My scary tube scan results explicitly say "no intracranial aneurysm"
The rest is under the cut, not because it's bad news but because I don't want people who are sensitive to medical speak to accidentally see it.
I can't express my relief. I've been terrified to have this scan done for well over a year because I was so afraid. For those unaware, my mother has this condition and experienced similar symptoms. It's really hard to describe the level of deep, existential fear that's just been lifted off me.
I haven't met with my doctor yet to review in full but based on the note it looks like I have a "high riding jugular bulb" which is likely the reason why I can hear my pulse in my right ear (clinically this is called pulsatile tinnitus). The irony is that the treatment for pulsatile tinnitus looks to be very similar to the treatment for a brain aneurysm. But I don't think I need treatment, not now at least. The sound is annoying but easily drowned out by almost any sound above "quiet office" noise levels.
Interestingly, my scan results also noted "partially empty sella." I had to google this but apparently this is a non-life threatening condition that often has no symptoms and needs no treatment, but can cause "less desire for sex and irregular periods."
Honestly did not expect an MRI scan to reveal a possible cause to issues that have plagued me for well over a decade. I don't know if it's treatable but it really doesn't matter, because for once in my life I can actually feel completely validated in actually having a fucked up sex drive.

my PA for ajovy got denied (booo) but the reason it did was bc a neurologist needed to prescribe it which i already had an apt set up for and gad yesterday (yay!) and the appointment went really well and i like the provider a LOT of migraine management (YAY!!!) so now i am once again waiting for an ajovy PA to be approved lmao
#nervous abt needles but jax offered to help plus ajovy has auto injectors so we are going for it#but i really liked the provider she was good and very easy to talk to and stuff!! and am excited to work w her and try things#also got a referral to their sleep clinic bc my sleep is still Bad and also sleep is a big trigger for me#so im hopeful to see what i can figure out/learn from that#this is the year of blue getting health shit figured out lmak gonna get my tonsils removed etc etc lets Go#she also said i could be a candidate for botox if that seemed interesting but i like how like. open ended she was w things#like she gave me a lot of good info but also everything was my call#she also was good when looking at my mri and explained why she isnt concerned abt ms based on mri and symptoms#and in a way where i actually agree lol like. esp bc i get so anxious i can just latch on to something#but she took the time to answer my questions and explain and show what theyd see if it was ms vs my brain lesions from migraines#and Yeah! i was super stressed and like gad cleared my whole afternoon in case it was a bad time#but it wasnt! im v happy w my new neurologist!#wild to have TWO whole doctors on my care team who i like and trust???? as a fat person??? wow#(trust is w a grain of salt lol i dint entirely trust any doctor But)

#speak friend and enter#let me preface this by saying that im doing everything in my power to not let mental illness wipe its greasy hands on me#however. im insane in the membrane and i can feel myself slipping back into lunatic mode#i have to go for an mri next week and i genuinely don't know if i can do it. i am so fucking terrified you have no idea#i'll spare y'all the grisly details but i was chronically ill as a kid (and not just like sick a lot it was touch and go there for a bit)#and as a result of certain procedures i had to undergo to abate the aforementioned chronic illness#i developed ptsd that manifests as an irrational but obscenely debilitating fear of hospitals#like i can't go in a hospital without having a psychotic episode. like clinically i just can't do it#but as part of my yearly post-whatever care i have to get imaging done and this year that entails an mri and. im just scared#i spent a significant portion of my time immediately post ptsd symptom presentation believing that my doctors were trying to kill me#like for sport. like i thought there was some larger deep state esque plan in place to enact further medical barbarism upon me for giggles#and obviously you and i both know that's a delusion with no basis in reality but that doesn't mean i can stop myself from believing it.#it's like a word-of-god thing. i know logically that it's not true but there's a voice in my head screaming 'they want to flay you alive'#and i am currently between therapists and also unmedicated bc my last therapist was too focused on inner child work#to give me the prozac and weed card i really need#like that's great that you think healing my inner child will solve this but my inner child is covered in her own viscera. can we pivot mayb#but anyway for the moment im just wallowing in my own fear and im doubly scared bc im finding myself falling into rabbit holes again#like empirically the worst thing that's gonna happen as a result of this mri is that they're gonna say i have to have another surgery#and the technology has advanced to a point where its way less invasive than what ive had previously#but the constant dull roar of my thoughts about the whole deal is just. increasingly delusional nonsense#and not to be overly morbid or anything but i decided a long time ago that if i ever had to be admitted to the hospital again i would rathe#well you know. and i don't wanna die. honestly i don't. but the idea of wading through that particular brand of hell again is torture#and im not gonna kill myself. im not. ive been working on that impulse for a long time and i don't want to undo all of that work#but im scared and i dont wanna spend the rest of my life in n out of the hospital or as a substance-abusing recluse. is that so much to ask#i want to fix this. i do. i don't wanna live in a hole anymore as fantastic mr fox would say. but the horrors persist#and i often find myself increasingly unable to cope. hence why i need the weed#anyway i'll be fine. eventually. i hope. but in the meantime i do want to say i appreciate you all. i mean it#i tend to regard myself (fairly or otherwise) as difficult to get along with in real life so despite the fact that i don't talk w y'all muc#i do appreciate y'all being there and making me feel like more of a person than i feel like i am lately <3

A case of moyamoya disease by Fei Yang Chaojie Zou in Journal of Clinical Case Reports Medical Images and Health Sciences
Introduction
Moyamoya disease (MMD) is a rare cause of stroke with progressive stenosis of the end of the internal carotid artery and compensatory capillaries. The symptoms and course of moyamoya disease depend on the age of the patient at the time of symptom onset and the type of first episode (ischemic or hemorrhagic), but vary in severity from transient ischemic episodes to persistent neurological deficits [1].
Case
Clinical data
The 57-year-old female patient was admitted to the hospital on May 10, 2023, due to "depression, nervousness, worry and numbness of the body for more than 2 months." The patient complained of numbness and discomfort in the left index finger, middle finger and ring finger two months ago, gradually developed numbness and discomfort in the mouth, indistinctness of speech, weakness in both lower limbs and instability in walking, which improved for about half an hour. On the same day, I went to the local hospital in Guiyang for treatment, and no special abnormalities were found after completing relevant examinations (specific details are unknown). These symptoms can occur after strenuous exercise. Patients began to repeatedly worry about their physical conditions, nervous irritability, easy to lose temper, panic discomfort. Gradually feel low mood, decreased interest, lack of motivation, negative suicidal thoughts, poor night sleep.
Past History
"High blood pressure" for 3 years, with a maximum blood pressure of 185/? mmHg, currently taking "amlodipine 20mg QD" for the treatment of private complaints of irregular medication, poor blood pressure control.
Physical Examination
T:36.2℃ P:93 times/min R:21 times/min Bp: 119/71mmg. No obvious abnormality was found in the heart, lung and abdomen, and the signs were difficult to stand up (+). Conscious, repeatedly stressing that he was unwell. Lead to severe depression and anxiety, slow thinking, lead to negative ideas; Did not induce hallucinations delusions. Loss of instantaneous memory, loss of computational power. No depression or alternation of mood.
Auxiliary Check
Post-admission scale HAMD:29 points (severe), HAMA:31 points (severe)
Blood biochemistry after admission: triglyceride 23.79mmol/L, free cholesterol 3.43 mmol/L, total cholesterol 6.57 mmol/L. Head MRI+MRA: bilateral internal carotid artery intracranial segment occlusion, bilateral anterior and middle main cerebral artery shallow and local occlusion, poor development in the far branch, CTA is recommended to determine whether there is smoke change. Bilateral cerebral p1-2 is more likely to be cold than peripheral. Head and neck CTA: (1). Bilateral internal carotid artery C1-C7 segments were not developed, and severe stricture-occlusion was considered. (2). There was no development in the M1 segment of the bilateral middle cerebral artery, and there were multiple tortuous collateral circulation small blood vessel shadows around, so Moyamoya disease could be considered. (3). Bilateral anterior cerebral arteries are slender; Right-anterior cerebral artery A2 stenosis is mild to moderate. (4). Severe stenosis of P1-P2 segment of the right posterior cerebral artery is possible; The left posterior cerebral artery P1-P2 lumen is moderately narrow. Increased distal branches of bilateral posterior cerebral arteries.
Diagnosis
Major depression without psychotic symptoms; Moyamoya disease; Hyperlipidemia
Discussion
DSA is the gold standard technique for detecting intracranial stenosis and cerebrovascular malformations. The Japanese guidelines for the diagnosis of Moyamoya disease include MRI and MRA as a non-invasive diagnostic method . The latest diagnostic criteria in Japan suggest that cerebral angiography may be omitted when MRI and MRA findings meet all of the following: (i) MRA shows intracranial internal carotid artery or anterior cerebral artery and/or middle cerebral artery proximal stenosis or occlusion. (ii) MRA showed abnormal basal ganglia vascular network . At present, the treatment of moyamoya disease mainly focuses on neuroprotection, cerebral blood flow reconstruction and neurological rehabilitation. Treatment strategies can be divided into medical and surgical interventions. The main purpose of surgical intervention for Moyamoya disease is to improve blood flow in the hypoperfusion brain, and revascularization is considered useful for reducing ischemic injury . A 10-year follow-up evaluation from a Japanese Research Council registered study showed that antiplatelet therapy did not affect the incidence of cerebral infarction in patients with moyamoya disease. Therefore, revascularization is the most effective treatment for hemorrhagic and ischemic moyamoya patients .

As someone who did MRIs just for fun to see my/our brains, yes it’s fun. But also NOOO!! This is such over diagnosis and actually harmful!
They had this idea to do this to every citizen here a while ago, to on one hand have previous findings of everyone - super helpful in case later in life something develops - and on the other hand have more and more data for research, which would be so great. The problem is just. You will find something. Not in everyone but in many people without any symptoms, without any indication for treatment. But how do you sleep at night knowing there is something? So treatment, treatment, treatment for little things, that wouldn't have needed treatment, because there was no symptom, no suffering to begin with! Treatment can mean medication, every medication has side effects and they are serious. Treatment can in these cases often mean surgery. Surgery has risks. Risks and complications, always. So now we have risky procedures and side effects from medication for people, not saying patients, because they are not really sick, who wouldn't have needed them, wouldn’t have needed the risks and side effects either. And worries, lots of worries for all those people. They had to stop that project soon after its start because the number of unnecessary procedures went sky rocket. There is (here) something called the right of ignorance. Of the bliss of ignorance.
When we put healthy people in MRIs for research, they have to sign this right off to us. Because otherwise we are in an ethical conflict, of telling or not telling should we see something. I had lots of people happily sign those forms for me, but also a few who actually weren't too happy about this. And it came as it had to come, the one person who had been really unhappy about giving up this right, had a brain, that didn’t look too textbook like. It ended up all good, it was benign, but that poor girl, she will not have spent a too calm night that week, that month, she won’t have forgotten that shock.
MRIs are great! They are so safe! We should have more of them, be able to use them more! For actual clinical problems! (and research). It sucks, that it’s as hard to get an MRI, when you need one, because you have symptoms, we should have more resources and more MRIs (that’s another problem with over diagnosed unnecessary treatments, while endless time and resources would be great, we don’t have those, so with every unnecessary procedure a dearly necessary one will be pushed or not possible to be done)!
But just having everyone do this would harm a good percentage of those examined, even if there is no radiation.
If you want to see your brain for fun, participate in research studies! It is always great to have volunteers and will be a great help, and possibly fun! Win-win for everyone!
