On 26th November 1917 Elsie Inglis, the Scottish doctor, nursing pioneer and suffragette, died.
Every Scot out there should read this with pride, Elsie Inglis and the other Scottish doctors and nurses faced prejudice and the horrors of war, but they did not flinch in what they saw as their duty.
Born in India in 1864, she was the daughter of John Inglis, a chief commissioner in the Indian civil service. She studied medicine at Dr Sophia Jex-Blake’s newly opened Edinburgh School of Medicine for Women and was one of the first women in Scotland to finish higher education, although she was not allowed to graduate. She went on to complete her training under Sir William Macewen at the Glasgow Royal Infirmary.
On the outbreak of WWI Elsie approached the War Office with the idea of either women-doctors co-operating with the Royal Army Medical Corps, or women's medical units being allowed to serve on the Western Front. The authorities were less than helpful and it is reported that an official said to her "My good lady, go home and sit still".
Despite attempts to repress her efforts—and those of many other women—to contribute, Elsie did not “sit still”. Instead, she persevered, setting up the Scottish women’s hospitals, which were all-female units that played a vital role with Britain’s allies, including the French, the Belgians and, particularly, the Serbs.
Elsie was 50 when war broke out and she defied British Government advice by setting up field hospitals close to the frontlines. She travelled to France within three months of the outbreak of war, and the all female staffed, Abbaye de Royaumont hospital, containing some 200 beds, was in place by the end of 1914. That was followed by a second hospital, at Villers Cotterets, in 1917. Tens of thousands were helped by the hospitals she set up in France, Serbia, Ukraine and Romania, acting with the support of the French and Serbian Governments.
Prior to that, Elsie was a strong advocate of women’s rights and a leading member of the suffragette movement in Scotland, playing a notable role in the establishment of the Scottish women’s suffrage federation in 1906. She fought energetically against prejudice and for the social and political emancipation of women, and had already made a huge impact in Edinburgh by working in some of the poorest parts of the city with women and babies who were in desperate need of help. Selflessly, she often waived the fees of patients who could not afford to pay.
Politically, Elsie was a staunch campaigner for votes for women, and her involvement in the suffragette movement prompted her to raise money to send out to female doctors, nurses, orderlies and drivers on the frontline. She recorded many great achievements, including setting up 14 hospitals during the war—staffed by 1,500 Scottish women, all volunteers. Most notably, Elsie raised the equivalent of £53 million in today’s money to fund greatly needed medical care for those on the frontline. Her efforts reached across the waters on another level, attracting volunteers from New Zealand, Australia and Canada. As I am sure everyone would agree, that showed fierce independence and capability from women who were well ahead of their time.
By 1917 Inglis knew she had cancer, and by the end of September was unable to work as a surgeon she sent a telegraph home saying, ‘Everything satisfactory and all well except me.’ Inglis and her unit landed in Newcastle and the following day, 26 November 1917, in the presence of her sisters, Inglis died.
In Edinburgh the response was huge and the streets were lined with people as her body was returned to the city. While there was no Victoria Cross for her at home, in Serbia she was the only woman to receive the Order of the White Eagle and is remembered by the nation every year in a ceremony at the memorial fountain built in her honour.
Before her body was interned in Dean Cemetery, Inglis’s body lay in state in St Giles’ Cathedral. The SWH continued its work for the duration of the war, sending out more units and raising money for the work. Remaining funds were used to establish the Elsie Inglis Memorial Maternity Hospital in Edinburgh in July 1925.

Asia tackles new monkeypox strain escalation
Asian countries from China to Pakistan have stepped up surveillance of a mutated monkeypox virus that is spreading outside Central Africa, where the World Health Organisation declared a public health emergency this month.
A case in Sweden last week underlines the international threat posed by the so-called 1B strain, which has reportedly killed more than 500 people, mostly children, in the Democratic Republic of Congo. Across Asia, travellers arriving from countries affected by the virus are being asked to report symptoms and hospitals are stepping up surveillance for the disease.
Unlike the smallpox-causing virus, which spread worldwide in 2022, the new strain is more dangerous, with a mortality rate of more than 3 per cent. It is also possible that infected people can transmit the virus days before symptoms appear, making it more difficult to control the disease by identifying and isolating cases.
China is asking incoming travellers from affected countries and regions, those who have been in contact with smallpox patients, and those experiencing symptoms to report to customs officials upon entering the country, where they may be required to undergo testing, the statement said.
Health authorities in India have ordered increased vigilance at international airports and ports for passengers with smallpox symptoms, and have designated hospitals and laboratories to deal with potential patients, the Economic Times reported. Indonesia and Malaysia have also taken similar measures.
While smallpox is back in the spotlight, health officials are reporting cases probably caused by the milder strain 2b. A case of infection with strain 2b was confirmed in Pakistan on 13 August. The Philippine Department of Health said it had identified the first case of smallpox in the country since December in a 33-year-old man who had not travelled recently. The strain that caused the disease is not yet known.
A 2022 UK study found that more than half of mpox cases were spread by infected people who had not yet developed symptoms. Emerging evidence suggests that people incubating the disease can transmit the smallpox virus for up to four days before symptoms appear.
Luxembourg’s health minister said last Friday that the risk of the Luxembourg population contracting smallpox is “low.”
WHO recommends special measures to control monkeypox outbreak
The World Health Organisation is recommending special measures to control the outbreak of mpox, also known as monkeypox, in five African countries – the Democratic Republic of Congo, Burundi, Kenya, Rwanda and Uganda.
According to WHO recommendations, states should strengthen national health emergency response mechanisms, improve coordination of mpox control efforts, improve laboratory diagnosis of the disease, and identify contacts of mpox-infected people with others. In addition, WHO is calling for increased border surveillance to identify those who have become ill, vaccination and better public awareness of the disease and control measures.
The African Union Health Service declared a public health emergency on the continent due to the spread of monkeypox on 13 August. The disease has been detected in 17 African countries, totalling more than 38,000 infections and more than 1,400 deaths. The epicentre of the outbreak has been the eastern Democratic Republic of Congo, where 92% of African cases have been reported.

A good day’s work and a celebration dinner
Tuesday 19th September
Breakfast here has different food on offer. Mo started the day with liver and plantain....
Our conference day began with lingering anxiety over the technology, then a breath of relief.
Our good friends Mahesh and Sujeet - both professors at Chester University and Cheshire & Wirral Partnership where I used to work - had kindly agreed to give us some teaching on Intellectual Disability and Neurodevelopmental Disorder. ID and NDD! This meant them getting up rather early to allow for the time difference between our two countries, and both ends needing faultless technology to make it happen, despite it being a two hour session over the internet.

ID and NDD are common around the world, and probably neglected more than any other mental health condition.
Happily all went well, and we had a very comprehensive learning session with two international experts. At one point Mahesh was helping us to think about the difficulty some people with NDD have in understanding what others are saying. His illustration was to talk to our students in a foreign language – his native language from a remote part of India, which no one had even heard of. The students were quite entertained as they listened; he turned out to be asking them what they’d had for breakfast.
Our session on epilepsy later in the morning was one of joint learning.
I challenged the students to discuss how we can do better – rather than just running huge clinics with a minute or two for each patient to review medication and little more, can we do better to help this person in their situation? How are their families managing the stigma? How much do community leaders know? How could we help to change attitudes amongst teachers, employers and the general public? How can we prevent epilepsy from even happening?
Meanwhile I learnt a whole lot more about traditional knowledge of epilepsy and its effects, and the perpetuation of the stigma surrounding the disease.
In April Uganda hosted a national conference on epilepsy. I didn’t know that until I was well on the way thinking about our teaching session! There are 770,000 individuals living with epilepsy in Uganda. Many others are not known to services. There are very few neurologists in the country (hence mental health teams manage people with the condition), and very limited access to investigations such as EEG, which would be routine in the UK.
Children with epilepsy are often denied education. If they do go to school they are frequently traumatised by the way they are treated – bullied, shamed and stigmatised. Many drop out. Getting employment is a big challenge, and if the seizures are not well controlled, then usually that means dismissal. On our last visit here we met a teacher and her daughter, both of whom have epilepsy. They have been unable to get work. You don’t have to look far to begin to understand some of the causes of poverty in a country like Uganda.

Linda spent the afternoon helping our students to understand psychological ways of working with serious mental conditions like psychosis.
Workshop Celebration Dinner and an important announcement
Our lovely nuns had prepared a special celebration dinner which as always included several different carbohydrates, and on this occasion, several proteins as well, including goat, chicken, and beef. In our experience goat can sometimes be a bit chewy, but this was very well cooked. Avril had never had goat before but she really enjoyed it.

As the meal ended, Hugh (Chair of Jamie’s Fund) gave a moving resume of all that has been achieved in partnership over the last 10 years:
Working with 25 hospitals across Uganda
Running professional development workshops in mental health
Training of seven new Psych Clinical Officers who will lead services for the future at their facilities
Over 1000 general health workers trained in mental health knowledge and skills
Over 1000 Community Leaders sensitized on mental health
Provision of new equipment – motor cycles, lap tops etc
He then made the difficult announcement that JF would be closing at the end of the year. As Hugh explained, we have not reached the decision easily. We promised that although the Fund would finish, our relationships would not. The trustees are all getting older as JF has been running for 10 years and the next generation of leaders has unfortunately not emerged. (Those of you on the JF supporters mailing list will receive the letter we have prepared in the near future, but we felt it was important to tell those who are most affected here in Uganda first.)
Ewan and I are certainly likely to return from time to time to see our friends.
Our wonderful young people received the news quietly but with great grace and appreciation. They promised that they would continue the great work they had been doing, and would indeed continue community mental health service development.
