Just to be clear:
"Intentionally" as in "non-accidentally/naturally." So pulling on a loose baby tooth to get it out would NOT count, but having a baby tooth surgically extracted because there was something wrong WOULD count. Getting in an accident and knocking some teeth out would NOT count, but if they were broken in an accident and needed to be removed the rest of the way for safety reasons, that WOULD count.
Baby, adult, and wisdom teeth count provided they fit the previous definition.
