Said this in the tags of another post a while back, but my #1 piece of advice for people trying to start HRT is that you should seriously consider lying to your doctor about the nature of your identity in any way necessary to adhere to the DSM-5's criteria for gender dysphoria, especially if your insurance requires a formal dysphoria diagnosis to cover hormones. Gender dysphoria evaluations are a means of selectively restricting access to care based on your capability to say the right things to check the right boxes to convince a doctor to write you a prescription. Fortunately, the trans community has developed a time-honored tradition to help you elide this barrier, and it's called Fucking Lying <3

I'm seeing him on Jan 2nd (such a short turn around! I made the appointment today!), here's hoping last time he was just having an off day and he comes across better this time

I think I’m done.
I’ve written many posts about my experience as a doctor. Some of them are funny, some of them sad. Some of them poignant.
But I think I’m done being a doctor in the current public healthcare system where I live.
I’m a Family Medicine resident. I rotated through two and a half years of hosptial residencies, and I’m in my first year of clinic-only practice. It should sound good; no more 26-38 hour shifts on top of 12 hour workdays.
But I’m currently seeing 33-37 patients from 9-1 pm daily and 24 patients from 2-5 pm daily. I also sometimes work 6-10 pm. Each of these clinic sessions also includes a solid 1.5 inches of seperate lab results I’m expected to screen during consultation time. I work Monday through to half of Saturday. Im expected to also complete a dermatology university diploma on top of this. I have exams in family medicine this year, the year after that, and two years after. Three and a half years more at least until I’m a specialist.
I can’t do it. I can’t keep giving each patient only 5 minutes of time as I’m doing now. I didn’t become a doctor to have to balance whether I should see my patient’s third complaint or ask them to book another session so I can have time to pee and eat.
Peeing and eating is like…the lowest level of Maslow’s hierarchy of needs. I’ve spent all of my adult working years, from 23 years of age til now, chasing that. The right to pee and eat and drink.
The department head has his head so far up his ass with bootlicking the government that last quarter he pulled workforce away from chronic cases for half-full covid clinics and made us each therefore have to review 56 cases per four hour session. When we raised the issue of patient safety one of his associate consultants said, “I know it’s hard! Let’s get through this together!”
He is famed for picking on whether residents punctuate the numbering of their past medical history lists with brackets or periods.
Nope. Nope. Nope.
This afternoon between patients, I suddenly realised I had lost my voice. I’d been speaking too much. Moreover, I think my autistic brain had finally clicked over into the “dude you better go non verbal or you’ll die” function. Of course, I couldn’t actually afford to be non verbal. I plowed through the remaining cases so dissociated I nearly blacked out.
If I pass these next two years’ exams, I become senior resident. I will have the privilege of not being entrusted with 33 cases every four hours, but 40.
40 cases; all of them episodic patients, not chronic. This means all of them come in with new complaints, as in multiple. Time per patient gets knocked down to four minutes.
Respectfully, Fuck That.
I’m waiting until I get contract gratuity this summer, do the damn exam that everyone agrees is insane, and then I’ll apply to a plethora of private clinics in September and kiss this bloody department goodbye.
I want to be able to spend more than two minutes comforting a crying patient, dammit.
If I don’t leave soon I’ll break, and that’ll compromise patient safety over anything else.

It dawns on me that the journey to the west took 5,040 days exactly, right, and while the book goes from tribulation to tribulation, there were still only 81 of those. And they were missing one when they got there. And some of the tribulations Tripitaka went through happened before the journey even started.
So, even being generous and saying that most of the tribulations that occurred during the actual journey could be said to have taken a few days to handle each, that's still only about 10% of the journey. A tribulation was anything that happened that put Tripitaka in danger or presented any sort of obstacle to him. Anything even remotely exciting would have fallen into that ~10%, and nothing else could have happened, because otherwise they wouldn't have gotten west one moderate inconvenience and/or major trauma short of the prize. (I mean, unless the thing that happened managed to not involve Tripitaka at all in any way, but that's very hard to do when you are all attached at the hip.)
Holy cow they really were just walking. ALL THAT TIME. No wonder Zhu Bajie was stirring the pot at any given opportunity. It was literally the only thing to do.
#jttw personal#how did they not kill each other#I was thinking about this while still picking at chapter 27#tripitaka was super gullible in that chapter in a sort of inexcusable way but also#it sooooort of makes sense when you think like#statistically#across the whole journey they could have gone moooonths between demons#years even#and suddenly sun wukong's claiming to have killed 3 (they didn't know it was the same demon) in a row in one morning?#even if tripitaka HAD believed him (or just harbored doubts) after the first one how likely was it the second was the case? or the THIRD?#obviously the evidence was in his face but couple the idea that their encounters with demons were actually SUPER rare#with the fact that tripitaka still had major trust issues with sun wukong from the fact he HAD trusted sun wukong previously#only to have that trust pretty solidly broken#and tripitaka's probably operating on a level of 'fool me once' hyper-vigilance against him that actually makes zhu bajie seem reasonable#I mean who are you going to trust? you and your own shitty judgement when you've already been wrong about the guy once before?#or the DEMON who probably knows more about DEMON MAGIC than you?#tripitaka's got TWO expert consults telling him two wildly opposing things but only ONE of them's seriously burned him in the past#(while the third expert consult and tie-breaker is notably abstaining. gdit sha wujing.)#anyway the characterization here is actually really good#tripitaka doesn't know the story framing - WE know something's up because otherwise we wouldn't have a story about it -#but tripitaka doesn't realize he's in a book#and I'm just saying tripitaka is being less foolish than the meta knowledge of being The Reader makes him seem#still a total brat though#he's definitely letting his own pride and hurt (and like...trauma) bias him against sun wukong unfairly#which is something he needs to work on and IS something that he pays for#(even with the bandits: expecting sun wukong to behave to tripitaka's standards of morality prior to TEACHING him those standards)#(wasn't fair. but also when he *tried* to address it sun wukong got angry and took off. and then tried to kill him. so.)#it's just interesting and whoever told this story originally was clearly putting a lot of thought into what it would be like#to actually be in these guys' shoes. Like ugh. HOW is this book so good?

Higgins has a network (he knows people who know people) but today we find out that a few of his little birdies are actually the players (Isaac & Colin) and Will Kitman?! I love it
I feel like he helped orchestrate the possibility of Nate coming back up to the team. Will probably said something about Nate’s apology to Higgins. Then Higgins gets to thinking and perhaps spoke a few well placed words in Isaac’s ear suggesting Nate could come back? Then Isaac the team captain brings it up for discussion. I love those himbos, but this has DFO* Higgins all over it. I just feel in my soul he sent them to the restaurant to Nate.
