Of course, I found information on the basics of weaning: preventing engorgement and finding alternative ways to continue bonding with the baby. But this wasn’t what I was experiencing. I took pregnancy tests (negative), visited my doctor (“It’s a phase”) and talked to other moms (huge variety of experiences). I was frustrated and I needed to know why I was feeling like garbage. My husband and I now refer to what happened to me as the “dark side” of weaning. Let’s start with the facts: Research does not, technically, show that postpartum depression or anxiety surges at this time. But that’s because mothers aren’t specifically screened for depression during weaning, as it’s usually a temporary phase and everyone weans at different times—it could be three months postpartum or three years postpartum. High-quality research simply does not exist yet. However, plenty of women report feeling the effects of the hormonal changes that occur during weaning. Reproductive psychiatrist Alexandra Sacks, author of What No One Tells You: A Guide to Your Emotions from Pregnancy to Motherhood and the host of the Motherhood Sessions podcast, explains it this way: “Some women are more sensitive to hormonal shifts than others; some have more dramatic mood changes around periods, during pregnancy, postpartum and around weaning, but these are individual sensitivities—so some people feel better, and some feel worse.” Others don’t notice any mood changes at all. A decline in oxytocin, the bonding hormone stimulated by breastfeeding, may lead to some women feeling low, says Sacks. She also explains that some women feel better after weaning if they had found breastfeeding to be stressful or disruptive to their own sleep (which may increase stress hormones). When you stop breastfeeding, prolactin (the milk-production hormone) drops off, estrogen shoots back up, and all of it sent me into a PMSing, semi-permanent state of terribleness. Verinder Sharma, a professor of psychiatry with a cross appointment to the Department of Obstetrics and Gynecology at the University of Western Ontario, confirms that the prolactin decrease is the issue, but it’s not the whole story: What matters is how the prolactin affects other changes in the brain and results in depression—or even mania. Sharma says to look for a “clustering of symptoms.” Some women may experience comparatively simple hormone changes with weaning, while others might be plunged into a full-blown postpartum depression. I sure had a cluster of symptoms, but I didn’t feel they were depression-like. It felt more physical for me. “We make a distinction between symptoms and syndromes,” says Sharma. For women, all “reproductive events” related to hormonal changes—this can include pregnancy and postpartum, as well as monthly PMS, menopause, and when you’re getting your period for the very first time—increase the risk of psychiatric problems, he says. For example, bipolar disorder mania is extremely affected by hormonal changes—one in three women with bipolar disorder will experience an onset within a year of starting puberty or getting their first period. It’s also the mental disorder most exacerbated by childbirth, according to Sharma.
How to stop breastfeedingIn the 18th century, doctors and psychiatrists did, in fact, distinguish between postpartum disorders and the “lactational period,” but in contemporary studies, they haven’t done this. Sharma says we are still evaluating men and women too similarly and that a paradigm shift needs to take place for us to more holistically consider a woman’s hormones in relation to her mental health. “That change should reflect the heightened risk around the time of reproductive events,” including during weaning, says Sharma. Due to the lack of research in this area, Sharma says there are no concrete numbers on how many women experience depression or other mental disorders during weaning. He thinks screenings for mood disorders during weaning should be as commonplace as mental health assessments during the early postpartum period. Ideally, doctors should be considering and connecting potential changes at each major hormonal shift in a woman’s life. While my so-called weaning fog was nothing like bipolar mania, I’m certain it was affected by the soup of hormones flowing from my pituitary gland and swirling throughout my body. In addition to this hormonal chaos—or maybe because of it—I was also feeling a little sad that breastfeeding was over, while simultaneously feeling glad that we had stopped. After nursing three babies, I felt a sense of nostalgia and freedom at the same time. It’s that classic push-pull feeling of parenthood: Looking forward yet yearning for the past, too. Mourning the breastfeeding period and feeling a sense of grief or loss is common. Catie Agave*, a 36-year-old mom in Toronto, felt it intensely, since she knew she was most likely going to have only one child. “The journey was ending for us, so that brought on sadness as well,” she says. While she weaned her three-year-old gradually, she started to feel foggy within two weeks of completely weaning. “I wasn’t prepared for the change,” she says. “I didn’t feel like myself. I was more exhausted even though he was finally sleeping more. By week three or four, I had a lack of interest in daily activities, which is difficult when you have a child of that age.” She kept her feelings to herself at first, and then did some googling, but she found very few research-based articles and a lack of support, even in her usual go-tos: her Facebook mom groups and breastfeeding forums. “Nobody talks about it.” “There are a lot of people talking about postpartum depression,” she says. “And reading their symptoms, I thought, yeah, this is what I have—this is depression. But nobody ever said you can have postpartum depression from weaning, too. I was very sad, and it lasted for a long time. I couldn’t find anyone else going through that,” she says. “It was a scary experience.” Agave says she was hesitant to talk to her doctor because she assumed postpartum depression (PPD) was for moms of infants, not moms of toddlers or preschoolers, and she worried she’d be judged for her choice to practise extended breastfeeding. She credits her sister with encouraging her to see a doctor, in spite of her fears. “The doctor was supportive and mentioned postpartum depression can happen up until three years,” she says. Sacks is working to popularize the term “matrescence,” originally coined by an anthropologist in the 1970s, as a better way to describe and fully capture the ongoing transitions of motherhood over time, even if your baby is now growing into a toddler. “It’s a helpful framing of new motherhood as a developmental phase, like ‘adolescence’—it’s not a coincidence that the words sound similar,” she says. “Both matrescence and adolescence describe shifts that are challenging because they involve changes in so many parts of life, ranging from the physical, hormonal, social, emotional and all the rest.” Adolescence is a gradual process—it isn’t instant in the way motherhood can be divided into pre-baby and post-baby life. But we need to be forgiving of ourselves, and to acknowledge that it might take time to adjust to all the shifts and challenges happening at once. Your body, your brain chemistry and your identity are all changing. Whether it’s a few months after birth or three years later, women shouldn’t feel ashamed if they experience the weaning fog, like me, or true depression symptoms, like Agave. We all have our own recovery period. Sacks encourages moms to remember that the end of breastfeeding doesn’t mean your baby needs you any less. Agave, who had struggled with anxiety in the past—but never depression—was ultimately referred to a treatment program where she improved through cognitive behavioural therapy (CBT). She was relieved to know CBT was an option, in addition to taking prescription medications, such as antidepressants. (She was prescribed an SNRI but chose to focus on CBT treatment instead.) “Eventually, my hormones regulated and the feeling of depression significantly decreased, but to this day, the anxiety piece is still there. I think a lot of it is the stress of being a mom.” Around the time I was preparing to pursue professional help, my own symptoms eased up, around two months after they began. I found myself reflecting back on the previous two months, asking, “What just happened?” It had felt like the flu, mixed with mild depression, combined with all those yo-yo-ing feelings about my relationship with my baby. I felt so thankful to feel “normal,” or like myself, again. Batya Grundland, a family physician with an emphasis in obstetrics and women’s health in Toronto, and the former head of maternal care at Women’s College Hospital Family Practice, says gradual versus cold-turkey weaning can play a part in the intensity of hormonal changes. She believes weaning is unlikely to be the sole cause; rather, it’s a complex puzzle with multiple additional factors happening all at once. “The tricky thing is that it would be hard to associate symptoms only with weaning,” she says. For many mothers, reductions to the nursing schedule often coincide with a return to full-time or part-time work. Some women will also experience the return of their period, with ovulation and menstrual cycles beginning to regulate again during the same time frame. “It could make sense that describe feeling pregnant. With the prolactin and estrogen changes, you could feel a whole bunch of things,” says Grundland. Not only are hormones changing drastically during this phase, but women may also be spending long days away from their babies, weaning by necessity (or attempting to pump at work), juggling full-time employment, adjusting to the work/daycare dash, not sleeping enough at night and forgetting to take care of themselves in all of this. “Moms are so busy—they need to be reminded that self-care is important, and we need to figure out ways to better support mothers,” says Sacks. She nudges parents to ask themselves how much they’ve slept and when they ate their last real meal. Do you have time to simply go to the bathroom and brush your teeth alone? Have you had time to yourself not engaged in childcare? Sacks says moms need to reconnect to who they are outside of parenting—like seeing friends, spending time with a romantic partner or pursuing non-child-related interests. “If you cut out the majority of activities that were essential to your routine before having a baby, you may feel disconnected from your identity.” Both Sacks and Grundland also recommend seeking help if temporary feelings of sadness become long-term or interfere with daily activities, but they agree that some sadness can be normal for some individuals. Most women can expect to feel physical and emotional changes for about four to six weeks, says Grundland. My journey through the weaning fog, and my version of self-care during the recovery period, meant seeking out meals with multiple food groups, a simple thing that had fallen off the priority list when I was caring for a colicky baby and keeping my other two toddlers alive and happy. I distinctly recall a three-course lunch I bought for myself, including a rack of ribs, that reminded me how to enjoy other things again, as a separate human from my baby. I had forgotten that I needed to eat real food, too. The end of breastfeeding doesn’t mean your baby needs you any less, emotionally, says Sacks. It’s like every other bittersweet aspect of parenting: “You feel a sense of longing when you see clothes your child no longer fits into, but you’re happy they’re growing. A baby is able to eat foods, but the ‘baby phase’ is now behind you. You can want two things at once.” I wanted to be the selfless, amazing super mom, but to also feel zero guilt treating myself to that rack of ribs—alone—instead of nursing a baby for the fourth time that day. I wanted to feel even-keeled and clear-headed again, yet still bond with my baby in the ways breastfeeding had magically provided. When the World Health Organization (WHO) decided to include “gaming disorder” in the 11th edition of the International Classification of Diseases(ICD-11) in mid-2018, it did not at all settle the long-standing debate whether electronic gaming — online, by video consoles, or on smartphones— could be addictive enough to be considered pathologic.
Rather, the decision further inflamed the controversy, polarizing researchers who have argued opposite sides and leaving many others trying to make sense of limited evidence.1 At even the most practical level, many researchers note the lack of accompanying guidance for the addition.
“We don’t have the appropriate diagnostic tools, and the WHO did not really bother to define any symptoms,” said Chris Ferguson, PhD, a professor of psychology at Stetson University in DeLand, Florida, who opposed the inclusion. “I think where things went off the rails is focusing on the behavior people are addicted to instead of traits of the individual,” Dr Ferguson told Psychiatry Advisor. Within public discourse, he said, people discuss video games as though they are inherently addictive, yet people overdo a wide range of behaviors.
He offered an example of the absurd: “We do not talk about cat addiction, but almost everything you could say about gaming addiction you could say about cats,” he said. “Stroking a cat tends to release dopamine, and cats have mechanisms to try to keep you petting them.” A quick Google search makes evident the problem of cat hoarding, he noted. “Is it something about video games that makes them different from shopping or exercise or food or sex or other things people can do excessively, or is it that individuals have difficulty regulating a fun thing?”
That was a key question the American Psychiatric Association wrangled with in updating the Diagnostic and Statistical Manual of Mental Disorders-5th edition(DSM-5). Ultimately, the American Psychiatric Association included “Internet Gaming Disorder” as a condition for further study rather than its own diagnosis. “There was not sufficient evidence to determine whether the condition is a unique mental disorder or the best criteria to classify it at the time the DSM-5 was published in 2013,” the organization wrote, but they proposed symptoms and potential criteria (5 or more symptoms within a year) for the disorder.
The WHO, meanwhile, announced in a September 2018 press release that gaming disorder becomes “a clinically recognizable and clinically significant syndrome when the pattern of gaming behavior is of such a nature and intensity that it results in marked distress or significant impairment in personal, family, social, educational or occupational functioning.” Mental health providers remain caught in the debate.
Pushback from Industry and the Profession
Not surprisingly, the Entertainment Software Association pushed back on the WHO’s designation even before its formal announcement. In March 2018, Michael D. Gallagher, president and CEO of the Entertainment Software Association, said, “The WHO’s process lacks transparency, is deeply flawed, and lacks objective scientific support.” The Entertainment Software Association drew attention to “A weak scientific basis for gaming disorder: Let us err on the side of caution,” a paper in the Journal of Behavioral Addictions that responded to an earlier open debate paper on the WHO’s ICD-11 proposal and argued against making gaming disorder its own diagnosis because of the weak evidence base.
Dr Ferguson was among the 36 authors who acknowledged the potential for video gaming to interfere with people’s lives but argued for an “extremely high burden of evidence and the clinical utility…because there is a genuine risk of abuse of diagnoses.” They described what additional research was needed to legitimize a diagnosis.
One central issue is whether gaming is a disorder unto itself or a manifestation, in the form of a coping mechanism or self-medication, of another mental health issue, such as anxiety, depression, attention-deficit/hyperactivity disorder or other established diagnoses. Neurobiology research does not clarify this question, according to Michelle Colder Carras, PhD, one of Dr Ferguson’s coauthors on the opposition paper and a postdoctoral fellow at Johns Hopkins Bloomberg School of Public Health in Baltimore.
“The way [many existing] studies are designed, we don’t have a way of telling the way the brain changes from video games compared to other pleasurable things, so it’s like we are comparing sex to drugs instead of comparing sex to some other decent thing that’s fulfilling,” Dr Carras told Psychiatry Advisor.
Dr Ferguson pointed out exaggerations regarding gaming’s effects on the brain. It’s true that gaming involves an anticipatory dopamine release in the brain — but so does “looking forward to going on a trip, having sex, or eating a pizza,” he said. “It sounds more ominous than it actually is.” The dopamine release associated with gaming is similar to those activities, whereas cocaine or methamphetamine release 3 and 12 times more dopamine, respectively.
The Framing Problem of “Addiction”
Another issue is the addiction paradigm itself: Is there a better way to frame problems of excessive use of a substance or excessive behaviors than saying the brain is addicted to that substance or behavior? Research suggests the underlying cognitive psychology of internet gaming disorder is complex and poorly understood.2
Dr Carras said she and colleagues have advocated for “broader category that could encompass different behavioral problems” when people lose control and excessive behaviors have a negative impact on their lives, although she acknowledged they lack a name for such a thing. “Compulsion” is an ego-dystonic behavior and hence not quite accurate: “You do not want to do it; you have to do it,” she explained.
Vladan Starcevic, MD, PhD, also an opposition paper coauthor and an associate professor of psychiatry at the University of Sydney in Australia, agreed, noting that compulsion “refers to having an urge to do a certain activity because you are afraid if you stop, there will be negative consequences.” Although people who cannot continue gaming may feel “restless, angry, or frustrated,” those are psychological symptoms, not physical withdrawal symptoms, which current evidence has not revealed so far.
For gamers, however, negative consequences exist in the form of lost status in the game, such as lost points or reduced rankings relative to other players, according to a presentation that Barbara Craig, MD, a child abuse pediatrician from Walter Reed National Military Medical Center in Bethesda, gave at the American Academy of Pediatrics annual meeting in November 2018. She agreed with gaming disorder as its own diagnostic problem and described pediatric abuse and neglect, including deaths, resulting from parents’ spending 12 or more hours a day playing games.
Still, is that behavior actually “addiction” per se? Dr Starcevic agrees with Dr Carras that “using the addiction paradigm is wrong” and called for caution in applying that label. “When you cross that boundary and something becomes a disorder, it has implications,” such as the stigma attached to any psychiatric diagnosis, Dr Starcevic told Psychiatry Advisor. He worries about making a pathological reason for excessive indulgence in everyday activities, such as shopping or sexual activity, while existing diagnoses already face challenges.
“There is not much support for many of our existing diagnoses,” Dr Starcevic said. “We are struggling to defend some of the existing diagnoses, and now we are introducing yet another that is difficult to defend.”
That does not mean problematic levels of gaming — or other behaviors — do not exist. However, boundaries distinguishing excessive or problematic gaming are not well defined, Dr Starcevic said, and these blurry boundaries can lead to stigma.
“There is a lot of literature on so-called moral panic and stigma associated with diagnostic labels,” Dr Starcevic said, which can be exacerbated without a “clear boundary of what could still be a variant of normal behavior.”
Supporters Recognize Risks Too
Even those who do believe gaming disorder should be its own diagnosis are cognizant of the risks. Petros Levounis, MD, MA, chairman of the psychiatry department at Rutgers New Jersey Medical School in Newark, believes gaming disorder does need its own diagnosis and told the New York Times that one positive consequence of the WHO’s ICD-11designation is the potential ability to get reimbursed for treating people. No pharmacologic treatments exist presently, he told Psychiatry Advisor, so treatment consists of psychotherapy, such as cognitive behavioral therapy, motivational interviewing, or 12-step programs, albeit without much evidence.3 Yet he agreed with being cautious.
“The issue in psychiatry is that there is so much stigma associated with mental illness that before you call something a disorder, you had better have your ducks in a row and know what you are talking about,” Dr Levounis said. However, stigma of other diagnoses — including psychiatric issues that potentially contribute to excessive gaming — may supersede stigma associated with gaming as a disease.
“Maybe the internet gaming is a response to the underlying condition,” Dr Levounis said. “It could be that it is easier for parents and young kids to formulate their troubles in terms of internet gaming instead of saying it maybe a sexual or gender or depression issue.”
That brings things full circle to whether making gaming disorder official runs the risk of leaving those problems undiagnosed and/or untreated. Dr Carras takes a pragmatic approach to this question in a way that breaks somewhat with her skeptical colleagues.
“If the behavior is leading to significant life problems — children not getting to school, not going to work,a spouse is going to leave you — then regardless of whether it is because of a disorder or addiction, it makes sense to treat the behavior,” Dr Carras said, agreeing that the stigma associated with treating gaming might be lesser than when treating other conditions.
Research Gaps Remain
The problem remains that the evidence on excessive gaming is spotty, with too many open questions.4Both Dr Carras and Dr Ferguson pointed out the problem of adequate measurement, for example.5Even vocabulary presents problems — including even whether gaming should be discussed within the context of the internet, as an offline technology, or both.6
“We need a way of defining technology-related problems that is able to keep up with changes in society,” Dr Carras said. She and Dr Ferguson pointed out the research base’s glaring lack of voices from gamers themselves and the industry, which can only worsen generational problems with characterizing the disorder. Dr Ferguson pointed out that most of the WHO researchers pushing for gaming disorder’s inclusion in ICD-11 are likely over age 50 and not familiar with gaming in general.
“They really missed an opportunity to get a more diverse view of this issue,” he said. “If all people who do not like video games came up with a mental health diagnosis, this would be it.” Dr Ferguson also brought up the complexities of cultural differences in perceptions of gaming and mental health and suspected the WHO felt pressure from some countries in Asia to make gaming disorder official.7
Jeffrey Snodgrass, PhD, a psychiatric anthropologist at Colorado State University in Fort Collins who has researched problematic gaming behaviors, told Psychiatry Advisor that the evidence does suggest “symptoms vary somewhat cross-culturally, pointing to a place for culture-specific ‘problem’ gaming that is not well captured by the ‘addiction’ frame.” Immersed in the research, he goes back and forth on the issue.
“I go where the data take me, and that varies from analysis to analysis,” he said, noting that even 2 of his most recent papers came to different conclusions on whether to support the addiction model.8,9 In fact, it is because Dr Snodgrass follows the data that he felt he was not the best person to offer a full-throated defense of either position in a point-counterpoint discussion. Perhaps that his research has not pointed solidly in one direction or another is a testament itself to how much more research is needed before a real consensus can begin to emerge.

In her book, Depression Is a Choice: Winning the Battle Without Drugs, A. B. Curtiss argues that most people can control the syndrome without the use of drugs and without the burden of endless therapy. In her book, she draws from her own experiences with depression, anecdotes from her practice, and a wealth of information about the history of the treatment of depression.[i] She acknowledges that depression in its most extreme forms is best treated through pharmaceutical and psychoanalytical intervention.
Clinical depression is a disease. I don’t wish to add to the stigmatization of depression.The stigmatization of depression is why approximately 80% of the people with depression don’t seek treatment.[ii]
If you have depression, I strongly encourage you to seek treatment. If you are unwilling to seek treatment, I hope that the information contained in this writing will help you. Depression is serious and shouldn’t be taken lightly.
Tony Robbins says he suffered from depression but was able to snap himself out of it by shifting his physical state and focusing on gratitude. I have found that gratitude is incompatible with negative emotions like fear, anger, frustration, and hopelessness. It is difficult to feel them while simultaneously being grateful.

His assertion that we can shift our mental state through changing our physical state is supported by science. Physiology has been shown to influence our psychology. Smiling has documented therapeutic effects. It is linked to lowered blood pressure, reduced levels of stress hormones like adrenaline and cortisol, and increased levels of endorphins and dopamine.[iii]Tony Robbins says that a research study at the University of California, Berkeley, asked clinically depressed people to smile for 20-minutes a day. He claims that after 20 days the study subjects no longer required medication.
Some have suggested that the paradigm we have been taught all our lives, that we smile when we are happy is backward. I don’t believe it is backward. I believe that it works both ways. I believe that our psychology and physiology are linked together in a push-pull relationship, with each affecting the other
Dr. John Ratey, a practicing psychiatrist and clinical associate professor of psychiatry at Harvard Medical School, presents numerous cases of scientific research and studies to demonstrate exercises ability to treat depression in his book, Spark: The Revolutionary New Science of Exercise and the Brain.
In Britain, doctors now use exercise as a first-line treatment for depression, but unfortunately, it is underutilized in the United States. Many doctors still prefer to prescribe a pill to an exercise regiment.

Exercise boosts dopamine, which improves mood, motivation, feelings of wellness and attention. Daily exercise increases dopamine storage in the brain and also triggers the production of enzymes that create dopamine receptors in the reward center of the brain, and this provides a feeling of satisfaction because we have accomplished something.
Serotonin is equally affected by regular exercise. Serotonin levels affect our mood, impulse control, and self-esteem. Frequent exercise improves our mood by making us feel better about ourselves. Exercise has been shown to help lift people out of depression. In many instances, people are able to treat their depression without medication.
Exercise immediately elevates levels of norepinephrine in certain areas of the brain. It wakes up the brain and gets it going and improves self-esteem, which is one component of depression. [iv]
Tony Robbins believes that depression is caused by a habitual pattern of thinking. Focusing on what we cannot control; focusing on what we don’t want, instead of what we already have.
He says he has helped many severely depressed people and has never lost anyone to suicide. He doesn’t have much faith in psychologists. He suggests we can solve many of our own issues. I remember him joking ‘How many psychologists does it take to change a lightbulb? Just one, but it will take a lot of time, cost a lot of money, and the light bulb has to want to change.’
In this powerful video he is able to get to the root of this woman’s depression. First, he identifies the patterns she uses to achieve a depressed state, her thoughts, her posture, and her tone of voice. Next, he discovers the human needs that she is satisfying by getting herself in a depressed state. Last, he helps her to change her patterns, to avoid becoming depressed.
