The Heilwald Loophole Dr. Randolph Headcanons:
Yippee!!! I love this game!
TW: Chronic/Terminal Illness, Thanatophobia (fear of death/dying), Arguments/Fighting, Body Horror/Hospital Horror, Surgery
•Dr. Randolph is generally a very smart man, but when it comes to medicine, he's not that great at it. Like, he knows how to provide treatment (surgery, medication, First Aid, etc), but not when it is needed. He'll look at someone with signs and symptoms of a simple cold, then say that they need like... lung surgery or something. He also just really likes to perform surgery, for some reason.
•He can speak fluent English, but for the life of him, he can't read or write it. I mainly got this idea from the "Benefits of the surgical removal of the lecur" paper (I think it is meant to be in English, due to there being moments when the player mentions that something is in German, like the maintenance sign). I am pretty sure he was trying to write "liver", since he is the guy that wants to remove livers. He can read his own "English" (read: gibberish), but no one else can. So, he'll give a whole speech in English, reading from a paper that he wrote, and none would be the wiser to how bad his writing is. He gets away with reading professional research papers in English by trucking someone else into reading it aloud for him. No one really knows that he can't actually read or write in English, since he is pretty good at hiding it.
•He is "tired" a lot of the time, to the point of extreme fatigue. He knows that his tiredness is actually fatigue, and not simply being tired, but doesn't want to admit it. This fatigue adds onto his temper, making him quick to snap at staff when he doesn't need to keep up appearances or is not in front of patients. This all culminated in an argument with Nurse Astrid that grew physical, which Dr. Hauser witnessed, over the paper she was writing about Dr. Randolph's work. This event is what caused Dr. Hauser to grow scared of Dr. Randolph, as well as causing Astrid to grow quiet and a "shell of her former self" according to her coworkers.
•Dr. Randolph's speech at the beginning of the game, where he mentions wanting to provide a better, or at least different option than death to the patients of the Heilwald Klinikum, is actually more about himself than the patients. The reason why he wants to remove your liver is because he, himself, is suffering from chronic liver failure. He was determined to work at the hospital, having heard that it was one of the best in Germany and, supposedly, had the capacity to work miracles due to the forest around it having special properties. He wanted to avoid his seemingly inevitable death from his disease, due to his severe Thanatophobia. In his opinion, there is no fate worse than death. His wish was fulfilled, since after a while, everyone became incapable of dying in the Heilwald Klinikum. They also couldn't leave. And... some of the staff started to change... but his wish to never die was fulfilled.
•On the idea of him stealing your liver... when he steals it, he does so with the intention to transplant it into himself. While he won't ever die from his condition, he still suffers from the symptoms of liver failure. So, he will gladly take yours and put it into himself. The newly transplanted livers always begin to fail soon after, but it does provide some relief for a little while. You also seem to just show up like nothing happened, anyway, so you aren't harmed by it, in his opinion.
•The physical changes to the staff are, depending on the staff member, representative of their state of mind. An example would be Nurse Astrid, who, after the incident with Dr. Randolph, became closed off. So, when she started changing, she basically grew an iron lung around herself to protect herself from harm, but she is still in pain. Other staff members that simply changed as a consequence of being in the hospital have less drastic changes, such as Nurse Sabine. Dr. Randolph's changes at the end of the game are a representation of all of his locked up anger at everything. His condition, how everything regarding his wishes turned out, and just everything. There is no specific thing or person he is angry at, so he just became a mangled mess of a... Whatever he became at that point...

Do fights ever break out between some of the Doctors? Do any of them have beef with each other?
The Receptionist nods, looking down at the papers in front of them. "Yes, there are quite a lot of fights. Thankfully, the majority of them are verbal and never escalate to physical. Sometimes, though, physical fights do happen. As far as I have been made aware, none have held a grudge..."
They suddenly sit straight as a board in their seat, their robotic face somehow growing pale. "There is, however... ONE issue that has been brought to my attention. A bit of a war of sorts of two differing opinions... It's about the idea of inviting a... certain doctor to our club. Let me find the papers that started it."
They rustle through some files, before pulling out a paper with messy penmanship written on it. The classic doctor handwriting. "You see, every doctor has the ability to request that a doctor be invited into our club, as long as you also write why you think they should be allowed to join. Then, the higher-ups will look over the requested doctor's work before deciding whether or not to invite them. You can request anonymously, or have your name written on the paper. This person requested anonymously... You know what? Let me just show you the paper." They place the paper on the desk, pushing it towards you.
"I would like to request that Dr. Josef Heiter be invited to join The Mad Doctor Appreciation Club for performing one of, if not, the most infamous and groundbreaking experiments in the history of mad medicine. He also succeeded in escaping the morality of medicine, having once been the leading surgeon in the separation of conjoined twins, before performing his experiment of sewing three people together to work as one unit."
The Receptionist quickly pulls the paper away before you can read anymore. "If you know him, you know him. Anyways... Dr. Heiter is someone a mad doctor either loves, or, more commonly, despises. His experiment is often seen as too vile and disturbing for even the most strong-willed of deranged minds. However, there are also those who admire his willingness to do whatever the hell he wants, by any means necessary." They look over the paper, continuing "So, once word of this request got out, the entirety of the club went absolutely apeshit. The Medic, you know, the one dressed in red gloves, was absolutely ecstatic to hear that Dr. Heiter might be invited. Dr. Maruki and Dr. Frankenstein, on the other hand, have been scared shitless - no pun intended."
The Receptionist then stands, placing the paper back where it belongs "It seems like The Medic, Dr. Randolph, and Dr. Wolfram are pretty excited about the possibility of Dr. Heiter joining the club. Dr. Frankenstein hates the idea, seeing it as something that might interfere with his own work of sewing dead body parts together to form new life. He once told me "The idea that a man would sew three people together in such a crude, disgusting manner sickens me to my core. What if people begin to see my work in such a vile light? It would taint the purpose of my work to be associated with the likes of him!" Dr. Maruki is just scared that he would fall victim to Dr. Heiter's experiments. I heard that Dr. Clef is actually the only person who doesn't seem to care, either way. According to The Medic, Dr. Clef just said "As long as I am not in the middle, I don't give a shit. Pun intended.", in response to the news of him possibly joining the club."
The Receptionist turns back to you, a weary smile on their face. "Unless you are Dr. Clef, you either despise the idea of Dr. Heiter joining the club, or are excited at the... well, the idea of seeing such a deranged lunatic in his element. Me? I don't care... I'm just here to keep things running and do the paperwork. This fighting is getting out of hand, though... I've never seen Dr. Maruki try to claw someone's eyes out in fear before now, but it was quite a sight! This paper probably caused the most physical, VIOLENT fights I have seen in this club, yet! It even got Dr. Maruki to pick up a scalpel... holy shit."
They look back and forth, only to quickly lean in and whisper "Between you and me... He's probably going to be let in. The higher-ups have no morality when it comes to who can join the club. The only reasons why I haven't told the others are because A. I haven't gotten a clear answer, yet, and B. I'm 100% certain that the knowledge of Dr. Heiter joining this club would cause an ACTUAL war in this building. I'm just waiting for the moment that one of these doctors actually gets killed... All over the morality of making a human centipede. Which, to be fair, I think is fucking disgusting-"

Circles (The Heilwald Loophole x GN Reader) Part 1

You feel yourself tense up as you look at the man in front of you. He isn't necessarily towering over you in height or covered in hulking muscles or anything. He just has this... aura around him. His brown eyes are staring directly at you, seeming to ignore the New Yorker hiding behind you. His dark hair is neatly slicked back, matching his posture good, but his face holds a sour look to it.
"What are you two doing back here? This place is off limits, especially for the likes of you two!" He looks between the both of you for a moment, and just as you were about to explain yourself, his eyes widen. He continues before you can get a word out. "Oh, is everything alright? You look... So pale..."
Jackson opens his mouth, leaning out from behind you to look at him. "I... I'm fine-" "Yeah, sweating like crazy, too!" The man cuts him off, disregarding his small comment about being fine. "Tell you what, follow me to my office; I'll see what I can do for you two."
He begins walking away, to what you can only assume is his office. You quickly grab Jackson's hand and follow the strange man, not wanting to upset this very... eerie person you just met. He seems to be in a hurry.
You feel the atmosphere growing tense, so you try to lighten it up by introducing yourself. "Umm... thank you for helping us, Herr. My name is (Y/N) (L/N). This man with me is Jackson... Jackson, what's your last name?" "Donner." "Jackson Donner. What's your name?"
He doesn't look back to either of you as he responds. Staring straight ahead as he keeps walking, he gives an insightful reply. "It is only my job. My name is Dr. Randolph." At least you have more evidence of this place being a hospital or clinic. If there are doctors here, it would only make sense for this place to be a hospital or clinic. You just hope you can figure out what exactly is going on!
Soon enough, the three of you are in a relatively small office. There's an odd painting on the wall, a desk, some filing cabinets... it seems pretty empty, but you aren't one to judge. You look around for a seat to sit in, only to hear Dr. Randolph say "Please, don't have a seat." You shrug your shoulders, peeking over to check on Jackson. He is practically shaking, possibly from anxiety, as he fumbles with the hem of his patient gown.
"Look... to be frank, I don't know either of you. I haven't seen either of you anywhere near this ward." Dr. Randolph begins. He pauses, thinking. You start to feel as though something terrible is going to happen. You can see that Jackson has the same feeling, as he takes a small step back and behind you, hiding again. Dr. Randolph continues, his tone slowly changing to something much darker. "I guess there is only one thing I can do for you two..."
His eyes widen, a wide grin splitting across his face as he excitedly announces "I'm now going to remove your livers!"
As if on an autopilot, you bolt out of the room. It's too late when you remember Jackson, as when you look behind you, you hear him let out a shout as he is pinned to the floor by the mad doctor. Within an instant, he is sedated and dragged into another room. You can't go back to help him, now, since you know that Dr. Randolph would probably take the opportunity to catch you, too.
You feel horrible, but silently hope that he is alright, at least, or that Dr. Randolph won't perform his surgery until he catches you, too. If you can find a way to take down Dr. Randolph before that happens, Jackson should be alright. A little shaken, yes, but alright. If just ONE staff member here is so... deranged, then who knows about the rest? You just want to help the one sane man you know here, so far.
Using the small amount of time that Dr. Randolph is distracted, you quickly look around the area. Opening doors, inspecting papers and boxes... you search for anything that can help you. However, you must admit... you have never been the best at finding things. Especially when you don't know what exactly it is that you are looking for. Something as vague as "something that can help you" doesn't bring up any specific object to search for, like a pen or chair.
Your know your headstart has run out once you hear a door open somewhere nearby, followed by footsteps. They are much slower than you expected, as if he is walking calmly while searching for you. You would've expected him to sprint throughout the area in a frantic attempt to catch you rather than act so calm about the situation.
You decide to risk it, not wanting to get cornered, and peek down the hallway you heard the door opening from. Luckily, he is walking away from you, his hands seemingly in his pockets, possibly gripping a syringe with your name written on it. You take a deep breath, crouching down and slowly making your way down the hall behind him. Before he can turn around, you duck into a doorway and search the room.
You shower through the papers in this office's cabinets, peek under the desk, look at the painting, and even pick up the potted plant to look under it. You are desperate, at this point, knowing that he could enter the room at any moment. You have no plans, still, other than find a way to stop him. A plan as general as that will do you no good, you know, so you need to find SOMETHING to help you make a better one! But nothing here seems to be useful. Why is this hospital ward so empty but so crowded at the same time?
You must have made a lot more noise than you realized, because it doesn't take long for Dr. Randolph to block the doorway. In an ironic twist, your attempt to not get cornered has only led to that exact situation happening. He lunges towards you as you decide to pick up the small chair to shield yourself, only for it to be wrenched from your arms in an instant. Next thing you know, he's tackled you to the floor, winding you, before jabbing a needle deep into your neck. The last thing you see before your vision goes black is the crazed look in his brown eyes.
You awaken in a small room. The walls and floor match that of the hospital ward you were just in, with a small window allowing light into the room. Oddly enough, you can't see what it looks like outside. A loud sound, much like the crunching of paper, fills your ears. Looking to your right, you see a paper on the bedside table, alongside another bed with Jackson sitting on it.
His eyes widen once he sees you. Then, his face forms an innocently happy expression, complete with a goofy grin. "(Y/N)! You... You're here! I was so scared! I just... Woke up here. You weren't here and I was too scared to go out alone, so I stayed put. I did try the door, though, but it was locked..." His face slowly drains of color, before he says "You just... Kinda popped into existence. I didn't see Dr. Randolph or anybody take you in here. You just... formed out of thin air! I assume he must have gotten to you too, right?"
You sit up in bed, rubbing your head. "Yeah. He cornered me in an office. Then I woke up here. Did you read that paper, by any chance?" He shakes his head. You tentatively pick it up, reading it aloud. "The Heilwald Klinikum welcomes Joseph Randolph, who is to take the role of chief doctor of the klinikum. The recruiter said that, although there were many applicants, Dr. Randolph was chosen above the rest." You look at the paper, flipping it over in confusion. You look over to Jackson, who is staring at you in anticipation. You shrug, saying "The rest is blank... It seems like an incomplete news article of some kind, though there isn't a label for any specific paper company."
Jackson sighs, nodding. "Well... everything seems to grow more confusing. Why would anybody choose someone like HIM to be a chief doctor? Or just a doctor in general? He saw us and the first thing he thought to do was remove our livers! He even chased you down, like... like... I don't know! A predator catching his prey!"
You shrug, again, responding in an equally as confused tone "I don't know! I am wondering the same thing! Although he seemed nice and cordial at first, even then I got an off-putting aura from him. I could understand him being upset that we were in an office limit area, but he changed his tone quickly. Maybe he just fooled everyone...?"
"I don't know, but I don't want to stick around here any longer. I feel like he might come in here any second." Jackson then looks around the room, standing on shaking legs. He points down at a broken vent, saying "I think I could fit in there! You probably could, too! It wouldn't hurt to check out..."
Despite your better judgement, you decide to follow him. Anywhere is better than nowhere, right? Although it's a tight squeeze, you manage to fit in the vent as your companion speeds ahead. You do your best to keep up, eventually making it to a large room with a hole in it. There is an odd machine, a singular chair, a small stand, and what looks like another plant across the room. Going to the door, you both find it to be locked.
"Jackson, do you see that?" "See what?" You point to the area right above the hole, explaining as best as you can "THAT. The strange light above the hole! It looks a bit like surgical lights, or something. Can you see it?" He looks to where you are pointing, nodding his head. "Yeah... I just thought I was going crazy. It's making an odd noise, too." He steps to the edge of the hole, continuing "I have this odd urge to jump in. We have no other options. I'd rather take my chances with this hole than Mr. "I'M NOW GOING TO REMOVE YOUR LIVER". You coming with me?"
You hold your breath for a moment, trying to calm yourself. Then, you release it in a heavy sigh. Whatever is going on here is much deeper than you can comprehend. A strange, mysterious hole, in an equally strange and mysterious hospital, all topped off by a crazed doctor with the urge to remove livers. Regardless, jumping in seems to be your only option. "Let's go in."
You grab his hand, looking at him for a moment, before hopping into the hole. Almost instantly after you fall through, you feel the painful sensation of rocks hitting you and dust entering your lungs, before a forceful impact against a floor. You take a second, waiting for the pain to alleviate, before standing. You look around the room, your blood running cold as you see familiar grey walls and cardboard boxes. You watch Jackson's face twist in disdain as he recognizes the room, too.
You can only muster up one question, formed perfectly with a single word: "Again?"

May I request some headcanons with Dr. Randolph and a reader who's *unbearably* kind and sweet? ^-^
You sure may! This was actually a bit hard since I rarely write sweet things, but I hope you like what I did:
At first, Randolph would be hostile towards you. He would not trust your wonderful nature. Not a single human being could be that sweet and selfish. There must be something you wanted from him, right? 
And while his hostility might fade after a while, he would still be very prone to avoiding you, still wary of your motives and straight up uncomfortable with all the attention directed at him. It will certainly take time for him to get used to you.
At first, he would flinch away every time you would reach out to touch him, thinking that now was the time you would finally make your move and strike him down. Imagine his surprise when you would just squeeze his shoulder to get his attention.
But then, an entire month after you first met, you did something that caused his thick walls to finally receive their first crack.
After he had overworked himself once again, which was quite common for him, you found him in his office, snoring and drooling all over his paperwork. Your heart had clenched at the sight, and you had reached for a blanket he kept hung over one of the chairs, carefully covering his back and broad shoulders with the soft material. You did this as carefully as you could so you would not awaken him, believing to have succeeded when you managed to sneak out without him moving, but in truth, Randolph was wide awake.
As soon as you had been out of sight and hearing range, he had sat up and turned his head to stare at the blanket, an absolutely dumbfounded expression on his face. He had remained in this position for what felt like hours, the gears in his brain turning as his mind had attempted to figure out what in Heilwald's name just happened and why?
After that sweet gesture, something changed, and he became more open towards you and your actions. You, being the observant person that you were, of course, immediately picked up on that and began to pamper him, using every opportunity presented to do something good for the monstrous man.
As soon as you picked up that he was not eating regularly, you began to sneak into Chef Sauer's kitchen and cook something for him. Once you were done (and survived), you would sneak into his office and leave the food on the desk in a lovely bag decorated with a sweet "Bon Appétit, from ___". You were unsure if he liked it first, but after the bag returned empty with a thick "Thank you" written onto it, you knew that you made the right decision.
Whenever it's colder than usual or if he is just feeling a little sick, you would sneak into the kitchen and make him a cup of tea or extra coffee, depending on the time of the day. At the beginning of the relationship, you, too, sneaked the cup into his office in fear of how he might react, but by now, you were bold enough to just hand it to him, sometimes even pressing your lips against his cheek for a short, fleeting kiss before running down the hallway.
If he overdoes it and actually falls sick, you would quite literally pamper him. You would bring him salves and rub them onto his chest and back, you would bring him every medication he would ask (demand) for and, if he he has to vomit, you would rub his back and whisper reassuring words into his ears when he would have his head stuck in a bucket or kiss the toiletseat. He would grumble and growl the entire time, trying to reassure you that he was fine and that he does not need your pity. You, however, would just reply with a long, sarcastic "suuuuurrrrreeee" before shoving another spoon of food or medication into his mouth, effectively shutting him up.
When he is stressed or angry, you would just come to his office and pull his head against your chest, pressing a soft kiss into his thick, dark brown hair before letting your fingers run through it. This causes him to calm down almost immediately, and he would lean into your touch, closing his reddened eyes.
When your relationship has been going on long enough for you to coax him to sleep in the same bed as you, you would hold him close and wrap your arms around his strong body. Sometimes, when you would run your hands over his bare back, (which was always exposed in bed due to him only sleeping in boxers, even in the harshest of winters) you would be able to feel the tense muscles caused by a shrimp position when sitting or the pure stress he was feeling 24/7. Depending if he has his back turned to you or not, you would tell him to shift so you would be able to reach it. The first few times, he would be very hesitant to do so, not entirely sure what your intentions were, but after a few wonderful massages that left him feel much more relaxed and brought salvation from the all-too-familiar pain, he soon began to do so without wasting a second thought.
To ease his stress at work, you had begun to help him with his syringes. You would make sure that he would never run out of them and fill them before he would start his shift. You even managed to force yourself out of bed before he would, making sure to prepare everything so all he would have to do it was to grab it and shove it into his pockets before he would leave. It took him no time to catch on to that, and he rewarded you with a soft kiss to your forehead and a low, rumbled "Thank you" before he would disappear out of the door to your personal quarters.
But while your sweet behavior was meant well, it sometimes gets on his nerves, and he does not shy away from telling you that. It, of course, hurts to hear that, but you know that he doesn't do it to be mean. Randolph is a man who needs his personal space, and he tends to become irritated when he does not get it. In response to his harsh yet well-meant words, you would try and better yourself, making sure to find out when it was "appropriate" for you to live our your sweet nature and when to hold back.
