I replayed recently and rewatched that last playthrough. It's tough to tell when Watson gives him the drug, but Sherlock stops freaking out when Watson says it's okay. If you look at the whole of the story in Awakened I think the point of the scene was to show the trust between Holmes and Watson.

Sherlock, who knows that his mother deteriorated under a strict drug regimen that included sedatives and hallucinogens just went through a case where the victims were given sedatives and hallucinogens, where he was knocked out by a drug in the asylum, and where he has has had hallucinations which may have been the result of the cultists drugging him somehow. So he sees the needle and starts to freak out, until Watson tells him it's all right and he calms down. He's not immediately knocked out, but calms down and goes to sleep.

Watson, who's been talking about starting his writing career all game, burns his manuscript at Sherlock's request.

The music is odd and foreboding, but it's not timed so that the needle seems as big a mistake as burning the manuscript does. If anything is foreboding and bad in this scene it's that both actions are a burial of the truth. The sedative is going to let Sherlock sleep without any lovecraftian dreams, and the manuscript will let the details of the case remain hidden.

It's interesting in light of Chapter One, where Sherlock doggedly searches for the truth and declares at any of the endings that knowing was better than forgetting. Here he doggedly searches for the truth again, Mycroft even compares it to his Cordona quest, and at the end requests two things of Watson: Destroy the manuscript, and help me sleep without these dreams. The sedative helps dull the dreams, and the manuscript gets burned.

I mean, it could lead to Sherlock's drug habit, but I don't see it as a "Oh, he had a sedative so now he'll go to cocaine." No, cocaine is a stimulant and self-medication for ADHD-like symptoms. This was to make him fall asleep. Instead, it will be the action of self-denial at the end, burying the truth about the Awakening, trying to forget what he's seen, that will lead him to cocaine use.

And the cocaine use has already been seeded in Chapter One. He already has the drug kit, and it came from Verner Vogel. Verner even SUGGESTED it for the same reason Holmes takes it in canon, as a way to relieve boredom. Boredom for Sherlock now has an additional danger after Chapter One and the Awakened, he might find his mind drifting back to the things that pained him.

So to me, I'm pretty sure if and when recreational drug use surfaces in this series, it will lead back to the scene with Verner Vogel at the party and not Watson's treatment. Sherlock would be the first to give Verner's name to Mycroft to get him away from Watson.

I am with you that Mycroft would freak at a sedative alone, since that's what they started his mother out with, I just think Watson has already proven he's not Otto Richter. It wouldn't be out of character as we've seen so far for him to show up to check on Sherlock after the Lighthouse. Someone had to clean up that mess, after all. Really, Watson's hesitation on the sedative could stem from already having had a visit from Mycroft and getting the family history.

Consider what Mycroft knows about Watson going into that encounter in the Awakened.

Veteran -- Not just that he has war service, but he knows the story that Watson told Sherlock a few scenes earlier, the one that he's so ashamed of

Doctor, General Practitioner

Watson's patient list -- Mycroft knows Watson treats Stenwick, he probably knows EVERYONE Watson treats.

Watson frequents Barnes' Bookstore -- where Mycroft finds him after the first meeting.

Watson's brother has a drinking problem (Also not in the game, but it's in A Sign of Four)

Watson had to move in with Sherlock because he had gambled his savings away and now had to live off his monthly pension. (Study in Scarlet)

Mycroft would have to distinguish between multiple profiles after gathering the facts on any person just like Sherlock does in-game, and for Watson here you'd get the choice between Cowardly Thrill-Seeker or Adventurous Humanitarian.

Mycroft settled on Cowardly Thrill-seeker. Then he waits for them in that sitting room, staring at that portrait of his FATHER and his MOTHER stewing on what Sherlock's just done with his disreputable DOCTOR friend and the first thing he does is call Watson a coward. Watson isn't pissed off by this. Instead, he forgets the insult the second he learns Mycroft can help them and asks for help. Mycroft insults him again and says the Crown won't help and THEN Watson lets him have it.

Should've picked Adventurous Humanitarian, Mycroft. I think that's why Mycroft tracks Watson down and reasons with him. And giving him the task and the last piece of evidence are major signs of trust, not desperation.

Neither Holmes brother forgets a mistake. Watson's filed under "People who can surprise you" in BOTH their minds, and I would expect Watson to be a bridge next time the brothers are at odds, even if Mycroft knows Watson used drugs for treatment.

Tonight on Thoughts That Fuck Me Up:

At some point, Mycroft probably came to the messed up realization that his little brother was safer outside than at home. Sherlock was already spending much of his time outside, and Mycroft encouraged it by sending him on all kinds of errands and assignments across Cordona, from stealing letters to searching for stupid rare coins. He had Sherlock spy on people for him, not because he couldn't do it himself, and not just because he wanted him to master the craft either, but because Mycroft would rather be the one to stay home and face the violent outbursts a million times over than have it be his baby brother. He let him be wild and free and spend as much time outside as possible even if it worried him sick (i wonder if sherlock ever told him about the slavers incident...), because, at the very least, the outside was an environment that Mycroft COULD control—manipulate in his favor so that, if for any reason he wasn't physically present to protect Sherlock, his reputation would. He'd been establishing connections with people everywhere he went since they arrived, compiling dossiers and dirt on everyone he met, securing the island for himself and his family. So at the very least, and especially when Mycroft was busy or away, Sherlock would be out in a public that either respected, feared, or owed Mycroft Holmes so many favors, watched over by his agents and other people he trusted, and not trapped inside a depressing manor with an unstable, abusive mother and a creepy doctor..
