Do you know how our understanding and treatment of diabetes has changed through history?
Oooh good question, anon!
As you may guess, diabetes mellitus is not new.
We've known about it since at least the Ebers Papyrus (1550 BCE) when the disease and a treatment was first described. This treatment was: "a liquid extract of bones, grain, grit, wheat, green lead and earth." I did not look these up, but I would guess they did not do a whole lot for the treatment of diabetes.
Later during the 6th century BCE it was first given a name when it was described by Hindu physician Sushruta as madhumeh or "honey urine."
Honey urine is a very apt descriptor for diabetes. In any type, one of the most measurable symptoms is that the person urinates a lot, and the urine tastes sweet (or, if one didn't feel like tasting, that it ferments, or that it attracts ants). This was also the first test for diabetes.
The reason for the sweetness of the urine (as well as a lot of other general info about diabetes) is spelled out more clearly in my "Don't Be That Guy Who Wrote Hansel and Gretel: Witch Hunters" post.
A Greek physician Apolonius of Memphis named it Diabetes, meaning "to siphon" (referring to the large amount of urine lost).
Roman physician Aretaeus later made the first precise description of diabetes. This included the classic symptoms of incessant thirst, copious urination, and constant hunger leading to emaciation and death. He also notes that if deprived of water, the patient will continue to urinate until they become so dehydrated that they die.
The term "Mellitus" was not added until the 1600s by an English physician Thomas Willis. This was again due to the sweetness of the expressed urine. Willis prescribed a diet of "slimy vegetables, rice, and white starch. He also suggested a milk drink which was distilled with cypress tops and egg whites, two powders (a mixture of gum arabic and gum dragant), rhubarb and cinnamon". Supposedly his patients improved if they kept to this diet, though few managed it long term. I honestly don't know how it would have worked, even temporarily.
A major breakthrough came in 1889 when it was discovered that if you removed the pancreas from a dog, the dog would become diabetic (particularly, that it would urinate large quantities of sweet urine). Up until this point it was thought that diabetes stemmed from the kidneys and bladder, or perhaps the lungs. This was the first time it had been shown experimentally that the pancreas was the problem.
Speaking of this, this was also part of a series of experiments where an English physician named Merkowski implanted a small amount of pancreas in the pancreas-less dog's fat, which reversed the diabetes temporarily. This proved that the pancreas was making something that helped regulate blood (and thus urine) sugar.
What this was wasn't figured out until 1921, when Canadian scientists Banting and Best (with help from McLeod and Collip) isolated something they called insletin (after the islets of langerhans, where the substance was being produced). It's important to note that all of these scientists hated each other so much they almost refused a Nobel Prize over it. Later, Collip would refine the substance and McLeod would rename it insulin.
Prior to insulin existing there was basically 1 vaguely useful treatment for diabetes. Unfortunately, that was starvation. So you could either die a slow and painful death by diabetes or you could die a slightly less slow but still painful death due to eating about 500 calories per day. Either way, diabetes was fatal, usually within a couple of years of diagnosis.
By 1923, the first commercial insulin product, Iletin, had been developed. Iletin was a U10 insulin (10 units per 1 milliliter- less potent than today's U100 and U500 insulins) and was made from pork pancreases. It took nearly a ton of pork pancreas to make 1oz of insulin. Fortunately, as a byproduct of the meat industry, pancreases were readily available.
Now, you might be thinking- no one has mentioned type 1 or type 2 yet in this entire post!
Well, you would be right, because diabetes wouldn't be split into 2 forms (insulin-dependent and non-insulin dependent) until 1979, and wouldn't be classified as types 1 and 2 until 1995. That's right- some of you were alive when there was only one kind of diabetes out there.
Now, there's more about the types in the Hansel and Gretel post, but essentially type 1 diabetes occurs when the pancreas itself stops producing insulin, usually in childhood. When this happens, the body stops being able to use sugar (insulin, a hormone, acts as a "key" to let sugar into cells for use). Without replacing that insulin, the person dies because their cells starve.
Type 2 diabetes occurs when the pancreas still produces insulin, but the cells stop responding to it correctly. This causes high sugar levels in the blood, which causes longer-term complications (infections, ulcers, blindness, neuropathy, heart and kidney disease, hyperosmolar syndrome, etc..) which eventually lead to death.
We started discovering oral drugs that worked on what would later become type 2 in the 1950s. Particularly those that worked by increasing the insulin output of the pancreas, but only when the pancreas was still producing some insulin.
Predicting which diabetics would benefit from oral therapies was challenging, but it was recognized that when the onset of diabetes was slow and came on in adulthood, the oral agents would work, while if it came on suddenly in childhood, the oral agents wouldn't. Terms like "adult onset" and "maturity onset" were common:
(Side note: if you have ever read Alas, Babylon (1955) there is a diabetic character who by today's standards clearly has type 1 diabetes, but wants to switch to the "new oral pill" (called "orinase" in the book, though they are likely referring to diabinese pictured above).)
From 1923 into the 1980s, insulin was given once or twice per day, and not particularly titrated to blood sugar. This was probably just because we didn't have a great way to measure blood sugar in real time. Pre-1970s, there was no way to test blood sugar outside of a lab setting.
Urine testing was common starting in the 1940s, but was cumbersome as it required a flame for heating the urine. By the 1950s, a test had been developed that didn't require a flame, but was still not practical for home use. In the 1960s, paper strips were developed that changed color for different amounts of sugar in the urine. The problem with this was that the strips couldn't change color until there was sugar in the urine- a blood sugar level of over 200 by today's measurements. Low blood sugar readings were impossible at this time, and had to be treated based on symptoms.
In the 1970s, blood sugar could finally be measured by putting a drop of blood on a test strip, wiping it off, and matching the color of the test strip to a chart. While less cumbersome than urine tests, this was still something that would generally only be done at a doctor's office.
In 1983, the first home blood glucometer is developed. Finally, it was practical to take one's sugar multiple times per day, and it becomes possible to experiment with "sliding scale" insulin injections that keep tighter control of blood sugar. By the late 90s, continuous glucose monitors became available- though unlike today's CGMs that allow readings in real time on a smartphone or monitor, these had to be downloaded to a computer at regular intervals.
The 1980s were the first decade where insulin pumps become widely available. The very first pump was large and had to be carried in a backpack, but it represented a huge step forward in glucose control, as it more closely mimicked the function of a working pancreas than once-daily injections.
For the next 30 or so years you really had to work to qualify for an insulin pump, but recently it's been found that pumps greatly improve compliance with blood glucose control whether or not the person had good compliance before getting the pumps, and insurance has gotten better about covering them (though CGMs are still a pain to get insurance to cover).
The 1980s was also the decade that recombinant human insulin (insulin made by genetically modified bacteria) was first used. Up until that point the only insulins were pork and beef insulins, which some people had allergic reactions to. Recombinant insulin was closer to regular human insulin than beef or pork, and represented a big change in how insulin was made.
Today for people who take insulin to manage their diabetes, insulin is usually given as a single injection of a long-acting basal insulin, coupled with smaller doses of ultra-short-acting insulins with meals or snacks. This is the closest we've gotten to mimicking the way a pancreas would work in the wild, and keeps very tight control of blood sugar. This can be done by fingerstick blood sugar tests and individual injections of insulin, or it can be done with a CGM and pump- it just depends on the resources available to the person and their personal preference.

I was sitting in the principal’s office when he came in, storming into the place like a hurricane. The door almost getting out of it’s place. I flinch at the loud sound and he doesn’t notice. He sits by my side with his eyebrows frowning and a look in his eyes that was kinda scary. I’m on my phone so it’s not awkward and there’s not weird looks in between us. He has his backpack across his chest and grabs it really tight like his life depends on it. I look at him slightly with my peripheral vision and notice his heavy breathing on my ears. His heart was probably going a mile per minute.
“you ok?” I ask without taking my eyes of my phone and the pictures I was watching. He takes notice of me and my question.
“you talking to me?” he sounds raspy like he was fighting to talk.
“do you see anyone else on the room I could be talking to instead?” my response comes out a bit more rude than I expected so I change my face and turn around.
“wow, rude girl, I see” he softens the grip on his backpack and turns too.
“sorry, I didn’t mean for it to come out like that” I really didn't.
“s’okay, I can imagine you got a bad day if you’re here” at least he’s nice.
“so do you right?” anyone who gets to meet the principal’s office is definitely having a bad one.
“you could say that, yeah” I get a confused look on my face as he stares at me. “what?” he asks in between laughs. Cute.
“what did you do?” I ask softly “to be here, what did you do?” I repeat.
“oh, uhm- I shouldn’t tell you, s’gross” he scrunches his nose as he speaks.
“ah come on, it can’t be worse than Jimmy Cooper sticking his used gum on Mrs. O’Donnell lunch last week. Can it?” there’s a little fear in my words as I can’t believe anything worse or more gross than that. He makes a face that indicates to me that it is, in fact, grosser than what Jimmy did. “what the fuck did you do, Munson?” I’m now on the edge of my seat.
“wait, you know my name? how come I don’t know yours but you know mine if I’ve never seen you before?” he’s at the edge of his now.
“you might have this rare disease called “not noticing pretty girls”, s’bad, if I were you I’ll be heading to the doctor right away after talking to the principal” I press my lips together in a fine line when I’m done talking and I nod, then laugh.
“ouch, I’m sorry lady. S’there a cure for this?” he ask with a hand on his chest, he takes it seriously as he can’t take his eyes off of mine.
“probably, haven’t been discovered yet tho. I know, s’shitty, m’sorry man, good luck at the doctor’s appointment anyways” we both chuckle after a few seconds. This is probably the best interaction I’ve had in years.
“no worries, I’ll call you when I get real sick” he says.
“what for?” I get confused.
“gonna need a nurse to take care of me and my poor state” his voice gets deeper as he talks like this.
“you should know I charge more for specialized treatment tho” I raise my brows at him, I’m not easy to get.
“I can pay you whatever way you need or want” he plays with me.
“oh now you’re just teasing. You’re awful Eddie, you killed the vibe” I get a finger in the air, accusing him in an also playful way.
“so there was a vibe, I wasn’t crazy after all” he moves the chair closer to me. “what did you do to be here” his eyes narrow on my way.
“don’t change the subject, I asked you first” I was not gonna forget that he never answered.
“well I don’t wanna tell but you insist so here we go. I got an f on one of my geometry test just because Mr. Larson doesn’t like me, I did good and he failed me, so I pissed his car door” I wish someone could take a picture of my face in this exact moment a frame it to put in the louvre “I told ya it was gross, you asked for it baby”
“don’t call me baby after you said that, eww” I scrunch my nose in disgust “although I think he deserves it, he’s pretty mean to me too”
“ohh we can’t have him being mean to baby right?” he teases again. I throw a look at him, letting him know what I mean. “sorry for the teasing, it’s who I am”
“wanna know what I did now?” I try my best to keep the topic of him teasing me as far as I can.
“yes, please”
“I got in a verbal fight with one of my classmates and I was in the right, but my teacher picked sides with my classmate and told me to shut up so I told her to go fuck herself. You should’ve seen her face, she could not believe what she heard. Was fucking hilarious” my foot was now closer to his, almost touching each other.
“savage” he says in a funny tone, but looking at the floor where our feet were now touching.
“I know, I know, I’m iconic” I joke, trying to make him laugh and I’m successful.
“I think is really brave to stand for yourself with an adult who doesn’t know what respect for the others is” most of his words come out shaky but genuine. I was about to say something to him but the door was opened and the secretary told us the principal was gonna be late for another half an hour, so we had to wait more.
“fucking awesome” I say. I watch as she closes door and Eddie goes to the principal’s chair and sits on it.
“he sure has better things to do right? Such a busy man” he sits straight and begins to talk again. “if it wasn’t for me this school would be a little shit dumpster. All this kids are nothing without me” his voice sounds funny since he’s trying his best to copy Mr. Kohl’s tone. “I’m a man of word, and you should trust when I tell you I’m gonna make this delinquents into decent people no matter how I have to do it” he continues.
“oh sir, but all that work must be so stressful, isn’t it?” I follow his game and he looks at me with open eyes.
“indeed it is sweet girl, why you ask?”
“I thought maybe…” I stand of my place and approach him. “maybe I could help you sir”
“you think you can young lady?” he says. I put one leg on each side of his sitting body. “what you think you’re doing?” he stutters in between words. I feel his body tense underneath me.
“I’m helping you sir, I can make all the stress go away and I’m gonna show you how” it’s not a secret I like Eddie, all of my friends know it. He’s the one that never noticed me, but I never cared; until now, where I find myself about to show him what he’s been missing.
“I-i- shit. I don’t think you shou- oh my god” all he can do is whimper from the way my hips are bucking into his right now.
“come on now sir, don’t you like me?” he probably should know I’m not playing anymore, this is true now. I pout and look intensely into his dark brown eyes.
“s’not that sweetheart, I-you shouldn’t be doing this here”
“you heard the secretary, he’ll be gone for half an hour more, we got time” he doesn’t seem to be comfortable with the situation as he keeps looking at the door. “I find the fact that someone could find us so fun, don’t you? I thought you were cool Munson”
“I am”
“not right now, you’re not being cool” my hands travel to the back of his neck and I kiss him deeply, desire building in my heart.
“fine” he says as soon as he can without stopping the kiss. One of my hands tries to unbuckle his belt with difficulty, and he helps me with his own hands. I lift my skirt to let him see my lilac and already wet panties. “holy fuck” the air I his lungs leaving is my favorite sound until now. Like he’s devoted to my body. I touch him over his boxer and I can’t believe he goes around packing like that. I don’t even exaggerate when I say he’s the biggest I’ve ever had. “found something you like?” I can’t take my eyes away from that part of him but I force myself and look up to him.
“maybe…” I say as I run a finger ever so slightly over his clothed length making him hiss at the contact.
“you’re such a tease” his tone let’s me know he doesn’t like it, he prefers if I just take it. But I’m not doing that just yet. This is too good to be so short.
“well, I was about to be put in detention for a reason sir” I keep my slight touch on his chest this time.

“I do sir, I misbehave so I can be with you in this tiny room, so I can be in your lap, so you can feel how good I wanna be for you” now where the fuck was that amount of words coming from? It’s like I was possessed and I liked it.
“my God, you’re so dirty baby” his voice is now so low, I can feel how he gets hard under me even more.

A 4-year-old from Detroit has a confirmed case of the measles, and potentially exposed others while seeking treatment for the highly infectious virus at DMC Children's Hospital of Michigan, an urgent care center and a pharmacy in the city, the Detroit Health Department announced Wednesday.
It is Michigan's fifth documented measles case this year.
The child was unvaccinated, said Dr. Claudia Richardson, the city health department's medical director, and had no history of international travel. Measles is so contagious, 90% of unvaccinated people who are exposed to the virus will become infected.
City health officials said they are working with the state health department to investigate where the child might have been exposed to the virus. So far, however, the case appears to be isolated, Richardson said.
"This is definitely an ongoing investigation," she said, "so we're in the middle of that. At this time, we do not have a source of the infection."
The child was not hospitalized, Richardson said, but was treated in the emergency department at Children's Hospital and is now following isolation protocol.
Anyone who was at the following places at these times and dates may have been exposed to the virus:
From noon to 3 p.m. April 1 at Acadian Urgent Care, 2117 Springwells, Detroit.
From 1:45-4 p.m. April 1 at the Rite Health Pharmacy, 5851 W. Vernor, Detroit.
From 5-10 a.m. April 3 in the emergency department at DMC Children’s Hospital of Michigan, 3901 Beaubien Blvd., Detroit.
Children’s Hospital of Michigan did not respond to questions from the Free Press about how many people may have been exposed to the measles in the emergency department on April 3 or about how the hospital is notifying them.
Health leaders urge anyone who might have been exposed to the virus April 1-3 to monitor themselves for symptoms for 21 days. If symptoms develop, call ahead before visiting a doctor, urgent care center or hospital emergency room so precautions may be taken to avoid exposing others.
"We're ... doing our routine communicable disease investigation, which requires us to reach out to anyone that may have had direct contact. So far, all we know are family members," Richardson said, adding: "We are trying to get the word out the best we can."
What are the symptoms of measles?
Measles symptoms typically begin seven to 14 days after exposure, but can first appear as long as 21 days after exposure and can include:
High fever that could reach 104 degrees or higher.
Cough.
Runny nose.
Red, watery eyes (conjunctivitis).
Tiny white spots on the inner cheeks, gums and roof of the mouth called Koplik Spots, which usually appear two to three days after symptoms begin.
A rash that starts on the face and spreads to the trunk, arms and legs, typically three to five days after symptoms begin. The rash is red, raised and blotchy.
"Infected people can spread measles before noticing any symptoms, including four days before and four days after the rash appears," Denise Fair Razo, chief public health officer for the Detroit health department, said in a statement. "Our best advice is to get vaccinated against measles if you have not already done so. It is part of the two-dose measles, mumps, rubella (MMR) series of childhood vaccines."
2 doses of MMR vaccine are 97% effective at preventing measles
Measles infections are preventable with widespread vaccination. Two doses of the measles, mumps and rubella vaccine (MMR), offer 97% protection from infection. A single dose is about 93% effective.
However, in Michigan, immunization rates have fallen to 66% among Michigan toddlers — the lowest point in more than a decade for recommended childhood immunizations, according to MDHHS.

What is the recommendation for measles vaccinations?
The U.S. Centers for Disease Control and Prevention recommends:
A first dose of the MMR vaccine for children ages 12-15 months old, with a booster dose administered between ages 4 and 6.
Anyone born in or after 1957 without evidence of immunity against measles or documentation of having been vaccinated with two doses of MMR vaccine should get vaccinated.
Two doses are recommended for adults at high risk, such as international travelers, college students and health care personnel born during or after 1957.
People exposed to measles who cannot document immunity against the virus should get post-exposure prophylaxis — a dose of the vaccine to potentially provide protection within 72 hours of initial exposure, or immunoglobulin within six days of exposure.
How serious is a measles infection?
Measles can cause serious illness, long-term disability and death in people of all ages.
About 1 in 5 people who get the measles end up hospitalized, according to the CDC, and 1 in 20 kids with measles develops pneumonia. Roughly 1 out of every 1,000 children who have measles infections will also have encephalitis. Brain swelling from encephalitis can lead to convulsions, permanent hearing loss, intellectual disability and death.
Anywhere from 1 to 3 out of every 1,000 children with measles dies from respiratory or neurologic complications from the virus.
"We've had the vaccine since 1963, and before the vaccine was available, every year across the U.S., between 400 and 500 people died," Dr. Natasha Bagdasarian, Michigan's chief medical officer, told the Free Press for a previous article. "Four hundred to 500 people may not seem like ... huge numbers, but it is a lot, especially if you're talking about your loved one or your child."
Notification too late for post-exposure treatment
The Detroit child's measles case was confirmed Tuesday, Richardson said, but public notification was not made until Wednesday afternoon, when anyone who was potentially exposed to the virus would be outside the window for treatment with post-exposure prophylaxis to prevent infection.
A dose of the MMR vaccine can be administered 72 hours of initial measles exposure and immunoglobulin can be given intravenously within six days of exposure to potentially prevent illness, according to the CDC.
By Wednesday afternoon, that window had closed.
"We are out of both of those windows and ... we were going through the dates very rigorously," Richardson said.
What can be done now, she said, is to educate the public about the potential exposures, what to do if they develop symptoms, and to urge vaccination.
"That's where we're focusing," Richardson said. "When situations like this arise, it brings preventable diseases such as measles to the forefront. ... It's the perfect opportunity for us to stress the importance of vaccination."

The Detroit Health Department offers vaccinations for measles and all childhood vaccination series, plus COVID-19, flu and Mpox at its Immunizations Clinic at 100 Mack Ave. Hours are: 8:30 a.m.-4:30 p.m. Mondays-Fridays by appointment. Walk-ins are welcome from 9 a.m.-3 p.m. Late hours are available on the first and third Thursdays of each month, when the clinic is open 10 a.m.-6:30 p.m.

Michigan is not at outbreak status
This is the fifth known measles case in Michigan in 2024.
Two cases were identified in March in Washtenaw County. The second person is believed to have been exposed to the virus at Trinity Health Ann Arbor's waiting and triage area, where another Washtenaw County resident contagious with measles had gone for care.
An adult from Wayne County who had traveled internationally and had no record of measles immunization had the virus in February.
An Oakland County child who was unvaccinated and had traveled internationally was the first known case of measles in Michigan this year, state health officials announced Feb. 23.
Although Michigan health leaders have reported five measles cases since February, the situation doesn't yet rise to the level of an outbreak, according to the definition set by the CDC, which identifies an outbreak as a chain of transmission including three or more cases linked in time and space.
As of April 4, there have been 113 cases of measles in the U.S. this year, according to the CDC, and 58% of the people infected with the virus nationally in 2024 have been hospitalized for isolation or for management of measles complications.
Among those 113 cases, the CDC has recognized seven measles outbreaks, which include 73% of cases identified in 2024.
Comparatively, there were 58 total U.S. measles cases in 2023 and four outbreaks.

Ciao lovelies! Today’s post is another REAL TALK post, meaning it includes anecdotes, and some content not suitable for younger readers (possibly, depending on your sensitivities.) Today’s REAL TALK is about living with Anxiety.
I wanna start by throwing out some general statements to outline the general idea here. Anxiety is a real problem. Many children, teens and adults in America face it, in fact, 5.7 % of adults in the USA have severe generalized anxiety, 25.1 % of teens 13-18 have anxiety that lingers through their lifetime, and 28.8 % of adults have any kind of anxiety, with the average age of onset being a mere eleven years old. Anxiety is a horribly prevalent disease, bordering on an epidemic. Yet many don’t talk about this subject, dismissing it as “just nerves” and the like. But Anxiety can really control and ruin your life. There are many different types of Anxiety, the most common being generalized and social.
I, myself, have Anxiety, and it is a constant challenge to overcome it to do the things I love.
I love to go to conventions and cosplay, but at first, my Social Anxiety made it impossible. I would have Anxiety and Panic attacks when going to conventions. I couldn’t go into the crowded dealers rooms and I certainly couldn’t ask my favorite cosplayers for a photo. My first few conventions were no fun because of my Anxiety.
But I wanted to enjoy my conventions. So I pushed myself into the situations that scared me. I even got to the point where I could host a panel!
Am I saying that you should force yourself into uncomfortable situations to overcome your Anxiety? NO. Anxiety attacks are horrible experiences that I wouldn’t wish on my worst enemy. The feeling of your chest tightening, the fight-or-flight response making your body shaky, the confusing and vulnerable headspace...It's awful.
No, to be honest, I don’t really know a healthy way to “overcome” Anxiety disorders. I know ways to help cope, however.
I personally prefer physical coping mechanisms. Things like fidget toys occupy my hands when they anxiously want to fidget, breathing techniques can help calm my chest, and even chewing gum can help curb anxious feelings.
When all else fails, I allot myself the time required to come down from an Anxiety Attack.
I also utilize ASMR videos to help with night time Anxiety, and insomnia and nightmares from Anxiety.
I think that everyone is different when it comes to what helps with Anxiety. There is not one universal method. But I encourage you to try out different methods! It may surprise you what works for you!

An implantable device could enable injection-free control of diabetes
The device contains encapsulated cells that produce insulin, plus a tiny oxygen-producing factory that keeps the cells healthy.

One promising approach to treating Type 1 diabetes is implanting pancreatic islet cells that can produce insulin when needed, which can free patients from giving themselves frequent insulin injections. However, one major obstacle to this approach is that once the cells are implanted, they eventually run out of oxygen and stop producing insulin.
To overcome that hurdle, MIT engineers have designed a new implantable device that not only carries hundreds of thousands of insulin-producing islet cells, but also has its own on-board oxygen factory, which generates oxygen by splitting water vapor found in the body.
The researchers showed that when implanted into diabetic mice, this device could keep the mice’s blood glucose levels stable for at least a month. The researchers now hope to create a larger version of the device, about the size of a stick of chewing gum, that could eventually be tested in people with Type 1 diabetes.
“You can think of this as a living medical device that is made from human cells that secrete insulin, along with an electronic life support-system. We’re excited by the progress so far, and we really are optimistic that this technology could end up helping patients,” says Daniel Anderson, a professor in MIT’s Department of Chemical Engineering, a member of MIT’s Koch Institute for Integrative Cancer Research and Institute for Medical Engineering and Science (IMES), and the senior author of the study.
While the researchers’ main focus is on diabetes treatment, they say that this kind of device could also be adapted to treat other diseases that require repeated delivery of therapeutic proteins.
MIT Research Scientist Siddharth Krishnan is the lead author of the paper, which appears today in the Proceedings of the National Academy of Sciences. The research team also includes several other researchers from MIT, including Robert Langer, the David H. Koch Institute Professor at MIT and a member of the Koch Institute, as well as researchers from Boston Children’s Hospital.
Replacing injections
Most patients with Type 1 diabetes have to monitor their blood glucose levels carefully and inject themselves with insulin at least once a day. However, this process doesn’t replicate the body’s natural ability to control blood glucose levels.
“The vast majority of diabetics that are insulin-dependent are injecting themselves with insulin, and doing their very best, but they do not have healthy blood sugar levels,” Anderson says. “If you look at their blood sugar levels, even for people that are very dedicated to being careful, they just can’t match what a living pancreas can do.”
A better alternative would be to transplant cells that produce insulin whenever they detect surges in the patient’s blood glucose levels. Some diabetes patients have received transplanted islet cells from human cadavers, which can achieve long-term control of diabetes; however, these patients have to take immunosuppressive drugs to prevent their body from rejecting the implanted cells.
More recently, researchers have shown similar success with islet cells derived from stem cells, but patients who receive those cells also need to take immunosuppressive drugs.
Another possibility, which could prevent the need for immunosuppressive drugs, is to encapsulate the transplanted cells within a flexible device that protects the cells from the immune system. However, finding a reliable oxygen supply for these encapsulated cells has proven challenging.
Some experimental devices, including one that has been tested in clinical trials, feature an oxygen chamber that can supply the cells, but this chamber needs to be reloaded periodically. Other researchers have developed implants that include chemical reagents that can generate oxygen, but these also run out eventually.
The MIT team took a different approach that could potentially generate oxygen indefinitely, by splitting water. This is done using a proton-exchange membrane — a technology originally deployed to generate hydrogen in fuel cells — located within the device. This membrane can split water vapor (found abundantly in the body) into hydrogen, which diffuses harmlessly away, and oxygen, which goes into a storage chamber that feeds the islet cells through a thin, oxygen-permeable membrane.
A significant advantage of this approach is that it does not require any wires or batteries. Splitting this water vapor requires a small voltage (about 2 volts), which is generated using a phenomenon known as resonant inductive coupling. A tuned magnetic coil located outside the body transmits power to a small, flexible antenna within the device, allowing for wireless power transfer. It does require an external coil, which the researchers anticipate could be worn as a patch on the patient’s skin.
Drugs on demand
After building their device, which is about the size of a U.S. quarter, the researchers tested it in diabetic mice. One group of mice received the device with the oxygen-generating, water-splitting membrane, while the other received a device that contained islet cells without any supplemental oxygen. The devices were implanted just under the skin, in mice with fully functional immune systems.
The researchers found that mice implanted with the oxygen-generating device were able to maintain normal blood glucose levels, comparable to healthy animals. However, mice that received the nonoxygenated device became hyperglycemic (with elevated blood sugar) within about two weeks.
Typically when any kind of medical device is implanted in the body, attack by the immune system leads to a buildup of scar tissue called fibrosis, which can reduce the devices’ effectiveness. This kind of scar tissue did form around the implants used in this study, but the device’s success in controlling blood glucose levels suggests that insulin was still able to diffuse out of the device, and glucose into it.
This approach could also be used to deliver cells that produce other types of therapeutic proteins that need to be given over long periods of time. In this study, the researchers showed that the device could also keep alive cells that produce erythropoietin, a protein that stimulates red blood cell production.
“We’re optimistic that it will be possible to make living medical devices that can reside in the body and produce drugs as needed,” Anderson says. “There are a variety of diseases where patients need to take proteins exogenously, sometimes very frequently. If we can replace the need for infusions every other week with a single implant that can act for a long time, I think that could really help a lot of patients.”
The researchers now plan to adapt the device for testing in larger animals and eventually humans. For human use, they hope to develop an implant that would be about the size of a stick of chewing gum. They also plan to test whether the device can remain in the body for longer periods of time.
“The materials we’ve used are inherently stable and long-lived, so I think that kind of long-term operation is within the realm of possibility, and that’s what we’re working on,” Krishnan says.
“We are very excited about these findings, which we believe could provide a whole new way of someday treating diabetes and possibly other diseases,” Langer adds.
The research was funded by JDRF, the Leona M. and Harry B. Helmsley Charitable Trust, and the National Institute of Biomedical Imaging and Bioengineering at the National Institutes of Health.
