Long COVID patients have similar brain activity to those with dementia, UK researchers find - Published Sept 10, 2024

Some patients with COVID see similar brain changes to people with such neurodegenerative disorders as Alzheimer’s, according to University of Kentucky researchers.
As the COVID pandemic raged across the world in 2020, researchers came together to start better understanding the new disease. At UK, Yang Jiang, a professor in the Department of Behavioral Science, led a study digging into the long-term effects of the virus.
“We’re together trying to understand how the COVID virus goes from getting into the nose and the lung, to somehow affecting the heart and the brain,” she said.
“We think there may be a long COVID, which we still don’t totally understand. It’s likely a risk factor for other neurodegenerative diseases.”
Looking at the effects of long COVID, Jiang sought help from the UK Sanders Brown Center on Aging to better understand the lingering mental effects of the virus.
“They understand some of the neuroinflammatory processes and how oxygen and blood will go through the blood brain barrier and interact with the virus, and how that alters brain functions,” she said. “So we began to sort of put two sides of evidence together.”
Chris Norris, a professor in the Department of Pharmacology and Nutritional Sciences and researcher at the Sanders Brown Center, said they were able to find brain cells, called astrocytes, in people diagnosed with COVID reacting similarly to brain cells of people with neurodecline.
“(Astrocytes) regulate blood flow to the brain, they regulate the shuttling of metabolites from the blood to the neurons, they support synaptic connections in the brain. When astrocytes become reactive and inflamed, like they do in COVID, all of those things – the metabolism, the blood flow, the synaptic communication – are adversely affected,” he said.
Those same cells also affect blood flow. Alzheimer’s patients and long COVID patients could both experience decreased blood flow in the brain, said Bob Simpol, an assistant professor in the Department of Pharmacology and Nutritional Sciences and researcher at the Sanders Brown institute.
He called COVID a “risk factor” that could contribute to long-term cognitive impairment or neurodegeneration.
People experiencing inflammation of astrocytes will see symptoms like brain fog, lapses in memory or forgetting the name of common items. Norris said these symptoms go beyond misplacing your keys, to something more serious – like forgetting you had your keys to begin with.
The research also showed that the brains of people with long COVID had similar electroencephalography (EEG) patterns to people with dementia.
Jiang said EEG’s measure “brain age” by looking at the activity of brain waves.
“Our brain is active all the time, even when you’re resting and when you sleep. EEG can capture the synchronized neural activity at the scalp. So what we observed, in COVID-19 patients, is the same pattern we see in dementia patients, which is the brain literally slows down,” Jiang said.
A COVID diagnosis does not mean you have dementia. Instead, people who have been diagnosed with COVID should have their brain function checked regularly, to catch signs of deterioration early and promote intervention.
With early intervention, 40-60% of neurodegeneration symptoms are reversible, Jiang said. With this research, the team is pushing for patients who have had a COVID diagnosis to get regular neurological check-ups.
“You can now look at brain function pretty easily and non invasively with EEG, just as easily as taking your blood pressure or listening to your lungs,” Norris said.
“After your symptomatic, it may be a good idea to have your brain activity assessed.”

A small new trial published in the journal Nature Medicine describes what would be two firsts for Parkinson's disease, if they pan out: a diagnostic test and a potential immune-based treatment that works similarly to a vaccine. The research is still early, but researchers are excited by the prospect of advances for a disease that lacks good diagnostics and treatments.
The target of both innovations is alpha synuclein, a protein that takes an abnormal form in Parkinson's patients—aggregating in their brains and destroying nerve cells involved in motor and some cognitive functions. While researchers have long known that these proteins are involved in the disease, finding ways to measure and target them has not been easy.
The (potential) Parkinson's vaccine
The Florida-based biotech company Vaxxinity developed a vaccine, or what it calls an active immune medicine, to train the immune system to attack only abnormal versions of the protein—which are improperly folded—and not the regular forms. This would essentially help people's bodies treat themselves.
“The idea is that patients should recognize their own misfolded proteins, and it is personalized because their own immune systems are doing the work,” says Dr. Mark Frasier, chief scientific officer at the Michael J. Fox Foundation for Parkinson’s Research, which funded the testing part of the study.
The Parkinson's test
The new diagnostic test for Parkinson’s, which was developed by researchers at University of Texas and Vaxxinity, uses samples of cerebrospinal fluid to measure a person's levels of abnormal alpha synuclein. If the U.S. Food and Drug Administration (FDA) grants it full approval, it will become the first test for diagnosing Parkinson's. (The FDA classified it as a breakthrough device in 2019, a status that expedites access to innovative technologies where there is unmet need.) “Without [such a test], you’re kind of shooting in the dark,” says Mei Mei Hu, CEO and co-founder of Vaxxinity.
Alpha synuclein has been tricky to measure in the body for several reasons, says Frasier. While everyone has the protein, abnormal forms of it occur in relatively small amounts, so they're harder to detect via imaging. This type of alpha synuclein also tends to clump inside cells rather than outside of them, making it even harder to see. If clumps are large enough to become detectable, they can look structurally similar to amyloid or tau—the proteins implicated in Alzheimer’s disease—so imaging tests might misdiagnose people with Alzheimer’s rather than Parkinson’s.

The test overcomes those hurdles by cleverly exploiting normal forms of the protein. Parkinson’s experts believe that tiny amounts of abnormal alpha synuclein circulate in the spinal fluid of patients, but are too small to be detected through imaging. To run the new test in the study, researchers take normal forms of the protein in the lab and add them to samples of spinal fluid from patients; that prompts any misfolded protein that might be present in the samples to pull the normal proteins into misfolded aggregates, amplifying the signal for the abnormal form. Scientists then use a fluorescent probe to detect how much antibody to the misfolded protein patients generated, resulting in a biomarker, or stand-in for the treatment effect.
This test would be a critical advance because it makes it possible to identify patients with abnormal alpha synuclein at the earliest stages of the disease, when treatments might be more effective.
With more data from patients, researchers hope to further refine what different levels mean, so that the test will be able to tell not just if a person has Parkinson's but whether someone might be at a greater risk of developing it. Currently the test is only used in research studies, but more results like these—as well as data on whether the same process can be applied to blood samples—could speed the test to getting approved for wider use.
What the study found
The trial—conducted by researchers at the University of Texas, the Mayo Clinic, the Michael J. Fox Foundation for Parkinson’s Research, and Vaxxinity—included 20 people with Parkinson’s. It was just designed to test the safety of the approach, so the study only provided hints about the treatment's effectiveness. Everyone received three shots over nearly a year; some contained the treatment at different doses, and some contained a placebo.
Overall, people receiving the vaccine generated more antibodies against the abnormal alpha synuclein protein than those vaccinated with placebo, as measured by the Parkinson's test. Antibodies started to ramp up about four months after the vaccinations began.
“What is unique about our technology is that it can stimulate the immune system to produce very, very specific antibodies against toxic forms of alpha synuclein, and do it in a safe way, which is reassuring,” says Jean-Cosme Dodart, senior vice president of research at Vaxxinity and senior author of the paper.
According to the test results, about half of the patients in the trial showed high levels of antibodies against the misfolded alpha synuclein, and most of these patients received the highest dose of the vaccine. They also scored the highest on motor and cognitive tests. There were too few patients to adequately assess any changes of Parkinson’s symptoms, but the researchers believe that longer follow-up with those tests, and potentially more frequent or higher doses of the vaccine, could lead to improvements in those scores. “The results are very, very encouraging,” says Dodart.
“This paper demonstrates that in a small number of people, the vaccine is having an impact on misfolded alpha synuclein, which is really exciting,” says Frasier. “We are now in the biological era for Parkinson’s disease."

With recent advances in imaging, genomics and other technologies, the life sciences are awash in data. If a biologist is studying cells taken from the brain tissue of Alzheimer’s patients, for example, there could be any number of characteristics they want to investigate — a cell’s type, the genes it’s expressing, its location within the tissue, or more. However, while cells can now be probed experimentally using different kinds of measurements simultaneously, when it comes to analyzing the data, scientists usually can only work with one type of measurement at a time.
Working with “multimodal” data, as it’s called, requires new computational tools, which is where Xinyi Zhang comes in.
The fourth-year MIT PhD student is bridging machine learning and biology to understand fundamental biological principles, especially in areas where conventional methods have hit limitations. Working in the lab of MIT Professor Caroline Uhler in the Department of Electrical Engineering and Computer Science and the Institute for Data, Systems, and Society, and collaborating with researchers at the Eric and Wendy Schmidt Center at the Broad Institute and elsewhere, Zhang has led multiple efforts to build computational frameworks and principles for understanding the regulatory mechanisms of cells.
“All of these are small steps toward the end goal of trying to answer how cells work, how tissues and organs work, why they have disease, and why they can sometimes be cured and sometimes not,” Zhang says.
The activities Zhang pursues in her down time are no less ambitious. The list of hobbies she has taken up at the Institute include sailing, skiing, ice skating, rock climbing, performing with MIT’s Concert Choir, and flying single-engine planes. (She earned her pilot’s license in November 2022.)
“I guess I like to go to places I’ve never been and do things I haven’t done before,” she says with signature understatement.
Uhler, her advisor, says that Zhang’s quiet humility leads to a surprise “in every conversation.”
“Every time, you learn something like, ‘Okay, so now she’s learning to fly,’” Uhler says. “It’s just amazing. Anything she does, she does for the right reasons. She wants to be good at the things she cares about, which I think is really exciting.”
Zhang first became interested in biology as a high school student in Hangzhou, China. She liked that her teachers couldn’t answer her questions in biology class, which led her to see it as the “most interesting” topic to study.
Her interest in biology eventually turned into an interest in bioengineering. After her parents, who were middle school teachers, suggested studying in the United States, she majored in the latter alongside electrical engineering and computer science as an undergraduate at the University of California at Berkeley.
Zhang was ready to dive straight into MIT’s EECS PhD program after graduating in 2020, but the Covid-19 pandemic delayed her first year. Despite that, in December 2022, she, Uhler, and two other co-authors published a paper in Nature Communications.
The groundwork for the paper was laid by Xiao Wang, one of the co-authors. She had previously done work with the Broad Institute in developing a form of spatial cell analysis that combined multiple forms of cell imaging and gene expression for the same cell while also mapping out the cell’s place in the tissue sample it came from — something that had never been done before.
This innovation had many potential applications, including enabling new ways of tracking the progression of various diseases, but there was no way to analyze all the multimodal data the method produced. In came Zhang, who became interested in designing a computational method that could.
The team focused on chromatin staining as their imaging method of choice, which is relatively cheap but still reveals a great deal of information about cells. The next step was integrating the spatial analysis techniques developed by Wang, and to do that, Zhang began designing an autoencoder.
Autoencoders are a type of neural network that typically encodes and shrinks large amounts of high-dimensional data, then expand the transformed data back to its original size. In this case, Zhang’s autoencoder did the reverse, taking the input data and making it higher-dimensional. This allowed them to combine data from different animals and remove technical variations that were not due to meaningful biological differences.
In the paper, they used this technology, abbreviated as STACI, to identify how cells and tissues reveal the progression of Alzheimer’s disease when observed under a number of spatial and imaging techniques. The model can also be used to analyze any number of diseases, Zhang says.
Given unlimited time and resources, her dream would be to build a fully complete model of human life. Unfortunately, both time and resources are limited. Her ambition isn’t, however, and she says she wants to keep applying her skills to solve the “most challenging questions that we don’t have the tools to answer.”
She’s currently working on wrapping up a couple of projects, one focused on studying neurodegeneration by analyzing frontal cortex imaging and another on predicting protein images from protein sequences and chromatin imaging.
“There are still many unanswered questions,” she says. “I want to pick questions that are biologically meaningful, that help us understand things we didn’t know before.”

i'm trying to get used to the idea that i can have my own opinion of a movie even if people think it's kinda shit, so here's my little review of Grave Encounters, which i watched just now late at night/in the wee hours of the morning.
i loved this movie so much, way more than i anticipated. i've seen my fair share of found-footage horror movies, and i was expecting to like it well enough alongside films like Cloverfield and As Above, So Below, or think it was not very good but enjoy it nonetheless like most of the Paranormal Activity movies or the Taking of Deborah Logan, but i unironically enjoyed it and thought it was unironically good, too. it's definitely not as good as found-footage movies like the Blair Witch Project, Creep or Rec, at least in my opinion, but i'd say it's one of the better ones for sure.
it had plenty of slightly cheap jumpscares- like i expected it to- but was really good at building a sense of tension and dread throughout. it's like if you take the last 20 minutes of a Paranormal Activity movie and extend it out to feature-length.
unlike with most found-footage movies, i really actively liked the characters, or found them entertaining at the very least. i found it way funnier than i should have when the characters would tell Houston to shut the fuck up whenever he said anything at all.
i also really liked how the film crew had to experience the sense of entrapment and mental decline that the patients must have felt, and how you only really get to sympathise with Lance when he's gradually isolated and psychologically tortured by the hospital. the first thing that comes to mind when looking for a comparison to this movie is the Taking of Deborah Logan, which i feel could have been way better if they went down the route of her possession better reflected the gradual decline experienced by Alzheimer patients, rather than 'ooo she's possessed by the ghost of a child predator' or whatever it was. like, with Grave Encounters, you get to see a group of people who are cut off from the world and tortured by a mental hospital like the people who were once thrown in there.
so yeah. good movie. i've been watching quite a few horror movies recently, so follow me on letterboxd so i can feel like my opinions on movies matter.

Alzheimer's Disease: biomarkers and neuroimaging markers cheatsheet for research articles
As Alzheimer's Disease (AD) research skews toward understanding the brain than the pathogenic proteins, studies exploring biomarkers and neuroimaging are hopeful toward developing a method for successful prevention of AD. A biomarker is a molecule, whose presence indicates abnormality or disease, and thus, is crucial in diagnostic procedures. Levels of certain molecules is notably altered in cerebrospinal fluid and in blood plasma, which helps in diagnosing the occurrence of AD. Neuroimaging involves the use of techniques such as magnetic resonance imaging and computed tomography to observe neuronal activity in the brain. This is good news, especially for AD, as the asymptomatic stage of the disease can be identified early enough.
Although the exact function and involvement in clinical practice is not profuse, altered concentrations of these biomarkers in plasma or cerebrospinal fluid encourage further research:
Amyloid and tau serve as the unsurprising biomarkers of AD pathology.
Neurofilament-light chain (NF-L) and visinin-like protein-1 (VILIP-1) are the most promising biomarkers of neuronal injury.
Post-synaptic protein neurogranin (Ng) and pre-synaptic proteins synaptosome-associated protein-25 (SNAP-25) and synaptotagmin-1 (Syt-1) are considered major biomarkers of synaptic injury.
Brain and CSF levels of tumor necrosis factor alpha (TNF-α) and increased levels of interleukin group of proteins (ILs) indicate intensified microglial response to neuroinflammation.
TREM2 receptor and YKL-40 glycoprotein are also reliable indicators of inflammation and impaired clearance of amyloid beta.
Heart-type fatty acid-binding protein (hFABP) could be a marker for pathology in blood vessels supplying the brain. Some vascular markers also show potential as markers of vascular injury in AD: von Willebrand factor (vWF) and monokine induced by γ-interferon (MIG, also known as CXCL-9).
Concentrations of TAR-DNA binding protein (TDP-43) in the brain and plasma and serum indicate, even contribute to, inflammation, mitochondrial dysfunction, and neuronal/synaptic injury in AD.
Neuroimaging techniques reveal structural, functional, and diffusion-related activities of the neurons. To identify them, markers are tracked in images obtained. Each marker is determined with the activity and biochemistry of the group of/individual neurons being studied.
Structural MRI will show location and severity of atrophy which can be identified in grey scale images by applying programs that create analogous color grading.
Functional MRI relies on blood oxygenation level dependent (BOLD) signal which reflects changes in blood oxygenation levels in response to neural activity.
Diffusion weighted imaging (DWI) focuses on diffusion of water molecules. A tensor model is applied to images obtained from DWI. The diffusion tensor imaging (DTI) metrics thus obtained help in studying connectivity through structural integrity of white matter tracts.
Tractography involves 3-D reconstruction of white matter as observed in DWI, which provides a more detailed look into a patient’s neural networks.
In positron emission tomography (PET), markers are identified and labelled so their features or functions can be traced during this procedure to obtain a resulting PET scan. The imaging procedure is named according to its marker: amyloid-PET, tau-PET, FDG-PET, inflammation-PET, receptor-PET.

I sighed as my alarm to get up went off, I silenced it and walked towards my bathroom to get ready. I had hardly slept since that nightmare and was extremely on edge, I called my therapist and left them a voicemail explaining what was happening, I was a nervous wreck.
I took my meds and brushed my teeth before pulling on my outfit of black scrubs and pulling my hair back into a tight bun. I looked at myself for a moment in the mirror,
"You need to get your shit together, you fought for this job you can't psych out now." You say to yourself in the mirror. You walk out of the bathroom and walk into the living room to see a man standing there, before you could take in his features he was gone, all you could remember was his piercing yellow eyes, you stand there frozen for a moment, your mind reeling. You grab your keys and drive to the facility as your try to wrap your head around what had just happened. It's not long before you reach your new place of work where you're once again greeted by the same receptionist.
"Hi! I'm guessing your here for your first day of training?" She asks and I nod
"Awesome, follow me." She says and I follow her down one of the hallways, we make a few turns before she stops outside of a door, I read the little sign next to it,

I grab the handle and open the door, thanking her before I walk in. Inside the room are three people I immediately recognize Dr.Loomis but the other two are unknown to me.
"Ah, just in time! (Yourname) meet Ashley and Bianca." Dr.Loomis says and introduces you to the others in the room, you give a small smile and shake their hands
"It's nice to meet you, will I be training with you both today?" I ask and they both nod
"Yes, today you'll be with me and then tomarrow Ashley will train you." Bianca says and you nod
"Sounds good to me, let's get started." You say and Bianca laughs
"Well I already like your attitude, c'mon" she says and motions for you to follow her.
"Now right now we're in the front of the building which in mostly unsecured so in this area there are no patients. You have to go through here doors here to get back to where the inpatient patience are held." Bianca says and you nod along trying to memorize what she's telling you.
"Once we're back in the secured facility you'll notice that the patient wings are in order. We have the Children's wing first which is comprised of children ages six to twelve, then the juvenile wing which houses teens from ages thirteen to seventeen, then we have the adult and elderly wing that houses anyone from eighteen to about Sixty-Five but that age does depend on the person and their mental condition most are sent to hospice one Alzheimers or Dementia set in." She explains and points down the different hallways,
"What about this wing?" I ask and she smiles
"That wing goes to the cafeteria, the therapy rooms, and other programs we have." She says
"Alright, so today you're with me on the children and juveniles wings, we're just gonna go over that and tomorrow you'll be with Ashley in the adults wing." She explains and you give her a thumbs up, she swipes her card and leads you through the secured door into the children's wing. You follow her back through a couple more doors until you reach a common area where children are sat doing different activities, some are coloring with tiny broken crayons, some are watching a show on a tv secured behind a plexiglass screen, and some are playing with stuffed animals with drawn on faces.
"Alright Kiddos, listen up, we have a new person here today, they're going to be asking you some questions and working with me, I want you all to be on your best behavior. Now will everyone please welcome (Yourname)?" Bianca asks
"Hi (Yourname)" the kids say in unison and you give them a polite wave,

Retail is a special kind of hell.
I've been a bartender. I've been a kitchen grunt in an assisted-living facility (couldn't handle the latter for long, but it was nothing to do with interacting socially with dementia patients. I was really good at that part. It was to do with my weird ability to smell a bunch of disease states including most degenerative ones and having an acute stress response to being around that kind of smell for so much of my day -- while I logically know that degenerative illnesses like alzheimers and parkinsons and seizure disorders aren't contagious, my instincts when confronted with the smell of them scream 'danger, get away, there's a horrible sickness here' and unfortunately I can't turn that off with logic).
As a bartender, you can tell the customers to fuck off if they get belligerent with you. You can kick them out temporarily or permanently depending on how severely and/or repeatedly they engage in verbal abuse. In an assisted-living facility, you can build up a rapport with the people you're interacting with every day, and you get special training on working with people with dementia that helps you prevent and defuse the ways they tend to get nasty, when they do get nasty (which isn't actually all that common). In retail you have no power and the worst of the customers know that and actively take advantage of it to abuse you. The managers also often abuse you. Plus the wage is completely insulting in comparison to the work you're expected to put in.
I'd bartend again. I would not work retail again.
People always gloss over how mentally damaging it can be to work in retail. I fucking hate that whenever I say “I could never work in retail again” someone has to reply “You snowflake millennials can’t take a starter job because you have to INTERACT with other people” No. Fuck you. I’ve worked as a planetarium host. I’ve worked as a public speaker. I’ve worked as a tutor and as a student teacher. I can work with people. I can work with crowds. Retail was fucking different. Retail was being treated as a subhuman. Retail was being treated so poorly that you have anxiety attacks before work. Having to work retail was a factor in my last suicide attempt. If I hear you say one fucking word about retail workers playing the victim I will personally break every bone in your body. Fuck You.

I'm so proud of them. I knew they could do it. They'll do even better next year, once they have an official adhd diagnosis and meds - which they will, as a quick goodle search showed me that all the pediatric psychiatrists in our area are now covered by our insurance, open after hours and on weekends, and accepting new patients.
Just then my cat looked up at me from my lap. "Hey," she said in a clear, human-esque voice, sounding embarrassed. "Thanks for being patient with me all these years. I promise, from now on I'll only poop and pee in the litter box. And I'll even try to be nicer to my..." she grimaced and forced out the last word. "Brothers."
I smiled and skritched her ears. "Thank you. I'd appreciate that."
She stretched. "Since I'm being such a good kitty, do you think we could have rotisserie chicken for dinner? And maybe you could share some with the best kitty ever?"
"I think we could make that happen. As luck would have it," I answered, "I just came into some money."
