Selamat Mengasihi, begitu ucapan yang mengalir seiring doa dan ucapan yang kuterima atas kelahiran Deana yangmana mungkin juga diterima ibu-ibu lainnya. Kurang lebih 2 bulan lalu ku terima kata-kata penuh makna, mengasihi punya tersendiri. Tulisan ini untuk memperingati 1 bulan keberhasilan Deana menyusui secara langsung/DBF.

Proses menyusui/mengasihi adalah satu dari sekian fase kata orang yang membutuhkan effort lebih dalam tumbuh kembang anak : mengasihi, tumbuh gigi, mpasi dan drama GTM, toilet training (monggo ibu-ibu yang lebih berpengalaman menambahkan). Padahal saya dulu pikir karena menyusui itu berkaitan dengan kebutuhan dasar manusia yakni makan, apalagi alhamdulillah Deana bisa diberi kesempatan Inisiasi Menyusui Dini selama satu jam yang kalau diinget lucu banget dia uget2 cari nenen 🤣. Kelucuan yang kemudian ambyar dimalam setelah tali pusar dipotong, Deana mulai konser akbar saat lapar, dan nyut nyut mulai terasa saat menyusui.
Nyut-nyut yang semakin menjadi hingga membuat fase mengasihi yang sekarang sedang saya lalui dan jauh dari semua iklan yang ada kaitan dengan menyusui dimana bayi terlihat tenang dan ibu terlihat anggun. Mengasihi bukan hanya sekadar menempel dan mengenyot. Fase ini buat saya luka dan air mata, berdamai dengan rasa sakit, serta merasa diri seperti sapi perah 🤣 (yangmana mungkin ibu lain juga mengalami). Mulai dari puting lecet dan berdarah (nipple cream lansinoh dan ASI penyelamatku), penyumbatan berhujung mastitis (lecitin penyelamatku) sudah saya rasakan dalan proses ini.
Dua jam sekali waktu menyusui serasa bom waktu bagi saya. Diminggu awal kelahiran Deana masih mencoba direct breastfeeding meski entah kenapa begitu payudara dan mulut Dea bersentuhan ada dua kemungkinan yang terjadi yaitu tangis kencang atau mulut rapat tidur lelap. Karena Dea saat lahir agak kuning akhirnya Saya mencoba dan fokus memberi ASIP melalui media selain dot karena saya pernah diceramahi panjang oleh adik tingkat saya bahaya dari dot yang bikin bingung puting, merubah struktur rahang, kemampuan oromotor dl.
Pemberian ASIP memberi saya sedikit ruang bernapas dari rasa sakit, tapi hati masih tidak tenang karena suara gleg gleg yang terlalu kencang setiap Deana minum dari pipet (berhenti memberikan lewat sendok karena tumpah-tumpah). Dan benar saja ketika jadwal konsul laktasi di klinik dibilang itu karena tidak bisa mengatur aliran dan bisa bikin kembung dan pipet ujung karet tidak direkomendasikan karena rawan pertumbuhan bakteri. Tapi koplak momentnya pas pijat laktasi ibuk terapisnya bilang "bu cobain pakai dot yang lebar kayak payudara aja, cucu saya pakai itu masih bisa nyusu badan", dan itu adalah titik permulaan Deana kenal dot dan lancar 🤣.
Hati masih ragu dan teringet ceramah adik tingkat dulu hehe walau dibesarkan hati oleh bu bidan "anak saya juga pakai dot, ya memang kalau dikonsuler laktasi dot itu kayak haram hukumnya, tapi coba kalau ibu bekerja.. apa iya yang mengasuh sabar kayak kita mau kasih susu pakai cup feeder, sendok, dkk.. kalau anak tersedak malah buyar, saya sih realistis aja dengan pengunaan dot". Dan kemudian dua atau tiga hari setelah Deana pakai dot terus mamaknya masih ragu... akhirnya aku kontak Mbak Ida Sholihah dan jreng jreng Deana udah bingung puting, tapi akhirnya dengan sabar diajarin lagi posisi menyusui (asli ini gak bisa online menurutku karena beda banget pas liat di webinar sama dipraktekin langsung), pentingnya pelekatan (inget banget sama suami selalu ngecek dagunya dea nempel gak, bibir bawahnya dower gak kayak ikan walau sampai sekarang bibir dea masih kayak melepuh yg katanya karena belum deep latch), skin to skin sama bayi sering-sering. Dan aku dikasih semangat kalau menyusui emang sulit buat buat bayi kayak Deana yang lahir 2.7kg dengan bobot termasuk mungil makanya sulit mangap dan naluri tidurnya masih buanyak. Alhamdulillah, abis kedatangan Mbak Ida, aku bisa DBF selama kurang lebih 3 hari sebelum akhirnya ambyar lagi Deana nangis gak sabar 🤣
Oh ya dalam menyusui ketika gagal tutup telinga rapat-rapat dari koment yang tidak berguna. Aku cukup stress ketika ditanya "kenapa gak nyusu langsung?" atau "kok pakai sendok?" atau "gak pakai dot aja lebih gampang" atau kalimat ketika yang melihat aku menyusui dan koment "Putingnya jelek tuh,,putingnya mendelep makanya anak susah nyusu". Pertama, kayaknya langka ibu yang gak punya kendala menyusui langsung terus memilih ngasih ASIP (wong saya merasakan indahnya gak perlu angetin susu, nyuci botol, nakar ml untuk diminum hhe alias tinggal buka baju pas berhasil DBF terlepas dikasus saya suakitnya bukan main) yah ASI kan buat anak, jadi ngapain tahan-tahan nyimpen dikulkas kalau bisa fresh dari badan, rata-rata ibu eping (exclusive pumping) pasti punya alasan tersendiri yang gak perlu ditanya juga. Kedua, kayak ceramah adik tingkat saya yang berkesan wkwkw hidup itu terus bergerak dan alternatif pemberian ASIP jadi banyak selain dot- bisa sendok, spoonfeeder, pipet, spuit, cupfeeder/sloki, bahkan ada finger feeding atau sns pakai selang gitu (Deana akhirnya mencoba semua, kalau ada yang mau nanya walau saya bukan konsuler laktasi boleh hehe dan disepakati Deana akan pakai dot saat saya mulai bekerja dengan harapan kemampuan untuk DBFnya udh lebih settle jadi gak bakal bingung puting). Ketiga, dari semua webinar dan konsultasi laktasi yang saya ikuti menyusu itu bukan diputing tapi diaerola yang coklat itu (cek deh anatomi payudara) nah kalau anaknya berhasil ngenyot pentil juga kebentuk seiring waktu, dan yang menariknya aliran ASI itu keluar dari berbagai titik di aerola bukan cuman 1 pancaran puting hehe.
Dunia mengasihi juga berkaitan dengan pumping yang juga bukan sembarang pumping, tapi ternyata kasusku putingku gak cocok sama corong pompa asi yang kupunya, disaranin pakai insert corong. Jadi selain brand/merk, kesesuaian corong, posisi alat juga mempengaruhi hasil pumping. Dan beruntunglah ibu-ibu yang cocok pakai handsfree jadi bisa multitasking, sejauh ini saya masih lebih banyak hasil pompanya pakai corong.
Aku inget banget Mbak Ida bilang "Deana ini hisapannya lumayan kuat loh, semangat nyusu dia". Kata - kata ini yang jadi pengharapan sekaligus cambuk yang kadang bikin saya melow "masak saya ibuknya gak bisa menyusui" (yang akhirnya bikin saya obsesi Dea harus nempel di payudara minimal 10 menit, dan gagal saya nangis sendiri). Meski pemberian ASIP alhamdulillah kuning Deana hilang dan berat badannya naik tapi hati ini pengen ngerasain bisa DBF lagi sama Dea. Apalagi pas naik gocar mau ke Bandara dikasih wejangan ama bapak driver "anaknya jangan dikasih sufor bu, ASI baik" yang awalnya ngira saya kasih Dea susu formula lewat dot terus saya jelasin ini ASIP (terlepas dari ASI paling baik, memang ada beberapa kondisi yang anak emang harus dibantu sufor jadi jangan dipersalahkan ya), obrolannya jadi lanjut dengan logat chinesse si bapak sambil cerita jenis obat-obat herbal yang diminum biar ASInya banyak
ASI banyak sebenarnya bukan solusi (saya doanya semoga ASI saya cukup diminum Dea hingga 2 tahun). Saya sempat baca di sosmed kondisi demikian bisa jadi bikin anak susah nyusu dan tersedak, belum lagi komposisi asi itu ada foremilk dan hindmilk (beda keduanya baca disini) nah kalau oversupply bisa jadi ga tau dapat keduanya kah. Saya memang bukan hyperlaktasi (sempat di diagnosis) tapi Deana yang hingga saat ini sudah DBF belum berhasil ngosongin payudara akhirnya tetap harus pumping aja bikin saya kewalahan ngatur jadwal, dan PUP deana juga bisa berubah tekstur karena menyusunya yang masih singkat jadi kalau baca diinternet kebanyakan foremilk.
Oh ya pengosongan payudara itu penting, karena ketersediaan ASI itu bergantung darisana. Jadi badan akan merespon kalau kosong berarti harus diisi lagi. Jadi misalnih dibotol 100ml airnya habis semua otomatis bakal kita isi penuh lagi, tapi kalau ada sisa ya paling diisi setengahnya aja, atau bahkan gak diisi. Katanya ketersediaan ASI itu ya berkaitan dengan demand jadi rajin-rajin menyusui dan pumping. Selain itu yang terpenting katanya ibu menyusui harus bahagia, cukup nutrisi dan air. Sedangkan untuk asi booster itu opsional aja bahkan beberapa bilang jatuhnya lebih ke sugesti menambah percaya diri, belum ada riset secara ilmiah yang benar-benar pasti.
Menjalani fase mengasihi ini saya sampai membutuhkan pendampingan konsuler laktasi bukan karena ala-ala atau kebanyakan duit tapi sesulit itu ternyata proses mengasihi. Total ada 4 (empat) konsuler dan saya sangat berterimakasih atas pendampingan serta kesabaran mereka, khususnya Mbak Ida Sholihah yang sabar dan selalu ngecek keadaan saya, membalas whatsapp saya sampai pertanyaan terkecil warna pup deana apakah baik-baik saja. Saya juga bisa menjalani fase mengasihi karena suami, keluarga khususnya kak dhana yang sampai jejelin cara menyusui, teman-teman kayak indri dan emir yang jadi pendengar dan mengingatkan saya bahwa menyusui telah diperintahkan dalam agama dan berganjarkan pahala (pas dikasih tahu ini agak mewek dan semangat untuk mencoba DBF hingga nantikala harus penggunaan media lain) . Saya selalu mendoakan yang terbaik dan Allah beri ganjaran sebaik-baiknya ganjaran atas perhatian dan bantuannya.
Dari hari itu 4 januari 2024 setelah lagi-lagi penyumbatan dan pijat laktasi, terapisnya yang sekaligus bidan kasihan liat aku pakai spoonfeeder ngereview lagi apa yang mbak ida ajarkan, tiba-tiba malamnya Deana menolak menyusu lewat sendok walau memang Dea udah bisa mangap tapi sakit yang saya rasa masih sama karena deana ada tonguetie dan liptie yang masih butuh asesment lebih lanjut, bahkan dihari berikut-berikutnya saya selalu merasa serasa ditusuk-tusuk dalam tulang dan nyut seputar dada yang bisa ke tulang punggung.
Ternyata dari pada tulisan haru biru mengingat 1 bulan (padahal orang biasa ketika 6 bulan menuju MPASI, tapi buat saya dengan liku-liku ini 1 bulan sudah merupakan waktu yang luar biasa) berhasil menyusui secara langsung, lebih jadi kenapak tilas yang ilmu mengasihinya masih butuh banyak belajar. Tapi sampai ketitik ini selalu terselip maaf ke Dea karena mama kurang sabar dan mengeluh sakit padahal Dea yang paling kepayahan. Dan hingga hari ini saya sampai di titik pemasrahan diri dalam proses mengasihi "selama Dea tidak sakit, berat badan stabil, tidak lesu dan rewel, pipisnya cukup.. insya allah proses mengasihi bisa dikatakan berhasil, mari kita lanjutkan dan selamat mengasihi sembari selalu berdoa kepada Allah untuk kemudahan dan kelancaran prosesnya serta terimakasih bantuannya untuk Dea".
