Overactive Bladder
What is an Overactive Bladder (OAB)?
Overactive bladder
(OAB) is a urinary symptom, it is not a disease. The most common symptom is sudden, uncontrolled need or urges to urinate. Another symptom is frequent urination during day and night.
What are the symptoms of Overactive Bladder?
Leak urine or urinary incontinence, this means you feel a sudden urge to urinate, this is completely different than stress urinary inconsistency. In this case, urine leakage occurs during sneezing, coughing, laughing or doing other physical activities.
Urinary frequency
Wake up at night times to pass urine
What are the causes of Overactive Bladder?
A urinary tract infections (UTIs)
Idiopathic
Neurological disorders, such as stroke and multiple sclerosis
Diabetes
Abnormalities in the bladder, such as bladder stones, bladder cancer, GUTB
Enlarged prostate, previous operations to treat other forms of incontinence
Excess consumption of caffeine or alcohol
The decrease in cognitive function due to aging also makes difficult for your bladder to understand the signals it receives from your brain
Difficult to walk; lead to bladder urgency if you're unable to get to the bathroom quickly
Incomplete bladder emptying also leads to overactive bladder
Diagnosis for Overactive Bladder
Bladder Diary
The doctor will ask you to keep a Bladder Diary for a few days. With this, you will note how often you are going to the bathroom and how many times you leak urine. This will help Urologists learn more about your symptoms. It helps you track:
How much fluid you drink
How much you urinate
How often you have urgency feeling
How much urine you may leak
This information helps in treatment.
Other Tests for Overactive Bladder
Urine test: Sample of your urine is taken to test for infection or blood·
Bladder scan: Ultrasound helps to find how much urine is still in the bladder after you go to the bathroom·
Cystoscopy: To find bladder lesions.·
Urodynamic study: If, regular treatment of OAB, not relieving symptoms.
Treatment for Overactive Bladder
Medications that relax bladder are helpful for relieving symptoms of overactive bladder and reducing times of urge incontinence. Drugs used for overactive bladder treatment: Anticholinergic & B3 – Agonists.

Goodpasture syndrome (GPS) also known as Goodpasture’s disease, anti-glomerular basement antibody disease, or anti-GBM disease) is a rare autoimmune disease in which antibodies attack the basement membrane in lungs and kidneys, leading to bleeding from the lungs and kidney failure. It is thought to attack the alpha-3 subunit of type IV collagen, which has therefore been referred to as Goodpasture’s antigen. Goodpasture syndrome may quickly result in permanent lung and kidney damage, often leading to death.
Signs and Symptoms The anti-glomerular basement membrane (GBM) antibodies primarily attack the kidneys and lungs, although, generalised symptoms like malaise, weight loss, fatigue, fever, and chills are also common, as are joint aches and pains
Lung symptoms usually antedate kidney symptoms and usually include: coughing up blood, chest pain, cough, and shortness of breath. Kidney symptoms usually include blood in the urine, protein in the urine, unexplained swelling of limbs or face, high amounts of urea in the blood, and high blood pressure.
Causes Its precise cause is unknown, but an insult to the blood vessels taking blood from and to the lungs is believed to be required to allow the anti-GBM antibodies to come into contact with the alveoli.
Exposure to organic solvents (e.g. chloroform)     or hydrocarbons
Exposure to tobacco smoke
Certain gene mutations (HLA-DR15)
Infection, such as influenza A

Haematuria | Blood In Urine | Kidney Disease | AINU
Haematuria is the presence of blood in the urine. There are two types of haematuria
Gross haematuria: - Person can see blood in the urine
Microscopic haematuria: - Person cannot see blood in the urine, it can be seen by a microscope.
Causes The most common causes of haematuria include Infections in bladder, kidney, and prostate, vigorous exercise, menstruation, endometriosis, trauma Serious causes of haematuria include Bladder or kidney cancer, blood-clotting disorders, sickle cell disease, nephritic syndromes
Haematuria is most commonly seen in? The person who affects with an enlarged prostate, Urinary stones, strenuous exercise, bacterial or viral infection, and having a family history of kidney disease.
Symptom People with gross haematuria has a urine colour that looks like pink, red, or brown. A small amount of blood in the urine can change colour. Other symptoms of gross haematuria are blood clots in urine that can cause pain in the bladder or pain in the back. People with microscopic haematuria usually doesn't have any symptoms.
Treatment and Test Treatment depends on the nature of illness which has to be evaluated. To find how the blood passing of urine doctor might ask you for the urine sample this helps to find out how blood passing in urine due to kidney disease, infection or other problem. Health care professionals diagnose haematuria with a urine test called a urinalysis.

Bladder cancer
Bladder cancer more begin in urothelial cells that will be interior side of bladder, it is the dedicated, muscler organ in your bottom abdomen the urine will be stored. It is common in bladder this type cancer may effect other parts of urinary/bladder system.
 How bladder cancer will ocur
Step 1: The lamina propria we see bladder cancer in the inner grown through the lining of bladder cancer , it is the thick muscle wall will not spread through lymph nodes or etc..
Step 2: slowly this cancer will start spreading into muscles of bladder that is called inessive disorder/inessive cancer, but in this step cancer will not reach/attach other glands.
Step 3: in this stage it starts spreading although the muscles of fatty layers of tissues around bladder
Step 4:  The tumour has grown into the peri vesical tissue or has spread to the prostate, uterus, or vagina, but has not spread to the lymph nodes or other organs
·        Step 5: in this step it starts growing/spreading started in lymph nodes and other parts
Step 6: in this stage the tumour go through every glands and tissues of urinary parts completely
·        Step 7: slowly it reaches to abdominal parts.
·        Step 8: This tumour spreads other organs in the body.
 Types of bladder cancer
3 Types of bladder cancer include:
·        Urothelial carcinoma. It is also called transitional cell carcinoma, attack the cells inside the bladder, this cell expands when urine is there compress when urine is let out. This cancer is the common type of bladder cancer
·        Squamous cell carcinoma. It is the chronic bladder irritation and pain in bladder. This can cells are very rarely seen. It's more common in parts of the world where a certain parasitic infection  is a common cause of bladder infections.
·        Adenocarcinoma. This cancer begin in the cells of bladder and spread throughout fatty tissues of bladder. Adenocarcinoma of the bladder is rare cancer many parts of the wo.

Chronic kidney disease:
Chronic kidney disease indicates a gradual loss of kidney function. The primary function of kidneys is to filter wastes and excess fluids from the blood. When chronic kidney disease reaches an advanced stage, dangerous levels of fluid, electrolytes and wastes can build up in your body.
In the early stages of CKD, very few symptoms manifest themselves. Chronic kidney disease may not become apparent until your kidney function is significantly impaired.
Treatment for CKD focuses on slowing the progression of the kidney damage, by keeping the underlying cause in control. CKD can progress to end-stage kidney failure, which can be fatal without a kidney transplant or dialysis
 Causes of CKD:
 ·       Diabetes
·       High blood pressure (hypertension
·       Having a family member with kidney disease
 Stages of Chronic kidney disease:
There are five stages of CKD
This stage depends on the function of the kidney and how it filters waste and extra fluid out of the blood.
Chronic kidney disease is measured in GFR (Glomerular filtration rate) specifically, it tells the quantity blood passes through the glomeruli each minute. Glomeruli in kidney filter waste from the blood.

Paediatric Urology Paediatric Urology is a surgical subspecialty of medicine dealing with the disorders of children’s genitourinary systems. Paediatric urologists provide care for both boys and girls ranging from birth to early adult age. The most common problems are those involving disorders of urination, reproductive organs and testes.
common problems: Most people are unaware of the common urological problems of children. This is because they are not often discussed outside the family. Most people have heard of urinary tract infections, but do not know that these infections are associated with anatomical abnormalities in approximately 30% of all children who have them, and even a higher percentage in boys. Among the genital defects, abnormalities of the penis are the most common followed by undescended testicles.
care for Paediatric Urological Problems: Paediatricians often manage medical problems of the urinary tract and genitalia. Nephrologists are specialists in medical diseases of the kidney, and endocrinologists specialize in endocrine problems affecting the kidneys, like diabetes, and of some problems of the genitalia, like ambiguous genitalia. Patients may be referred to urology after seeing a paediatrician.
Some of the problems deal with are:
Bladder control problems such as bedwetting and daytime urinary incontinence
Undescended     testes
Hypospadias
Epispadias
Urolithiasis
Chordee  and other minor malformations of the penis
Phimosis
Urinary   obstruction and vesicoureteral reflux
Neurogenic  bladder (e.g., associated with spina bifida)
Antenatal  hydronephrosis
Tumours  and cancers of the kidneys
Repair of genitourinary trauma
Genitourinary  malformations and birth defects
Prune  belly syndrome
Cloacal  exstrophy, bladder exstrophy, and epispadias

The urethra is a tube that carries urine from the bladder so it can be expelled from the body. Usually the urethra is wide enough for urine to flow freely through it. When the urethra narrows, it can restrict urinary flow. This is known as a urethral stricture. Urethral stricture is a medical condition that mainly affects men.
Causes
·       trauma to the urethra
·       infection such as a sexually transmitted disease
·       damage from surgical tools
·       conditions that cause swelling
In most cases, no cause can be found. In adults, urethral strictures are most often due to:
injury from a fall onto the scrotum or     perineum
prostate surgery
kidney stone removal
urinary catheterization
other surgical tools
Symptoms
weak urine flow or reduction in the volume of     urine
sudden, frequent urges to urinate
a feeling of incomplete bladder emptying after     urination
frequent starting and stopping urinary stream
pain or burning during urination
inability to control urination (incontinence)
pain in the pelvic or lower abdominal area
urethral discharge
penile swelling and pain
presence of blood in the semen or urine
darkening of the urine
inability to urinate (this is very serious and     requires immediate medical attention)
Diagnosis Doctors may use several approaches to diagnose urethral stricture.
Reviewing your symptoms and medical history     You can self-report the symptoms mentioned above. Your doctor may also ask     about past illnesses and medical procedures to determine whether one or     more risk factors are present.
Performing a physical examination     A simple physical examination of the penis area can help the doctor     identify the presence of a urinary stricture. For instance, the doctor     will be able to readily observe redness (or urethral discharge) and find     out if one or more areas are hard or swollen.
Treatment There are many options depending on the size of the blockage and how much scar tissue is involved. Treatments include:
dilation – enlarging the stricture with gradual stretching
urethrotomy – cutting the stricture with a laser or knife through a scope
open surgery – surgical removal of the stricture with reconnection and     reconstruction, possibly with grafts (urethroplasty)
Asian Institute of Nephrology and Urology (AINU) is the best kidney hospital in India, providing world-class treatment for all your renal problems. AINU hospitals successfully completed 500 robotic surgeries and proven that they are the best kidney transplant hospital in India.

DIABETIC NEPHROPATHY
Diabetes is the most silent and devastating health issue which, if neglected can lead to many long-term complications like Diabetic nephropathy (affecting kidneys), cardiovascular diseases affecting the heart, Diabetic retinopathy (affecting the eyes), Diabetic neuropathy (affecting the nervous system) etc.
Among the diabetic complications, Kidney disease is a major complication as it can lead to end-stage kidney disease requiring dialysis or kidney transplantation.
How does Diabetes hurt or affect my Kidneys?
The main job of the kidneys is to filter wastes and extra water out of the blood to make urine. Kidneys also help control blood pressure and produce hormones that your body needs to stay healthy.
Kidneys have minute blood vessels that aid in filtering the blood. High blood sugar levels can make these vessels become narrow and clogged leading to decreased filtration capacity, causing the excretion of albumin (a type of protein) through urine. If diabetes is uncontrolled for prolonged periods, it leads to high blood pressure, which can further damage the kidneys.
Loss of protein in the urine is one of the foremost symptoms of kidney disease. If the corrective measures are not taken at this stage, it can progress over the few years leading to irreversible damage to the kidneys.
Diabetic Nephropathy Symptoms
·        High blood pressure
·        Protein in the urine
·        Ankle/Leg swelling
·        Excessive urination, especially during the night-time
Diagnostic Tests to screen for Diabetic Nephropathy:
·        Urinalysis (test to detect the presence of protein in urine)
·        Complete Urine Examination (CUE).
·        Serum Creatinine.
·        Estimated Glomerular Filtration Rate (eGFR)
·        Urine Albumin Creatinine Ratio
How to Prevent Diabetic Nephropathy
·        Control on Diabetes, Blood pressure, Cholesterol levels.
·        Avoiding High protein diet and strict adherence to Diet chart
·        Regular screening for diabetic nephropathy.
·        Lifestyle modifications like smoking cessation, weight control, avoiding consumption of alcohol, and adequate exercise.

What do the kidneys do?
Kidneys are bean-shaped organs; they are located below the rib cage.
Kidneys filter about a half cup of blood every minute, filtering waste and excess of water to make urine. Urine flows from the kidneys to the bladder through two tubes called ureters, one on each side of the bladder. Bladder stores urine. Kidneys, ureters, and bladder are part of the urinary tract.
Jobs that kidney do
1. Remove wastes and extra fluid
Kidneys act as a filter that removes wastes and extra fluid from your body. Kidneys filter blood to make urine. Urine contains wastes and extra fluid. This prevents the gather of wastes and fluid to keep the body healthy.
2. Control blood pressure
Kidneys need the pressure to work properly. Kidneys can ask for higher pressure if it seems too low, or try to lower pressure if it seems too high by controlling fluid levels and making the hormone that causes blood vessels to constrict.
3. Make red blood cells
Kidneys make the hormone called erythropoietin.  Erythropoietin helps the bone marrow to make red blood cells. Red blood cells carry oxygen from the lungs to all body parts. Red blood cells give the energy that you need for daily activities.
4. Keep bones healthy
Kidneys will make an active form of vitamin D. You need vitamin D to absorb calcium and phosphorus. Calcium and phosphorus are important minerals that make bones strong. Kidneys also balance calcium and phosphorus in the body has the right amount.
5. Control pH Levels
pH is a measure of acid and base. Kidneys maintain a healthy balance of the chemicals to control acid levels. As cells break down, they make acids. The foods you eat can either increase or lower the amount of acid in your body. Your kidneys balance the pH of your body by either removing or adjusting the right amounts of acid and buffering agents.

Overview
TURBT: Endoscopic resection of bladder tumors or laser fulguration of tiny lesions Robotic or open Radical cystectomy: Removal of cancerous bladder and surrounding lymph nodes. Urine diversion is done by an ileal conduit (urine tubes or ureters are attached to a segment of the small intestine and this is brought out through the tummy wall as stoma to which a urine bag is applied). In selected patients neobladder can be created where the intestinal segment is used to create a reservoir similar to urinary bladder, facilitating urine passage naturally.
