i know you said that generally appearance suffix aren't used and can be insulting. are there any instances of a leader who gives a new warrior an appearance suffix just because they have personal beef?
Oh, for sure.
Rainfall named Minktail -tail due to his parentage, rather than his skills. Dude literally has a balance issue; his name was in poor taste.
Lizardstripe was renamed by Raggedclaw. Formerly Lizardheart, for the way she treated her son, Raggedclaw made a full ceremony out of it. Renaming a fully grown cat, especially when that cat has no desire to change their own name? Not only is it practically unheard of, it’s also just plain mean.
Wigeonfur was named in bad faith by her older half-brother, Rhema Turtlemask. Bad blood and all that, rivalry, bitterness… some cats just shouldn’t be offered positions of power, especially not when they might hold it over the heads of others.
That’s just a few via current characters, but throughout the fealty’s history, there’s been many, many more cases of such naming taking place.

Springstep - A fiesty and energetic she-cat. She is known for her boundless energy and will often be found showing off. She hates to lose, but often losing to someone strengthens her resolve to be the best. 15/10
Troutshine - A nurturing and kind she-cat. Troutshine is very motherly to all her clanmates, many seeking her out for advice and comfort. She will often be found around the nursery helping to look after the kits, but she often will visit the apprentices too, always greeted by the ‘paws clamouring for her attention and telling her about their training, 14/10
Berrystripe - This kind tom likes to take things slowly so he can fully take in his surroundings. His movements are careful and precise and he is always sure to consider his words before he speaks. Some younger cats are under the impression he is slow and a little harebrained but most warriors consider him wise, 13/10
Wigeonfur - This she-cat is intelligent and highly skilled and is considered a great warrior. She is diplomatic and many cats expect one day she will become leader. She cares deeply for her clanmates and will always go out of her way to help any of them, 13/10
Creekheart - This calm and stoic tom is often silent unless spoken to. He will never speak up in meetings unless his opinions are asked for, since he is such a good listener he often gives great advice. He can seem cold and uncaring to those that don’t know him, but in reality he would lay down his life for those he loves, 14/10
Ashspark - This bold and daring tom will often run headfirst into trouble, both intentionally and unintentionally. He will often rush into things at the slightest hint of trouble in an attempt to help, this can sometimes lead him to situations in which he becomes the one that needs rescuing, once it’s all over he will shrug it off with a purr as long as no ones hurt, before rushing off to his next adventure, 15/10

berryclaw about wigeonfur?
Berryclaw sees Wigeonfur as a valuable clanmate and though they don’t talk very often, a friend. Even from the first day she came to Nettleclan she was nothing but friendly to her, and so Berryclaw never saw her as an outsider. That may have been in part because Swanface accepted Wigeonpaw, but who knows. I feel like in another timeline they could have even ended up as mates, but that window has come and gone, at least for any foreseeable future. Their personalities mesh well tho!

what decides if a cat gets a pattern suffix vs. fur/pelt? Because i can see tortoiseshell cats on the allegiances who got -fur/pelt instead of -dapple, like Wigeonfur and Mosspelt (that is, if i understand the meaning of "dapple" as a non native speaker xD) and bicolor cats who didnt get -patch
The usual rule is that if the prefix already describes a pattern, the suffix also describing that pattern would be redundant. For example, Cherry- means "a brown tortoiseshell", so it's very unlikely for that cat to be named Cherrydapple or Cherryspeck because, well, you already know she's dappled/speckled just from her prefix. Same goes for bicolor cats - a lot of bicolor names are specific to that singular definition of (color) with white, so using -patch is more rare than you'd expect (RiverClan does often use names that don't imply white coloring for the sake of variety in their very-much-bi-or-tricolored community, but since everyone kind of expects that from them and knows to guess that Gingerpaw of RiverClan is actually ginger-and-white, -patch doesn't see a lot of use there either).
There are exceptions to this rule, as there are to many things in this culture - if a particular name sounds good when paired with a pattern suffix, the leader may be inclined to name the up-and-coming warrior accordingly (consulting them first, of course); or a cat may develop starker markings as they age, meaning their prefix doesn't quite showcase what they look like as an adult. But overall, you're very unlikely to meet a Brindlespot or Starkstripe. It's like someone's first and middle/last name being the same thing. Doesn't always work that well.

Riverward Prefixes
As with all faction-specific prefixes, the Riverwardens name their members after their general surroundings first and foremost.
For the Riverward Faction specifically, their prefixes generally relate to the freshwater fens, mangroves, fenlands, and moist forests of their territory, and everything in it. This includes various trees, riverside or water-dwelling plants and animals (most commonly: fish, amphibians, and water-dwelling predators), mosses, and herbs.
Desirable traits, sounds, and more complex subjects are also quite popular, and found almost exclusively within their ranks due to the other faction’s views on how “ditzy” they sound.
More specific examples:
