What Husbands Can Do When Their Wives Have Postpartum Depression

When your wife has postpartum depression (PPD), which affects about 1 in 7 women after childbirth, you might feel confused, annoyed, scared, sad, worried or any combination of these. My husband certainly did. You might be thinking, "Why can't she just be happy? What's wrong with her? Aren't new moms supposed to be happy now that the baby's finally here? What's going on?"

Remember that PPD is a biochemical disorder which is no one's fault - not yours or hers. Although you can't fix it like you can a broken cabinet or leaky faucet, it's your job to support her as she recovers. Warning signs of PPD include anxiety, lack of energy, frequent crying, inability to sleep at night even when the baby's sleeping, low self-esteem, guilt feelings, appetite problems, irritability or anger, overwhelmed feelings, forgetfulness, decreased sex drive, and hopelessness. The normal Baby Blues should be gone by two weeks postpartum, so if she's still feeling weepy, she needs help. Or, if the symptoms are more severe than the mild Baby Blues even during the first two weeks, don't wait - get her help right away. You or she should call a healthcare practitioner you trust and ask for a referral to a therapist who specializes in postpartum depression.

Here are some pointers that will help you to help her and your relationship: (Excerpts from Beyond the Blues: A Guide to Understanding and Treating Prenatal and Postpartum Depression by Bennett and Indman)

Just being there with her is doing a great deal.

Letting her know you support her is often all she'll need. Ask her what words she needs to hear for reassurance, and say those words to her often. Things like, "We'll get through this. I'm here for you. I love you very much. You're a great mom. The baby loves you. You'll get yourself back. The PPD is temporary. I'm sorry you're suffering - that must feel awful. This isn't your fault."

Share at-home responsibilities.

Even a non-depressed new mom can't realistically be expected to cook dinner and clean house. She may be guilt-tripping herself about not measuring up to her own expectations and worrying that you'll also be disappointed with her. Remind her that parenting your child(ren) and taking care of your home is also your job, not just hers. Your relationship will emerge from this crisis stronger than ever.

Let her sleep at night.

She needs at least 5 hour of uninterrupted sleep per night to receive a full sleep cycle and restore her biorhythms (Chapter 11 of Postpartum Depression For Dummies* explains in detail how splitting the night can work even if she's breastfeeding or you need to leave the house early for work.) If you want your wife back quicker, be on duty for this time without disturbing her. Many dads have expressed how much closer they are to their children because of nighttime caretaking. If you can't be up at night taking care of your baby, hire someone who can take your place. A temporary baby nurse will be worth her weight in gold.

Get the support you need so you can be there for her.

Often a husband becomes depressed during or after his wife's depression. You can help protect yourself by getting your own support from friends, family, or professionals. Regular exercise or other stress-relieving activity is important, so you can remain the solid support for your wife. Provide a stand-in support person for her while you're gone. o Don't take it personally. Irritability is common with PPD. Don't allow yourself to become a verbal punching bag. It's not healthy for anyone concerned. She feels guilty after saying hurtful things to you and it's not good for her. If you feel you didn't deserve to be snapped at, calmly explain that to her. Back her up in her decision- making.

If your wife needs to see various practitioners, take medication, join a PPD support group, stop breastfeeding, or whatever else, she needs to know you're behind her 100 percent. You can certainly participate in the decision-making process, but the decisions themselves are ultimately hers. It can be helpful for you to accompany her to a therapy or doctor's appointment so you can ask any questions you may have regarding her treatment. As a therapist, I find the partner's attendance useful and I encourage it at least once. My client is always relieved to know that her husband is getting support and now understands more about her situation and the illness. o Don't mention how much her care costs. She's already feeling guilty about what she's costing the family, both emotionally and financially. Without your wife's mental health postpartum, nothing else matters. During PPD recovery, couples may use up savings and take out loans - consider it an investment in launching your new family in a healthy way. Be open to doing (and spending) whatever it takes to get her the right, specialized help, not just whoever is covered by the insurance plan.

Practice the work/life balance.

You've probably read your employee handbook about your company's work/life balance program. Now's the time to make it work for you. Tell your manager what's going on at home, that you need to leave work every evening on time, and that you can't take expended business trips for the foreseeable future. You may see this practice as career suicide, but it isn't. Many of my clients' husbands have taken parental leave, and have made the effort to be at home on time every night during this difficult period. Federal law provides husbands job-protected time off from work following the birth of a baby or to care for a seriously ill spouse. If you're a domestic partner, it depends on the state in which you live whether or not you'll be covered. If necessary, go ahead and move off the corporate fast track to help your partner recover. Your physical presence to her is more important than the next promotion, and years from now, when you look back on your life, you'll never regret having chosen family over work. I hear over and over from my clients that they don't care about the big house (with the big mortgage). They just want their husbands at home. So, if you're thinking that it's for her and your kids that you're working long hours, traveling, and so forth, you may want to ask her what she thinks - you many be surprised.

Maintain intimacy.

As you and your wife walk the road to recovery, it's important to maintain intimacy, even if it's (for now) void of any sexual activity. You may be rolling your eyes with the thought of "just cuddling." After all, what's the point of cuddling if it doesn't lead to anything? But for her, just being close to you and being held by you is comforting and healing. She may also have some physical healing to do following the birth process. Remember not to take her lack of interest in sex personally. This isn't a rejection of you - it's mainly about hormones, brain chemicals, and life changes. If you're the one returning from work at the end of the day, make sure you greet your wife first, before you greet any other member of the family (including the furry, four-legged ones). The relationship with her is the most important one and without it, no other little person would be there (see Chapter 15 for other sex and intimacy issues). Refer to the first bullet for ideas of what to say to your wife that will truly help her.

There are also some clear no-no's to avoid. Here are a few: DO NOT say:

"Think about everything you have to feel happy about." She already knows everything she has to feel happy about. One of the reasons she feels so guilty is that she's depressed despite these things.

"Just relax." This suggestion usually produces the opposite effect! She's already frustrated at not being able to relax in spite of all the coping mechanisms that have worked in the past. Anxiety produces hormones that can cause physical reactions such as increased heart rate, shakiness, and muscle tension. This is not something she can just will away.

"Snap out of it." If she could, she would have already. She wouldn't wish this on anyone. She can't snap out of PPD any easier than she can snap out of the flu. Be patient, non-judgmental, and upbeat. With the right kind of professional help along with your consistent and loving support, your wife will recover and your marriage will likely be stronger than ever.

Colombia grants citizenship to 24,000 children born to Venezuelan mothers

BOGOTA — Colombia granted citizenship Monday to more than 24,000 children born to Venezuelan migrants on its territory since 2015, as well as to all those who will be born in the next two years, in an effort to address a growing problem in one of the world’s largest refu­gee crises: undocumented babies.

“Today we can say, amid difficulties, that the way of xenophobia is not the right way,” President Iván Duque told an assembly of U.N. officials, representatives of other nongovernmental organizations and Colombian officials at the presidential palace. “In defense of brotherhood, we will attend to this situation.”

Most nations in the Americas confer citizenship on babies born on their territory automatically, but Colombia requires at least one parent to have legal residence. Duque’s decree loosens that standard, the latest move aimed at accommodating the continuing rush of refugees from neighboring Venezuela into this country.

More than 4 million have fled the hyperinflation, blackouts, and shortages of water, food and medicine of Venezuela under President Nicolás Maduro, according to the U.N. refu­gee agency. Nearly 1.5 million have ended up in Colombia.

Officials here say Duque’s decree will protect a generation of children.

“The Colombian government has been firm in its commitment to rise to the height of ethical responsibility and offer help to the Venezuelans who feel forced to leave their country,” Felipe Muñoz, appointed by Duque to manage the Venezuelan border, wrote in response to questions from The Washington Post. “Let’s remember that decades ago it was they who received us Colombians in their country, when the situation was inverse and it was us fleeing difficult local conditions.”

Poverty and conflict have displaced more people in the world today than at any time in human history. Some countries, including the United States, have responded by tightening borders and immigration policies. Amal de Chickera, co-director of the Institute on Statelessness and Inclusion in London, praised Colombia for taking the opposite approach.

“In the current global climate, to see Colombia actually going the other way and taking a decision to meet its obligations under international law, it’s incredibly heartening,” he said.

Nine-month-old Rossani Tua is now one of Colombia’s newest citizens.

Her parents, Nixon, 35, and Rossana, 33, traveled 10 hours by bus from their home in the northwestern state of Lara, Venezuela, in late 2017. Then they walked and hitchhiked to Bogota, the capital. They slept in a park for a time, then found shelter with a good Samaritan before eventually settling in a brick shack in La Magdalena, a bustling slum where people survive by collecting, sorting, cleaning and recycling the city’s garbage.

Rossana gave birth to Rossani in November. If she’d had a Colombian visa, the child would have been a citizen at birth. But because she has no nationality, Rossani’s parents have been unable to obtain the documentation, such as a national ID card or passport, that would enable her to access public services or travel.

“We have her practically in limbo,” said Rossana, holding the baby in the door of their small home. “She’s not Venezuelan or Colombian. She doesn’t have a right to anything. We don’t know what to do.

Miriam Rivera is director of the Karol Wojtyla Foundation, an organization in Bogota that provides legal guidance to immigrants.

Not only will the decree benefit the children, she said, but it also will provide a path to citizenship for immigrant parents, who under Colombian law can earn nationality through their children.

Colombian Foreign Minister Carlos Holmes Trujillo said the measure will be reviewed in two years and could be prolonged.

Trujillo blamed the problem on “insuperable obstacles” created by the Venezuelan consular service to register the children for Venezuelan citizenship.

“Those children have a right to Venezuelan nationality,” he said. “But it’s been practically impossible.”

It’s the latest effort by the government here to accommodate the Venezuelan influx.

The country has loosened ID requirements for Venezuelans to enter, offered special residence and work permits to migrants, and opened public schools to their children.

The special relationship between the two countries goes back two centuries, leaders here often recount, when Simón Bolívar led a ragtag army from Venezuela to liberate Bogota from Spanish rule. For a time, they were both part of a single nation — Greater Colombia — that also included present-day Ecuador, Panama and parts of Brazil, Guyana and Peru.

More recently, tensions between a succession of center-right governments in Bogota and the socialist governments of Maduro and Hugo Chávez have been strained. But Duque still speaks of “our Venezuelan brothers and sisters” when addressing migration.

“Today we are giving a light of hope to thousands of kids and to thousands of families,” Duque said Monday. “We can show that in these moments of difficulty, fraternity prevails.”

Colombia’s policies on Venezuelan migrants have increasingly set it apart. Peru, with nearly 800,000 Venezuelans, and Chile and Ecuador, with nearly 300,000 each, have tightened restrictions on Venezuelans crossing their borders — although all grant birthright citizenship.

Humanitarian groups have applauded Colombia’s approach.

Marianne Menjivar, Colombia director for the International Rescue Committee, called the citizenship decree “a testament to Colombia’s pro-refugee stance, one which is increasingly under threat in the region and which deserves international support.”

She said other South American countries should prioritize registration and legal status for Venezuelan refugees, followed by programs for social and economic integration.

The Organization of American States last month predicted that the Venezuelan exodus would surpass the 6.7 million people who fled the Syrian civil war by 2020 to become the world’s largest.

The organization said international funding for the Venezuelan crisis amounts to about $100 per person displaced. For the Syrians, the figure peaked at $5,000.

That has left Colombia mostly on its own to foot the bill for the humanitarian crisis spilling over its border.

Duque called on other nations to offer more support.

“This requires that the world does not remain indifferent,” he said.

Children have proved costly, drawing state funding for birth services and space in public schools. Thousands of Venezuelan mothers have migrated to Colombia specifically to give birth.

For Osmari Cemplin, 18, delivering in her Venezuelan hometown of Maracaibo would have meant buying medical supplies she couldn’t afford, so her family pooled their money to help her move in with a cousin in Bogota, where she gave birth last year.

Her son, Yonaiker, has no citizenship. When he was 2 months old, he came down with a fever, and some months later, his face and eyes swelled up. But undocumented immigrants are eligible to receive only emergency treatment free, and the family couldn’t afford anything more.

“They said I’d have to bring her in almost dead to tend to her,” she said. “We all went home and cried.”

Maikely Rojas, 18, delivered Matias in Bogota two years ago. But she and her husband struggle to feed themselves, and their son wears clothing for a 6-month-old.

Rojas said that she has asked for nutritional assistance and pediatric care from state hospitals, but her son doesn’t qualify.

As a Colombian citizen, he will.

A Daily Pill to Treat Covid Could Be Just Months Away, Scientists Say

Within a day of testing positive for covid-19 in June, Miranda Kelly was sick enough to be scared. At 44, with diabetes and high blood pressure, Kelly, a certified nursing assistant, was having trouble breathing, symptoms serious enough to send her to the emergency room.

When her husband, Joe, 46, fell ill with the virus, too, she really got worried, especially about their five teenagers at home: “I thought, ‘I hope to God we don’t wind up on ventilators. We have children. Who’s going to raise these kids?”

But the Kellys, who live in Seattle, had agreed just after their diagnoses to join a clinical trial at the nearby Fred Hutch cancer research center that’s part of an international effort to test an antiviral treatment that could halt covid early in its course.

By the next day, the couple were taking four pills, twice a day. Though they weren’t told whether they had received an active medication or placebo, within a week, they said, their symptoms were better. Within two weeks, they had recovered.

“I don’t know if we got the treatment, but I kind of feel like we did,” Miranda Kelly said. “To have all these underlying conditions, I felt like the recovery was very quick.”

The Kellys have a role in developing what could be the world’s next chance to thwart covid: a short-term regimen of daily pills that can fight the virus early after diagnosis and conceivably prevent symptoms from developing after exposure.

“Oral antivirals have the potential to not only curtail the duration of one’s covid-19 syndrome, but also have the potential to limit transmission to people in your household if you are sick,” said Timothy Sheahan, a virologist at the University of North Carolina-Chapel Hill who has helped pioneer these therapies.

Antivirals are already essential treatments for other viral infections, including hepatitis C and HIV. One of the best known is Tamiflu, the widely prescribed pill that can shorten the duration of influenza and reduce the risk of hospitalization if given quickly.

The medications, developed to treat and prevent viral infections in people and animals, work differently depending on the type. But they can be engineered to boost the immune system to fight infection, block receptors so viruses can’t enter healthy cells, or lower the amount of active virus in the body.

At least three promising antivirals for covid are being tested in clinical trials, with results expected as soon as late fall or winter, said Carl Dieffenbach, director of the Division of AIDS at the National Institute of Allergy and Infectious Diseases, who is overseeing antiviral development.

“I think that we will have answers as to what these pills are capable of within the next several months,” Dieffenbach said.

The top contender is a medication from Merck & Co. and Ridgeback Biotherapeutics called molnupiravir, Dieffenbach said. This is the product being tested in the Kellys’ Seattle trial. Two others include a candidate from Pfizer, known as PF-07321332, and AT-527, an antiviral produced by Roche and Atea Pharmaceuticals.

They work by interfering with the virus’s ability to replicate in human cells. In the case of molnupiravir, the enzyme that copies the viral genetic material is forced to make so many mistakes that the virus can’t reproduce. That, in turn, reduces the patient’s viral load, shortening infection time and preventing the kind of dangerous immune response that can cause serious illness or death.

So far, only one antiviral drug, remdesivir, has been approved to treat covid. But it is given intravenously to patients ill enough to be hospitalized, and is not intended for early, widespread use. By contrast, the top contenders under study can be packaged as pills.

Sheahan, who also performed preclinical work on remdesivir, led an early study in mice that showed that molnupiravir could prevent early disease caused by SARS-CoV-2, the virus that causes covid. The formula was discovered at Emory University and later acquired by Ridgeback and Merck.

Clinical trials have followed, including an early trial of 202 participants last spring that showed that molnupiravir rapidly reduced the levels of infectious virus. Merck chief executive Robert Davis said this month that the company expects data from its larger phase 3 trials in the coming weeks, with the potential to seek emergency use authorization from the Food and Drug Administration “before year-end.”

Pfizer launched a combined phase 2 and 3 trial of its product Sept. 1, and Atea officials said they expect results from phase 2 and phase 3 trials later this year.

If the results are positive and emergency use is granted for any product, Dieffenbach said, “distribution could begin quickly.”

That would mean millions of Americans soon could have access to a daily orally administered medication, ideally a single pill, that could be taken for five to 10 days at the first confirmation of covid infection.

“When we get there, that’s the idea,” said Dr. Daniel Griffin, an infectious diseases and immunology expert at Columbia University. “To have this all around the country, so that people get it the same day they get diagnosed.”

Once sidelined for lack of interest, oral antivirals to treat coronavirus infections are now a subject of fierce competition and funding. In June, the Biden administration announced it had agreed to obtain about 1.7 million treatment courses of Merck’s molnupiravir, at a cost of $1.2 billion, if the product receives emergency authorization or full approval. The same month, the administration said it would invest $3.2 billion in the Antiviral Program for Pandemics, which aims to develop antivirals for the covid crisis and beyond, Dieffenbach said.

The pandemic kick-started a long-neglected effort to develop potent antiviral treatments for coronaviruses, said Sheahan. Though the original SARS virus in 2003 gave scientists a scare — followed by Middle East respiratory syndrome, or MERS, in 2012 — research efforts slowed when those outbreaks did not persist.

“The commercial drive to develop any products just went down the tubes,” said Sheahan.

Widely available antiviral drugs would join the monoclonal antibody therapies already used to treat and prevent serious illness and hospitalizations caused by covid. The lab-produced monoclonal antibodies, which mimic the body’s natural response to infection, were easier to develop but must be given primarily through intravenous infusions.

The federal government is covering the cost of most monoclonal products at $2,000 a dose. It’s still too early to know how the price of antivirals might compare.

Like the monoclonal antibodies, antiviral pills would be no substitute for vaccination, said Griffin. They would be another tool to fight covid. “It’s nice to have another option,” he said.

One challenge in developing antiviral drugs quickly has been recruiting enough participants for the clinical trials, each of which needs to enroll many hundreds of people, said Dr. Elizabeth Duke, a Fred Hutch research associate overseeing its molnupiravir trial.

Participants must be unvaccinated and enrolled in the trial within five days of a positive covid test. Any given day, interns make 100 calls to newly covid-positive people in the Seattle area — and most say no.

“Just generally speaking, there’s a lot of mistrust about the scientific process,” Duke said. “And some of the people are saying kind of nasty things to the interns.”

If the antiviral pills prove effective, the next challenge will be ramping up a distribution system that can rush them to people as soon as they test positive. Griffin said it will take something akin to the program set up last year by UnitedHealthcare, which sped Tamiflu kits to 200,000 at-risk patients enrolled in the insurer’s Medicare Advantage plans.

Merck officials predicted the company could produce more than 10 million courses of therapy by the end of the year. Atea and Pfizer have not released similar estimates.

Even more promising? Studies evaluating whether antivirals can prevent infection after exposure.

“Think about that,” said Duke, who is also overseeing a prophylactic trial. “You could give it to everyone in a household, or everyone in a school. Then we’re talking about a return to, maybe, normal life.”

Every politician is a liar, every businessman a crook. The champion athlete is on steroids, the sunny comedian a secret drinker, and the sobbing husband at the press conference always, always the one who did it.The celebrated US journalist Janet Malcolm once characterised her trade as ‘morally indefensible’ and while I wouldn’t go quite that far, spend long enough hanging around courts, committee rooms or crime scenes and you begin to harden your heart with misanthropic bravado and a cold, black sense of humour.It’s not the done thing to admit this but last week, for the first time in a long time, a story got to me. It was the tragic tale of Gerard Brown, the Glasgow man who died after waiting 40 hours for an ambulance to arrive. Given Mr Brown was 65, had survived cancer, and was afflicted by other health problems, it would have been appalling if he had had to wait 40 minutes for medical assistance. Then I read an interview with his son Dylan, who relayed what the family GP told him: ‘Dylan, I can assure you that if they'd got to him your dad would still be here.’That broke me. When you write about politics day in and day out it is easy to think of it as one big football tournament. Is the yellow team or the blue team at the top of the league this week? Did Labour’s star striker break through the Scottish Government’s defences and will the Lib Dems’ new gaffer switch up their tactics?

The sad, needless way Gerard Brown left this world, and similar stories of vulnerable patients caught up in the ambulance crisis, are a reminder we shouldn’t need that politics — the bits of it that actually matter — is about people and whether or not they are being treated right.Nicola Sturgeon would no doubt disapprove of the phrase ‘ambulance crisis’, given how she danced around the term at First Minister’s Questions. That is unfortunately a hallmark of her premiership: the naming of a problem is more important than the fixing of it.The First Minister is one of those people who puts a great deal of faith in language and perhaps as someone who earns his crust with a pen I shouldn’t object too vigorously. But this mindset, which took grip in the 1990s and reflects fashionable theories inculcated at universities in the two or three decades beforehand, has been ruinous to the notion that you go into politics to do things rather than talk about them.

Postmodern politics, a tradition to which Sturgeon very much belongs, prizes rhetoric above everything, even reality. This is what lies behind her approach to government, with its relentless emphasis on acknowledging issues, defining challenges, expressing sympathies — but never getting round to attacking the underlying causes.If there is a hashtag to be tweeted or a lapel pin to be worn, Sturgeon is second to none in capturing the mood and striking an appropriate tone. If it is the material conditions of the matter that you want changed, she is not your woman.The ambulance crisis — and it is a crisis — is an object lesson in the limits of the Sturgeon school of politics. For her five years as health secretary and seven as First Minister have been a tornado of words about the NHS — and a limp wind when it comes to actions.This present crisis, for example, can’t be the fault of single-crewing of ambulances. That practice has been eliminated. We know this because Nicola Sturgeon told us so.In June 2008, she instructed the Scottish Ambulance Service ‘to achieve the elimination of single manning’. By January 2012, she was telling the Scottish Parliament that ‘single crewing… has been substantially eliminated’, ‘single crewing has, as of late 2010, effectively been eliminated’ and that, ‘[t]hankfully, single crewing is no longer routine in the Ambulance Service’. Between 2013/14 and 2019/20, there were 18,999 shifts with only one crew member in an ambulance.

Nor can the problem lie with excessive waiting times for hospital treatment. Those, too, have been abolished and we can say that with confidence because, once again, Nicola Sturgeon told us so.The 2007 manifesto that brought the SNP to power promised voters ‘you will have your own personal and legally guaranteed waiting time’. Following her party’s victory, the new health secretary told Holyrood: ‘[T]he point of legally binding guarantees is to ensure that patients’ rights are meaningful, that guarantees are not breached, and that patients get the treatment they deserve’. Sturgeon enshrined her Treatment Time Guarantee in the Patient Rights (Scotland) Act 2011, which, per her government’s statutory guidance, meant health boards would now ‘be required in law to ensure that patients start their treatment within 12 weeks’. Since it came into force, the legally binding waiting time has been breached more than 300,000 times.

Ministers routinely invoke the pandemic to explain away their failings and because most people have lives and jobs and no particular affinity for the NHS statistical archives, they shrug and figure that this sounds plausible enough. Not only is it not plausible, one perverse facet of Covid-19 is that it has actually helped ministers meet some targets.Take the A&E waiting time standard, which says that 95 per cent of patients should receive emergency care within four hours of arriving at hospital. That target was missed every single month from September 2017 to April 2020, then, suddenly, it was met in May, June and July of that year.

What changed? Did the Scottish Government draft in more doctors and nurses or invest in additional capacity in emergency wards? No, the words ‘coronavirus’ and ‘lockdown’ came into our daily vocabulary and the number of patients presenting at A&E dropped off a cliff. America Has Been Through An Opioid Crisis Before

According to most accounts, America’s opioid crisis dates back to the late 1990s. That’s when predatory Big Pharma companies like Purdue Pharma lied to doctors, falsely claiming that opioid painkillers like Oxycontin and Opana were safe and non-addictive. Prescriptions for addictive opioids skyrocketed during the 2000s, sparking an epidemic of drug addiction that quickly spiraled out of control. 

But while this narrative might be convenient, it isn’t the full story, because, devastating though it is, this is not America’s first opioid crisis. Its roots go far deeper. America’s earliest opioid crisis occurred 150 years ago, in the aftermath of the American Civil War, when an epidemic of prescription opioid addiction killed tens of thousands of Army veterans.  

Tragically, America’s vintage opioid addiction epidemic shares surprising and disturbing parallels with today’s opioid crisis, calling into question the familiar stories we tell ourselves about how the opioid crisis began and what it will take to solve the epidemic. 

Doctors have been prescribing opiate painkillers since the Bronze Age, and by the time the Civil War broke out in 1861, opiates were the most commonly prescribed medicines in America. 

During and after America’s bloodiest war, in which 750,000 people were killed with millions of survivors left in chronic pain, Army doctors saw opiate medicines as the most “indispensable drug[s] on the battlefield—important to the surgeon, as gunpowder to the ordinance.” Syringe-wielding surgeons gave morphine injections—cutting-edge technology back then—to dull the unbearable pain stemming from gruesome gunshot wounds and last-ditch, hacksaw amputations.

Doctors doled out opium pills and morphine shots for practically any ailment. Off the battlefield, opium was a surprisingly effective treatment for diarrheal diseases like dysentery, which often hit troops living in squalid conditions.

Naturally, thousands of Civil War veterans got addicted to opiates. Like today, most addiction started with prescriptions. Military doctors introduced soldiers to opium and morphine during the war, and sick and injured survivors simply kept on taking the medicines after leaving the Army. Like today, doctors didn’t prescribe opiate painkillers equitably to white and Black soldiers. Some racist doctors believed their Black patients couldn’t feel pain and didn’t need painkillers, while other surgeons took for themselves the painkillers that were meant for injured black soldiers. Consequently, most Civil War veterans who got addicted were white, a product of the shoddy medical care provided for Black troops.

There were no regulations on narcotics during the Civil War era, so the drugs were cheap and easy to get. Opium pills, morphine, and hypodermic needle kits were sold in pharmacies over the counter. You could even order them by mail from Sears. Unsurprisingly, addicted veterans were everywhere in post-Civil War America, and the opioid crisis was covered widely in the press.

But those who got addicted quickly turned in the public eye from respectable war veterans to something lesser, being derisively labeled “opium eaters,” “morphine habitués,” and “opiomaniacs” by doctors and the media. Having fought a war over the fate of slavery, Civil War veterans referred to themselves as “opium slaves.” One Union veteran, Joseph C. Darrow of Michigan, wrote that he was prescribed opium after getting sick during the war. In no time, he found himself “a slave to the Habit of using Morphine, with not a ray of hope of ever being emancipated.” 

The outlook for addicted Civil War veterans was bleak. Thousands of ex-soldiers died from overdoses between the war’s end and the early 1900s. Like today, when addicted veterans overdosed, their families and communities were devastated. One teenage soldier, Frank Clewell, left his family in North Carolina to fight in the Civil War. He ended up sitting out much of the war in a POW camp, where he apparently got sick and started taking morphine. Clewell got released when the war ended, but before he could make it home, he died of a midnight drug overdose, alone in a chilly St. Louis hotel room. Clewell’s friends were shocked to discover his corpse the next day, and the death made national headlines.

Opium and morphine abuse were so common among Civil War veterans that addiction rippled through all facets of postwar American medicine and society. Addiction medicine became big business for the first time in American history, as veterans were desperate for addiction treatments. Between the 1860s and 1890s, hundreds of medical “inebriety” clinics catering to opioid addicts opened around the country offering addiction care. These were America’s earliest drug rehabs. 

But like today’s rehabs, these early clinics were expensive, unreliable, and out of reach for most Americans. The medical care provided at the early rehabs wasn’t very effective, either. Today, the gold standard for addiction therapy, medication-assisted treatment, relies on medicines such as methadone and buprenorphine. But this treatment wasn’t developed until the 1960s, a century after the Civil War. Instead, the methods used to treat Civil War veterans in early rehabs were primitive, usually involving slow, agonizing detox under doctors’ supervision. The idea was to gradually taper down the patient’s usual dose of morphine or opium over a few weeks. Once patients detoxed, they could go back home and get on with their lives.

This method sounds doable  on paper, but it only worked some of the time. Opioid addiction causes powerful cravings that can potentially strike any time, even years after detox and into recovery. These cravings were so powerful that, without methadone or buprenorphine, relapses were common.

When veterans relapsed and started taking opioids again, they were shamed  and denounced in the media. Any drug addict who relapsed, according to one newspaper, was a sinner who simply “did not wish to be cured.” 

This unsympathetic mentality influenced the government’s approach to the postwar addiction crisis. Grover Cleveland, president during the 1880s and ‘90s, thought addicted veterans—even those who had been war heroes—were simply bad people, and he went out of his way to punish them and their families. 

When a Union veteran, Clinton Smith, fatally overdosed in 1888, his widow Eliza applied for a military pension, like most women in the same situation. The government paid out millions of dollars in pensions every year to the wives of dead Civil War soldiers. But Cleveland thought those who were addicted and their widows didn’t deserve these benefits. He refused to grant the pension to Eliza because her dead husband “was in the habit of taking large doses of morphine.” Cleveland’s condescending , punitive mindset was the norm in the Civil War era, and, according to contemporary media coverage, most Americans agreed that addicted drug users did not deserve sympathy or government help.

The victim-blaming that emerged out of the Civil War era has influenced U.S. drug policy for more than a century. In 2017, an Ohio sheriff made headlines for refusing to let his deputies carry the overdose reversal medicine Narcan. “I’m not the one that decides if people live or die. They decide that when they stick that needle in their arm,” the sheriff explained, echoing President Cleveland’s harsh words from the 1880s. Anti-addiction rhetoric born in the Civil War era is still with us today, as strong as ever. 

The stigma surrounding opiate use meant that addicted Civil War veterans often landed in jails and even mental institutions. Brutal prison wardens had no sympathy, abusing prisoners with drug addictions by forcing them into cold turkey withdrawal. One veteran, Cyrus Harmon, got addicted to morphine to deaden the pain of a half-healed Civil War gunshot injury. Harmon was arrested in Ohio in 1901 and sent to the state penitentiary, where the warden confiscated his morphine. When Harmon entered detox, the warden denied medical assistance, leaving the elderly veteran writhing in agony on the floor of his cell until he seized to death a few hours later. Harmon’s old Army buddies were livid, and they protested his death at the state capital. Today, over a century later, some American jails still refuse to provide medication-assisted therapy to addicted inmates.  

Considering their bleak prospects, most addicted Civil War veterans were desperate to stop using drugs. Like today, however, reputable medical care in rehabs was out of reach for most. So they tried anything, leaving the door open for predatory medical hucksters who invented bogus miracle cures for addiction. These so-called opium “antidotes,” which promised to cure addiction in the privacy of customers’ homes, were pervasive. Newspapers and magazines from the 1870s and ‘80s were loaded with advertisements for these antidotes. People suspected of being “addicts” even received unsolicited postcards—the Gilded Age version of spam emails—hawking these potions.

Predictably, these miracle cures were too good to be true. Desperate consumers didn’t realize it, but Civil War-era addiction antidotes actually contained opiates. Antidote peddlers promised that their “cures” relied on non-opiate active ingredients, but they lied. So instead of curing addiction, the fake cures actually prolonged it, unbeknown to the customers who shelled out thousands of dollars on the expensive products, only to be left in poverty and still addicted. In an era before the FDA or robust consumer protection laws, there was no check on medical scam artists, who made fortunes profiting on Civil War veterans’ addiction. Today, a new wave of miracle cures falsely promise to cure opioid addiction, evoking Civil War-era opium antidote scams. Dangerous, expensive detox schemes, like the Church of Scientology’s Narconon and Purif and unproven plant-based substances like ibogaine and kratom claim to cure opioid addiction. But customers often end up even worse off than before, just like Civil War veterans who shelled out for opium antidotes.

Doctors never managed to solve the Civil War’s opioid crisis. The tragedy only ended when addicted veterans died off slowly over time. Most veterans never managed to escape addiction, and swallowing opium pills and injecting morphine year-after-year took a major toll on veterans’ bodies and health. The injections left men emaciated, weak, and prone to life-threatening infections. “Morphinists” were notorious for sporting infected skin abscesses caused by dirty needles. (This was before the “germ theory of disease” was widely known, so most people didn’t recognize the need to use clean needles). Decades before the development of modern antibiotics in the 1940s, these infections could be fatal. Eventually, enough addicted Civil War veterans got sick and died or fatally overdosed that ex-soldiers faded from view.

During the early 1900s, Civil War veterans were replaced by other cohorts of drug-users. The pharmaceutical company Bayer introduced heroin around 1900, marketing  the drug as a cough medicine. Heroin  was more than double the strength of morphine, so it quickly became popular among younger Americans who preferred  heroin over less-potent opiates . Soon heroin users began catching the attention of cops in big cities like New York and Philadelphia. About the same time, xenophobes cited opium smoking among some Chinese immigrants—in reality a small problem, compared to medicinal opiate abuse among whites—to justify racist anti-immigration laws, sparking  a news panic over the issue. The racist belief that Chinese opium dens snared white people into addiction also inspired some of the nation’s first drug laws in the 1870s and ‘80s, which were aimed more at upholding racial segregation than actually stamping out opiate use. After the U.S. invaded and colonized the Philippines in 1898, it banned the opium trade, doubling down on racist drug laws and keeping the issue in the news cycle. Eventually, heroin users and opium smokers replaced aging Civil War veterans as the stereotypical American drug addict. By the 1910s, a national panic over heroin abuse and opium smoking sparked the U.S.’s first heavy-handed drug laws. By that point, people began to forget about America’s Civil War-era opioid crisis. 

But forgetting about America’s first opioid crisis helped spark today’s epidemic. Just like after the Civil War, today’s opioid crisis stemmed in part from doctors’ careless overprescribing. Similarly, the lack of effective regulations on the medical marketplace allowed for rampant fraud in addiction medicine, both in the late 1800s and today. In Civil War America, the stigmatization of drug addiction, pervasive victim-blaming, and the jailing of addicted people, proved totally ineffective solutions. These responses only made the post-Civil War addiction epidemic worse.

If American doctors and drug policy makers had taken a hard look at Civil War America’s opioid crisis, they might have learned the lessons this dark history has to offer. Maybe the U.S. could have avoided the tragedy of today’s opioid crisis. Instead, Americans forgot about the U.S.’s first opioid crisis, and we ended up repeating it 150 years later.

How Rhode Island Fell to the Coronavirus PROVIDENCE, R.I. — The numbers began ticking up in September. After a quiet summer, doctors at Rhode Island Hospital began seeing one or two patients with Covid-19 on each shift — and soon three. Then four. Cases climbed steadily until early December, when Rhode Island earned the dubious distinction of having more cases and deaths per 100,000 people than any other state in the country. The case rate still puts it among the top five states. Where did this tightly knit state go wrong? Former Gov. Gina Raimondo’s “pauses” on economic activity were short-lived and partial, leaving open indoor dining, shopping malls and bowling alleys. But the shutdowns were no patchier than those in many other states. Until late summer, she was lauded for reining in the virus. Even now, few residents blame her for the bleak numbers. (Ms. Raimondo was sworn in as the secretary of commerce on Wednesday night.) Experts point instead to myriad other factors, all of which have played out elsewhere in the country but converged into a bigger crisis here. The fall chill sent people indoors, where risk from the virus is highest, and the holidays brought people together. Rhode Island is tiny — you can traverse it in 45 minutes. But crammed into that smallish area are a million people, for a population density second only to that of New Jersey. If everyone in the world is connected by six degrees of separation, Rhode Islanders seem to be connected by maybe two. Central Falls, the epicenter of Rhode Island’s epidemic, has a density of 16,000 people per square mile, almost twice that of Providence. “Just imagine, 16,000 people per square mile — I mean, that’s amazing,” said Dr. Pablo Rodriguez, a member of the government committee that guides Covid vaccine distribution in Rhode Island. “It doesn’t take much for the spark to create an outbreak.” Apart from its density, Rhode Island has a high percentage of elderly residents in nursing homes, accounting for the bulk of deaths. Packed into the state are multiple urban areas — Central Falls, Pawtucket, Providence — where language barriers, mistrust and jobs have left immigrant families in multigenerational homes particularly vulnerable. The state is also home to multiple colleges that set off chains of infection in the early fall. For months, the hospitals in Rhode Island were understaffed and overwhelmed. Doctors and nurses were trying to cope with rising caseloads, often without the protective equipment they needed, with constantly shifting guidelines and with their own resilience stretched to the limit. Dr. Megan Ranney, a researcher and public health advocate, is also an emergency room physician at Rhode Island Hospital who has witnessed the full scope of the state’s crisis firsthand. What she saw unfold over a single shift offers a window into what happened. Plowing Through It One day in late December, as the crisis reached new heights, Dr. Ranney girded for a long eight-hour shift. The sores behind her ears, where her glasses and the straps of the N95 and surgical masks dug in, still had not healed. But how could she complain, Dr. Ranney said, when her medical residents “eat, sleep, breathe Covid” five days a week? The patients had it worse, she knew. Anxious and isolated, they became even more discomfited by the masked and unrecognizable doctors and nurses rushing around them. During Dr. Ranney’s shift the prior week, she had seen a broad spectrum: elderly people on a downward spiral, otherwise healthy young Latino men, Cape Verdean immigrants with limited English comprehension. These demographics are partly what made Rhode Island particularly susceptible, said Dr. Ashish Jha, dean of the School of Public Health at Brown University in Providence: “Certainly in New England, it is the poorest state — so a lot of poverty, and a lot of multigenerational poverty.” As in most of the country, the Latino community has borne the brunt of the epidemic. In Rhode Island, Latinos have 6.7 times the risk for hospitalization and 2.5 times the risk of death, compared with white people. In the days before her shift, Dr. Ranney had been working in a part of the hospital intended to deal with non-Covid cases. But even people with other ailments, like ankle fractures, turned out to be positive for the virus, she found. “I never know from day to day how bad the surge will be,” she said. “I’ve just got to plow through it.” It turned out to be an extraordinarily busy day. “The E.R. is full, the hospital is full, the intensive care unit is full,” Dr. Ranney said. “All of our units are moving as quickly as they can, but the patients keep coming in.” Every time she took off masks during a shift, she ran the risk of contaminating herself. She had had four cups of coffee before this shift, and nothing since. The average age of the patients that night was about 70. One elderly woman who had trouble breathing could not isolate because she lived with her children and grandchildren. At any rate, she arrived at the hospital 10 days into her illness, too late for isolation to matter. Updated  March 5, 2021, 5:03 a.m. ET Rhode Island’s epidemic has been disastrous for immigrant families in multigenerational households. “How do you isolate from someone when you have one bathroom?” Dr. Ranney said. It’s a problem throughout this diverse state. When Djini Tavares, 60, became infected in July, she was prepared to spend about $120 a night at a hotel — a sum many in her Cape Verdean community cannot afford — to isolate from her vulnerable 86-year-old father. Even before the pandemic, Ms. Tavares was fastidious about hygiene, keeping mounds of wipes and cleaning supplies in the house at all times. She could not imagine where she had picked up the virus. The loss of her godmother and a friend to Covid-19 had shaken her. Cape Verdeans are a close-knit community, and not being able to mourn the dead has been painful, Ms. Tavares said: “Culturally, I think it’s causing us to hurt even more.” On her shift, Dr. Ranney encountered Covid-19 patients who had blood clots or heart problems, or who still needed oxygen weeks after their diagnosis. Many patients had been very careful — or said they had — but were infected after a family member brought the virus into the household. The story is told too often in Rhode Island. Abby Burchfield, 58, lost her mother and stepfather to Covid-19 within days of each other at an assisted living center in New Jersey in April. Devastated and afraid, she and her family stayed away from restaurants, washed their hands often, and tried to wear masks everywhere. It wasn’t enough. Ms. Burchfield’s younger daughter, Lily, 21, became infected at her college in Virginia in August and was hospitalized. Then, in late October, her husband, Jimmy, 58, caught the virus from a co-worker who was infected but did not wear a mask. Despite Ms. Burchfield’s best efforts she, too, was infected. She was hospitalized after she collapsed suddenly in the family kitchen. She recovered, but her husband, who was also admitted to the hospital, still has no taste, a limited sense of smell, and continuing fatigue. “My biggest fear right now is protecting my older daughter,” Ms. Burchfield said. Workplace exposures have especially hurt the Latino and Cape Verdean community, many of whom hold jobs that cannot be done from home. But in state surveys, it also became obvious that people still were holding get-togethers of 15 to 20 people even as the virus spread, said Dr. James McDonald, medical director of the Covid-19 unit at the Rhode Island Department of Health. Dr. Ranney said there were several such cases in the emergency room that night. “It’s frustrating to see patients come in from car crashes when they were not wearing a seatbelt, or to see patients with a firearm injury because the firearm wasn’t stored safely,” she said. “It’s like that to see folks with Covid.” The Masks ‘Were Disgusting’ Some nights in emergency medicine, the diagnoses and treatments are immediately obvious. But on this shift, Dr. Ranney said, “there was very, very little that was straightforward or smooth.” A number of patients with substance abuse problems appeared, as well as people with mental illness who had become a danger to themselves. And “we’re seeing a lot of people who are just lonely,” she said. Dr. Ranney would get a respite, but many medical residents and nurses in Rhode Island were already burning out. Some felt that hospital administrators had not protected them. Early in the pandemic, most health care workers in Rhode Island, as in other parts of the country, did not have N95 masks. The masks are single-use, but when the nurses received an N95 each, they were asked to place them in paper bags at the end of their shift and put them back on again the next day. “They stunk, they were slimy, they were disgusting. They made your face break out,” said a nurse at Rhode Island Hospital, who spoke on condition of anonymity because the hospital had instructed employees not to speak to the news media. If a strap broke, the mask would be returned with new straps stapled on. “The staples would dig into your face,” the nurse said. Many nurses got just 40 hours of sick time a year, which roughly translated to three 12-hour shifts; a fourth day out might earn a reprimand. Because of this, many nurses weren’t tested, and some came to work even when they were sick. At Eleanor Slater Hospital in Cranston, R.I., ill staff members led to an outbreak of at least 29 employees and nine patients. It’s a phenomenon seen in hospitals throughout the United States. The rules for patients don’t always accord with the science, said one nurse at Rhode Island Hospital. At first, the hospital did not allow anybody up from the E.R. until test results were back. But as the first surge ebbed, the rules became lax. Patients were sent up with pending test results, potentially exposing other patients as well as the nurses who cared for them. After treating one such patient, at least nine nurses tested positive for the virus, the nurse said. The policy at most hospitals in Rhode Island now is to have health care workers wear N95 respirators or similar reusable masks at all times, and to test anyone suspected of having Covid-19. But that does not account for patients who might be asymptomatic and who come in for other ailments. Rhode Island has adopted one unusual approach: Officials are distributing vaccines to anyone who will take them in Central Falls, regardless of age. It’s a strategy that few other jurisdictions have tried. “We decided to do that because of the horrific toll of the pandemic in those communities,” said Dr. Rodriguez, the vaccine committee member. Twenty percent of the adult residents have received at least one dose at local clinics, not including those who may have been immunized at work or elsewhere. The state’s plan to immunize those at highest risk by age and geography, he added, “will put out the fire where it is burning the most intensely.” In recent weeks, the number of cases in Rhode Island have fallen, as they have in the rest of the country. And fewer health care workers are getting sick because they have been immunized, so hospital shifts are better than they used to be, Dr. Ranney said. But cases in the state are still the third highest per capita in the country.
