are going to be very excited,” Carter said. “Both of them light up when they see him.”
After Walker and his peers are released, they’ll get some additional help from Frontline Dads. On deck are free haircuts, clothes, job placement assistance and counseling services.
Still in its first year, Jones’ Father’s Day program hasn’t come together without at least one snag. Earlier this week, the bailout fund released a dad who won’t actually get together with his kids for the holiday at all — because he doesn’t have a good relationship with their mother.
The nonprofit director plans to use this misstep as an opportunity. Frontline Dads will provide the father with services to better his parenting skills and eventually get more time with his kids.
“The separation is a sad story on his part,” Jones added. “We’re going to help him navigate through the family court system to change that.”
In total, Frontline Dads is paying $10k to release the handful of Philly fathers — with about $1,500 coming directly from a community crowdfunding effort.
Though the donations will make an impact, they’re not quite as much as Jones wanted to bring in. He originally hoped to raise closer to $50,000 to bring even more dads home to their families. The lack of support, he said, is evidence that people often underestimate the importance of paternal care.
In its inaugural year, the Mama’s Day Bailout raised $60,000.
“It speaks to the stereotypes of this country, that mothers are primarily viewed as caregivers of children and fathers are looked at as more disposable,” Jones said. “There’s no outrage around fathers not being in the home for an important holiday.”

Latkes 2017!

Tonight is the first night of Hanukkah, and I find myself at home recovering from a broken left wrist! 
A week and a half ago my husband and I decided to resume our tennis careers (very loosely speaking), and go hit the ball around at the new courts at the local high school.  
As trying to volley after being away from the game for almost 30 years was not as much fun as we had hoped, we agreed to leave after just one more.
Unfortunately, and due to my stupidity competitive spirit, I decided to go for an out of reach ball, tripped on my own feet, and fell to the ground, racquet flying.
Within fifteen minutes we were at the emergency room of the hospital.
Diagnosis: a serious break requiring surgery which I had this past Friday with a fairly long recovery ahead.
I am fortunate to have excellent medical care available to me, and appreciate all the skilled nurses, caregivers, doctors, anesthesiologists, and surgeon who have treated me along the way.
It goes without saying I will miss cooking, baking and posting Hanukkah treats on this blog for 2017. 
So in the interest of trying to make the best of things I have decided to share some recipes with you, so you might enjoy them.   I will look forward to making them next Hanukkah!
Today, being the first I have been able to sit at my computer and type with one hand, I will be able to share Gail Simmon’s Mom’s Latkes with you!
I happened to see her on The Rachel Ray Show this morning as I was lying in bed channel surfing and resting.

A Week at Farm Sanctuary: Winding Down the Summer Days With Rescued Farm Animal Friends
Students across the country are heading back to school, and here at Farm Sanctuary’s New York Shelter, we’re feeling the early traces of fall arriving just around the bend! For our “humanimal” friends, this time of year can be one of new beginnings — a chance to recommit to aligning our actions with our values and making choices we can feel good about. New “Kids” on the Block
This week brought big changes for two of our residents in particular: Darius and Halbert goats! As you may recall, we rescued these boys last year from a so-called “humane” farm-to-table operation in Cattaraugus County, NY. But there was nothing humane about the circumstances from whence they came: The boys were emaciated, riddled with parasites, and too frightened to even venture outdoors. After receiving care and kindness at Farm Sanctuary, however, they began to heal and learn to love again.

Sweet Darius. These affectionate boys want nothing more than to love and be loved. They playfully wag their tails and lick our faces, just like dogs!
Darius and Halbert were doing so well that we decided to place them in a loving home through our Farm Animal Adoption Network! We are so grateful for the compassionate families who open their homes and hearts to rescued farm animals — helping us to rescue and rehabilitate even more individuals in need.
Sometimes, though, circumstances happen that are beyond our control. A few months ago, Darius injured his leg, and his adoptive family contacted us to connect him with the very best care available. We brought Darius to the Nemo Farm Animal Hospital at Cornell University (along with his best friend Halbert for company, of course!), and he convalesced with his buddy at our onsite Melrose Small Animal Hospital. Thankfully, Darius has healed nicely — and with his adopter’s blessing, Darius and Halbert will stay at Farm Sanctuary in order to give Darius easy access to any follow-up care that he needs.
This week, Darius was doing so well that we moved the boys from the hospital to a pasture of their very own! In their new spot, they spend their days climbing, running, and grazing — free to just be themselves.

This gentle giant is a friend to all, demonstrating that farm animals just like him are each someone, not something.
Merlin is a great example for calves like Stanton! Along with his adoptive moms Jackie and Nancy, Stanton loves living in our special-needs herd and learning how to be a calf.
