A large trial investigating the effectiveness of a celery-seed-derived neuroprotectant drug for use in ischemic stroke cases has shown significant promise.
Patients given the medicine called butylphthalide had a 70% better chance of improved neurological and general living outcomes 90 days post-stroke, a finding generated from a little over 1,000 cases.
Butylphthalide is already approved for use in treating strokes in China. Currently though, the FDA has not approved it for any use.
How butylphthalide works isn’t clear, with animal studies suggesting various possible mechanisms. It may help protect brain tissue from damage when the supply of oxygenated blood is cut off and it works alongside existing clot-busting drugs or procedures to remove blood clots in ischemic stroke conditions.
The trial consisted of 1,216 patients who were admitted for stroke in China where the drug is approved for use, and given it alongside a tissue plasminogen activator (tPA) and/or a mechanical clot removal procedure—two primary means of dealing with ischemic, or clot strokes.
Every year, more than 795,000 people in the United States have a stroke, with 87% of these being ischemic ones. The burden of these events is large, and they are a leading cause of disability in the elderly.

On November 21 I spoke to another  class of Carol Carielli’s students learning to be teachers who had seen “A Stroke Of Endurance”. One student asked if the real professor who “A Stroke Of Endurance” is based on had his stroke in class, and I told them he had the stroke on campus but not during our class. Is radon linked to health condition other than lung cancer?
Radon is a known mutagen, meaning it can cause genetic mutations, it is also a growing concern because this gas makes its way into homes, and exposure is increasing as modern construction practices make homes more airtight.
We wanted to determine if exposures to high levels of radon are linked to this condition called CHIP, caused by genetic mutations in hematopoietic stem cells.
The condition, called clonal hematopoiesis of indeterminate potential (CHIP), develops when some hematopoietic stem cells, the building blocks for all blood cells, undergo genetic mutations as a person ages.
Cells with such mutations replicate more quickly than cells without them.
Previous research has shown people with CHIP may have a higher risk of blood cancers like leukemia and cardiovascular disease including stroke.
Researchers found participants with ischemic stroke living in areas with the highest concentrations of radon had a 46% increased risk of CHIP, while those living in areas with medium concentrations had a 39% increased risk compared to those living in areas with the lowest concentrations of radon.
Risk was not increased in participants without stroke.

Objective:
To share an intriguing case of a young transgender male patient receiving testosterone therapy who developed locked-in syndrome due to an acute ischemic stroke and to highlight potential risk factors for stroke in the LGBTQI+ community
Background:
There are many studies identifying risk factors for stroke in racially and ethnically diverse populations. However, there is little existing data for stroke risk factors in the LGBTQI+ community. Prior research has shown testosterone therapy in cis-gender men with initially low levels of testosterone increases the risk of stroke, especially in the first 2 years of use1. While testosterone therapy has been shown to increase the risk of venous thrombosis, its role in arterial thrombosis is unclear2. A proposed mechanism for thrombosis with testosterone replacement includes erythrocytosis, but the potential contribution of an independent pro-coagulant effect is yet to be determined3.
Design/Methods:
Literature review and case report.
Results:
An otherwise healthy 23-year-old transgender male on one year of testosterone therapy presented in an obtunded state. Examination revealed complete quadriplegia with sparing of vertical eye movements, consistent with locked-in syndrome. Imaging revealed complete occlusion of the basilar artery with distal reconstitution at the superior cerebellar arteries, and a large bilateral ischemic infarct of the pons. Computed tomography angiography did not demonstrate other large vessel disease or structural vascular abnormalities. Unfortunately, the patient was out of the time window for any acute stroke interventions. A hypercoagulable workup was performed but results were unrevealing and hematocrit was normal. Further investigation with transthoracic echocardiogram, transesophageal echocardiogram, and telemetry were negative for thrombus, patent foramen ovale, and atrial fibrillation.
Conclusions:
Acute ischemic stroke may be an under recognized complication of testosterone therapy in transgender males independent of degree of erythrocytosis. Further research is needed to establish a safety profile of testosterone therapy in this understudied population.
