#CaseOfTheWeek: A 46-year-old male was brought to the casualty of Nanavati Super Speciality Hospital. He complained of pain and deformity of his right hip with an inability to bear weight. X-rays & CT scan confirmed the clinical diagnosis of a hip dislocation and an associated fracture of the femoral head. The patient was taken up the next day for fixation of the fracture. The safe surgical dislocation approach as described by Ganz was used. Patient was discharged on the fifth day & gradually progressed to full weight bearing by three months. Many theories have been given for possible cause of hip dislocation like intrauterine position of baby, less amount of amniotic fluid in the mother’s womb.
Risk factors like first baby, female child, breech position of baby, family history of same disorder should be watch carefully.

Treatment of Hip dislocation
Goal: Reduce in <6 hours to decrease the risk of avascular necrosis
Reduction:
conscious sedation (need lots for muscle relaxation!)
Allis Maneuver
Many other techniques
After Reduction:
check leg length, rotation
repeat Xray
repeat neurovascular exam
(if attempt unsuccessful, get CT to see if there are any bony fragments getting in the way) 
immobilize legs in slight abduction 
Patients likely need to be admitted even after successful reduction.
Indications for surgery:
associated fracture
associated sciatic injury
unable to reduce or keep reduction
delayed presentation of dislocation

Hip Dislocations - Physical Exam
high amount of force needed to dislocate hip, so look for other injuries
anterior dislocation: hip and knee flexed / hip abducted and externally rotated
posterior dislocation / hip adducted and internally rotated (may be able to palpate femoral head in buttock region) 
neurovascular exam before and after reduction
vascular: dp and pt pulses
sciatic nerve: ankle dorsiflexion (deep peroneal nerve branch innervating tibialis anterior), ankle plantar flexion (tibial branch) + sensation over the posterior leg. 
femoral nerve - sensation over thigh, quad muscles
