Problem of blindness in Bihar and Uttar Pradesh
1 out of 4 blind people in the world live in India, and the states of Bihar and Uttar Pradesh share a significant burden of this visually impaired population. Within India, the combined population of Bihar and Uttar Pradesh (UP) can make it the third most populous country in the world, and in contrast, these are the two states with the highest population of visually impaired and blind people.
The cloudy reality of Bihar
Bihar, India's most densely populated state with a population of around 13 crores and the lowest GDP per capita, is the worst vision impairment-hit state, with an estimated 33 lakh visually impaired people. Around 4,65,120 people are blind in Bihar. Cataract is the primary reason for blindness in Bihar. Cataract is the clouding of the natural lens of the eye leading to partial or complete blindness, mostly occurs in the older population. Only 18% of those with refractive errors in rural areas of Bihar receive vision correction treatment, compared to 82% in urban areas.
People in Bihar lacks access to eye hospitals and eye care facilities. The visually impaired population of Bihar has limited access to proper eye care. The older population is the most vulnerable. Bihar has the highest deficit of qualified clinical professionals such as ophthalmologists and optometrists. The majority of eye hospitals are concentrated in urban areas, leaving rural populations underserved. For example, 75% of health infrastructure is in urban areas, where only 27% of the population resides. Economic constraints are a major barrier to seeking eye care in rural areas, contributing to the higher prevalence of blindness and visual impairment. Available eye care services are not affordable to the mass. Transportation infrastructure is also very poor which also contributes to patient not availing eye care services. Lower levels of awareness about eye health in the people in rural areas of Bihar contribute to delayed diagnosis and treatment.
Women in rural Bihar are at higher risk of visual impairment. Women in rural India are 1.51 times more likely to be blind compared to men. Men has better access to eye care services. Men are 2.3 times more likely to attend eye camps than women in rural areas.
Visual impairment leads to reduced productivity and increases the economic burden on families and caregivers. Visual impairment and blindness have a negative impact on livelihoods and contribute to poverty. Once an old person becomes blind due to cataract, he becomes a burden on the family that is already struggling to exist due to poverty. The person loses his/ her livelihood and stays confined within the four walls. When a child develops visual impairment, it hinders the overall growth of the child.

The school-screening team visits government run schools in villages to screen and provide primary eye care to the children free of charge. Spectacles are distributed to the visually impaired students at no cost to correct their refractive errors. Children needing surgical interventions are referred to the nearest surgical centres and provided free of cost surgeries. The hospital also engages Anganwadi workers, trains them to screen children and refer patients to the hospital needing eye care interventions.
