Aspirin is widely known for its heart attack prevention benefits, but a new study has also linked the common drug to a reduced risk of colorectal cancer.The study analyzed data from 107,655 men and women who participated in the Nurses’ Health Study and the Health Professionals Follow-Up Study over a three-decade span. Rare sexually transmitted ringworm found in US

Medical experts are warning doctors and the public of a rare, highly contagious fungal strain after the first report of sexually transmitted ringworm in the U.S.
According to a report released Wednesday in JAMA Dermatology, the first case involved a New York City man in his 30s who developed tinea, also known as ringworm, after having sex with men during trips to California, England and Greece.

When the man returned home, according to the case study, he developed an itchy, red rash on his groin, genitals and buttocks.
Tests confirmed he had contracted the rare, sexually transmitted fungus Trichophyton mentagrophytes type VII.
'Healthcare providers should be aware that Trichophyton mentagrophytes type VII is the latest in a group of severe skin infections to have now reached the United States,' Avrom Caplan, the report’s lead author and a dermatologist at New York University Grossman School of Medicine, wrote in a university news release.
Trichophyton mentagrophytes type VII is a drug-resistant fungus that causes skin disease in animals and humans and can be acquired through sexual contact, according to the Centers for Disease Control and Prevention.
In the report, Caplan wrote that the rash may look more like an eczema flare-up than a ringworm infection, which usually forms in a circular pattern on the skin.
The dermatologist said the infection is not fatal but can cause permanent scarring.
Caplan said he identified the first two cases of a different ringworm infection last year.
Those infections, caused by Trichophyton indotineae, are not sexually transmitted infections but are contagious and drug-resistant, the doctor reported.
Since then, Caplan’s NYU Langone Health team has identified 11 cases of Trichophyton indotineae ringworm in New York City, man yof them linked to travel in South Asia, according to a report published in JAMA Dermatology in May.
The report indicates the fungus is an emerging public health threat because it is resistant to standard therapies, including first-line antifungals, such as terbinafine.
Terbinafine, according to the Mayo Clinic, is a prescription tablet used to treat jock itch, athlete’s foot and other fungus infections of the body, scalp, fingernails and toenails

The Lancet Psychiatry Looks at Black Mental Health in the US

In December of 2023, The Lancet Psychiatry journal published three literature reviews on the state of Black mental health in the United States, as well as an essay written by Black Americans on their lived experience in confronting systemic racism. Together, these papers comprehensively analyze the impact and effects of racism across a half century.
The authors were led by Ayana Jordan, MD, PhD, a physician scientist who serves as Barbara Wilson Associate Professor in the NYU Grossman School of Medicine’s Department of Psychiatry. Dr. Jordan is also community engagement pillar co-lead at the NYU Langone Health Institute for Excellence in Health Equity.
According to Dr. Jordan, the issue is multifaceted and challenging to solve. Centuries of racism and dehumanization, rooted in diaspora and slavery, have exposed many Black Americans to mental illness risk factors that include exposure to violence, police brutality, elevated homelessness, and living in poverty-stricken neighborhoods.
Compounding this, the current health care system tends to dismiss such backgrounds and thus invalidate the exposure to risk factors that many Black Americans face. Perceiving the system of care as both inequitable and not culturally informed, Black people are significantly less likely than their white counterparts to seek out mental health care.
The issue is no less acute in the medical community, with Black people historically excluded from taking leadership positions in health care research. Of those receiving US National Institutes of Health funding for independent research, fewer than 1 percent are Black Americans.
The authors also take a close look at the way in which myths of “whiteness and white supremacy” impact how mental health and mental illness have been framed. Differences in rates of substance use and mental illness are often utilized as evidence in propounding cultural and/or biological distinctions. What is lacking in this one-sided debate is a coherent explanation of the avoidable and unjust systemic economic and social policies that created such inequities.
An example of this inequity centers on the classification system, Diagnostic and Statistical Manual of Mental Disorders (DSM-5), used in conceptualizing mental illness in the academic community. DSM-5 situates mental illness as an issue defined by the behavior and physiology of the individual, and then works outward to incorporate race. This approach ignores the underlying impact of racism, and being racialized as Black, on mental health and reinforces prevalent systemic biases.
In working toward a set of solutions, the authors highlight the need for more community-based participatory research. This involves Black community members, as well as scientists, working together to improve Black Americans’ mental well-being. This inclusive approach is unique in encouraging the active participation of those with well-rounded cultural knowledge that can be of most use in supporting those with mental problems. When Black people have a voice, new and more nuanced models of care and intervention can be proposed, evaluated, and implemented.
On a practical level, the authors urge the launch of a large-scale study that will identify publicly available datasets spanning economic factors, political districting, and environmental exposure as they relate to racism. At the same time, such a study will integrate individual-level data in areas like emotional well-being and use this as a basis for further analysis. The achievable output will be a risk index that presents and quantifies exposures to racism, and provides a tool for understanding how this impacts emotional well-being and mental health.

A New Study Says Gray Hair May Be Reversible! Bad News For Just For Men.

Study links graying hair to stem cells getting stuck, unable to color new hair growth.
Stem cells travel back and forth within compartments, but when they get stuck in one compartment, they cannot regenerate into pigment cells.
Restoring mobility of the cells could allow the continuation of pigment production, eliminating graying hair.
If only our pesky stem cells didn’t get stuck in place after a while, maybe we wouldn’t have gray hair. Really. Thanks to a new study published in the journal Nature and led by researchers from New York University’s Grossman School of Medicine, the team revealed how stuck melanocyte stem cells (McSCs) can’t make the protein needed to pigment hair, potentially explaining gray hair.
Using mice as the research subjects, the team found that McSCs travel between compartments of developing hair follicles in a healthy situation. The differing compartments allow the McSCs to mature and pick up the protein that can regenerate into pigment cells and continually color hair as it grows. These McSCs shift back and forth between maturity levels over time as they continually move between the compartments, a unique aspect of the McSCs.
But in some cases, the McSCs can get stuck in the hair follicle bulge compartment and become unable to move back to the germ compartment, where the WNT proteins encourage the cells to regenerate into pigment cells. Getting stuck means no pigment cells, which means gray hair.
“It is the loss of chameleon-like function in melanocyte stems cells that may be responsible for graying and loss of hair color,” Mayumi Ito, study senior investigator and professor in the Ronald O. Perelman Department of Dermatology and Department of Cell Biology at NYU Langone Health, says in a news release. “These findings suggest that melanocyte stem cell motility and reversible differentiation are key to keeping hair healthy and colored.”
Basically, if the McSCs can keep moving—or, if they get stuck, we can somehow prod them to get going again—we should expect more colorful heads of hair the world over.
“Our study adds to our basic understanding of how melanocyte stem cells work to color hair,” Qi Sun, postdoctoral fellow at NYU Langone Health, says in a news release. “The newfound mechanisms raise the possibility that the same fixed positioning of melanocyte stem cells may exist in humans. If so, it presents a potential pathway for reversing or preventing the graying of human hair by helping jammed cells to move again between developing hair follicle compartments.”
The McSCs focus on pigment-making, and are different from cells responsible for hair growth, Sun says, so hair can to continue growing even without pigment. The NYU study showed that as the hair regrowth process aged, the number of McSC’s lodged in the follicle bulge continued to increase. At some points, this non-pigment producing follicle bulge contains roughly 50 percent of all McSCs.
The McSCs that remained mobile retained their ability to produce pigment, but with the ever-shifting requirements of McSCs breaking down over time, the rise of gray hair coincided with aging. And while stress has also been associated with graying hair, unrelated Harvard research says that stress simply increases the hair regrowth pattern, speeding up the aging process for hair follicles.
“For unknown reasons, the melanocyte stem cell system fails earlier than other adult stem cell populations, which leads to hair graying in most humans and mice,” according to the NYU study.
The next step for the NYU team involves looking at how we can get McSCs, once stuck, moving again. Because once they move, they create pigment. And that could mean the end of gray.

Certain stem cells have a unique ability to move between the growth compartments of hair follicles, but as people age they get stuck and thus lose their ability to mature and maintain hair color, a new study shows.
Led by researchers at NYU Grossman School of Medicine, the new work focused on cells in the skin of mice and also found cells in humans called melanocyte stem cells, or McSCs. The new study, published online April 19 in the journal Nature, shows that McSCs have remarkable plasticity. This means that during normal hair growth, as these cells move between the compartments of the developing follicle, they are constantly moving back and forth along the maturing shaft. It is within these compartments that McSCs are exposed to varying levels of protein signals that affect maturation.
Specifically, the team found that McSCs switch between their most primitive stem cell state and the next stage of maturation (the trans-amplification state), depending on their location.
The researchers found that as hair ages, falls out, and then repeatedly grows back, more and more McSCs get stuck in a stem cell compartment called the follicular bulge. They stay there, neither maturing to the transitional state nor returning to their original position in the germ chamber, where the WNT protein prompts them to regenerate into pigment cells.
The plasticity of McSCs is not present in other self-regenerating stem cells, such as those that make up the hair follicle itself, which are known to move in only one direction along a given time axis as they mature, the researchers said. For example, translocation-amplified hair follicle cells never reverted to their original stem cell state. This helps, in part, explain why hair continues to grow even when pigmentation fails, Sun said.
Earlier work by the same group at NYU showed that WNT signaling is required to stimulate McSCs to mature and produce pigment. The study also showed that McSCs were trillions of times less exposed to WNT signaling in the follicular bulge than in the hair germ compartment located just below the bulge.
In recent experiments in mice whose hair was physically aged by plucking and forced regrowth, the number of follicles containing McSCs in the follicular bulge increased from 15 percent before plucking to nearly half after forced aging. These cells still fail to regenerate or mature into pigment-producing melanocytes.
The researchers found that the stuck McSCs stopped their regenerative behavior because they were no longer exposed to high levels of WNT signaling, so they were unable to produce pigment in the new hair follicles that continued to grow.
In contrast, other McSCs that continued to move back and forth between the hair follicle bulge and hair germ retained the ability to regenerate into McSCs, mature into melanocytes, and produce pigment throughout the two-year study period.
"Loss of chameleon-like function in melanocyte stem cells may be responsible for hair graying and discoloration," said study senior investigator Mayumi Ito, Ph.D., professor in the Ronald O. Perelman Department of Dermatology and NYU Langone Ph.D. in Cell Biology, Faculty of Health.
"These findings suggest that melanocyte stem cell motility and reversible differentiation are key to maintaining healthy and colored hair," said Ito, who is also a professor in the Department of Cell Biology at NYU Langone.
The team plans to investigate ways to rejuvenate McSCs, or physically move them back to their germ chambers, where they can produce pigment, Ito said.
In the study, the researchers used recent 3D intravital imaging and scRNA-seq techniques to track cells aging and moving within each hair follicle in near real time.

Dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) is often used as a screening tool in high-risk patients and its applications are expanding. The tool can help doctors investigate potentially suspicious lesions or evaluate the extent of disease in newly diagnosed patients. This type of medical imaging can also help inform a doctor’s treatment plan, including whether to perform a biopsy or the extent of surgery needed. Both scenarios influence short- and long-term patient outcomes. According to the researchers, MRIs have untapped potential in predicting disease pathology and achieving a better understanding of tumor biology. Developing AI models from large, well-annotated datasets could be key in refining the sensitivity of these scans and reducing unnecessary biopsies. The researchers produced an AI model that improves the accuracy of breast cancer diagnosis using a dataset extracted from clinical exams performed at a NYU Langone Health breast imaging site. The dataset consisted of bilateral DCE-MRI studies from patients categorized as high-risk screening, preoperative planning, routine surveillance, or follow-up after a suspicious finding. They trained an ensemble of deep neural networks with 3D convolutions for detecting spatiotemporal features using 14,198 labeled MRI examinations with mixed precision on the NVIDIA Apex open-source library. According to the team, the use of this library made it possible to increase the batch size during training.

Averaging the AI and radiologist predictions together increased overall accuracy by at least 5%, suggesting a hybrid approach may be most beneficial. The model was also as accurate among patients with various subtypes of cancer, even among less common malignancies. Patient demographics, such as age and race, did not influence the AI system, despite the little training data available for some groups. The model output can also be combined with the personal preference of a clinician or patient deciding whether to pursue a biopsy after a suspicious finding. By default, all suspicious lesions classified with category BI-RADS 4 are recommended for biopsy, which leads to a substantial number of false positives. The AI model predictions can help avoid benign biopsies in up to 20% of all BI-RADS category 4 patients. The authors note that the work has a few limitations, including understanding how a hybrid approach could impact a radiologist’s decision in a hospital setting or how the model makes its predictions. “Although we only looked at retrospective data in our study, the results are strong enough that we are confident in the accuracy of the model. The patient, David Bennett, a 57-year-old Maryland handyman, knew there was no guarantee the experiment would work but he was dying, ineligible for a human heart transplant and had no other option, his son said.
“It was either die or do this transplant. I want to live. I know it’s a shot in the dark, but it’s my last choice,” Bennett said a day before the surgery, according to a statement provided by the University of Maryland School of Medicine.
On Monday, Bennett was breathing on his own while still connected to a heart-lung machine to help his new heart. The next few weeks will be critical as Bennett recovers from the surgery and doctors carefully monitor how his heart is faring.
There’s a huge shortage of human organs donated for transplant, driving scientists to try to figure out how to use animal organs instead. Last year, there were just over 3,800 heart transplants in the U.S., a record number, according to the United Network for Organ Sharing, which oversees the nation’s transplant system.
"If this works, there will be an endless supply of these organs for patients who are suffering,” said Dr. Muhammad Mohiuddin, scientific director of the Maryland university's animal-to-human transplant program.
But prior attempts at such transplants — or xenotransplantation — have failed, largely because patients’ bodies rapidly rejected the animal organ. Notably, in 1984, Baby Fae, a dying infant, lived 21 days with a baboon heart.
The difference this time: The Maryland surgeons used a heart from a pig that had undergone gene-editing to remove a sugar in its cells that’s responsible for that hyper-fast organ rejection. Several biotech companies are developing pig organs for human transplant; the one used for Friday's operation came from Revivicor, a subsidiary of United Therapeutics.
“I think you can characterize it as a watershed event,” Dr. David Klassen, UNOS’ chief medical officer, said of the Maryland transplant.
Still, Klassen cautioned that it’s only a first tentative step into exploring whether this time around, xenotransplantation might finally work.
The Food and Drug Administration, which oversees such experiments, allowed the surgery under what’s called a “compassionate use” emergency authorization, available when a patient with a life-threatening condition has no other options.
It will be crucial to share the data gathered from this transplant before extending it to more patients, said Karen Maschke, a research scholar at the Hastings Center, who is helping develop ethics and policy recommendations for the first clinical trials under a grant from the National Institutes of Health.
“Rushing into animal-to-human transplants without this information would not be advisable,” Maschke said.
Over the years, scientists have turned from primates to pigs, tinkering with their genes.
Just last September, researchers in New York performed an experiment suggesting these kinds of pigs might offer promise for animal-to-human transplants. Doctors temporarily attached a pig’s kidney to a deceased human body and watched it begin to work.
The Maryland transplant takes their experiment to the next level, said Dr. Robert Montgomery, who led that work at NYU Langone Health.
“This is a truly remarkable breakthrough," he said in a statement. "As a heart transplant recipient, myself with a genetic heart disorder, I am thrilled by this news and the hope it gives to my family and other patients who will eventually be saved by this breakthrough.”
The surgery last Friday took seven hours at the Baltimore hospital. Dr. Bartley Griffith, who performed the surgery, said the patient’s condition — heart failure and an irregular heartbeat — made him ineligible for a human heart transplant or a heart pump.
Griffith had transplanted pig hearts into about 50 baboons over five years, before offering the option to Bennett.
“We’re learning a lot every day with this gentleman,” Griffith said. “And so far, we’re happy with our decision to move forward. And he is as well: Big smile on his face today.”
Pig heart valves also have been used successfully for decades in humans, and Bennett’s son said his father had received one about a decade ago.
As for the heart transplant, “He realizes the magnitude of what was done and he really realizes the importance of it,” David Bennett Jr. said. “He could not live, or he could last a day, or he could last a couple of days. I mean, we’re in the unknown at this point.”

First Pig-Heart Transplant Patient Dies 2 Months After Surgery

According to the statement, the transplant worked very well for "several weeks" and there was no sign of rejection.
