Fuck global warming, it’s 52 in February in Michigan! All I needed this morning was my hoodie and it was fucking HOT in the hospital but I was cheeky and didn’t wear a bra today because of my oversized hoodie and man did I regret it, my big naturals need a bra lol I had my annual heart appointment this morning though so I would have had to take it off anyway so I said fuck it entirely. This morning was so stressful, my blood draw was really easy so very thankful for that but the hospital app was down this morning so we didn’t know where to go and the front desk sent us to the main hospital’s PET scan department when I apparently needed to be in the PET scan department in the cardiovascular center. So, I had to walk all over the various buildings to finally get to the right department and was a tad late but thankfully I was the only appointment so it wasn’t a big deal. For fucking once in my life they got the IV in the first try 😭 I almost cried, I’m such a hard stick it usually takes 4-5 attempts to start an IV on me. So, I was very thankful it went in so easily, it burned really badly though, or at least the medication did. They inject you with radioactive dye to make everything show up and then give you medication to simulate exercise all while you’re in an MRI machine to get imaging of the heart. It’s such a weird sensation, you go from half falling asleep to suddenly feeling like your sprinting in seconds, it’s not necessarily unpleasant but it is odd but like I mentioned earlier the medication burned quite a bit going in. The imaging was apparently sufficient even though the drug didn’t do its job as well as it was supposed to because they sent me home in a timely fashion. I got back home and took a very long nap and apparently the girl who wrapped my IV didn’t wrap it tight enough because the dresssing was soaked in blood when I took it off later. It’s bruised but not awfully. All in all things went better than they could have. Downside my labs aren’t good at all. My tacrolimus level is an 8 it’s supposed to be between 4-6, so that’s not good but reasonably easy to fix, they’ll just lower my dose again and repeat labs next week most likely. The really bad thing though is my liver is crazy out of whack again, which really blows. It might be as easily explained as my enzymes are spiked because I just got over the flu but last time it was this high is when I had CMV. I’ll get my results for the CMV test tomorrow. In more news no one cares about but I over share is my parathyroid is very high which is actually good because it finally explains my chronic fatigue and weird appetite lately. Hopefully they can get it under control and my energy levels will return and I might be able to do things again. In small good things my kidneys are stable, they’re stage 3b kidney failure but they’ve also been there for like 7 years so they’re not getting worse, dance party time.
TL:DR: I’m still dying slowly but instead of just my heart and kidneys doing the dirty work now my liver is back on board for murder too.

When you find a post/blog by someone who seems knowledgeable about your understudied chronic illness and you get hopeful that they might have advice for symptom management but then they start going into how using essential oils to "detoxify" your body can improve symptoms -_-
#like great i don't trust literally anything else you said now#it's the worst when the quack shit is tacked on at the end cause it gets you all hopeful for a moment#like man i just want ways to manage post-exertional malaise#i get why so many people with chronic illnesses are taken in by pseudoscience cause it offers an easy fix that real medicine won't give#and the people hawking it often give the appearance of listening to your woes more than most doctors do#but it's still frustrating#like no this isn't going to help you and may in fact make you worse. you've been scammed at best and lured into a pyramid scheme at worst#i never see any of these fucks properly warn people either that if you have pets you absolutely cannot use most of those products#i hate the term ''detox'' the most bc it's the most demonstrably bullshit one out there#like do you have a working liver and kidneys? congrats! you don't need detoxing#and if you don't well that's a big problem and you probably need an organ transplant#medical detoxing is called ''dialysis''

BIG NEWS
I'm not sure who's left around these parts that doesn't follow me on facebook or whatever but for anyone who knows the struggle --
Yesterday/last night my mom finally received her double organ transplant after waiting 5 years. The surgery was successful and her new liver and kidney are healthy and the doctors are very happy. She was up and talking today already even after a 15+ hour transplant surgery.
I want to send big love to her team of doctors, and the most most most love to the family who lost their child and chose to give the gift of life during their tragedy.
Please consider being an organ donor!

I was visiting my sister and she covertly took pics of me and my brother in law playing with the kids, but he knew she was taking pictures and posed like rose in titanic in that paint me like your french girls scene, while I look absolutely unhinged cause I was in a ball pit performing a fake vivisection on their oldest child (her favourite for some reason)
#i didnt realise she was taking pictures#i was too busy checking if the liver and kidneys are transplant ready#which just means i poke my niece in the stomach#but for some reason this is her favourite like playtime activity

Organ Transplant: Saving Lives Through Donation and Medical Advancement
The History of Organ Transplant
The first successful organ transplant took place in 1954 when a kidney was transplanted between identical twins in Boston. This groundbreaking surgery paved the way for other transplant procedures. Throughout the 1950s and 60s, doctors continued experimenting with kidney transplants with varying success rates due to the body's natural immune response. It wasn't until the 1980s that anti-rejection drugs like cyclosporine were developed, allowing transplants between non-related donors and recipients to become more widely performed and successful. Types of Transplants
Kidneys are the most commonly transplanted organs, with over 80,000 kidney transplants performed in the United States alone between 1988-2018 according to the Organ Procurement and Transplantation Network. Other organs that can be transplanted include the liver, heart, lungs, pancreas, intestine, and thymus gland. Tissue such as corneas, skin, bone marrow, heart valves, veins and ligaments can also be donated. Organ Donation
For a Organ Transplant to occur, a donor must first agree to donate their organs upon death. In the United States, individuals can register as organ donors through their state's department of motor vehicles or on the Organ Donor Registry. Family consent is also required before organs can be recovered. Once a donor is declared brain dead but their heart is still beating, doctors work to preserve the organs until they can be surgically removed and transplanted into recipients waiting for a life-saving gift. Waiting for an Organ
The need for donated organs far outweighs the available supply. As of January 2020, there were over 113,000 candidates on the U.S. national transplant waiting list according to the Organ Procurement and Transplantation Network. The waiting time for a donated organ varies depending on blood type, organ size match and medical urgency. On average, 20 people die each day from the lack of available organs. Children have a higher mortality rate while waiting, with nearly 5,000 children in need of a transplant. Advancements in Living Donation
While deceased donor transplants remain the standard, advancements have been made to increase living donation options. In kidney and portions of liver or lung donation, a healthy person can elect to donate an organ or parts of organs to recipients in need. The recipient's prognosis is often better when the donated organ is still functioning at the time of transplant rather than being recovered from a deceased donor. Some donors are emotionally motivated to help a loved one, while others choose to altruistically donate to a stranger. Either way, living donation has elevated many from waiting lists to restored health. The Future of 3D Bioprinting and Xenotransplantation
Scientists are exploring new frontiers to close the organ supply gap through bioprinting and cross-species transplantation possibilities. 3D bioprinting uses inkjet-like printers to deposit cells layer by layer, building functional organs. Researchers have printed heart tissue, liver buds and kidney structures, with the goal of fully printed transplantable organs in development. Meanwhile, some scientists are testing the use of organs from genetically-modified pigs, known as xenotransplantation. Pig heart valves and skin are already used as human transplants with promising research underway for whole organs. If effective, bioprinting and xenotransplantation could revolutionize transplant availability in coming decades. The Impact of a Transplant
For recipients who receive that lifesaving gift from a donor, organ transplantation means a second chance at life. Kidney transplant patients can discontinue dialysis treatments and greatly improve their quality of living without being tethered to a machine. Liver recipients are cured of end-stage liver disease and related conditions. Heart transplant patients are no longer constrained by heart failure.
lung transplants restore breathing abilities for people suffering from cystic fibrosis, COPD or pulmonary hypertension. The impact of organ donation should not be underestimated - for both recipients who receive more days with their families, and donors who give the ultimate gift so others may live on. Continued medical progress and donation awareness will help even more individuals overcome organ failure in the future.

In other non MK related news my mental health is really in the shitter today. I know most people who see this won’t care but I use tumblr as an outlet for my emotions. I’m really reeling from my hepatology appointment yesterday, I’ve known my kidneys were bad for a while because tacrolimus is a horrible, wicked, two-sided drug, on the one hand it keeps my body from destroying my heart but on the other hand it ruins my kidneys but I had no clue anything was wrong with my liver. The doctor said living with my childhood condition of left ventricular failure for 14 years caused irreversible liver damage. I just, I don’t know what to do. I just want to cry but for some reason I can’t. I’ve always known I was living on borrowed time ever since my heart transplant but now with having so many issues with so many vital organs it seems like my time is running out sooner than I expected. I guess 25 years was a good run but it seems so fucking ironic and unfair that I finally don’t want to die and I will. I get the feeling my celebration trip I planned for later this year will probably be my final vacation, so I guess I better try and enjoy it as much as I can. I’m not necessarily sad for myself but Denny won’t understand why I’m gone and that really breaks my heart. He’s such a good boy and I love him so much I don’t want to do anything that will make him sad. At least I’ll get to be reunited with Jäger if anything in life is even remotely fair.

Advancements in Surgical Techniques for Liver Transplantation
Liver transplantation has evolved significantly since its inception, offering hope to countless patients suffering from end-stage liver disease. As a leading Liver Transplant Surgeon in Pune, Dr. Aniruddha Bhosale has witnessed and contributed to the remarkable advancements in this life-saving procedure. This article delves into the latest surgical techniques that are enhancing outcomes and improving the quality of life for liver transplant recipients.
Historical Context
The first successful liver transplant was performed in 1967 by Dr. Thomas Starzl. Since then, the field has progressed from high-risk experimental surgeries to a well-established and routinely performed procedure. The introduction of immunosuppressive drugs like cyclosporine in the 1980s marked a significant milestone, reducing the risk of organ rejection and improving patient survival rates. Today, ongoing innovations in surgical techniques continue to push the boundaries of what is possible in liver transplantation.
Laparoscopic Liver Transplantation
Laparoscopic surgery, a minimally invasive technique, has revolutionized various surgical fields, including liver transplantation. This approach involves making small incisions through which specialized instruments and a camera are inserted, allowing the surgeon to operate with enhanced precision. The benefits of laparoscopic liver transplantation are manifold:
Reduced Blood Loss: Smaller incisions lead to less bleeding during surgery.
Shorter Recovery Time: Patients experience less postoperative pain and a faster return to normal activities.
Lower Infection Risk: The minimally invasive nature of the procedure reduces the likelihood of infection.
Dr. Aniruddha Bhosale has been at the forefront of adopting laparoscopic techniques, ensuring that his patients benefit from these advancements.
Robotic-Assisted Liver Transplantation
Robotic-assisted surgery represents the cutting edge of surgical innovation. This technique utilizes robotic systems to assist surgeons in performing complex procedures with greater accuracy and control. The da Vinci Surgical System is one such example, providing a 3D high-definition view and enhanced dexterity through its robotic arms.
Enhanced Precision: Robotic systems allow for meticulous dissection and suturing, minimizing the risk of complications.
Greater Flexibility: Surgeons can perform intricate maneuvers that are difficult to achieve with traditional methods.
Improved Outcomes: Studies have shown that robotic-assisted liver transplants can lead to better graft survival and reduced postoperative complications.
Dr. Aniruddha Bhosale's expertise in robotic-assisted liver transplantation ensures that patients in Pune receive the highest standard of care.
Living Donor Liver Transplantation (LDLT)
Living Donor Liver Transplantation has emerged as a viable solution to the shortage of deceased donor organs. In LDLT, a portion of a healthy donor's liver is transplanted into the recipient. The donor's liver regenerates to its original size within a few months, as does the transplanted portion in the recipient.
Reduced Waiting Time: LDLT significantly reduces the time patients spend on the transplant waiting list.
Improved Graft Function: Living donor livers are typically healthier and function better than deceased donor livers.
Planned Surgery: The transplant can be scheduled electively, ensuring optimal timing for both donor and recipient.
Dr. Aniruddha Bhosale's proficiency in LDLT has provided many patients with a timely and effective treatment option, demonstrating excellent outcomes in terms of graft survival and patient recovery.
Split Liver Transplantation
Split liver transplantation is another innovative technique addressing organ scarcity. In this procedure, a deceased donor liver is divided into two segments, each transplanted into a different recipient. Typically, the larger segment goes to an adult, while the smaller segment is suitable for a pediatric patient.
Maximizing Donor Organs: This approach allows a single donor liver to save two lives.
Efficient Use of Resources: It optimizes the use of available donor organs, reducing waiting times for patients.
Dr. Aniruddha Bhosale's involvement in split liver transplantation highlights his commitment to maximizing the lifesaving potential of donor organs.
Normothermic Machine Perfusion (NMP)
Normothermic Machine Perfusion is a groundbreaking technique that preserves donor livers outside the body by perfusing them with oxygenated blood at body temperature. This method offers several advantages over traditional cold storage.
Extended Preservation Time: NMP allows for longer preservation times, facilitating better logistical planning for transplant surgeries.
Improved Organ Quality: The continuous perfusion maintains liver function and reduces ischemia-reperfusion injury.
Assessment and Repair: Surgeons can assess the liver's viability and even perform therapeutic interventions to improve organ quality before transplantation.
Dr. Aniruddha Bhosale's use of NMP reflects his dedication to leveraging cutting-edge technology to enhance transplant outcomes.
Innovations in Immunosuppression
Advancements in immunosuppressive therapies are crucial for the long-term success of liver transplants. Traditional immunosuppressive drugs, while effective, can have significant side effects. Recent innovations aim to minimize these adverse effects while maintaining efficacy.
Targeted Therapies: Newer drugs specifically target immune pathways involved in organ rejection, reducing collateral damage to the patient's immune system.
Personalized Medicine: Genetic and biomarker studies enable personalized immunosuppressive regimens tailored to individual patient profiles.
Tolerogenic Approaches: Research into inducing immune tolerance aims to reduce or eliminate the need for lifelong immunosuppression.
Dr. Aniruddha Bhosale stays abreast of these advancements, ensuring his patients receive the most current and effective treatment protocols.
Postoperative Care and Rehabilitation
The success of liver transplantation extends beyond the operating room. Comprehensive postoperative care and rehabilitation are critical for optimal recovery and long-term health. This includes:
Multidisciplinary Approach: Coordinated care involving hepatologists, transplant surgeons, nutritionists, and physical therapists.
Patient Education: Informing patients about medication adherence, lifestyle modifications, and recognizing signs of complications.
Regular Monitoring: Routine follow-ups and lab tests to detect any issues early and adjust treatment plans accordingly.
Dr. Aniruddha Bhosale emphasizes a holistic approach to postoperative care, recognizing its importance in ensuring successful liver transplant outcomes.
Conclusion
The field of liver transplantation is continually evolving, with advancements in surgical techniques playing a pivotal role in improving patient outcomes. From laparoscopic and robotic-assisted surgeries to innovative preservation methods and personalized immunosuppressive therapies, these innovations are transforming the lives of patients with liver disease.
As a leading Liver Transplant Surgeon in Pune, Dr. Aniruddha Bhosale remains at the forefront of these advancements, committed to providing his patients with the highest standard of care. His expertise and dedication ensure that liver transplant recipients receive the best possible chance for a healthy, fulfilling life post-transplant.For more information or to schedule a consultation, visit Dr. Aniruddha Bhosale's clinic and take the first step towards a new lease on life.
