holding on {alex karev}
plot: you and alex aren't friends but he's the person that sits by your hospital bed day and night until you wake up.
character: alex karev (early seasons) x reader
The steady beeping of the various machines was something that Alex had grown tired of two days ago, the machine's volumes had been turned to 0 but his anxious eyes kept flickering to them every few seconds just to be sure. The background noise of the hospital was something he was used to and it was an oddly comforting sound. Now, the silence of being in the room with you had been nice at first but now that he was here, with you, waiting... just waiting... the silence was unnerving him.
He tapped his foot, checking the clock on the wall. Bailey should've been here by now, she promised him that she'd check on you every two hours. She was late. Anger surged through his body causing his heart to pound and his fists to clench.
"You're such an idiot," he could hear you scolding him in his mind, "if you just stopped dealing with your problems with sheer anger then maybe, maybe people would actually start to like you."
He scoffed.
You and him had hardly been friends. You and the rest of Bailey's interns were the best of friends, all living together in Mer's mom's house so why wasn't George or Izzie or Cristina or Mer here? Why was it Alex? That's all the four of them had been whispering about. Cristina asked Alex, Izzie asked Alex... hell, Bailey even asked Alex. Alex had ignored each of their questions and instead gave some snarky asshole comment with an eye roll. Alex didn't even know why he was here - why he'd purposefully demanded the week off to be by your bedside day and night sleeping on a camping bed with the scratchiest sheets in the world. He didn't know and yet, here he was.
You were annoying. You annoyed him. But since the news of the accident and since you'd been in a coma, Alex couldn't stop thinking about the way you laughed as you teased him. He couldn't get one specific moment out of his head.
You and Alex had been working on a case together - much to your dismay - and Alex had opened up slightly, letting you see that he was much more than what you previously thought.
"So... you're not just an asshole with the emotional range of a teaspoon, who knew?" You helped yourself to the bar stool next to Karev. Joe glanced at you, asking if you wanted your usual to which you nodded.
Alex rolled his eyes, "Whatever."
There was silence for a few seconds before you tried again, "I know you have this hard 'I don't care' exterior," you started, "and I know it's probably because of some past trauma in your life, Karev - believe me we've all got some shit - but..."
"Are you gonna keep giving me a stupid high school girl pep talk or are you gonna shut up and drink?"
It was your turn to roll your eyes now, "Joe, another round please."
As Joe poured the two of you more drinks, Alex sighed and looked at you, "Thanks," he murmured quietly, "I'm not- I don't..." he cleared his throat, "I don't mean to be an asshole all the time... I don't really know... Social shit isn't really my thing."
"Now who's acting like an emotional high school girl?" You teased. Alex laughed, a genuine smile stretched onto his face. Yeah... maybe he wasn't so bad after all.
So after the accident, Alex stayed.
It was then Bailey strode in, chart in hand, "Karev," she said glancing up for a second, "you look like hell. Don't you think you should go home get a proper sleep? Take a damn shower?" She could see the worry in him, she could see how stressed out he was; the dark circles under his eyes, his nails chewed down. Alex might not even know it yet but he cared about you.
"I'm staying," he said with a nod standing to look over her shoulder at your chart, "Any updates?"
"You tell me, you're the one who's been here since she got admitted." Bailey moved to you, turning the volume up on the machines, checking your levels.
"Oxygen levels were a little low at 3am, managed to level them out... No issues since." He nodded, arms crossed with a hand rubbing at his jawline, "Why hasn't she woken up yet, Bailey? She should be-"
"Karev," Bailey said, voice strong, "Go get yourself a cup of coffee, now."
"I don't-"
"Now, Karev. Let me do my damn job and stop hanging over me. Coffee."
With a few harsh words which made Bailey surprisingly laugh, Alex stormed out of your hospital room, storming past O'Malley and Stevens who had come to check in with Bailey on how you were doing.
Bailey leaned down closed to you, "If you die, god help us all... that boy..." she looked to the door where Alex had left from, "he'll be lost forever. So don't you dare, you hear me?"
The coffee machine was a minute's walk away from your room so Alex would know if anything were to happen to you, he would know but he kept checking over his shoulder anyway just in case. He was exhausted, he couldn't remember the last time he'd drank or even the last time he'd eaten. You had consumed him for the last two days; making sure that you were okay was his first priority.
He stopped at the coffee machine punching the button for a crappy black coffee that he wasn't going to drink anyway, "Come on," he grumbled as the cup dropped and the coffee began to pour in slowly, "Damn piece of crap machine, hurry the hell up!" He yelled suddenly, slamming his fist into the plastic front. Around him, people stared but he didn't care. When the coffee finally stopped, he pulled the cup out when he heard it.
"Code blue! I need a crash cart! Room 2203!" It was Bailey. It was you.
Boiling hot coffee splashed over the floor, the cup dropped and on the ground as Alex Karev took off running.
His heart pounded, usually the thrill was the thing he loved the most but this wasn't a thrill, no, this was dread. When he burst into your room, the first thing he heard was, "Clear!" and heard the noise of the defibrillator.
"What's going on?!" He yelled over the chaos.
"Get him outta here!" Bailey yelled, "Charge to 200! Get him outta here, O'Malley!"
George tried but a determined Alex was a strong Alex. He resisted George's grip, shoving him back every chance he tried to take him out. It got to the point that George gave up, "Dr Bailey!" He exclaimed, hopelessly as Alex barged to your bedside. Bailey couldn't do anything, she was busy trying to save your life, she couldn't deal with Karev as well so she let him be.
"Don't you dare die on me," Alex hissed, eyes flooding with tears, "don't you dare. Can't do that to me, (y/n). Can't have me sitting here waiting for two days to just die on me-" he looked to Bailey, "Save her... please."
Bailey's eyes met Alex's and she found a lump in her throat, "You hear him?" She asked you as the paddles charged, "don't you dare die on us, (y/n)." With one final shock, the monitor started to beep again, "Heart rate is coming back up," she said with a relieved sigh, "Thank the Lord. Levels are stabilising."
Alex collapsed into the chair at your bedside, hand clamped around yours, as his eyes closed, letting the relief wash over him. You were alive; you were stable.
"What- what caused it?"
Bailey shook her head, "Don't know, levels were fine but as soon as you left the room they started to drop so do me a favour, Karev," she looked pointedly at him, "don't leave this room again." Normally he would've bit back, said a comment about her forcing him out but instead, he just nodded falling back into his chair, hand still in yours, "I'll check every hour, okay? You page me immediately, got it?" Again, he nodded and then the room cleared out.
Alex didn't turn the monitors down, he needed to hear the steady beep to know that you were okay, you were alive and you were breathing. For the last three hours that he'd sat here, he had prayed to every god he could remember the name of - he didn't know if it counted but even started praying to some Greek Gods as well. Why have God in the title if it doesn't count? His hand was still firmly in yours.
Bailey had checked five times in the three hours, checking on you but also on him. She brought him a soda, a sandwich and a muffin and didn't leave until he'd drained half the can and eaten one of the sandwiches. He hadn't realised how hungry he was until he'd started eating, he devoured the rest of meal once she'd left. You were still stable but you weren't awake yet. Bailey was optimistic but Alex wasn't. He was dreading the worst, expecting your levels to become unstable again but as he was dosing off, he felt your hand twitch in his.
He shot up, "(y/n)?" He asked staring at your hand and then at you and much to his relief, your eyes began to flutter open. He let out a sound that was half laugh, half sob. Utter relief crashed over him, "You're awake," he grinned, "you're actually awake."
"A-Alex?" You croaked.
"Here," he said gently as he grabbed a plastic cup and straw and filled it with water from the jug on your bedside unit, "Drink up. How you feeling?"
"Sore."
"Multiple ruptured organs and a few broken bones'll do that to you," Alex teased with a smile. You noticed his hand was still in yours, warm and strong. He saw your eyes narrow at your joined hands and he was quick to pull his back despite everything inside him not wanting to, "I- I'm gonna page Bailey, you drink up." He helped you take the water and left. He was just outside, close enough to make sure that you were still safe - still alive.
It was as he left you looked around the room and you noticed the camping cot which was set up on the floor next to your bed. You frowned. Someone had been staying here. Was it... no, it couldn't have been Alex; Alex hated you.
Your thoughts were cut off by Bailey bursting into your room, "Oh thank the Lord," she grinned, "it's good to see you awake. You scared us." As Bailey checked you over, Alex returned to the seat next to you. Bailey saw your confused expression seeing him sat there, normal clothes not working, "Karev," she said, "go and get (y/n) a sandwich, will you? She's hungry." Alex went to argue, to tell Bailey she told him not to leave your room but Bailey's pointed look made him stop and nod. He left a second later giving you one last worried look, "She's fine now go."
You looked up at Bailey who sighed and looked down at you, "You had that boy scared to death, you know."
"Who? Alex?!"
Bailey nodded, "You're not the only one who's surprised. As soon as you were admitted he was here. It was his day off and he was here. Soon as you got outta surgery he was set up in your room. He hasn't left since Tuesday."
You looked down to the cot next to you, "He's been here the whole time?"
Bailey nodded, "I don't know what's going on between the two of you - if anything - but I'd say that there's something." Your frown deepened and Bailey smiled, "Just... be patient with him."
When Alex came back, Bailey gave you a secret nod with a knowing smile before she left promising to come check on you every hour and to not dare think about going back into a coma otherwise she would kill you. "I'm a doctor, I know how to save people but I know how to kill them too."
"Hey," Alex said as he placed a sandwich and soda on the unit beside your bed, "You okay?"
You nodded, finding yourself rather overwhelmed and touched by his actions. He - Alex Karev - had stayed by your side since the accident. What did that mean? What did Dr Bailey mean? You nodded quickly, "Yeah," you said softly, "just tired."
Alex puffed out a long breath as he sat in the seat next to your bed, "Yeah, you must be. Gave me- gave us all a fright."
Silence fell and the two of you fell into the comfort of the sounds of the hospital. You sipped at the soda Alex had brought before curiosity got the better of you, "Alex... why did you stay with me?"
You could've sworn his cheeks flushed a darker shade of pink but he rubbed his hands over his tired looking face so you couldn't have been sure, "Hell if I know," he muttered, "it's not like we're friends but... I didn't want you to be alone. You're the only one that's almost like a friend and... I dunno." He shrugged, "I don't really understand it myself." Maybe there was something deeper lurking under the surface but he didn't know. That was something you'd have to navigate together, "I know you'd have probably preferred Cristina or Mer-"
You took his hand and gave it a gentle squeeze, "Thank you, Alex," you whispered, "for everything. Thank you." Now, this time you could see the tips of his ears go pink. You smiled, "Now when are you going to shower cause boy you are looking rough-"
"Shut up!" Alex rolled his eyes but he laughed with you and for once, it felt nice to joke around with him. It felt normal. You didn't know what was going to happen but you somehow knew that he would be beside you, figuring out this crazy journey together and somehow, that made it a little less scary.

Dating Alex Karev headcanons
Paring: Alex Karev x Grey!reader
Summary: headcanons about dating Alex Karev -SMUT warning
A/n: I had fun writing this one :)
🩷MasterList ML2 🩷Dating Mood Board
Enemies to lovers kinda...
Your first encounter with him was your first day intern year. His locker was next to yours and you could feel him staring at you as he sat on one of the benches.
“nice tattoos” he smirked, shining a flashlight on your tattooed covered arm. Before you could replie, your Cousin Meredith pulled you away. “don't talk to her” she said a little too defensively
You and Alex butted heads a lot at first, you didn't like his attitude and you thought he was a total ass. Then you thought you were ridiculous for thinking he was hot.
After a few months you guys did become buddies. You'd make dark or dirty jokes together.
He would ignore his feelings for you. He knew you deserved better and he didn't want to add anymore darkness to his life.
He really didn't start showing his softer side until your aunt Elis came to the hospital. You opened up to him about how your aunt adopted you, but never really cared about you. He wanted to tell you about his crapy childhood, but wasn't ready to open up just yet.
He was just ready to be your shoulder to cry on.
You did ended up going to the hospital prom together. You asked him. “you wanna crash the prom together?” he down played his excitement, he was over the moon excited on the inside but on the out side he shrugged and said this nonchalantly: “sure, I guess we'll suffer together”
But that he wasn't suffering at all, he was so happy to have you in his arms all night. And for the first time he didn't care he was showing his softer side to the hospital. He just enjoyed himself and danced with you most of the night. “wow, Alex Karev can dance”
He just smirked and twirled you, he brought you back to his chest shaking his head playfully. “Shut up”
The first song you guys danced to was Jeff Healey's Angel Eyes, it would later be your guys song. At first he'd never admit out loud how much the word hit him right in the heart, lyrics like:
'There's just one more thing that I need to know, If this is love why does it scare me so? It must be somethin' only you can see, 'Cause girl I feel it when you look at me'
You weren't gonna tell him how nervous you felt to be there with him, you never went to your high school prom. It was nice to be surrounded by all the fairy lights and dance with a guy you've liked for a long time. Alex did catch on quickly and wanted to know what was on your mind. “what's wrong?”
You shrugged, feeling a bit embarrassed. “this is my first prom... I never really experienced my high school prom” Alex looked suprised. “why's that?”
“no guy ever really liked me or found me attractive enough to give me a chance... I was kinda of a geek back then”
“thoes guys are idiots. I would have killed to be with someone like you in high school”
You definitely saw something there in him that wasn't there before. You knew he was more than just a closed off jerk and that night only confirmed it. That same night he kissed you.
The kiss wasn't rough like you expected, it was gentle and sweet. He held the side of your face and kissed like you were the only person he wanted.
After that night he constantly flirted with you, making you blush or smirk.
Even when you started dating he would intentionally give you awful pick up lines just to make you laugh. “I hope you know CPR ‘cuz you just took my breath away”
“Alex... We're dating you don't need pick-up lines” he would just shrug and kiss your cheek. “I know, I just like making you laugh”
He's a total softy around you. He might seem like an ass to everyone else and people(especially George) might not understand why your with him, but he treats you like a queen.
Meredith did put the fear of god in him when you started dating. Like really did, Meredith and him were in a OR and she was holding a scaple and threatening his life when she found you and Alex were dating. “calm down, Mer. You think I'm gonna lose the most amazing thing in my life”
You were glad he was such a good friend to Izzie, especially when she lost Denny, but you couldn't help but be worried about their friendship later on. You and Alex had learned the hard way to be open with each other, so when it become a problem you decide to talk to him. “your just friends... Right?” you asked nervously. Alex gave a confused look and before he could awnser you talked over him in a nervous pace. “because she's a Model, Alex... She tall, blonde, and Beautiful... Any guy would want her... I'm short, I have tattoos, and dyed hair”
Alex cut you off by gently holding your face in his hands. “your what I want... Got it? I think your sexy as hell... And as for the dye job and tattoos I think is beautiful on you”
If your in the kitchen cooking he's under your feet sampling everything. It's even worse when you get a dog because you have to move around them both. “Alex, quit eating! I'm trying to cook”
“woman, I'm hungry!”
He probably didn't get decent home cooked meals till he started dating you. He loves your cooking.
When there's nothing on TV you'll watch old re-runs of Walker Texas Ranger. Which leads to walker and Chuck Norris inside jokes.
You also had conversations that's only normal to you two. Your friend learned that when you guys were in a OR gallery eating chips and ignoring the sugury. “who'd win in a fight Walker or Patrick Swayze?”
“if we're talkin' 'Road House' Swayze then, Patrick” Alex replied like it was the most obvious thing.
“but Chuck Norris knows martial arts” you reminded Alex. “but Swayze ripped a guy's throat out in 'Road house' ” Alex fired back. Alex loves 'Road house', it has Swayze and Jeff Healey(who sings your guys song).
“weirdest conversation” Izzie said, over hearing it. “always” Meredith added.
You guys stay in Derek's trailer for a bit and it wasn't a good experience for either one of you. He hates bears and you hate Snakes. “Alex! There's a fucking snake in the porch!”
“I'm not going out there! there's bears out there too, what do you want me to do about?”
You shrugged and annoyed with him and scared by the snake. “What? Are we just supposed to spend the rest of our lives in here?”
You stopped listing to the song House of Pain by Faster Pussycat around Alex because you can tell the words hit a little too hard for him and upset him.
Alex shrugged. “sounds good to me” he walked pasted you and towrds the bed. “wonder if Walker is on tv”
Since he's Evil Spawn his nickname for you is Angel.
He'd confide in you and open up to you about his past and family. He wanted to be a better man than his father was and he sure as hell wanted to treat you better than his father treated his mother.
The day his father showd up at hospital as a patient he kepted you far away from him. “that baaterd doesn't deserve to meet or know someone as amazing as you... Promise me you'll stay away from his hospital room”
He's a girl dog dad, plain and simple. He might only admit it to you though. You brought home a female Chiweenie puppy one day and he immediately fell in love with the pup. But they also have a love hate relationship. “that little bitch bit me” and then next hour their cuddling on the couch. “such a sweet little girl” he smiled scratching behind the puppy's ear.
You named her Baby after Jennifer Grey's character in Dirty Dancing
After the puppy calmed down and was more calmer, you guys got a license to bring her in for the Peds patients to see and cuddle while they were in the hospital.
Even though your an amazing cook to him, you guys get take-out alot due to your busy work schedule.
Sometimes you'll so tired to even eat something big and you'll end up sharing a large order of French fries.
Physical touch is his love language.
This man need physical contact a lot too. He's a big cuddler and just needs you in his arms, he's really just a big ol' teddy bear.
You joke about that, he can be such a cocky jerk and tough but he's really just a big teddy bear.
“I'm not a teddy bear” he said trying to not to smile. You shook your head and held him tighter. “your right... Your MY teddy bear”
He also loves forehead kisses, it's just such a soft and he loves doing gentle gesture like that to you. It just makes your relationship so much more intimate and real to him.
Alex refuses to sleep unless you have fallen asleep first, he just needs to know your okay before he can have a good night sleep. He really stated doing this after the hospital shooting.
He may be protective of you, but your definitely more protective. He finds is adorable and hot when you get all made and try to be his guard dog. “listen here Nurse syphilis...”
He calls you his angry Chihuahua just to mess with you.
He's not afraid to defend you either, he'd be damed if someone talked bad or down to you. He knows you and Christina don't get along and he's not afraid to put her in her place if she goes to far. “Back off Yang! I mean it”
He's a rough kisser, he's way taller than you so usually he hold your chin between his fingers and lift for head up to kiss you.
NSFW headcanons:
He has a tendency to grip the headboard when he's close to cumming.
He's a soft/mean Dom, it just depends on his mood. But no matter what he always makes sure your comfortable. He loves to take control in the bed but would never push you.
He loves missionary, keeping eye contact while he fucks you. He loves how you dig your nails into his back and wrap your legs around his waist to bring him impossibly closer.
Loves eating you out, your legs around his head. He loves your legs in general and loves leaving kisses on the insides of your thighs. He'd rather pleasure you for hours than receive.
He love getting head. I feel like he’d like having you on your knees. Plus, feeling your lips around him pushes him closer to cumming in your mouth.
Has a soft spot for sex in the shower, he loves holding you up against the wall, roughly while he drives his cock into you.
He has big chocking kink, he won't be too rough about but he loves wrapping his fingers around your neck and feeling your pulse when he's ramming into you.
He'll mark your thighs with his teeth and biting hard enough to leave a light bruise.
He doesn’t keep quiet. A good amount of teasing, dirty talk and the way he grunts...
Alex loves gripping your hips as you ride him, leaving his impressions on your thighs as you move, chasing your highs.
His favorite thing is probably you riding him. He loves gripping your hips, controlling your pace.
You may be in top, but he's in control. But that doesn't mean he loves the fact your using him to chase your high.
he loves grabbing you by the hips, literally squeezing his fingertips against your soft skin, holding you in the place or using them to help guide the pace.
One of his biggest kink is cumming inside you, seeing you filled up just makes him go crazy.
God, he loves anything to do with your breasts. He loves it when your sitting on his lap, grinding against his bulge while his face is buryed in your breasts, kissing and marking them up.
Honestly, you grinding yourself on him in someway and getting yourself off turns him on so much.
He loves using his fingers to get you to cum. “Look at you, whimpering for my fingers already”
This man is amazing when it comes to aftercare. He knows exactly what you need. After your both cleaned up, he'll pull you to his chest to cuddle.

alex karev x fem!reader
summary: evil spawn isn’t so evil when he discovers you struggling in a closet
warnings: panic attack
-
You were a new intern at Seattle Grace and today you had been chosen to help perform in your first surgery. You were extremely excited but your overall emotion was nerves. What if you messed up? What if the patient died? What if you kill the patient? You knew you were overthinking it but you couldn’t help the spiral of your thoughts. You knew you were a good surgeon, it was a simple procedure that you had practiced many times in med school. This was easy.
All week you had been stressing over it. You would say dreading it but you were looking forward to it but you were just so anxious.
And that is what lead you to hiding in the nearest janitor’s closet whilst tears welled up in your eyes. You were supposed to be heading down to the OR in five minutes but you needed a moment to collect yourself. With your mind racing, you paced around the tiny area with out falling over the cleaning supplies.
Bailey had sent Alex or ‘evil spawn’ to come and find you as you were due in the OR now. He had paged you and told you to go to the OR but he was yet to see you. Meanwhile, you were taking deep breaths and then you left the closet. On your way to OR 3, you stumbled upon Alex who seemed to notice your nervous demeanour. “Are you alright, Bambi?”, he questioned with genuine concern laced in his voice. The nickname had developed when you knocked into him on your first day and spilt your water all over him. He obviously being evil spawn didn’t take it too well but he eventually got over it.
“Yep, I’m fine.”, you said in a hushed voice as you struggled to breathe properly. “You don’t sound it.”, he stated. “Come here.”, he guided you into another closet. “Take some deep breaths, follow me, 1… 2… 3…”, he continued as you tried to follow him. He only noticed you getting worse as more tears began to stream down your face and your breathing fastened.
“Okay, okay, look at me, baby.”, he gently grabbed your chin, forcing you to look into his eyes. He then repeated his breathing steps for you to follow. “In through your nose and out through your mouth. Well done.”
Never in a million years would you have expected Alex Karev to be so good at comforting someone. Right now he was so not living up to his nickname. When you had calmed down, you slowly got up. “Thank you so much, Alex. It genuinely means so much.”, you thanked him awkwardly.
“No problem, anyway, should we head off to perform the surgery now?”, he questioned. He realised your face had become full of nerves again. He cupped your cheeks and stated with certainty: “You can do this, you’re one of the best interns here, maybe even the best. You’ll pass this with flying colours, trust me.”
You smiled at him and your cheeks started blazing. You loved how sweet and gentle he was with you. Also, you couldn’t deny his good looks but you couldn’t give him the ego boost. However, one thing that would damage his ego is the news that evil spawn has a soft spot.
Who knew Alex Karev would have gone soft for an anxious girl in need of some guidance?

𝘼/𝙣: Really, I don't know how to apologize for the wait, this time has been crazy, but I'm back and I hope you'll like this!! 🫶🏻
Your internship had started perfectly. You were a year under your brother George and all his friends, but you could say you were the best between them all. It's also been easy being friend with George's group, they had welcomed you well and unlike your brother you had a tougher character than him. There was this guy, Alex, he was surely the most interesting since you've seen him for the first time. You knew he was livesick of you, you just liked teasing him. Let's say it, you were very hot, his gaze couldn't help to be always focused on you. You enjoyed it.
While the others were spending time complaining about their days you both were too busy flirting with each other most of the time, and then you finally gave him the satisfaction of going out for an evening. George told you he didn't like Alex that much for you, but you didn't care. Sometimes he was too protective.
By the way, finally you accepted to spend an evening with him. You were kinda excited.
It hadn't been anything special, just enough, in his car eating some Chinese food ordered to the nearest restaurant since out was raining and both of you were very hungry. It was like the first time you were just talking spending time normally and not flirting or something else. You just talked about your first days of work and started knowing each other better. The he drove you to your house. Before you get out from his car you looked at him. Yeah he was still staring at you, his eyes looked at you lips but you just smiled.
"Good night Evil Spawn" you smirked and got up. You felt him grab your arm and pull you back inside.
"Not this time" he whispered before press his lips against yours in a pretty passionate kiss. You just kissed him back since this was all you were waiting for.
"Wait.." you stopped the kiss. "In my house there's more space" you said smirking. He smirked as well.
In a few time you were already inside, you pushed him towards the bedroom as you kept kissing. Suddently his lips were on your neck, biting your skin softly. You let out a little moan and then you pushed him onto your bed. You took off your top, he grabbed your hips to bring you closer and kissed you again. You put your hands under his shirt to signal for him to take it off, he did it and then went on top of you.
"Fuck the things that will happen in this room" he whispered. Only these words made you get more wet. You smirked at him and bit you lip when you felt his hand on your stomach.
"Never been so ready" you said.
He took off your bra, starting to kiss your chest and going down with his lips on your breasts, biting them softly but enough to make you gasp.
One moment later you were completely naked, you on top of him, moving your hips against his body. His hands were holding and squeezing you closer, your moans filled the room as you were looking at Alex. His eyes remained glued on you, enjoying that wonderful sight.
"God, I don't think I'll ever get enough of you" he said panting and feeling the contact of your bodies collide. You leaned down to kiss his chest, moving up his neck and finally meeting his eyes.
"Oh, really?" You smirked.
He kissed your lips, in a very passionate way, he positioned himself remaining sitting with you on his lap. He kept pushing you against him grabbing your ass. You moaned again feeling him coming inside you. Both sighed before letting your bodies fall onto the mattress. You let out a giggle.
"Say it, you had fun boy" you then whispered very close to his face.
"Who told you I've finished?" He asked smirking and going on top of you. He slipped under the blanket, you knew where he was going, so you just let your head fall back on the pillow looking at the ceiling, until you felt him teasing your weakest point with his tongue. He began to go deeper and sucking, causing you to moan, you liked that he was a little rough.
"Mmh.." you started moaning louder, the way he kept pleasuring you drove you crazy. He arrived to your clit, intensifying the movements with his tongue. "Alex..." you moaned, clinging to the blanket.
Then he started to move up, kissing your stomach, and reaching your face, savoring the taste of you on his lips.
"I will never finish with you, baby girl"
You would surely have had many more similar nights.

It started as a fling-no promises, no complications, just a way to blow off steam during the long, grueling hours at Seattle Grace. Alex Karev wasn’t the type to settle down, and you weren’t the type to let yourself get caught up in messy entanglements. You were both driven, ambitious, and in control. At least, that’s what you told yourselves. The on-call room was your safe haven, the place where you could steal moments together, away from prying eyes and the judgmental whispers that filled the hospital corridors. Late at night, when the rest of the world was sleeping, you’d find each other in the dimly lit room, the only sounds being the hum of the air conditioning and the soft rustling of scrubs.
“Hey, you” you whispered as you slipped into the room, your heart racing in a way it hadn’t in years. Alex was already there, leaning against the wall with that familiar smirk that always made your stomach flutter. “Hey yourself” he replied, his voice low and teasing. “Long night?”. “Isn’t it always?” You sighed, walking over to him. The stress of the day seemed to melt away the moment you were in his arms, your bodies fitting together perfectly. You kissed, slowly at first, savoring the few minutes you had together. It wasn’t just about the physical connection, though that was part of it. There was something more between you both, something unspoken that neither of you were ready to admit.
As you pulled away, you looked into Alex’s eyes, seeing a vulnerability there that he rarely showed. “We need to be careful” you said, your voice tinged with concern. “If anyone finds out…” “I know” Alex cut you off, his tone more serious now. “But I don’t want to stop”. “Neither do I” you admitted, your resolve wavering. “But we both know how it’ll look. I’m an attending, and you’re… well, you”. “Thanks for the vote of confidence, Doctor”. Alex quipped, his smirk returning, but there was a tension in his jaw that you didn’t miss. Your expression softened, reaching up to brush a strand of hair from his forehead. “You’re a damn good doctor, Alex. One of the best. But people talk, and the last thing I want is for anyone to think you’re getting special treatment because of… this”.
Alex’s eyes darkened, and for a moment, you thought you’d pushed too far. But then he leaned in, pressing a kiss to your forehead. “I don’t give a damn what anyone thinks. I know I’ve earned my place here. And so do you”. His words sent a warmth through you and you found herself smiling despite the situation. “You’re right” you said softly. “We’ll just have to be smart about it”. “Yeah, smart” Alex echoed, though there was something in his tone that made you wonder if he was already planning on breaking that rule.
As the weeks went by, you managed to keep your relationship under wraps. You were careful. No stolen glances during rounds, no lingering touches in the halls. But there were moments when the facade almost slipped, when Alex’s hand would brush against yours, or when you would catch yourself watching him a little too intently as he worked.
It was during a particularly stressful day in the ER that you had your first real scare. A trauma case had come in, a young boy, barely ten, with a gunshot wound to the chest. It was all hands on deck, and the pressure was palpable as the team worked to stabilize him. Alex was in the thick of it, his hands steady as he assisted Dr. Bailey with the surgery. You watched from the sidelines, your heart in her throat as you fought to save the boy’s life. When it was finally over, and the boy was stable, the tension broke like a dam, relief flooding the room. “Good work, Karev” Bailey said gruffly, her way of acknowledging a job well done. Alex nodded, pulling off his gloves and letting out a breath he didn’t know he’d been holding. As he turned to leave, he caught your eye. For a split second, the world seemed to narrow down to just the two of you, the intensity of the moment pulling them together like a magnet. Without thinking, Alex reached out, his hand brushing against yours in a gesture that was too familiar, too intimate for the public setting. Your heart skipped a beat, and you quickly pulled away, your eyes darting around to see if anyone had noticed. Dr. Bailey’s sharp gaze was on you both, and you felt a pang of panic. But before she could say anything, Bailey turned away, muttering something about paperwork and leaving the room.
“Close call” Alex murmured as you walked out of the OR, his voice laced with a mix of relief and amusement. “Too close” you replied, your pulse still racing. “We have to be more careful, Alex. If Bailey had said something..” “But she didn’t,” Alex interrupted, his expression serious. “Look, I get it. We have to keep this quiet. But I’m not going to act like I don’t care about you. Not in here” he gestured to the hallway around them, “and not out there”. You wanted to argue, to tell him that you had to keep your distance, but the look in his eyes stopped you. There was a determination there, a certainty that you couldn’t deny. So instead, you nodded, letting yourself believe, just for a moment, that everything would be okay.
It was a few weeks later when everything came to a head. You’d been playing the game for so long, sneaking around, keeping your relationship hidden, that it was starting to wear on both of you. Alex was growing more frustrated, his usual bravado masking the tension that simmered beneath the surface. You too were feeling the strain, the constant fear of being discovered gnawing at your nerves. The final straw came during a particularly hectic day in the clinic. Alex had been working nonstop, dealing with a never ending stream of patients, and you could see the exhaustion in his eyes. You wanted to reach out, to offer him a moment of comfort, but there were too many eyes, too many people who could see.
And then it happened.
One of the other attendings, Dr. Stevens, made a comment, a casual remark about how Alex was getting all the best cases, how he always seemed to be in the right place at the right time. It was said in jest, but the implication was clear: favoritism. You felt your stomach drop, the color draining from your face as the words hung in the air. You waited for Alex to brush it off, to let it slide like he usually did. But instead, he snapped.
“Maybe I get the best cases because I’m a damn good doctor” Alex retorted, his voice cutting through the room like a knife. “Or is that too hard for you to believe?”. The room went silent, the tension thick enough to choke on. You could see the shock on everyone’s faces, the way they exchanged glances, as if suddenly realizing there was more going on than they’d thought. Dr. Stevens opened his mouth to respond, but Alex didn’t give him the chance. “You know what? Screw this. Yeah, I’m seeing Dr. Y/L/N We’ve been together for a while now. And it’s got nothing to do with my work. So if anyone’s got a problem with that, say it now”.
Your heart stopped, your mind racing as you processed what he’d just done. Alex had just blown your cover, exposed your relationship to the entire room. But as you looked at him, standing there with that defiant look in his eyes, you realised something. You didn’t care. You weren’t ashamed of what you had, and you weren’t going to let anyone make you feel like you should be. Taking a deep breath, you stepped forward, your voice steady as you spoke. “Alex is right. Our relationship is personal, and it has no bearing on the work we do here. If anyone has a problem with that, you can take it up with me.”
There was a murmur of surprise, but no one challenged you. Dr. Stevens, for his part, looked chastened, muttering an apology under his breath. As the room slowly returned to its usual hum of activity, Alex turned to you, a small smile tugging at the corner of his mouth. “You sure about this?” he asked, his voice softer now. “Yeah” you replied, feeling a weight lift off your shoulders. “I’m sure”.
And just like that, the secret was out. It wasn’t easy, and there were still challenges to face. Whispers in the halls, disapproving looks from some of your colleagues but you faced it together, no longer hiding, no longer pretending. Because in the end, it wasn’t about what anyone else thought. It was about the connection you shared, the trust you had in each other. And that was something worth owning, something worth fighting for.

Healing Hearts
includes: alex karev in the beginning of his peds residency, a seattle grace mercy west merger which involves a new mercy west transfer....
black fem surgical resident! reader x alex karev
song inspiration: ivy- taylor swift
---------------------
Y/N walked into Seattle Grace, the bustling environment a sharp contrast to her previous residency. As a new resident transfer, she knew she had to prove herself. Her heart pounded with a mix of excitement and nervousness as she reported for her first day on Arizona Robbins' pediatric service.
"You're the new resident, Y/N?" Arizona greeted her with a warm smile. "Welcome to Peds. You'll be working closely with Dr. Karev today. He knows my service and he'll help you if you have any questions."
Y/N had heard of Alex Karev's reputation. He was known for his exceptional skills with children but also for his abrasive personality. She was determined not to let his reputed arrogance faze her.
When she met Alex in the pediatric ward, she was struck by his intense eyes and the air of confidence he exuded. "You're the new transfer?" he asked, not looking up from the chart he was reviewing.
"Yes, I'm Y/N," she replied, trying to keep her tone professional.
"Okay, let's get one thing straight. I don't have time to babysit," Alex said, finally looking at her. "Just stay out of my way, and we'll be fine."
Y/N bit back a retort. "I'm here to learn and help. Let's just focus on the patients."
Their first case together was a young boy named Liam, suffering from cardiomyopathy. The child's condition was delicate, and his treatment required careful coordination. Despite their initial friction, both Y/N and Alex were deeply invested in Liam's well-being.
"His latest tests show worsening heart function," Y/N said, frowning at the results. "We need to act fast."
Alex nodded. "We need to discuss his case with Dr. Robbins. If we don't come up with a new plan, he won't make it."
They presented their findings to Arizona, who decided on a risky but potentially life-saving surgery. Throughout the procedure, Y/N and Alex worked in perfect sync, their combined skills bringing the best possible care to Liam. In the days following the surgery, Y/N and Alex found themselves spending more time together. They worked late into the night, monitoring Liam's progress and making sure he was stable.
----------------------
One evening, after a particularly exhausting shift, they found themselves alone in the residents' lounge. Alex, surprisingly, broke the silence.
"So, you're a Mercy West transfer?" he asked, his tone less harsh than usual.
"Yeah," Y/N replied, sipping her coffee. "Since the merger. What about you? You've been here a while, right?"
"Yeah, since my intern year." Alex said. "It's a great program. "
Y/N nodded. "I can see that. But I like it so far. The team is great, and the cases are challenging."
Alex looked at her, a hint of a smile playing on his lips. "You handled Liam's case well. Most new residents would have freaked out."
Y/N felt a warm flush of pride. "Thanks. You weren't so bad yourself."
--------------------
As the weeks passed, they continued to work together, their initial animosity giving way to mutual respect. They shared stories about their backgrounds, their dreams, and their fears. Y/N found herself looking forward to their shifts together, her heart skipping a beat whenever Alex smiled at her. One day, as they were preparing for another surgery, Y/N noticed Alex seemed distracted. "Everything okay?" she asked gently.
Alex hesitated before answering. "Just...family stuff. My brother's in town, and it's complicated."
Y/N placed a reassuring hand on his arm. "If you need to talk, I'm here."
Alex looked at her, surprise and gratitude in his eyes. "Thanks, Y/N. I might take you up on that."
Their bond continued to grow, each moment together strengthening their connection. They laughed more, confided in each other, and began to realize that the line between professional and personal was blurring.
One night, after another successful surgery, they found themselves standing on the hospital roof, the city lights sparkling below them.
"You know," Alex said, his voice soft, "I never thought I'd say this, but I'm glad you're here."
Y/N smiled, her heart swelling with emotion. "Me too, Alex. Me too."
As they stood there, side by side, they knew that whatever challenges lay ahead, they would face them together. In the midst of the chaos and the heartbreak of Seattle Grace Mercy West, they had found something unexpected: each other.
--------------------
During the weeks following their initial meeting, Y/N and Alex found themselves thrown together in a variety of challenging cases. Each case revealed more about their personalities and work ethics, slowly breaking down the walls they had built around themselves.
In one particularly intense case, they treated a toddler with a rare genetic disorder. The child's condition was rapidly deteriorating, and both Y/N and Alex had to rely on each other to come up with a treatment plan.
"His genetic markers indicate a possible enzyme deficiency," Y/N pointed out during one of their late-night strategy sessions. She was surrounded by stacks of medical journals and lab reports.
Alex leaned over the table, scanning the data. "Good catch. Let's run a specific panel to confirm and then we can start the enzyme replacement therapy."
Their combined efforts paid off, and the toddler began to show signs of improvement. The victory was a turning point in their relationship. They began to see each other as partners rather than competitors.
One day, after a long shift, Y/N and Alex found themselves in the on-call room, too exhausted to go home. They lay on opposite bunks, the room dimly lit by a small bedside lamp.
"Why did you choose pediatrics?" Y/N asked, breaking the comfortable silence.
Alex turned his head to look at her. "Kids are resilient. They can go through hell and still bounce back. I guess I wanted to be a part of that."
Y/N nodded, understanding. "I get that. I love their spirit, their will to fight. It’s inspiring."
As the days turned into weeks, they shared more personal stories. Y/N learned about Alex’s difficult childhood and his struggles with his family. In return, Y/N opened up about her own challenges, including the pressures she faced at her previous residency.
Their late-night conversations became a routine, each one drawing them closer. They began to anticipate each other's needs in the operating room, moving with a synchronicity that amazed their colleagues.
One evening, after successfully performing a complex surgery on a newborn, they sat in the hospital cafeteria, finally allowing themselves to relax.
"Do you remember the first time we met?" Y/N asked with a smile.
Alex chuckled. "Yeah, I wasn't exactly welcoming."
"That's an understatement," Y/N replied, laughing. "But you've grown on me, Karev."
"You too, Y/N. You too."
Their eyes locked, and for a moment, the bustling cafeteria faded away. They both knew something significant had shifted between them.
As the relationship evolved, so did their feelings. They began to steal glances at each other in the hallways, their touches lingered a little longer, and their conversations took on a deeper, more intimate tone.
One night, after another grueling shift, they found themselves alone on the hospital roof again, the cool night air providing a welcome respite.
"Sometimes, I wonder how I got so lucky to have you as a partner," Alex said, his voice sincere.
Y/N turned to him, her heart pounding. "I feel the same way, Alex."
He reached out, taking her hand in his. The touch was electrifying, sending a shiver down Y/N’s spine. They stood there, hand in hand, the city lights below them, knowing that they had found something rare and precious in each other.
He stepped closer, cupping her face in his hands. "Then let's see where this goes."
They shared their first kiss under the starlit sky. From that moment on, they faced every challenge together, their love growing stronger with each passing day. In the chaotic world of Seattle Grace Mercy West, they had found their calm, their solace, and their love.

It’s you I want
(George O’Malley x gn!reader angst Alex Karev x gn!reader fluff)
Details: Nurse Y/N has fallen for Dr. O’Malley, but is overlooked time and again.
(Reader is described as shorter than Karev and O’Malley)
Word Count: 3.28k
You worked as a nurse at Seattle Grace for the past four years. You’ve seen your fair share of interns, some have stayed, some left, some are obnoxious and arrogant, and others are so passive that they get flushed out.
At first, this is what you thought would happen with George O’Malley. There were many times when you were at the nurses station, watching him get overshadowed by his fellow interns. You felt bad for him, and there was his whole puppy thing going on, so there were times when you would help him with his scut work, giving him pointers here and there.
After a while, you felt your feelings grow from pity, to friendship, to admiration. George was sweet, and caring. He would tell you about his family during downtimes, and you would talk about yours. You felt a connection with him that you haven’t felt in a while. But he also talked to you about Meredith. He would talk, and talk, and talk some more. He would gush about her beauty and her intelligence, how funny she was, every single quality was attractive to him. She was everything you weren’t. You knew nursing was in no way lower than being a doctor, but you felt somehow smaller than her.
But even with his feelings for Meredith you couldn’t put aside your own feelings for George. How he would bring you coffee in the mornings, buy you a little snack whenever you were having a rough day, hold you while you cried if you lost a patient. It was so hard to not fall for George O’Malley, and tried as you might you did.
You were venting to another nurse about your feelings for George.
“I just can’t get George out of my head,” you sighed, “he’s just so damn cute and so caring.”
“Y/N, honey, you have to tell him.” your friend responded.
“I can’t,” you shook your head, “not when he has eyes for someone else.”
A sweet voice butted in, “Who has eyes for someone else?”
You jumped and swiveled in your chair to see George standing there, leaning against the station. The other nurse took the opportunity to get back to her work. Your cheeks heated and eyes widened, “How much of that did you hear?”
“Not much, just the end part about having eyes for someone else.” George explained, “So who’s the lucky guy?”
You breathed a subtle sigh of relief to this, “Nobody, really, it doesn’t matter.” You tried to go about your work.
“C’mon, Y/N,” he groaned, “this is the first time you’ve expressed interest in someone!”
That he knew.
You could tell he wasn’t about to let this go. You looked around, hoping to get an idea of what to say.
Eventually your eyes land on Dr. Karev talking to Dr. Stevens.
“Karev.” You stated, hoping he believed it.
George widened his eyes and shook his head quickly at that. “Alex? Really? He doesn’t seem like your type.”
He’s not.
“I don’t know something about him,” you lied, “he’s just so dreamy.”
You internally vomited. Alex Karev was a manwhore and hellspawn. He’s slept with half of the nurses, and insulted the other half. He hadn’t done either in his short interactions with you, luckily, but that didn’t help your opinion of him.
George was silent for a few seconds, blinking a bit rapidly, before he smiled at you.
“How about I set you up on a date with him?” he suggested.
Now you were gonna vomit.
“Oh no,” you shook your head and your hands, “no need, besides I don’t think he’d go for someone like me?” Your chest tightened in dejection.
“Someone like you?” George asked in surprise, “Y/N, you’re amazing! I can’t imagine anyone would turn you down.”
You bit your lip, chest tightening even more. George didn’t realize how he was squeezing your heart, threatening to destroy it. You thought about it for a second before you closed your eyes.
“Fine,” you sighed.
“Alright!” he jeered, “Y/N, I know what it’s like not to be noticed. You deserve this, even if I don’t really understand your interest in him.”
You sighed once more before George gave you two thumbs up and backed away before rushing off.
Once he was out of sight you groaned and dropped your head into your hands.
You heard the swivel of a chair once more, “you’ve gotten yourself into a big mess here, Y/N.”
You turned your head to look to your friend again, “Who knows, maybe Alex will help me get my mind off George.”
Not letting your mind linger on the thought much longer, you went back to work checking in on patients and updating charts. Before long you forgot about George’s suggestion, focusing completely on your work.
The end of the day came and you were getting ready to punch out before a knock on the desk alerted you to the presence of someone in front of you. You looked up to see none other than Alex Karev. Your eyes widened and you saw George not too far behind him giving you two thumbs up.
“Y/N,” Alex smiled, “a little birdie told me you may have taken a bit of an interest in me.”
You gave him a thin smile back, blinking at his boldness.
“Did they? And what do you think of that?” you asked, hoping for rejection.
“I think,” he leaned in, “I should take you out to dinner.”
Your cheek flushed. You weren’t used to someone taking an interest in you, let alone be so forward with it.
“Um, yeah, sure.” you stammered, smiling shyly.
“Drinks tomorrow? Meet here at 8?” he suggested with a sly smile.
“Sure,” you agreed, “it’s a date.”
Alex smiled and knocked once more, walking away.
Your heart was beating faster than you thought it would. Unexpectedly so.
“Sooo…” George inquired, worried.
“We’re getting drinks tomorrow.” you smiled a bit wistfully.
“What?”
“What?”
“You’re going on a date?” George asked incredulously.
You recoiled a bit at that, “What’s that supposed to mean?”
“Nothing just…” he trailed
“No, go on.” you insisted, getting a bit heated, “what do you mean?”
“You just don’t seem like his type.” he admitted.
You blinked at him a few times and licked your teeth.
“Am I undateable?” you urged, nodding your head.
“What?!” George almost shouted, “No, that’s not what I meant.”
“That’s what it sounded like.” you countered, “I’m not a nun. Men find me attractive.”
“That’s not what I’m saying, Y/N.”
“No, you’re just surprised that he would go on a date with me.” you stated roughly, “You know what George, I’m heading out.”
You clocked out, grabbing your bag and rushed away.
The drive home had you stewing, angry punk music playing over the radio. When you got home you slammed the apartment door closed, thinking of George’s reaction to your date with Alex. He was the one who suggested a date, why would he do that if he didn’t think it would happen? Where did he find the audacity to tell you that you weren’t Alex’s type? You had listened to him go on and on about Meredith, so why was he acting like this when something romantic was happening to you?
All of these thoughts ran through your head before another solidified within your mind. You were going to treat tomorrow like an actual date. Whether you were originally interested or not, you are now. You went to the bathroom to prepare for the next day, shaving, exfoliating, and a face mask. You went to bed determined to make the date successful.
The morning came and you had a shift right before the date. You wore your scrubs but packed your best outfit for after, making sure that it wouldn’t get too wrinkled.
You went about your day as you would usually, except for avoiding George here and there. Any time that you would see him approaching you shot him a look that led him away. You were still pissed off and didn’t want to see him.
The day came to an end and you changed into your outfit. It fit you in all the right places, putting your best features on display. You looked into the mirror, fixed a few pieces of hair and sighed. You were actually anxious, and not in a I-don’t-wanna-go way but in a I-want-this-to-go-well way. Thinking about it throughout the day you realized that Alex Karev was attractive, and even though you had heard bad things about him, he hadn’t done anything to you. You were starting to get a bit giddy.
You walked out to the nurses station and saw Alex waiting for you in a nice pair of jeans, a t-shirt, and a flannel. When he looked up to see you, a small smile spread across his face. He took a few steps to meet you where you were, a small gesture but one you noticed.
“You look,” Alex breathed, “good.”
You blushed and smiled under his gaze, “you do too.”
Alex scratched the back of his neck and let out a small chuckle.
“Joe’s?” you suggested.
“Yeah.” he smiled, “let’s go.”
You both walked in comfortable silence, shoulders bumping now and then.
When you got to the bar it was a bit crowded but not too loud. You scanned the bar for seats and locked eyes with George, who was sitting with Meredith and Christina. You quickly looked away and found a booth in the corner, away from the crowd.
You tugged on Alex’s sleeve and nodded towards the booth. He nodded back before taking your hand and leading you through the crowd of people. The action was shocking to you, and made your heart race. Once you got there, Alex sat you down and asked what you wanted. You gave him the drink, he nodded before going to the bar to order.
You sat there and stared at the table, feeling your cheeks with the back of your hand, surprised to feel them flushed. You smiled to yourself, surprised. So far Alex was nothing if not a gentleman and you were a bit happy you decided to go on the date.
After a beat you could feel holes being bored into your skull, you turned your head to see the trio from earlier staring at you. Meredith was staring with amusement, Christina with curiosity, and George with an expression you couldn’t read. They all looked away quickly, Christina and Meredith giggling a bit and George silent. You shook your head, not letting them get to you, and Alex returned with your drinks and a basket of peanuts.
You thanked him as he set your drink in front of you and took a small sip. He nodded and smiled in response. You decided that if this date was going to be successful you had to be bold.
“So, Alex,” you started, “why ask me on a date?”
He looked at you curiously, a mischievous smile playing on his face.
“Well, one, you were interested,” he counted off, to which you rolled your eyes, “and two you’re… admirable.”
You blinked back at him, smile splitting across your face, “Admirable?”
“Yeah, I don’t know,” he shakes his head, “your presence is commanding. You have the respect of the doctors, of your peers, hell you have my respect. You advocate for your patients and you actually care for each and every one of them. You’re admirable.”
You sat there a bit stunned. Alex admired you. He respected you. He saw you. He saw you. For the first time in a long time you felt appreciated and proud.
“Now you,” he insisted, “why me? I mean I do have a reputation.”
You didn’t think he would ask you that, which was a bit stupid considering you just asked him. You thought for a second. Why were you excited earlier? Why not turn down the date? Thinking back on your interactions with Alex Karev, you had an answer.
“One,” you numbered, mirroring him, “you’re pretty easy on the eyes.” He chuckled at your admission.
“And two, you work hard. Harder than most. Sure you walk around all cocky but I can see that you don’t just want to be a doctor, but a good doctor.”
He looked away smiling, scratching his neck again, a nervous habit you noted.
As the night went on, the conversation flowed rather freely. You both talked about interesting cases, hobbies outside of work, traded funny stories. You didn’t expect it but the two of you clicked rather easily. He wasn’t afraid to express his attraction to you and you liked the attention. He made you laugh. Really laugh, even if it was teasingly at your expense. You ribbed him just as hard, to which he would laugh. You couldn’t tell if it was the alcohol or the attention or just Alex, but you were genuinely enjoying yourself.
It was getting late and as you were just getting off your shift it was time to go home. You and Karev walked out the bar together, arms touching, practically holding hands. He hailed you a cab and opened the door for you.
“Thank you for tonight,” you started, “it was… really great.” You smiled up at him, cheeks flushed.
“I had a good time too,” Alex admitted, “I would, uh, like to do it again sometime.”
“Me too,” you agreed.
You bit your lip a bit sheepishly before leaning forward and planting a kiss on his cheek. He looked a bit shocked but smiled.
“See you later, Alex,” you waved goodbye, closing the door and giving the driver your address.
You closed the door to your apartment and leaned against it, covering your mouth and smiling a bit. Being around Alex seemed… easy. He was more caring and thoughtful than he looked. It wasn’t hard to like him. The conversation never got boring. As all of these thoughts ran through your mind, you realized something. You hadn’t thought of George almost the entire night. Maybe, you didn’t need to.
You took a shower, letting the hot water run over you. All you could think about was Alex and when you’d see him next. You had the next day off, but you would see him when you worked next. The idea of it planted a smile on your face and butterflies in your stomach. That night you went to sleep content.
The next few weeks had you floating. You and George were talking again after he apologized but that’s not why. Alex Karev was the reason. He would come visit you at the nurses station, flirting and joking. On the occasion that you had lunch at the same time, you would eat together, sharing your food at times. You went on a second date and then a third. After the third, you felt bad about how everything started. About not being honest with Alex. After work one day, you pulled Alex aside into an on-call room.
Your heart was going to beat out of your chest. What if this hurt him? What if he thought you were pathetic? What if he wanted to break things off?
“Y/N? What’s wrong?” Alex asked, “are you okay?”
The concern in his voice made you feel even worse, you were pacing a bit.
“I have to tell you something,” you sighed, “I haven’t been completely honest with you. And it’s eating me up.”
“What is it?” Alex placed a hand on your shoulder.
You looked into his eyes and frowned before looking down.
“So our first date,” you started, “you know how George told you I was interested in you? I wasn’t. I had feelings for George and he walked up on me talking about it and asked who I was talking about and I said you and I didn’t expect him to set us up. But I really did want to go on that date with you and I really did enjoy it, and it’s not just me getting over him I really really like you, and I felt so bad not being honest.”
“Y/N-“ he interrupted.
“Please don’t hate me.” you pleaded.
“Y/N!” He put both of his hands on your shoulders, “I know.”
“What?” you breathed.
“I know you had feelings for George,” he confessed, “I could see the way you looked at him. I was surprised when he said you had feelings for me. I figured I’d ask you out, you’d say no, and that was that. But you said yes, and I had the best time. And then we went on a second date and suddenly, you didn’t look at George the way you used to. I don’t care if you had feelings for George, as long as I’m the one you want now.”
You smiled up at him wistfully, bringing his face down to yours for a kiss.
You had kissed before, but they were just a few quick pecks at the end of the date. This was something more, it was soft and deep. Passionate but not lustful. It was everything you could have ever hoped it would be. The two of you broke away and you looked up at him, arms around his neck, “Alex Karev, you are the one I want.” He smiled before leaning down and kissing you again.
That day was the day your relationship with Alex truly started. He introduced you officially to the rest of the interns, aside from George, who you already knew. You’d sit with the five of them during lunch from time to time. There were times when you would catch George looking at you weird or rolling his eyes at you and Alex.
All of this came to a head one day when George caught you as you were about to leave. You were going to have a movie night with Alex, the two of you holding hands about to walk out the door.
“Y/N,” George called as he jogged up to you, “I need to talk to you.”
You looked to Alex and told him to go ahead, giving him a kiss on the cheek before approaching George.
“What’s up George?” you asked.
He pulled you aside into a on-call room for a bit of privacy. He looked around and took a heavy sigh.
“Break up with Karev.” he stated, as if he were mentioning the weather.
“I’m sorry, what?” you almost shouted.
“Look, Alex isn’t right for you,” he explained, “I didn’t realize it before but.. Y/N I want you. I need you.”
You stood there, staring at the man in front of you, trying to find any hint of a joke in his expression. When you couldn’t find any you felt a wave of anger wash over you.
“Are you fucking kidding me?” you actually shouted this time, “you need me? I fucking pine after you for months, listening to you gush about Meredith, and now that I’m happy with Alex you fucking NEED me. No. No, I can’t, George. You are far too late. Alex actually sees me. He’s direct and makes me feel secure. He’s loving and caring and funny and I can’t go through what you put me through again. I can’t.”
You turned around and left the room, slamming the door behind you. You sped out the door and into the parking lot, seeing Alex waiting for you by his car. You walked up and wrapped your arms around him, pulling him close.
“What was that about?” he asked.
“I’ll tell you when we’re home, you’ll thank me later.” you smiled up, kissing him.

hangouts or dates? alex karev x reader
prompt: hangouts starting to feel like dates until the last hangout when alex confesses tha he likes you
at first, meeting up with alex during midnight on shifts when the both of you had to stay overnight had only happened that one time the two of you were the only ones together at the hospital. mer, cristina, callie, april, jackson, and the rest were off that night, having the privilege of going back to their comfy, warm beds. while you and alex on the other hand, had to stay back and stick with your schedules.
so alex that day asked if you wanted to hang with him at the cafeteria later that night for some midnight snacks. anyways, you both were bound on a slow but important night since you had patients to check up on every so often during the night. and of course, you guys were really close already so it wouldn't have been a problem.
doing that with alex was really fun. he'd show up with lattes and you'd show up with cupcakes then the both of you would catch up with each other, have deep talks as if nobody was around, and laugh so loud until you realize it was time to quickly check up on your patients.
it was really fun to the point where almost every other day when you two had the same night shifts at the hospital, the both of you would seriously dedicate your "midnight snacks" to the point where your friends would tease you for being too busy for date night with alex, suspecting there was more going on.
"hey, y/n, wanna get drinks with us!" lexi invited you to get drinks with her mark, mer, derek, and cristina as they were all bound for home afterwards anyways.
you smiled apologetically, "sorry, i'm doing an overnight shift tonight and i-"
"have a date with karev as usual," mark winked, getting you to roll your eyes.
"it is NOT a date, mark! it's our-"
"daily midnight snack blah blah blah," cristina sarcastically understood, "i honestly don't understand how you guys are not dating yet when it's clearly a date!"
"i know you'll miss me when you're out drinking later, cristina," you waved your friends goodbye as they all exchanged playful looks with each other.
it was 10:00pm now, and you had two hours left till you could hangout with alex.
while you were doing some computer work to get time to pass, there was a thought that got you thinking.
alex.
moments of the many hangouts you've been having with your friend have been flashing on your mind. but they were certain moments about alex.
the way the corners of his mouth go up when you make him smile.
the way his laugh lines appear when you crack a joke with him.
his direct eye contact with you when you talk about something whether deep or silly.
suddenly, after recalling these moments, you started feeling your cheeks heat up a little, as there were pink spots forming around. with that, your eyes widened, realizing you had feelings forming for your friend.
now realizing how your friends saw the two of you together from their perspective made you feel all so silly, putting a small smile on your face. you like alex!
what made you smile even more publicly, was when you received a message on your phone from alex.
"see ya later cupcake," he sent. that was a new nickname that surprised you and made you feel butterflies in your stomach.
"that's new.." you replied, following with, "see you!" there was no way right now you'd reply back with a nickname, not now at least...
--
midnight. the midnight snack hangout was about to commence.
after buying cupcakes from the cafeteria, you turned around and already saw alex with lattes on both hands, with a small smile on his face.
"hey cupcake," he greeted, as he started to sit down with you in the table.
"i hope you were talking to the cupcakes and then you could look so food deprived," you sarcastically noted, trying to conceal the flustered feeling you instantly had at the moment.
"i could be talking to the both of ya" he teased.
--
after an hour or two of talking loud and silly with alex, things started to mellow down.
you were drinking the last of your lattes with alex quietly until you asked a really good question that got to him.
"so which patient are you watching overnight?"
"hm?" he shot you a quick and nervous look all of a sudden. "how about you?"
"mr. evans up in the icu, but i asked you first, alex!" you said with an upbeat but suspicious tone.
"uh, some lady's newborn up in the nicu."
"does that lady or a newborn have a name you should know about, dr. karev?"
"yeah, uh-"
"you're lying, don't even both think of a ridiculous name!" you laughed, lightly punching him in the shoulder. "what's up with you, alex, do you even have a patient to watch tonight?"
he looked around quickly for a brief moment, then finally sighed and looked directly at you. "no," he said fidgeting, "look, i just wanted to do overtime again and hangout with you, alright?"
you raised your eyebrows the minute he mentioned, 'again'.
"i'm sorry, 'again' you said?"
alex then looked down at the floor out of guilt, "yeah, i just really liked spending more time with you, and i guess apart of me's been too chicken to ask you out on a real date outside this place, cause i really like you, y/n."
if only alex could see your soft and warm expression upon hearing the sweetest thing you could ever hear from him. you turned to get a cupcake from the table and turned back, lowering it for him to see.
he took the cupcake, looking back at you now as he saw a glimmering look on your face.
"i like you alex, actually a little more than the cupcakes i always buy us."
now, there was the smile that you were looking forward to seeing from alex. it was a small and slightly red embarrassed smile, from your friend.
he slowly unwrapped the wrappings of the cupcake, eyes still on you. "do you wanna continue this into a little date, cupcake?" his new nickname for you.
"i hope you're referring to me this time, karev," you tease, getting a bite from alex's cupcake as he lets out a light laugh, adoring you and this moment.
i suppose cupcake dates and more are to be expected from now on...

LOOK AT ME! alex karev x fem!reader
SUMMARY: in which you struggle after the loss of a patient, and alex is there to calm you down
WARNINGS: panic attack, grief, mentions of death
WORD COUNT: 0.8k +
The moment you heard the machine flatline, you swore you felt your heart in your throat. You also felt the sweat dripping down your brow as you immediately shook your head, locking your hands and starting compressions on the patients chest. You couldn’t lose her, you just couldn’t. She had four kids, for God’s sake. No matter how many times you pressed against her chest, she just wasn’t coming back.
“No,” You whispered, warm tears threatening to spool from your eyelids. “C’mon,” you breathed.
Dr. Bailey came up behind you, a similar look of grief in her eyes as she gently grasped your shoulder, attempting to pull you away. “Dr. L/N,”
“C’mon!” You yelled out with a shaky, broken voice.
“Dr. L/N,” Miranda tried again, a little more forcefully this time, but you just didn’t stop. The rest of the surgeons in the OR stayed silent, sharing looks, none of them wanting to interfere with your clear distress. “Dr. L/N, she’s gone.”
You let out a pained sob and your body suddenly went limp. Dr. Bailey stepped in front of you, pulling your hands away from the patient as you were positioned next to Dr. Shepherd. He rested a hand on your shoulder whilst you fixated your eyes on Miranda, inhaling as she looked up at the clock.
“Time of death: 21:04.”
Dr. Bailey’s words rattled through you so fast, that you felt yourself struggling to breath. “No, no, no,” The heat began to spread through your body, it felt like the walls were closing in, like you you were going to explode. You had to get out of there.
Ripping off your scrub cap, you burst through the OR doors, running out into the hall and collapsing against the wall. “No,” you whispered once more, clutching your chest with both of your hands, trying your best to steady your breathing. All you could hear was the screeching sound of the machine filling your ears, and the hospital corridor around you was now a blur.
“Oh my god, Y/N.” You ever so slightly managed to hear the familiar voice of your boyfriend calling out your name, as he knelt down beside you, concern spread all across his face.
“Y/N, babe,” Alex watched as you sat slumped against the wall, your hands pressed to your chest and you heaved. He extended out his arms, one tightly gripping your shoulder, and the other cupping your cheek at an attempt to reassure you. “Y/N.” He repeated, keeping his tone soft not to stress you further. “Y/N, I need you to look at me, please. Can you do that?”
When your struggles for breath got faster, Alex quickly started to panic, “Y/N, look at me!” He sternly said, shaking you slightly and pulling you harshly back into reality. He watched as your eyelids flickered open, as your rapid breaths calmed down when you looked at him. There you were. You never told him what happened - you couldn’t. Nor did you need to, the look in your eyes told him all he needed to know.
You studied his face. Alex seemed sad. For the patient and her family, but more so for you. He knew that you were blaming yourself, but it wasn’t your fault. And he needed you to know that. He pulled you in for a hug, allowing you to sob into his shoulder the second he wrapped his arms around you. You felt his hands rub soft circles on your back, whilst he pressed gentle kisses to your temple. “Shh, it’s okay. I’m here. Need you to calm down for me.”
“It’s all my fault.” You croaked out between sobs as you heavy breaths finally slowed down.
“Hey,” Alex scolded, pulling away from the embrace. He cupped your cheeks with both his hands, pulling your chin up to ensure you were looking at him. “Don’t say that, don’t you ever say that.” His tone was stern yet sincere, “There was nothing else you could’ve done. She was sick, she’d been sick for a long time. It was not your fault, Y/N. Okay?”
“Okay,” you whispered under your breath, but loud enough that he could still hear you, “Okay.”
Alex continued to hold you as you calmed down, it felt as though the the whole world was crumbling away from underneath you both. As the two of you sat there against the wall, thoughts began to swirl around your head again, thoughts of what was going to happen next. “Oh god,” you whimpered, sitting up slightly as your eyes filled once more, “I have to tell her husband.”
Alex tightened his grip on you. He’s just calmed you down, if you got stressed out again you’d only hurt yourself. “No.” He was quick to shake his head, “No, you don’t. Bailey can do it. You need to stay here with me and calm down or you’ll make yourself sick.”

Something Gained - Jackson Avery
Word Count: 1515
Summary: Time never stands still but it flies when sudden things happen, doesn't it?
The Seattle skyline loomed over the bustling city as the rain pattered softly against the windows of Grey Sloan Memorial Hospital.
Inside, the familiar hum of the hospital was interrupted by the echo of high heels against the polished floors.
Heads turned, whispers rippled through the corridors, and eyes widened in surprise as you walked through the front entrance, a determined expression on your face.
It had been a year since you had last stepped foot in Seattle, and even longer since you had seen him.
A lot had changed in that time.
You had left Seattle abruptly, your departure fueled by a mix of heartbreak, confusion, and the need to escape the painful memories.
The wedding of Jo and Alex should have been a happy occasion, a celebration of love, but for you, it had been the final straw.
You and Jackson Avery, the man you had fallen so deeply in love with, had ended things that night in a whirlwind of emotions and misunderstandings.
The pain had been too much to bear, and so you fled, seeking solace far from the city that had been your home.
But now, you were back.
And you weren’t alone.
You glanced down at the baby carrier in your hand, where your three-month-old daughter, slept soundly, oblivious to the storm brewing around you.
You had returned to Seattle with a purpose, a strength that had grown in you during your time away.
You were no longer the broken woman who had left, but a mother with a fierce determination to create a future for your daughter.
As you walked through the hospital, you felt the weight of the stares, the curiosity of your colleagues, and the unspoken questions that hung in the air.
It wasn’t long before you reached the elevators, pressing the button with a shaky hand.
You took a deep breath, preparing yourself for the inevitable reunion with your past.
The doors slid open, and you stepped inside, the ride up to the surgical floor feeling like an eternity.
The memories of your time here flooded back—countless hours spent in surgery, late-night coffee runs, the friendship of the surgical team, and the undeniable chemistry you had shared with Jackson.
Jackson.
His name alone was enough to send a shiver down your spine, a mixture of longing and pain.
You had tried to move on, to forget him, but he had always been there, a constant presence in your mind.
And now, you would have to face him again.
The elevator dinged, and the doors opened to the familiar sight of the surgical floor.
Nurses and doctors moved quickly through the halls, the sound of beeping monitors and distant conversations filling the air.
You stepped out, your heart pounding in your chest as you made your way toward the nurse’s station.
It didn’t take long for you to spot the familiar faces, Meredith Grey, your long-time friend, was in deep conversation with Bailey.
They both looked up as you approached, their eyes widening in surprise.
“Y/n?” Meredith’s voice was filled with disbelief. “What are you doing here?”
You forced a smile, though it didn’t quite reach your eyes. “I’m back,” you said simply, your voice steady despite the turmoil inside. “And… I have someone I’d like you to meet.”
You set the carrier down gently on the counter, revealing the sleeping baby inside.
Meredith and Miranda exchanged a look of shock before their gazes softened as they took in the sight of the tiny infant.
“Oh my God,” Meredith breathed, her eyes locking onto yours. “She’s beautiful.”
“Thank you,” you replied, your voice catching slightly. “Three months so far.”
Bailey’s stern expression softened as she looked at the baby. “Congratulations, y/n. She’s precious.”
“Thank you, Bailey,” you said, your heart swelling with pride as you looked down at your daughter.
Meredith’s eyes narrowed slightly as she studied your face. “Is she…?” She hesitated, as if unsure whether to finish the question.
But you knew what she was asking. “Yes,” you said quietly, your voice barely above a whisper. “She’s Jackson’s.”
The silence that followed was deafening, the weight of your words hanging in the air like a heavy cloud.
Meredith and Bailey exchanged another look, this one filled with concern and uncertainty.
“Does he know?” Meredith asked gently.
You shook your head, tears threatening to spill over. “No. He doesn’t know.”
Bailey crossed her arms, her expression a mix of frustration and understanding. “You need to tell him, y/n. He has a right to know.”
“I know,” you replied, your voice trembling. “I just… I didn’t know how. Or when. And now… I’m back, and I have to face him.”
Before Meredith or Bailey could respond, a familiar voice cut through the tension. “Face who?”
Your heart stopped as you turned to see Jackson standing just a few feet away, his eyes locked onto yours.
The world seemed to blur around you, the noise of the hospital fading into the background as you stood frozen in time.
Jackson’s expression was a mix of surprise and confusion, his gaze shifting from you to the baby carrier on the counter.
He took a step closer, his brow furrowing as he took in the sight of the sleeping baby.
“Y/n?” His voice was filled with disbelief. “What’s going on?”
Your mouth went dry, your heart racing as you struggled to find the words.
This was the moment you had been dreading, the moment you had tried to prepare for but could never fully imagine.
“I… I’m back,” you said, your voice barely above a whisper.
Jackson’s eyes softened as he looked at you, the confusion giving way to something else—something deeper, more vulnerable. “And… who’s this?”
You swallowed hard, the tears you had been holding back finally spilling over. “She's uh–” you said, your voice breaking. “She’s… she’s our daughter.”
The silence that followed was thick with emotion, the weight of your confession settling over you both like a heavy blanket.
Jackson’s eyes widened, his gaze darting from you to the baby and back again.
“Our… daughter?” His voice was barely audible, filled with a mixture of shock, disbelief, and something else—something that made your heart ache.
“Yes,” you whispered, your voice trembling. “I found out I was pregnant after I left. I… I didn’t know how to tell you. And then… I just couldn’t.”
Jackson took a step closer, his eyes locked onto the tiny form of your daughter, his hand reaching out hesitantly as if afraid to touch her. “She’s… she’s beautiful,” he whispered, his voice thick with emotion.
You nodded, tears streaming down your face as you watched him, your heart breaking at the sight of the man you had once loved—still loved—finally meeting your daughter.
“I’m so sorry, Jackson,” you whispered, your voice filled with pain. “I never wanted to keep her from you. I just… I didn’t know how to face you after everything that happened.”
Jackson’s eyes met yours, and in that moment, you saw the depth of his emotions—the hurt, the anger, the confusion, but also the love.
The love that had never truly faded, despite the distance and the time that had passed.
“I should have been there,” he said, his voice shaking. “I should have been there for you, for her. But I didn’t even know…”
You shook your head, fresh tears falling. “It’s not your fault. I made a choice, and I’ve regretted it every day since.”
Jackson’s hand finally made contact with your daughter's tiny fingers, and as he gently held her hand, a single tear slid down his cheek. “She’s perfect,” he whispered, his voice filled with awe. “And she’s ours.”
The sight of Jackson holding your daughter’s hand was almost too much for you to bear.
You had imagined this moment so many times, but it had never been this raw, this real.
“Can we… can we start over?” Jackson asked, his voice breaking the silence. “For her? For us?”
You looked into his eyes, seeing the hope, the desperation, and the love that had always been there.
It was a moment of clarity, of understanding, and you knew that despite everything that had happened, despite all the pain and the mistakes, you couldn’t walk away from him again.
“I’d like that,” you whispered, your voice filled with a tentative hope. “I’d like that very much.”
Jackson nodded, a small, tentative smile forming on his lips.
He reached out, gently brushing a tear from your cheek. “We’ll figure this out,” he said, his voice filled with determination. “Together.”
You nodded, your heart swelling with a mixture of fear and hope. “Together.”
As you stood there, your daughter sleeping peacefully between you, the noise of the hospital gradually returned, the world slowly coming back into focus.
The future was uncertain, and there were still so many challenges ahead, but for the first time in a long time, you felt a sense of peace.
You had a long road ahead of you, but you would face it together—one step at a time.
And this time, you wouldn’t let go.

something sugary {alex karev}
Plot: Reader has diabetes and Alex always keeps a stash of sugary food and drink for them in his locker.
requested by anon
Character: Alex Karev x Female Reader
Alex nudged your side for the third time in the span of twenty seconds. You shot him a dirty look, "Stop," you hissed under your breath.
Bailey pulled a look of 'shut your mouth right now or I'll kill you' at the two of you but still, ten seconds later, Alex nudged your side, "Are you feeling alright?" He asked quietly, "You're looking a little clammy..."
You looked at him, feeling a rather large surge of anger bubble inside you, "Alex, you're a piece of-" and then you realised that he was right. You'd been feeling rather warm for the last five or so minutes and when you stopped to listen to your body, you felt sluggish and a little shaky. You were having a hypo. "-shit!" You cursed as you began to feel the familiar tendrils of a hypo taking over your body. It had a way of sneaking up on you and then making you go down fast. You had to check your bloods and get something sugary into your body and fast.
Bailey, who'd been in the middle of telling you about a patient, raised her eyebrows, "Excuse me?"
"Sorry, Doctor Bailey, I need to go check my levels, think I might be-"
She flapped a hand at you, "Alright, go, go!"
You rushed out of the room quickly, trying to get to your locker before the hypo got worse. You always kept your kit on you but the snacks were always stored in your locker (or the vending machine since you usually forgot to get more) - maybe not the most ideal but that's the way it had to be. You'd been so preoccupied with getting out that you hadn't realised someone else left behind you.
By the time you got to the locker room, you felt rather worse for wear. A dull headache was spreading across your forehead that was slick with sweat and your body felt like one more step forwards would cause it to collapse. Despite that, you pushed through and sat on the bench. You needed to know your levels, as you did this, you barely processed the fact that Alex Karev had now joined you in the locker room and was rifling through his locker for something.
2.2mmol/L flashed on screen.
"Shit," you whispered before standing and going to your own locker but before you could, Alex caught your wrist and twirled you round. Annoyed, you snapped, "Alex, I don't have time for-"
He pushed something into your hands, "Eat." You frowned at him and then looked down at what he'd put in your hands... Candy?
"What is this?"
"Shut up and eat," he rolled his eyes, "I told you that you looked funny, you're having a hypo. Something sugary is what you need. Eat."
You didn't question him again and instead sat down on the bench and began to eat a few of the sour candies he'd given you. He sat next to you in silence.
As you ate, you could feel the headache ease off a bit and you didn't feel as warm, "How did you know?"
"I'm a Doctor, genius."
You rolled your eyes, "Seriously, Alex, how did you know?"
He shrugged, "We've been doing this together for like a year. I know when you're having a hypo or you're about to. It's why I've stashed so much sugary stuff in my locker for you."
It took you a moment to process what he'd just said. He'd been observing you enough to know when you were in a hypo and he kept snacks in his locker for you? You and Alex were friends, barely. He was an asshole half the time but sometimes, in rare moments, you connected. You liked to drink with him, he had the wildest stories and could always make you laugh.
"Alex..."
He gave you a small smile, "Don't mention it but maybe you should lay off calling me a piece of shit and just let me help you instead, yeah?"
"... Why do you keep snacks in your locker for me?"
"Give me your locker key." He held out his hand and you gave him your keys. He stood and moved to your locker before swinging it open. Inside, you had a few changes of clothes, some skin care, shampoo, tampons but no snacks, "Like I said, we've been working together for a year. I know that even though you're one of the best Doctors I know, you're hopeless at keeping on top of buying yourself snacks to keep in work. Too many times I've seen you send O'Malley to the vending machines." He shrugged, "I thought I'd keep a stash for easiness."
You were touched. You were insanely appreciative of Alex Karev in this moment, your heart surged and felt rather full and warm inside, "Alex, I don't know what to say."
He shook his head, "It's alright," he sat beside you again, "here, let me check your levels." He took your hand and you let him. You were a little too shocked to stop him if you wanted to but you'd never felt this seen before, least of all by Alex Karev. You watched him in awe as he expertly pricked your finger and took your levels, "There," he grinned, "Perfect."
He packed your kit away and handed it back to you and dug around in his scrub pocket to pull out another key, "It's my spare locker key." He looped it onto your keychain and you noticed it had AK in Sharpie on either side, "If you ever need sugary stuff, help yourself... it's all for you anyway."
"Thank you, Alex," you said softly. Your cheeks felt hot again but not because of a hypo but more so because you didn't know what to do now. Having him be here, having him care for you like this... it sent your heart and stomach into a frenzy and you weren't sure how to react now.
He rolled his eyes as he laughed, "What?" He asked, "I give you candy and that's got you all blushing and shy? What would you be like if I took you on a real date, mm?" He was teasing but there was a genuine question under the humour. A date - a real date with him. He was testing the waters, wanting to gauge your reaction to him saying that.
You laughed, not freaked out or disgusted at all by his suggestion, "You need to work harder to get a date with me, Karev," you stood up and the two of you began to walk back to the elevators.
"Yeah? Name your price, I'll do it."
"Coffee, every morning for the next two weeks. Not crappy hospital coffee-"
"Nah, you want the real stuff from that coffee shop across the road. I've seen you in there before your shift starts." You nodded in agreement. Alex smiled, "Easy, done."
You saw a completely different side to Alex Karev that afternoon and it didn't stop there. Once you'd seen it once, you'd seen his softer side one hundred times. You began to see him just like he had seen you. And yes, for the next two weeks, he got you coffee and a muffin every morning (and then every morning afterwards also).

Hi! Could I request some Alex Karev smut? If possible? :)
Dr. Feelgood(Alex Karev)
Paring: Alex Karev x Fem!Reader
Summary: after being forced to plan a 'prom' at the hospital for Webber's niece, Alex and the reader decided to go together. They had nothing better to do, but their desire can't go unnoticed.
Warrings: SMUT, oral(female receiving), unprotected sex, pull-out method, sorta public sex?
MasterList ML2
“Everyone goes to the prom!” Webber shouts. “Everybody!”
And that's when we become party planners instead of surgeons, well we became that after the LVAD wire cutting incident, but that's not the point.
And when you have someone like me who never went to prom and someone like Alex who couldn't give a shit to help plane, nothing could get done quick enough.
Camille's friends just wouldn't shut up. Alex groaned and put his head on the table. The girls continued to ramble about colors and lights and Alex groaned louder like they were stabbing him.
“we could go all white” one the girls said, but then stopped when Alex made more noise of anguish.
“are you having a Seizure?” I asked, more annoyed with his groaning than the teenagers yapping.
“Will it get me out of here?” Alex asked, setting up. “Fine. Let's go with seizure”
Then after a while George got Bailey to help. She got every decoration down to a Tee and got everything ordered. I just didn't want to deal with this lovey-dovey crap. I just broke up with Derek Shepherd because I found out he was actually married, everything was just getting to me. Even a dying guy like Denny proposed to Izzie.
I could tell Alex had had enough of everything too. He blamed Izzie for our intern group's suspension and put on decoration duty, he was just pissed. I couldn't blame him, others might but I always saw a side of him no one else was. To be honest he was my person as Meredith would say… even if he looks ready to punch someone.
I let go of an unclosed balloon full of helium, making it fly towards Alex as it deflates. “Are you serious?!” He grabs the balloon. He was very agitated.
“Calm down, I'm just messing with ya” I told Alex.
“No! Don’t mess with me, have you seen these girls? These decorations! And now this!?”
I shrugged, already accepting our fate. I looked around as the hospital slowly turned into some prom nightmare. My high school prom was hell… If I had one, guy asked me and I ever really got the full experience.
“I really don’t want to be here. I can’t believe Webber forced us to do this because LVAD shit” Alex complained.
“No one wants to do this, but it's for the chief's niece” I said gently.
“Yeah, I know, but he forced the whole hospital staff to attend is stupid as hell”
I shrugged. I didn't really care about most things these last few months because of the Derek and Addison crap. I don't know what possessed me but it turned towards Alex. “you wanna go together tonight? need to be around friends” I don't know how I felt about Derek, but I wasn't ready to see him at his damn prom with Addison.
Alex turned to me, surprised. Like he hadn't expected me to say that. He thought about it for a moment and shrugged “Sure, why not?” he said with a smirk.
“Great,” I smiled.
--------(3rd pov)--------
The prom went off without a hitch. Y/n looked up at all the lights and decorations as her and Alex slowly swayed to the music.
“I can't believe I'm saying this, but this is nice” Alex said softly as his hands stayed on her hips, moving to the music. He was enjoying every second of this, all because of her. And he felt like he was getting back at Derek for hurting her. He felt good just being with her, seeing her in a beautiful dress instead of those blue scrubs. Even the decorations weren't pissing him off anymore, just being there with her ment everything.
“Yeah…” I looked away from Alex to look at everything around us, all the lights and colors.
“what's up?” Alex asked, she looked back at him with a soft smile. “nothing… I Just never went to prom before”
“Really?” he was surprised, in his eyes y/n was beautiful and a pretty smart doctor. He assumed she probably had a ton of guys wanting her. And he thought Derek was a complete idiot for losing her.
She shook her head. “no one really wanted to ask me…”
“I would have…” He continued to sway to the music, not breaking eye contact with her. Alex looks into her eyes. She had the prettiest hazel eyes he ever saw and the more he looked the more he could see himself falling for her. He couldn’t help but keep looking, they were like pools of honey. It was a nice way to get his mind off of work and the LVAD wire cutting incident. His eyes were locked into hers. He couldn’t help but admire her beauty.
“I was one of those nerdy girls no guy was interested in…” she said softly.
He put his hand on her cheek. She was just so beautiful, he had never thought about her like this before. And now the whole idea of her seemed so new to him and it was making him like her more. “That’s ridiculous. Look how beautiful you are. I don’t know why guys did not ask you out… I don't know why shepherd was stupid enough to lose you”
Her heart pounded against her chest when he said that. She hid her burning cheeks and rested her head on his shoulder as he moved to the music. “I got bumped up a couple of grades because I was smart… I played D&D… I was the weird kid because I didn't want to go out and get drunk”
Alex couldn't believe what he was hearing. He playfully chuckles as they slowly danced around oh... “oh, you were one of those geeks,” he said playfully.
Y/n scoffed playfully, resting her head on his shoulder gently “playing D&D isn't the geekiest thing in the world…”
Alex couldn't help but smile at her reaction. He rubbed the small of her back as they danced. “I was on the wrestling team… Who am I to judge”
“Wow, a wrestler” she laughed softly and playfully into his shoulder. “yeah... I was in marching band, school newspaper... All that nerdy stuff”
“Marching band huh?” he teased her, he lifted their arms up and he slowly spun her around before bringing her back to his chest. “you were a band geek, who knew”
“who knew you could dance” she teased back.
He smirked at her, holding her hand and continued to dance with her with zero embarrassment. “Shut up” he said playfully.
“ya know… If we went to the same high school, I would have been proud to go to prom with you.” he had a serious look on his face now. He was not messing around anymore. His friends would have made fun of him but the more he thought about it, the more he realized he didn’t care.
Her heart fluttered as she smiled, she wrapped her arms around his neck and played with the hair at the neap of his neck. “I would have been proud to go out with you too... All of my geeky friends would have been terrified of you though” she joked softly as we swayed to the music
He smiled softly and wrapped an arm around her waist. The other hand is still resting on her lower back. He leaned in to nuzzle her neck slightly. “I was always the scary one.” he whispered. “But you’re not afraid of me now are you?”
She ran her fingers through his hair as he nuzzled her neck. “I never was”
He smiled against her skin and kissed it gently. “Good.” he whispered before pulling back to look at her. His eyes searching hers for something, he couldn’t quite put his finger on what though. There was just something about her that he hadn't seen before, it drew him in and made him want more.
Y/n smiled as she stared into his chocolate brown eyes. Deep down she always knew there was more to him than him just being a surgery hungry asshole intern that slept around with nurses.
“Would you believe me if I said I always had a thing for nerds like you?” he said in a playful tone, he wasn’t joking. He genuinely liked her intelligence. She was beautiful and smart and that's what Alex liked in a woman.
“I don't know,” she said in a playful tone. Alex lifted her arm up and gently spun her around then pulled her back to his chest.
He leaned in to nibble on her earlobe softly, they both continued to sway to the music so their conversation didn't look suspicious. “I’m serious.” he whispered before trailing kisses down her neck. He could feel her pulse racing under his lips.
Alex smirked against her blushing skin. He loved how responsive she was to him. He lifted his head slightly to look at her once more. His eyes searching hers. "Do you want me to stop?”
She looked him in the eyes and shook her head. “N-no…” she said softly.
Alex's smirk grew as he felt her hesitation turn into desire. He leaned in once more, his lips pressing against hers with a soft yet hungry kiss, making her heart pounded against her chest . His hands slid up her back, pulling her closer as he deepened the kiss as her hand held his jaw.
The loud music masked the small moan that slipped past her lips as Alex's hands slid down her back, gripping her ass. Alex pulled her even closer, his tongue slipped into her mouth, dancing with hers in a heated duel. He could taste the sweetness of her lips and it only made him want more. She wrapped her arms around his neck, they were barley dancing now and the 'prom' was forgotten.
Alex groaned into the kiss, loving how responsive she was. He felt his blood rushing through his veins, making him hard against her body. He pressed her closer to him, rolling his hips in a small motion, making a small moan get caught in her throat. Y/n slowly pulled away from the kiss and rested her forehead against his, trying to catch her breath.
Alex closed his eyes, breathing heavily as he felt her against him. He wanted her so badly but he knew they couldn't do this here. “We should go somewhere else,” he murmured against her lips before leaning in to steal another kiss.
“Let's get outta here,” she said quietly.
Alex's hands never left her waist as he led her through the crowd, making their way towards an empty examination room. Once inside, he locked the door and pushed her against it, forcing a moan to escape past her. She tugged on his hair as his lips crashed into hers again, his tongue slipping past her lips as he devoured her mouth.
Alex's hands roamed up her thighs, hitching her dress higher. He could feel how wet she already was and it only made him harder. He slid his hand between their bodies, rubbing against her swollen clit through her underwear. “Fuck,”
“Alex” she moaned, her head falling back against the wall as his fingers danced around her most sensitive area.
Alex growled low in his throat, slipping a finger under the elastic of her panties to slowly push them down her thighs as he fell to his knees in front of her. He replaced his finger with his tongue, lapping at her folds and tasting her sweet nectar. She let out another moan, tugging at his hair and arching her hips off the wall.
Alex hummed against her folds, loving the way she responded to his touch. He suckled on her clit, flicking his tongue over it as he circled his finger around her entrance. He groaned when he felt how wet and swollen she was for him.
Her legs got wobbly and she arched her hips, moaning. “A-Alex…”
Alex kept sucking and flicking her clit, driving her wild with his expert tongue. He hummed against her, loving the way she squirmed underneath him. He placed his hand on her thighs, squeezing gently. "Shh...I got you, baby.”
Y/n’s heart pounded against her chest as felt herself getting closer and closer to the edge. “A-Alex, I'm c-close” she moaned as her head fell back in pleasure, eyes screwed shut.
Alex wanted her to come undone for him. He lifted up her leg and set It on his shoulder and sucked harder on her clit, swirling his tongue around it as he thrust his finger deeper into her wet heat. “Come for me,” He growled, not relenting until she did.
“Alex!” she moaned louder, tugging at his hair as she came undone on his tongue.
Alex hummed around her clit, enjoying the sweet taste of her as she came undone. He kept sucking and flicking until he felt her muscles start to relax. “Fuck, Angel” he groaned, pulling away slowly and kissing his way up her body as her fingers trailed down his body.
Alex kissed her deeply, she could taste herself on his lips. He pulled her closer, grinding against her as he grew harder. “I need you,” he whispered against her mouth. He quickly pulled the zipper of her dress down, making it pool to their feet. He lifted her up, pushing her onto the examination table.
“Take me” she grabbed his tie, pulling him between her legs. He slammed his lips against hers, the kiss was messy and rushed as she unbuttoned his shirt and pushed it off his shoulders.
Alex looked down at her, his eyes dark and intense as he quickly unbuckled his belt and pushed his pants down. He positioned himself at her entrance and slid in slowly, watching her face for any sign of discomfort. “Tell me if it hurts,” he whispered against her lips before thrusting deeper.
She moaned, arching her hips against his as the pain quickly turned into pleasure. “oh, fuck”
Alex groaned, feeling her walls clenching around him. He started to move faster, his hips slapping against hers as he took her hard and fast. “That's it,” he growled, nipping at her neck. “Fuck, you feel so good.”
Shemoaned against his neck, digging her nails into his back. “A-Alex!” she wrapped her legs around his waist, pushing him deeper inside her.
Alex cried out, feeling her tighten around him. He lost himself in the sensation, thrusting harder and faster as they became one. His hips slapped against hers in a rhythmic cadence that echoed through the room. “God, you're so fucking tight,”
She tugged at his hair. “Alex, I'm gonna cum!” she cried out desperately and tugged at his hair.
Alex groaned, feeling her inner walls pulsing around him. He slammed into her one last time, she cried out and bit his shoulder. He slammed into her one last time before pulling out, shooting his hot seed across her stomach and chest. “Fuck,” he panted.
Alex collapsed on top of her, their bodies sticky with sweat and each other's fluids. He nuzzled into her neck, breathing heavily as he tried to catch his breath. “That was... intense,” he murmured with a chuckle.
“A-amazing” she moaned breathlessly.
Alex grinned against her skin before slowly pulling out of her. He reached down and gently wiped some of his seed off her stomach with his tie, chuckling softly. “You're a mess,” he teased as he knelt down and picked up her jumbled up dress.
“It's your fault” she teased as Alex helped her step into her dress.
Alex laughed softly, his eyes trailing down her curves as he zipped the dress back up for her. “I suppose it is,” he agreed as he placed a kiss on her neck. “But I'd do it again in a heartbeat.” He smirked, pulling his shirt back over his shoulder, before he buttoned it back up he leaned in and pressed a gentle kiss to her forehead.
Alex pulled back and smiled back at her, his gaze softening as he studied her face. “You're beautiful,” he murmured before leaning in to capture her lips in a slow, passionate kiss. He savored the taste of her on his tongue, wishing this moment could last forever.
She pulled away, smirking. “who knew Alex Karev was such a softie”
He let out an airy chuckle. “only for you” he smirked, kissing her on the nose.

just visiting
♡ pairing: alex karev x female! reader
♡ genre: angst to fluff
♡ warnings: small season 9 spoilers (plane crash) & family issues
♡ part two, part three, part four, special scenes
♡ check out my grey's anatomy masterlist here
going back to seattle will never be one of your plans. but, your mother had to pick seattle as one of the destinations for your so called family road trip.
to add more, you had to stay in town for a few days. it's not like you hate the city.
it's where you've met your soulmate. your person, alex.
but when you were on your last year residency, you had to leave. not only because of all the crisis that hit seattle grave mercy west that time, you were also offered a job at hopkins, your dream hospital. to start your fellowship.
fortunately, alex was also offered to start a fellowship there.
but, the plane crash happened.
he couldn't just leave the hospital. especially with what happened to arizona.
"you can't be for real y/n." he said following you onto your shared bedroom.
he proposed that morning and now, you're packing your stuff up.
"we said we'd stay!"
but, now you can't.
"is this about your family y/n? is that why you can't answer me? i swear if they're talking crap about me again i'll–"
"you want to know how they're treating me because of us?" stopping at by the doors.
"you know about this. alex i–"
"and yet you listen to them huh." scoffing at what you said.
"if you walk out of that door, we're done y/n."
after five years, you were back. you convinced yourself you were just visiting and checking the sights you never seen or had the chance to see when you were still living in the city.
you were on your hotel room when your four year old boy, lucas, who just came back from a walk with your parents suddenly threw up on the carpet.
"i don't feel good mama."
you called for an ambulance and of course it brought you to where you started your career as a doctor.
as your son was brought in the emergency room. one of your former colleague and a very close friend noticed you.
"y/n?"
"meredith!"
she walked towards you. "how are you? and you've got a kid!" she said hugging you.
"i'm fine, mer." you replied, hugging her back.
"are you staying long?"
"does alex—" she was interrupted when your son started to cry and vomit again.
you came back to his side and started to rub his back for comfort.
"no. he doesn't know i'm here." you replied shortly.
"just so you know he's the one on-call on peds today."
"and let's catch up after work." with that she hugged you again before going back to her patient.
while waiting for the doctor on-call to come down. you made the time to pray that it's arizona or another doctor will come down.
"where's the–" he stopped in the middle of the emergency room.
he certainly did not expect you to be somewhat in seattle. and of course, he did not expect for you to have a kid.
he saw the way you were talking to the child. promising something which made the kid smile. but shortly after, the kid reached for the basin and vomited again.
from that, he got out of his daze and walked towards the two of you.
"how are you feeling little champ?" he said standing on the bedside.
"i don't like it." he said turning himself towards you.
alex looked at you and asked you what happened. "he was fine before he went out with my parents. he said he ate something from the sea."
"from the sea huh." alex said examining your kid.
"i tried to ask which is it but he's really not in the mood to be specific." you said brushing his hair out of his face.
after alex looked at him more "he's already getting fluids and i already ordered some meds but i'd like to keep him overnight for observation."
you agreed. "y/n i–" he was interrupted when lucas started to call for you.
"i'll check up on him again later." he said before letting himself out of the emergency room.
after settling in and calling your parents. you had the chance to take a breather. walking in the halls with some of the staff recognizing you. you finally reached the cafeteria.
"can we talk?" alex's voice came from behind. you motioned for him to take the vacant seat in front of you.
"so...you've got a kid huh." he said starting the conversation.
"yeah. he's smart and funny..."
"i met him three years ago from a program we did back at boston. and instantly fell in love with him. can't believe that i'd be falling in love an infant though." both of you letting out a laugh.
"you know, i waited for you text or something." he spoke again.
"i waited over there too." you said smiling sadly at him.
"so you're the chief of peds surgery now huh." you said pointing at his coat. "yeah. robbins gave me the job, so she can do neonatal."
"are you staying?"
you stood up from the table before answering. "we're just visiting."
after that talk with alex, you went back to lucas' room. he's finally asleep after vomiting all afternoon.
"you can go back to the hotel and rest up." sitting at the end of the bed. "lucas' doctor, karev. isn't he that ex-boyfriend of yours?" your father asked as they were gathering their things.
"it's a good thing you listened to us cause you're doing way better than him, i mean have you seen him? hopkins is way better than this–"
"can we drop this conversation dad?" you said sighing as took as seat beside your child's bed.
"i'm just saying that you're doing way better than–" with your father not dropping the subject, it made you mad. all those years of torment of them hearing how great you are now just because you listened to them and left seattle.
"do you think i was fine after i left this hospital? i listened to everything the both of you said because i can't bear how you were treating me."
"the only good thing that happened to me over there is lucas! and don't think i didn't know what you said about me adopting him cause i–" you received a slap from your mother.
"how dare you speak to your father like that? we were only looking out for you and you adopting a kid you just saw at a program was–"
"out. i've heard enough." your mother protesting woke up lucas. "mama... where's grandma and grandpa going?"
"the-they're going home sweetie. we'll just visit them after we go home tomorrow okay?" rushing to his side so you could help him go back to sleep.
after your parents left, meredith was standing by the door. "are you okay? cause this is just my cup of tea y/n, i mean, mommy and daddy issues?"
laughing from what she said, you let her take a seat on the chair by bedside. "so you heard the whole thing."
"pretty much."
"well, hopkins was great. but you know, this place was my home, mer." sighing, and thinking that all of the things that happened today made you tired enough for days.
"i could hire you." she said nonchalantly. "are you serious?" you said in disbelief.
"you don't think i'm serious? i kinda own this place now."
"we could head down and sign the contract." looking at her, checking to see if she's really saying the truth.
"no. i–i can't." standing up and hushing your child as he started to turn again. "why? is it alex?"
"it's kinda stupid but yeah."
"what's stupid is the both of you are still in love with each other and you're not together."
"pretty sure he moved on mer. the minute i walked out on him. we were done." you said getting a cup of water and some apple slices which you presume was for your child.
"no. he still loves you."
"i left him after he proposed mer."
meredith stopped at what she was about, shocked at what she's hearing. "he proposed?"
"then i left him." you sternly added. "so that's why he always have this small box on his coat or pocket."
"what?"
"i think you weren't supposed to know that." she suddenly changed the topic about you and your program at hopkins and it turned to how you met your kid and how she loves her kids. just the both of you catching up.
the next day, meredith came back to check on the both of you for the third time. "are you sure you don't want to work back here, cause i told bailey and she's more than glad to have you back."
"mer–"
"we're moving here mama?" your kid said looking up from where he was playing with his toys.
"do you want to live here, sweetheart?" meredith asked him as she sat beside him.
"can we live near auntie mer?" lucas asked too excited. meredith looked at you with a smug grin on her face.
"bailey has an offer that tops hopkins." she said as she started to play with your kid again.
"fine. i'll talk to bailey, but i don't promise anything."
"i'll take it."
you didn't know how she convinced you to talk to bailey about your job here. but boy, meredith and bailey pulled out the big guns.
"look, i love the offer dr. bailey, but i just got here yesterday and–"
"we'll keep the position open for you dr. y/l/n, it would be really great to have you back with us." with that, you left the office wondering how you and lucas would be happier here.
to your surprise, alex was the one playing with your child.
"hey, where's aunt mer?" asking as you walked in. "she has a person to cure, mama." your kid said not looking up.
"your mama's back now, champ. i'm going now." alex said as he ruffled lucas' hair. "don't go! i love playing with you!"
"baby i'm sure dr. alex has some patients to cure too." your kid started to have tears forming on his eyes, but he tried his best not to let one tear out.
"how about we, uhh--" you can't find a pretty good excuse for lucas to feel better.
"hey how about this. you and you're mom are going home in a few hours, right?" alex asked and he nodded. "you play with your mama first, and i'll come back after curing my patients?" lucas looked at you for a signal and you smiled at him which he took and nodded at alex. "you promise uncle alex?"
alex was quite taken aback by the way he called him. the whole time the both of you were around, he always called him by 'dr. alex'
"yeah. i'll be quick."
you mouthed thank you at alex and went to your kid's side, starting to play with him.
"ma?"
"yes?" the both of you are now eating your lunch, well you're doing most of the eating.
"auntie mer said uncle alex is special to you." almost choking at what he said, you tried to hide the coughing. "she told you that?"
sipping water trying to compose yourself. "h-he is, sweetheart."
