Understand the benefits of bariatric surgery
Bariatric surgery, also known as weight loss surgery, is a medical procedure designed to help people who are severely obese lose weight and improve their overall health. There are several different types of bariatric surgeries, but they all aim to reduce the size of the stomach or bypass a portion of the digestive tract, leading to reduced food intake and nutrient absorption. The benefits of bariatric surgery include:
Tumblr media
Significant and sustained weight loss: Bariatric surgery is highly effective in achieving substantial weight loss, especially in individuals who have struggled to lose weight through conventional means like diet and exercise. The amount of weight loss varies depending on the type of surgery and the individual's commitment to lifestyle changes, but most patients experience significant and long-term weight reduction.
Improved obesity-related health conditions: Bariatric surgery can lead to the resolution or improvement of various obesity-related health conditions, such as type 2 diabetes, high blood pressure, sleep apnea, high cholesterol, and joint pain. This can greatly enhance the patient's overall health and quality of life.
Enhanced cardiovascular health: With significant weight loss, patients often experience improvements in their cardiovascular health, including a reduction in heart disease risk factors like high blood pressure and high cholesterol levels.
Remission of type 2 diabetes: Some patients with type 2 diabetes may achieve remission or significant improvement in their condition after bariatric surgery. This occurs even before substantial weight loss and is thought to be related to hormonal changes that affect insulin sensitivity.
Long-term sustainability: Unlike many fad diets, bariatric surgery provides a more sustainable approach to weight loss. Patients tend to maintain their weight loss over the long term, leading to better health outcomes and a reduced risk of weight-related complications.
Enhanced mental health and well-being: Many patients experience improvements in their mental health, self-esteem, and overall well-being after successful weight loss. This can result in increased confidence and a more positive outlook on life.
Increased mobility and physical activity: As patients shed excess weight, their physical mobility often improves, making it easier to engage in physical activities and lead a more active lifestyle.
Reduced mortality risk: Bariatric surgery has been associated with a decrease in overall mortality risk, particularly in patients with severe obesity. By addressing  obesity-related health conditions and promoting weight loss, the surgery can help improve life expectancy.
Despite these benefits, it's important to note that bariatric surgery is a significant medical intervention and not suitable for everyone. Candidates for the procedure are carefully assessed by healthcare professionals to ensure that the potential benefits outweigh the risks. Additionally, successful outcomes require a commitment to lifelong dietary and lifestyle changes to maintain weight loss and optimize health benefits.

Splenectomy Treatment
What is Spleen?
The spleen is a delicate, fist-sized organ in the upper left region of the abdomen. To be precise rests under the left rib cage near the stomach, which is an important part of the defence system of the body. It plays a role in the body’s blood production and immune system since it contains special white blood cells. Spleen also helps to remove or filter old red blood cells from the blood in circulation.

BEST OBESITY TREATMENT HOSPITAL IN INDIA.
What is splenectomy?
Splenectomy is a surgical procedure to remove the whole spleen. The procedure is called a partial splenectomy, where only a part of the spleen is removed,
What to expect during the splenectomy surgery?
The Splenectomy procedure is performed under general anaesthesia. The surgeon places a cannula that is a hollow tube, into the abdomen to inflate it with carbon dioxide gas to create a space for the operation. A laparoscope, a tiny telescope connected to a video camera, is put through one of the cannulas which projects the video of the internal organs and spleen on a video monitor. Several cannulas are placed in through multiple small incisions on the patient’s abdomen to allow the surgeon to place instruments inside the belly to operate and remove the spleen. A search and removal of any accessory that is extra spleens are performed since 15% of people have small, extra spleens. After the spleen is separated by cutting it from all that it is connected to, it is placed inside a special bag, morcellated that is broken into pieces and pulled out in the bag from the largest incision on the abdomen to remove it completely. Depending on the condition of the patient, the laparoscopic method may be converted to the open procedure such as a history of prior abdominal surgery causing dense scar tissue. The decision to perform the open procedure or to convert from the laparoscopic approach is made by the surgeon either before or during the actual operation, based on the patient’s condition.
What to expect after the splenectomy surgery?
After surgery patient is given Intravenous Fluids in the arm and pain medication to relieve the discomfort experienced from the small incisions. The surgeon gives the patients, discharge from the hospital when the patients resume oral food intake, urination and taking care of their own basic needs. Most patients return to their normal routine in a week after returning home.

Comparing Sleeve Gastrectomy to Other Bariatric Surgery
Tumblr media
Bariatric surgery is one of the most efficient ways of treating obesity and sleeve gastrectomy is one such type of bariatric surgery. During gastric sleeve surgery, a portion the stomach is removed which results in the remaining stomach has a much smaller capacity, which physically limits the amount of a person can consume at one time. Your remaining stomach is narrower, around the shape and size of a banana. This results in feeling full faster and allows you to control your hunger and eat less food.
In this blog we will explore the advantages of sleeve gastrectomy over other bariatric surgeries with insights from Dr.Shashank Shah:
Sleeve Gastrectomy vs Intragastric Balloon
As compared to the Gastric balloon, the sleeve gastrectomy leads to drastic weight loss. Sleeve gastrectomy is suitable for people who want to lose more than 40 pounds.
2. Sleeve Gastrectomy vs Roux-en-Y Gastric Bypass Surgery (RYGB)
As compared to sleeve gastrectomy, Roux-en-Y Gastric Bypass Surgery (RYGB) is much more complex as it involves creating a small stomach pouch and rerouting the intestines. RYGB also bypasses a portion of the small intestine, potentially leading to a higher risk of nutritional deficiencies over time.
3.Sleeve Gastrectomy vs Mini Gastric Bypass
Sleeve gastrectomy is more simple than mini gastric bypass. This results in less complications during surgery. Also, the risk of dumping syndrome after mini gastric bypass is much greater than sleeve gastrectomy.
4.Sleeve gastrectomy vs Duodenal Switch
SG presents a reduced likelihood of nutritional deficiencies in contrast to DS. DS entails bypassing a substantial segment of the small intestine, potentially resulting in inadequate absorption of essential vitamins and minerals, thus requiring ongoing supplementation. SG is commonly viewed as being more easily reversible compared to DS. When necessary, SG can be transformed into DS or a different bariatric procedure, providing a range of treatment choices.

He has the following qualifications, M.B.B.S., M.S., FAIS, Honorary FMAS, Honorary FIAGES, Honorary FALS, Honorary FMBS, Dip. Visiting Prof. France, Medical Council Reg. №61837. Intragastric Balloon for weight loss
What is intragastric balloon :
Intragastric balloon placement has been started in India at Laparo Obeso Centre since 2006 and LOC is known to be one of the pioneer balloon clinics in India. Since then, LOC has trained and certified many endoscopists and nutritionists. Intragastric balloon is a non-invasive modality for weight loss. However, since the balloon needs to be removed after a period of time, it is accepted as a temporary method of weight loss.It is popular as a nonsurgical weight loss method for those who do not wish to undergo bariatric surgery or do not need surgery. A silicon or similar balloon is filled with saline and occupies the space in the stomach. The material of the balloon prevents it from being digested and hence stays in the stomach.Balloon can be swallowed or endoscopically placed in the stomach under sedation.
Who should undergo intragastric balloon for weight loss?
Those who can’t lose weight after diet and exercise
Ideal for those with an excess weight up to 10 to 15 kg
PCOS
Those with infertility needing weight loss
Failed IVF and with obesity
Cosmetic reasons
Balloon therapy is not ideal for patients with morbid obesity who require bariatric surgery.
How does gastric balloon work?
The balloon occupies the space in the stomach limiting the food intake. However, a person needs to be careful with the caloric intake to get the maximum benefit of weight loss. The balloon in the stomach gives a feeling of fullness and reduces hunger and cravings. However, the effect of balloon starts wearing off slowly and the person needs advice from a qualified bariatric nutritionist to learn to keep the food intake and calories to the lowest.
Is balloon a day care procedure?
Intragastric balloon placement is known to be a day care procedure and one can be discharged in about 3 to 4 hours. Balloon is placed under short sedation and filled with saline under vision through an endoscope or gastroscope. could be some discomfort or pain for the first few days since the stomach would contract and attempt to digest the balloon. Some patients may need intravenous medications to suppress nausea and discomfort or pain after the balloon insertion. The discomfort usually disappears after about two to four days.
How much weight can be lost after the balloon?
It is generally said that a person can lose about 5 to 10 percent of weight. The results are variable based on the excess weight, BMI, metabolism and dietary compliance. The weight loss is maximum in the initial 6 weeks and may slow down over time. Maintenance of weight loss is necessary with a proper follow up.
What is a swallowable balloon or a balloon pill?
These balloons are available as large capsules and can be swallowed by a person, with some difficulty. It is then confirmed under X ray vision that the balloon is inside the stomach. The balloon is attached to a tube through which saline can be instilled inside the balloon and the tube can be pulled. Some balloons rupture and pass out in about two to three months. The weight loss effect lasts for a very short time and needs to be maintained. This method is expensive and weight regain could be very rapid after the balloon passes out.
Can a gastric balloon be kept inside the stomach for a year?
There are balloons which can be left inside the stomach for one year, so that the weight loss benefits can be sustained for longer period. ‘Spatz’ balloons are adjustable balloons which can be left inside for a year. The word adjustable means that the same balloon can be further inflated with more saline so the effect of fullness and satiety can be augmented if the weight loss slows down or stops during the period of one year. There are different types of balloons which can be kept up to 3,6 or 12 months. The effect of the balloon lasts longer if the balloon can be kept inside for a longer period.
Balloon removal procedure:
It is an outpatient procedure. Patient reaches the clinic fasting and under short sedation the saline/ fluid inside the balloon is sucked out with a special needle passed through the endoscope. Patient d
Can balloon rupture or leak?
The incidence of balloon leak is very low. However, the balloon is filled with saline tinged with a water-soluble dye known as methylene blue. If a balloon leaks, the methylene blue leaks in the stomach, absorbed and excreted through the urine. Urine turns bluish green and the person can immediately know it. In case of a leaked balloon, it can be safely removed or replaced with a new one. Balloons are made with silicon and hence can not rupture.
What are the contraindications for a balloon?
People with acidity or reflux disease are not recommended to have gastric balloon placed. Ideally, an endoscopy is recommended to ensure whether it can suit a person. Those on aspirin, blood thinners, pain killers are generally requested to avoid balloon placement.
What are the side effects of gastric balloon?
Side effects of balloon are usually mild and well tolerated. Nausea, vomiting, abdominal discomfort or pain are the most common side effects seen initially and can be controlled easily with medications. Acidity is sometimes a challenge especially after spicy food and should be treated with antacids. Vitamin and protein supplements are needed to keep the nutritional status healthy.

The Science Behind Duodenal Switch Surgery: How It Leads to Weight Loss and Improved Health-By Dr.Shashank Shah-The best bariatric surgeon in Mumbai and Pune
Tumblr media
Duodenal Switch (DS) surgery is a powerful and effective bariatric procedure that has helped countless individuals achieve significant weight loss and improve their overall health. But how exactly does this surgery work, and what makes it so effective? In this blog, we will delve into the mechanisms behind DS surgery, exploring how it leads to weight loss and the myriad health benefits it offers.Duodenal Switch
Understanding the Duodenal Switch Procedure
The Duodenal Switch is a complex bariatric surgery that combines restrictive and malabsorptive elements to promote weight loss. It involves two main components:
Sleeve Gastrectomy: The first part of the surgery involves removing a large portion of the stomach, creating a smaller, tube-like stomach (sleeve). This significantly reduces the stomach’s capacity, limiting the amount of food you can eat at one time and promoting early satiety (feeling full sooner).
Intestinal Bypass: The second part of the procedure involves rerouting a portion of the small intestine. The duodenum (the first part of the small intestine) is divided, and the lower part of the small intestine is connected to the new stomach pouch. This creates a bypass, allowing food to bypass a significant portion of the small intestine, where most nutrient absorption occurs.
How DS Surgery Leads to Weight Loss
The Duodenal Switch promotes weight loss through several key mechanisms:
Restriction: By reducing the size of the stomach, the DS limits the amount of food you can consume in one sitting. This helps you feel full more quickly and reduces overall calorie intake. The smaller stomach also slows the passage of food, helping you stay full for longer periods.
Malabsorption: The intestinal bypass reduces the length of the small intestine available for nutrient absorption. This means that fewer calories and nutrients are absorbed from the food you eat. While this can lead to significant weight loss, it also necessitates careful management of vitamin and mineral intake to prevent deficiencies.
Hormonal Changes: The removal of a portion of the stomach and the rerouting of the intestines lead to changes in gut hormones that regulate hunger and satiety. For example, levels of ghrelin (a hormone that stimulates appetite) are reduced, while levels of peptide YY (a hormone that promotes satiety) are increased. These hormonal changes help decrease hunger and improve feelings of fullness, supporting weight loss efforts.
Health Benefits of Duodenal Switch Surgery
Beyond weight loss, the Duodenal Switch offers numerous health benefits, particularly for individuals struggling with obesity-related conditions. Some of the key health improvements include:
Improved Diabetes Management: DS surgery is highly effective in improving blood sugar control and, in many cases, can lead to the remission of type 2 diabetes. The hormonal changes and reduced nutrient absorption contribute to better insulin sensitivity and glucose metabolism.
Resolution of Metabolic Syndrome: Metabolic syndrome, characterized by a cluster of conditions like high blood pressure, high blood sugar, and abnormal cholesterol levels, is often resolved after DS surgery. The weight loss and metabolic changes help reduce the risk of cardiovascular diseases.
Better Cardiovascular Health: Significant weight loss and improved metabolic health lead to lower blood pressure, improved cholesterol levels, and a reduced risk of heart disease and stroke. These benefits contribute to overall cardiovascular health and longevity.
Enhanced Mobility and Joint Health: Carrying excess weight puts tremendous strain on the joints, leading to conditions like osteoarthritis. Weight loss after DS surgery alleviates this pressure, resulting in reduced pain and improved mobility.
Improved Sleep Apnea: Obstructive sleep apnea, a condition characterized by interrupted breathing during sleep, is commonly associated with obesity. DS surgery can lead to a significant reduction in sleep apnea symptoms or even complete resolution, improving sleep quality and overall well-being.
Enhanced Quality of Life: The physical and emotional burden of obesity can impact all aspects of life. By achieving significant weight loss and improving health, individuals often experience enhanced self-esteem, better mental health, and a higher quality of life.
Long-Term Commitment to Health
While the Duodenal Switch surgery provides powerful tools for weight loss and health improvement, it requires a lifelong commitment to healthy habits. Patients must adhere to a nutrient-rich diet, take vitamin and mineral supplements, and engage in regular physical activity to maintain their weight loss and overall health. Regular follow-ups with healthcare providers are essential to monitor progress and address any potential complications.
