The trope of a character being totally incapacitated/overwhelmed by pain is living rent free in my mind rn.
Like all they can do is scream or cry, it hurts so bad they can’t even form words. They might be shaking or hyperventilating, breath hitching if another wave of pain hits them. They can’t stop the tears flowing down their face. Caretaker/team tries to soothe them but nothing works—they don’t even respond.
Or, maybe they do, but its a quiet, pleading “please, make it stop,” between cries. Maybe they beg. “Just shoot me.”
Caretaker/team probably wouldn’t know how to respond. “We’re trying,” they say. Or, “No. I won’t do that.”

There’s something about a whumpee just sitting down. Not fainting, necessarily. Maybe they’re just about to faint, and they quietly just kneel on the ground at a time and place that doesn’t make sense. They don’t even have the capacity or willingness to articulate why they need to abruptly stop and sit. Maybe they’re catatonic while the others look at them.
Maybe a caretaker can see the dull, vacant look in their eyes and immediately senses that something is seriously wrong. Maybe the fainting comes just a few moments later.

Obsessed with sick cuddles. I love it when people have a bad cold and just long for affection and warmth. I love it when someone is sniffly and tired and feverish and cold, their body begging to be held and comforted. I love it when they lean into the touch of their caregiver/significant other with such pleading affection, they just need to be held. I love it when said caregiver holds them close and lets them rest their feverish head on their chest, whispering sweet nothings into their ear, because the poor thing just doesn’t feel good.

Cold weather prompts
It’s so cold here right now so this is very personal
Sick Whumpee working in freezing weather, only making them sicker.
Caretaker rubbing Whumpee’s hands together and blowing on them to warm them up. Huddling together because it’s the only warmth either of them has.
Whumpee being found half-buried in the snow. (By Caretaker or Whumper?)
Whumpee who refused to dress for the weather arrives home numb and shivering. Queue Caretaker having to scold them but also make sure they get warmed up as soon as possible.
Injured Whumpee running and hiding from Whumper, but leaving a trail of blood in the snow.
A frozen lake breaking beneath Whumpee’s feet while ice skating — a perfectly innocent moment gone wrong.
Whumpee who was never allowed to play in the snow now jumps at the opportunity to do so.
Alternatively: Whumpee who has always loved playing in the snow now isn’t allowed to (or otherwise can’t).
Whumpee being too afraid to mention how cold they are until Caretaker finally notices their shivering and wraps their own scarf around Whumpee’s neck.
Whumpee losing a precious item in the snow/a storm and not having the chance to retrieve it.

One of my fave sickfic scenarios:
✨Sickie fainting in the shower or bath✨🥰
Shower has been my lifetime favorite. Sickie with a ragingly high fever (that no one else knows about) wants to take a shower to warm up because they are freezing. They can barely make it from their room into the shower, stumbling and swaying the whole way, but they’re too delirious to be logical. Then unsurprisingly the heat of the shower combined with their body heat and lightheadedness results in them collapsing. (Likely hitting their head on the way down as a major bonus, or injuring something). But the best part is they’re unconscious and under the spray of the shower for so long it eventually turns ICE COLD. And that’s where Caretaker finds them…already terribly sick, now laying fully exposed under ice cold water for an undetermined amount of time.
And now lately I have this totally unrealistic dream scenario of Caretaker getting Sickie in the bath and while laying in the bath, Sickie faints…and in this unrealistic scenario Caretaker is distracted for a moment or goes to get something, and that’s all the time it takes for Sickie to slip below the water. They’re fully unconscious and they inhale water before Caretaker sees what’s happened and frantically has to get a completely limp and soaking wet Sickie out of the bath. Maybe the shock of being pulled out of the water starts to pull Sickie from unconsciousness, but their eyes are fluttering and rolling back and they are terrified and disoriented and suffocating and barely hanging on.
Just so many possibilities 😍🤤

Military whump prompts
(because Reddit is down and I’m chronically online)
Hearing the whumpee’s screams over the radio as they’re captured, the rest of the team frozen in horror.
• Patch job in the middle of the battlefield, someone pressing down on a wound with their bare hands, whispering, “Stay with me, okay? Just stay.”
• Post-mission debrief, but the whumpee is barely holding it together, swaying on their feet as the adrenaline wears off.
• The whumpee refusing pain meds because they need to “stay sharp,” only to pass out mid-conversation.
• “I don’t leave people behind,” they growl, limping and bleeding but refusing to abandon the unconscious teammate over their shoulder.
• Waking up in a field hospital, disoriented and panicked, pulling out the IV before being restrained by a firm but gentle voice.
• “We’re not going to make it.” “Yes, we are.” Cue one of them doing something reckless to ensure the other gets out alive.
• Cold, soaked to the bone, huddled in a ruined building during a downpour, one of them feverish while the other desperately tries to keep them awake.
• Hiding an injury to avoid being benched, only for it to get worse at the worst possible moment.
• “You don’t understand—I can’t go back. I can’t be discharged.”
• A high-ranking officer demanding a mission debrief while the whumpee is barely conscious, words slurring, bleeding through their uniform.
• A medic struggling to save the whumpee in the back of a jolting vehicle, yelling for the driver to go faster.
• Post-rescue, sitting by the whumpee’s bedside, counting every beep of the heart monitor like a prayer.

DISCLAIMER: I HAVE NO CLUE IF THIS IS MEDICALLY ACCURATE
Something that whump writers don’t consider:
IVs feel cold. Can you imagine a room temperature liquid going directly into the bloodstream of someone who’s 97-104 degrees? It’s hellish. You can’t get warm no matter how much external heat you receive.
Imagine a delirious whumpee whimpering and clawing at an IV while being restrained and reassured by Caretaker.
“No no no, that stays in”
“Hey, hey. I know it hurts, but it’ll help you feel better”
Maybe the whumpee’s hallucinating, thinking that they’re being tortured. When Caretaker’s words fail to get through, they have to use gentle touches and singing. Or, if you want to be mean, you can have the Caretaker being forced to restrain Whumpee to prevent them from hurting themselves until they run out of energy and pass out.

B has been feeling a little weird all day. A little shivery, a little tired, a little achey, starting to get sniffly, but overall okay. They don’t feel sick per se, just a little off.
Cut to B having a little movie night with their partner A, who is cradling B’s head in their lap. A is carding softly through their hair as they feel B’s breathing start to grow deeper and slower. They smile and move their fingers down towards B’s eyes, only to brush their forehead and stop. Is it just them, or is B feeling a little warm? They move their hand to B’s cheek, their other cupping the nape of their neck.
“You feeling okay? You feel really warm,” A asked, feeling B roll a little bit in their lap.
B mumbled something before burying their hot face deeper into the blankets and sniffling liquidly.
“Aw honey, let me get you some Tylenol so you can get some rest.”

We use being cold, hungry, and tired a lot in our writings, and that makes sense! Those aren't pleasant feelings, but what's not used enough is all of the other symptoms that can occur under those conditions.
Being malnourished not only causes the feeling of hunger, but can give you headaches, and make you feel weak, faint, sick, cold, and tired. It makes you irritable, unable to concentrate, and can even cause wounds to take longer to heal, while brusing much more easily. After a while you even develop an aversion to food, and don't want to eat. You feel nauseous just thinking about it, and breaking a fast isn't something you can do with a flick of your fingers. Your body isn't used to eating, and it may not sit well with you.
Not eating can also cause your blood sugar to drop, which is a whole entire thing in and of itself, and you don't have to be a diabetic to experience it. Low blood sugar is horrible 0/10 do NOT reccomend (but definitely do in whump!)
As for being cold, it can not only be uncomfortable, but it can make you feel physically ill. Especially when paired with a lack of vitamin D from low sun exposure. You just constantly feel sick, and may even begin to feel hot. It is absolutely FREEZING in my room all the time and I always feel sick. Horrible. Do it to your whumpees and tell them if they're good they can go outside and lay in the sun. That's shit's awesome when you're cold.
And of course, with sleep deprivation, it not only makes you drowsy, but it basically screws everything up. Your ability to think, your coordination, your strength. All you wanna do is curl up into a ball and go to sleep, no matter where you are. There's so much you can do with a tired whumpee.
The most fun thing about all of these is that they can generally go together all at once, inflicting symptoms of the others in an endless loop of torment. So yeah, go nuts! It's miserable!
