Pelvic Floor Health for Detrans Women
A disclaimer before we start: I am not a doctor, a PT, or an expert of any kind, I just noticed there was a lack of information about pelvic floor health in the detrans (and trans) communities and I wanted to compile the information that I’ve gained. If there are any issues or you have any wisdom to share, please DM me! I plan to add to this post and edit it over time as I learn more.
95% of females who have been on testosterone report pelvic health issues, which can include urinary leakage or retention, bladder pain, difficulty emptying the bladder, general pelvic pain, pain with sex, vaginal dryness, vaginal atrophy, vaginismus, anal issues, and more. (Source)
Here’s a discussion with a pelvic floor expert on the issues faced by females who have been on testosterone.
If you’ve experienced any of these problems, you’re not alone, and there are things you can do!
Vaginal Atrophy
The vast majority of detransitioning women (and females who are transitioning) have vaginal atrophy, which is a thinning and weakening of the tissues that line the vaginal wall. Atrophy can lead to pain during sex, or with regular movement, bleeding due to small tears in the vaginal lining, narrowing of the vaginal canal, urinary issues, and more.
Because testosterone affects our ovaries, we can think of this issue as something similar to GSM (Genitourinary Syndrome of Menopause). Many of our symptoms mirror what happens to women as they age and their estrogen production decreases.
Treatments for Atrophy
1. Vaginal estrogen comes in the form of creams, suppositories, and insertable rings.
2. Vitamin E suppositories have been found to be as effective as vaginal estrogen in some studies. (Source)
3. Sea Buckthorn oil capsules have been shown to be effective in vaginal health. (Source)
4. Regular sexual activity can help by filling the vaginal wall tissues with blood, which can help to revitalize those tissues.
There are also many options for dryness, including vaginal moisturizers, aloe, coconut oil, and more. Sometimes the simplest natural options can be the most effective! Always talk to your gynecologist and do your own research on products you’re considering buying and make sure the ingredients are safe. Some people may experience yeast infections and other issues when using certain products.
Vaginal atrophy itself is to blame in many cases for the urinary symptoms that many of us report, and treating the atrophy may be all that’s needed in order to improve the urinary symptoms.
In other cases, we also need to look at overall pelvic floor health. I would argue that taking care of your pelvic floor is essential for any woman at any stage of life, since it can help with so many things!
Pelvic Floor Muscle Issues
Pelvic floor health issues can be divided into 2 types - Hypotonicity and Hypertonicity. Both types can lead to bladder issues, among other things.
Hypotonicity is the classic type many women experience after having children or during menopause. It’s also described as having a weak pelvic floor, and kegels are often the best treatment. The YouTube playlist at the end of this post includes videos for beginner and advanced kegel exercise methods and yoga.
Hypertonicity is the opposite type, where the pelvic muscles are chronically tight. For this, the treatment is to use muscle release methods to relax the pelvic floor. Remember - Relaxed muscles are the best at doing their job.
Anxiety can also be a factor in hypertonicity! If you’re often anxious, get used to checking how that feels in your pelvic floor. Many young women experience bladder leakage or discomfort, feeling like you have to go when you don’t, or pain with sexual activity, due to anxiety which causes overly tight muscles.
You wouldn’t think at first that Hypertonicity could lead to urinary symptoms like leakage, but when your muscles are overly tight, they just don’t work the way they should.
Sitting a lot and generally not getting much exercise also causes muscles in the pelvic floor, hips, and hamstrings to tighten and become shorter, so stretching these areas is very helpful.
And when you have a urinary issue, or you’re dealing with the aftermath of childbirth, surgery, or any other medical trauma to the pelvic floor, there can be a tendency to reflexively tighten your muscles all the time, for fear of what might happen if you don’t. Some people with hypertonicity also experience their symptoms getting worse if they do a lot of kegel exercise. In these situations, kegels can become counterintuitive.
That said, using methods to address both types can be the best option for some people. As long as you listen to your body, keep track of how each method makes you feel, and talk with a doctor or pelvic floor PT if you have serious concerns or don’t understand how to do something, you should be able to figure out something that will help!
Vaginismus is also a very common condition that’s connected to hypertonicity and potential mental causes. You’ll know you have Vaginismus if you’ve always had trouble inserting things into your vagina, or if your partner has had trouble with it. Many women describe it as a sensation of the vagina closing up when faced with something trying to get in. You may find that at certain times or with certain objects, you have no problem, and at other times or with other objects, you do. Stretches and massages for hypertonicity can often help with Vaginismus.
Prolapse is a relatively common issue in women who have had kids and older women in menopause. This can also cause urinary symptoms. The incidence of pelvic floor prolapse in females on testosterone is not known, but due to atrophy weakening the walls of the vagina, it’s possible that testosterone will increase your risk. It’s also more common in people who have had a hysterectomy.
Tools
1. Vaginal dilators can be helpful for people who have trouble with Vaginismus or feel like their vagina is small. These are also helpful for people who have difficulty inserting fingers
2. A pelvic wand or vibrator can help you with massage to loosen muscles, if needed
4. Kegel trainers come in various types and can help you perform kegels more effectively if you know that your issue is hypotoniticy
5. Pessaries can help in cases where atrophy has led to pelvic floor prolapse. Make sure you get diagnosed before using one!
6. Your hands! Don’t underestimate the power of using your hands for external or internal massage
The biggest thing to take away from this post is this - Don’t be afraid of your vagina or pelvic floor! Don’t be afraid to try things that may help you improve whatever issues you’re having.
Your vagina is a normal part of your body, and especially when you’re experiencing issues, that’s when it’s time to really learn about it and understand what’s going on. If you’re anything like me, you’ve gone your whole life being too afraid or too uninformed to do certain things or explore your body in certain ways. We need to reduce the fear, stigma, and awkwardness of vaginal and pelvic floor issues, and the first step is to get to know your body. 💪

Welcome to the Great UTI Scandal, a story of unnecessary suffering for millions, needless hospital admissions, antibiotic resistance, sepsis-related deaths and basic ignorance of the science around female bodies. Women are 30 times more likely to get a urinary tract infection than men, and UTIs are agonising and occasionally fatal. In the past five years, there were 1.8m hospital admissions involving UTIs in England alone, plus even more GP appointments. This is not just a gender health gap – it’s a dangerous crevasse. But is there another way? ... Younger women do suffer from infections, dehydration and post-sex cystitis, but the brunt is borne by older women. UTI rates shoot upwards when women hit 45 and are perimenopause. The loss of the hormone oestrogen leaves the vulva dry in menopause (and sometimes post-pregnancy), a condition previously called “vaginal atrophy” – perhaps because early medics were only focused on those lady parts useful to men. Now, the hormone deficiency has been renamed Genitourinary Syndrome of Menopause (GSM) and Dr Rachel Rubin, a campaigning urologist [...] explained why. “This is not just vaginal dryness. GSM is a very serious condition. Without hormones in the local environment of the vagina and the bladder, you are susceptible to both genital and urinary symptoms. So, yes, it’s about pain with sex, decreased orgasm, decreased arousal. As a sex doctor, those are important to me. However, as a urologist, the much more serious issues are discomfort, pain when sitting, irritation, burning and itching of the vulva, urinary frequency and urgency. And the thing that kills elderly people all the time is urinary tract infections, which can lead to sepsis, worsened dementia, and death.” The NHS says the death rate for hospital UTIs is 4 in 100, rising to 1 in 10 in those aged 95 and over. Just as we have a gut microbiome, we have a vaginal microbiome. Lack of oestrogen means the lactobacilli and other good bacteria in the vaginal microbiome can be replaced by pathogenic ones. Oestrogen also helps collagen production, and collagen disappears from the vulva by up to 30% in the five years after menopause. We invest millions in collagen supplements and anti-ageing serums for our faces, but it never occurs to us that precisely the same process goes on down below. ... But guess what? We can prevent GSM for women in perimenopause and menopause by giving them a safe, incredibly low dose of vaginal oestrogen, which plumps the tissue back up again, feeds the vaginal microbiome and reduces UTIs by 50%, according to the American Journal of Obstetrics & Gynecology. It’s safe for most breast cancer patients, too, it’s a win-win on the sex front and you can take it alongside normal HRT. Vaginal hormones come as a cream, gel or pessary and are cheap, costing the NHS around £5 for the cream. Meanwhile hospital admissions for people with UTIs stand at around £400m a year. Although some GPs and urologists are getting the message and prescribing vaginal oestrogen, more work needs to be done. In one California study in 2003 of over 5,000 women (average age 70), more than half had reduced UTIs after a year on vaginal oestrogen and a third had none whatsoever. Dr Rubin said: “We have millions of people in nursing homes who are dying of UTIs [in the US] and we have lots of data since the 1990s to show that vaginal hormones massively decrease urinary tract infections. We have new data. We have old data. We have so much data. The problem is that nobody’s talking about it.”

When time, read: [ ] double barrel wet colostomy [ ] appendiceal cancer [ ] malignant bowel protocol [ ] May Thurner syndrome [ ] Po vs IV vs rectal vs vaginal contrast As always, so, so tired. Last "work" night of 5 weeks with a call shift tomorrow. So the night goes oncology night shift > present research @ 0930 > sleep > graduation @ 1800 > call shift.
I'm dreading the whole freaking day. My research proposal is not good. My slides are messed up. It's confusing. I think the study itself will be fine, but a 5 week night float was not the time or place to get my shit together for this. I just hope the program coordinator can find it within herself to update my slides in the morning, or else I'm fucked. And then I'm gonna get such LITTLE SLEEP before I have to go to graduation for 4 hrs and pretend to be in a good mood and socialize. At least my call shift right afterwards is normal R2 crap, antepartum/benign gynecology stuff.
To be honest covering the oncology service has been chill enough the past shift-and-half, except for when it's bad it's BAD and probably the worst service to be on. I like onc nights more than days, though, because there's not a lot of extra people and I can just sit in my little work room upstairs and be alone and read through things. It's mostly covering the OR at the end of the day shift, seeing overnight direct admits and transports, and following up vitals and miscellaneous labs, post-op checks, etc. And then sometimes people surgical emergencies, or people crump or straight up die.
So, a medicine service +/- OR time.
My first night I had two transports: a malignant large bowel obstruction iso newly diagnosed HGSOC that is being managed conservatively given its size <13 cm but also her significant neutropenia due to her neoadjuvant Avastin therapy (her prognosis.... isn't amazing), and a "frequent flier" with recurrent vulvar cancer (and the gnarliest genitourinary anatomy I've seen) s/p MULTIPLE resections, EUAs, ablations admitted with c/f sepsis in the setting of a new perirectal abscess. All things considered it went well enough but my presentations were rocky, like I'd expect them to be for a new R2, but I'm almost R3. I was flustered because they both came at the same time and the LBO made me nervous, so I felt rushed trying to get the other transport tucked in so I only had to call the attending once.
Tonight started out with the potential to be a NIGHTMARE. There were two ORs running late, an exlap followed by an EUA with one attending, and a robotic hyst with another, and then I got sign out on four (4) incoming transports, and I also had four (4) post-op checks all due around the same time. I ended up not having to go to the OR because the exlap said I don't need to scrub anyone out, the EUA was cancelled, and one of the transports came at the same time so my R3 said to see that instead of scrubbing her out.
It's a very sad case, though. I mean, all onc cases are sad, but she was with her dad which just tore at my heart. It's a woman <50 yo, no hx cancer, with 3-4 months of back pain who finally had an ultrasound and CT that showed 3 large abdominopelvic masses and widespread mets. :c She's getting omental biopsies tomorrow. We ultimately think it's appendiceal or some other gastrointestinal malignancy because her CA-125 is only about 180, which for a pre-menopausal woman is low. Another one where the prognosis is not very good.
Actually, none of these people I admitted have good prognoses. I guess that's just how gyne onc is. :-/
The other transports probably aren't coming but I'm going to try and prep them in a second. There's a pelvic fluid collection in a woman 5d s/p a hyst with a post-op course c/b May Thurner syndrome and PE s/p extensive thrombectomy now on Eliquis, another vulvar cancer patient whose left drain fell out and now there is c/f infection in addition to just general failure to thrive, and someone who probably won't come who was found to be hypokalemic apparently on outpatient labs. And then some mystery woman in her 80s that may or may not show up on the ED board with recurrent HGSOC and a GTube for gastrointestinal issues.
3 h til signout, and then I have about 2-2.5 hr to sleep and practice my presentation. I'm ready for this weekend to be overrrrrrrrrrrr

Gonna add to this, as a trans masc dude that’s been on T for nearly 8 years. Look up ALL of the possible symptoms of menopause. Sometimes you can end up with something called GSM (genitourinary syndrome of menopause, also called atrophic vaginitis). It can cause bleeding because of the thinning of the vaginal canal, which is caused by a reduction in estrogen levels. You’re not guaranteed to deal with it, and it CAN be eased with a variety of options, both otc and prescribed (it’ll be a localized estrogen cream applied vaginally, but it doesn’t absorb into your bloodstream).

So, back over the summer, I made a post about how I was Going Through It and I have ... an addition? A something. It's TMI, so it's under a cut.
So, I continue to deal with atrophic vaginitis (or, GSM - Genitourinary Syndrome of Menopause), which for a while I'd had good results with. I was using the estrogen cream and switched moisturizers from the Sliquid Satin to Replens which has an application schedule of about every three days (I use it twice a week). And for a while, everything was going well. I was only occasionally dealing with bleeding, and when I was, it was very little and extremely infrequently.
But, here's the rub.
I smoke weed. I smoke about as frequently as I can afford to, and can usually go through 3.5g in about a month if I'm methodical enough with it. Unfortunately, I discovered that ... that might be causing the issue I'm having wrt the bleeding I'm having. So, my options are 1. stop smoking, and significantly decrease the issue I'm having (thus removing the one indulgence I get to have occasionally) or 2. continue smoking and significantly reduce the issue (thus, giving me peace of mind).
I know the right answer is to stop. And, given that I'm out and likely won't have any real money any time soon, that's probably what's gonna happen. But, again, smoking is the one small indulgence I actually get to have. And now, I'm miffed.
Anyway, yeah.

Definition – Menopausal hormone therapy (MHT) is the broad term used to describe both unopposed estrogen use for women who have undergone hysterectomy, and combined estrogen-progestin therapy (EPT) for women with an intact uterus who need a progestin to prevent estrogen-associated endometrial hyperplasia.
●Goals – The primary goal of MHT is to relieve vasomotor symptoms (hot flashes). Other symptoms associated with perimenopause and menopause that respond to estrogen include sleep disturbances, depression/anxiety, and, in some cases, joint aches and pains.
Estrogen is also indicated for the management of genitourinary syndrome of menopause (GSM); however, low-dose vaginal estrogen should be used rather than systemic estrogen.
●Patient selection – For healthy, peri/postmenopausal women with moderate to severe vasomotor symptoms impacting sleep, quality of life, or ability to function, and who are within 10 years of menopause (or <60 years of age), we suggest MHT (Grade 2B). For most women, the benefits of MHT outweigh the risks (figure 1). Exceptions include women with a history of breast cancer, coronary heart disease (CHD), a previous venous thromboembolic (VTE) event or stroke, active liver disease, or those at high risk for these complications.
We no longer use MHT for the prevention of chronic disease (osteoporosis, CHD, or dementia). However, there are some data to suggest that use of estrogen within the first 10 years after clinical menopause may reduce the risks of CHD and mortality.
●Preferred regimens
•Estrogens – All types and routes of estrogen are equally effective for hot flashes. We prefer 17-beta estradiol over other estrogens (such as conjugated equine estrogens [CEE]) because it is structurally identical (bioidentical) to the main estrogen secreted by the ovary. We suggest against the use of compounded bioidentical hormone therapy (Grade 2C). Concerns include the lack of efficacy and safety data, and the contents, dose, quality, and sterility of these products are not subject to regulatory oversight.
The transdermal route is particularly important in women with hypertriglyceridemia, active gallbladder disease, or known thrombophilias such as factor V Leiden (without a personal history of VTE). The baseline risk of both VTE and stroke is very low in otherwise healthy, young postmenopausal women. We therefore consider oral estradiol to be a safe and reasonable option for patients who prefer an oral preparation over a transdermal one (cost or personal preference).
●Progestins – For women with an intact uterus who are starting MHT and therefore require a progestin, we suggest micronized progesterone as our first-line progestin (Grade 2C). It is effective for endometrial hyperplasia, is metabolically neutral, and does not appear to increase the risk of either breast cancer or CHD, although data are limited.
●Duration of MHT
•Standard recommendations for duration of use are three to five years. For women who experience recurrent, bothersome hot flashes after stopping estrogen, we initially try nonhormonal options. However, if this approach is unsuccessful and symptoms persist, we resume MHT at the lowest dose possible in carefully selected women with persistent, severe hot flashes.
•Women with primary ovarian insufficiency (POI) should continue MHT until the average of menopause, eg, age 50 to 51 years, to decrease the risk of premature CHD, stroke, osteoporosis, and dementia.
Nonhormonal pharmacotherapy — For women with moderate to severe hot flashes who are not candidates for hormone therapy based upon their breast cancer, CHD, or VTE risk and for those who choose not to take MHT, we suggest nonhormonal agents [41,53]. The best studied agents with positive results include SSRIs, serotonin-norepinephrine reuptake inhibitors (SNRIs), anti-epileptics, clonidine and oxybutynin, and centrally acting drugs.
These drugs are also often tried in women who experience recurrent hot flashes after stopping MHT. Our choice of drug depends upon whether the patient is taking tamoxifen, the pattern of hot flashes, and the presence of a mood disorder or sleep problem.
