Muckle-Wells Syndrome (MWS) is a rare autosomal dominant genetic disorder of IL-1 regulation, thought to be due to a mutation in the gene for the NLRP3 protein. It causes periodic fevers, an urticarial rash, conjunctivitis (>90%) arthralgias (100%), myalgias (50%), chronic fatigue (100%), and sometimes secondary amyloidosis (25%). Typical onset is 70%) go on to develop sensorineural hearing loss in later childhood. MWS is one of the three cryopyrin-associated periodic syndromes (CAPS), the other two being Familial Cold Autoinflammatory Syndrome (FCAS) and some cases of Neonatal-Onset Multisystem Inflammatory Disease (NOMID). Unlike Familial Cold Autoimflammatory Syndrome, triggers for MWS attacks include cold, heat, infection, surgery, and many other other stressors. Treatment includes steroids for acute attacks, NSAIDs + a PPI for joint symptoms, and a chronic IL-1 blocker like anakinra.
