8 Important Facts about Dentist Insurance

Lol I have no idea what Shadowrun is, did a quick google and it’s something like magic + cyberpunk + vaguely futuristic post-apocalyptic setting + virtual reality?? Omg it’s too complicated to write just from reading the wiki lmao, I’ll just give you a cyberpunk fantasy AU.

1. Kisuke is your average humble toymaker in the Slums who may or may not supply the underground Robin Hood-esque rebel faction Visored with not-so-average weapons and prosthetics and various repairs and upgrades. The Visored are pretty much wanted in every kingdom in existence, partly because half of them used to be nobles themselves and were part of the minority who hated the way they treated their citizens, mostly because they keep fucking with the other nobles, but no one except said nobles actually wants them to stop because everybody who isn’t nobility pretty much hates those who are. Mostly, it’s the three neighbouring kingdoms - Seireitei, Silbern, and Las Noches - sitting cozy up in their walled off flourishing cities up high, constantly at war with each other but with plenty to eat and plenty of money to fund their large-scale chess games, and paying almost zero attention to the poor and homeless outside their walls. That doesn’t stop them from forcibly conscripting the lower class as cannon fodder or using the Slums as their personal dumping grounds or imprisoning or executing anyone they decide is breaking one law or another. Kisuke’s stayed under the radar so far, so nobody knows he’s the man even more wanted than the Visored, if only for his prodigal skills with cybertech. He’s the one who built their equipment and vehicles, who repaired Hiyori’s spine after she’d been left unable to walk from an ambush and produced a new arm for Hachi after it was cut off in a skirmish, both of which work just as well as the original parts. Most of the nobles either want to kill him or “hire” him. But a toymaker in the Slums who cobbles together tiny cats and dragons and chickens and pixies out of scrap metal for children to play with isn’t anything to look twice at.
2. Here is a secret only a handful of people know - Shihouin Yoruichi was born a cripple. She couldn’t walk, at all, and even the best prosthetics money could buy from the various cybertech companies were clunky and awkward and only allowed her to limp a certain distance. Her family, one of the great noble houses of Seireitei that specialized in seduction and assassination, hid her away out of shame, right up until a rebellious teenaged Yoruichi had had enough and snuck out one night on nothing but her wobbly fake legs and a crutch. She’d spent enough time on her own for the majority of her childhood to know just about every passageway and secret door that snaked through the length and width of Seireitei. She didn’t stop until she appeared in the Slums, and she fainted from exhaustion and hunger only a few days later, but she never looked back. Kisuke found her, took her in, and then made her legs on a whim, upgrading them every time he figured out something new and better that he could add to them. In exchange, Yoruichi used her newfound mobility to retrieve better materials for Kisuke, robbing delivery trucks en route to Seireitei’s cybertech companies or outright stealing from her own family’s weapons storage. Anything they could buy, Kisuke could reverse-engineer and make better. Ten years after she left her old life behind, her legs are a work of art, connected to her nerves to give her complete control over them but granting her superhuman speed and jumping ability, and she’d practiced enough with them over the years that her mind had no problems keeping up with both. The prosthetics are lightweight but strong enough to withstand the swing of a blade or the impact of a bullet, and she would give a lot to see her family’s faces if they ever realize just who has been ransacking their vaults.
3. Most people carry some kind of weapon these days, but the best - for those who can afford them, or can call Kisuke a friend - can take the form of a companion when not in combat. Shinji’s is in the shape of a sphinx, all sleek lines and feline flexibility, but one that shifts into a sword in a silent whir of pulsing blue lines and polished metal at his command. Mashiro’s is a pixie, not unlike the toys commonly seen in Kisuke’s part of the Slums, except hers includes translucent wings threaded with pale green wiring. It’s perpetually perched on her shoulder, but in a fight, the pixie fuses with her hands and legs, the wings melting and sliding over her skin like liquid mercury to form gloves and boots that increase the power of her kicks and punches.
Yoruichi’s is a black cat but nobody actually knows what kind of weapon it can turn into. More often than not, Yoruichi sends it off as a spy because the thing is so realistic nobody can actually tell it’s not a real animal unless they get close enough to see the delicate wiring in its yellow eyes.
Nobody’s ever seen Kisuke’s either, weapon or otherwise, until a spy from a cybertech company snoops too closely around his shop. Then the other occupants get front-row seats to the bright red threads that extend from his hands - hands that light up with the many, many upgrades inside, a complicated maze of crimson circuits swirling beneath his flesh - and attach themselves to their target like strings on a puppet. At least he takes it out back before he literally rips the spy apart.
4. Once upon a time, before Yoruichi was even born, there were five noble houses instead of four. But the fall of the Shiba Clan is never talked about, and most don’t even remember the details anymore, only that most were put to the sword and the rest were scattered. One of the runners in Kisuke’s employ - the many who scrounge through the Slums’s trash heaps for parts Kisuke might find useful - is a boy on the cusp of twenty who looks uncannily like the last Shiba clan head before the family’s collapse. He goes by Ichigo and doesn’t seem aware of his lineage, and if he notices the way Shinji almost always makes an appearance when he comes in with his haul, and his payment always ends up including several extra portions of food and some high-grade medical supplies and even a new change of clothes now and then, he never says anything. After they find out he has two little sisters to feed, a handful of toys get bundled in as well, free of charge.
Kisuke wouldn’t know a Shiba from a Shihouin, and Yoruichi’s family never bothered teaching her all the things an heir or even just an average noble-born child would’ve needed to know, so neither of them treats Ichigo differently because of his blood or background. They do treat him differently because none of Kisuke’s runners have lasted as long as Ichigo. Sooner or later, they disappear, arrested by guards or killed in a back alley scuffle. Ichigo slinks into the shop at fifteen and still comes around every week like clockwork five years later. He always shows up with a decent haul too, and once, Yoruichi follows him, just to see where he’s getting his loot because surely most of the trash pits in the area have been picked clean over the years? There’s always more added to them, but not at the rate Ichigo is scrounging materials. So Yoruichi follows him one day when he leaves and that’s how they find out about his sisters and the makeshift hole in the wall they live in, shabby-looking on the outside but clean and cozy on the inside and insulated well from the cold. That’s also how they find out about all the enhancements Ichigo has, because Yoruichi makes the mistake of underestimating him and almost gets beheaded when he disappears and almost shivs her from behind with a hand-turned-blade, teeth bared like an animal as his eyes burn with golden circuitry.
(The Shiba Clan had been widely feared, once upon a time, for their genius in the more explosive weaponry and their talent with artificial intelligence and robotics and other biological cybertech enhancements. It was why they’d been so swiftly sentenced to death when they’d come down on the side of the poorfolk. Even one Shiba would’ve been equivalent to having a small army in one’s arsenal.)
Ichigo moved faster, jumped higher, hit harder, than anything Yoruichi had ever come up against. The crack of his heel coming down against the ground shattered rock and cement everywhere, and the only thing that saved her life that day was her dodging ability and a quickly shouted explanation for why she’d followed him in the first place. Ichigo wasn’t unreasonable, even if he wasn’t entirely human. His enhancements explained how he could move further through the Slums for loot and still put down roots in the area. It took some coaxing and several dozen more months of coming and going from the shop, but eventually, he’d also admitted that he didn’t know where his enhancements had come from, he couldn’t remember anything from before waking up the Slums with two regular human toddlers who called him brother depending on him. The only thing imprinted in his memory were the directives: 1) Take Care of Your Sisters, and 2) Survive.
But he was the most powerful thing around for miles, and Kisuke was fascinated because the work done on Ichigo was only vaguely like his own, and far more advanced than anything the nobility churned out these days. Yoruichi didn’t care as much, but she liked having a new sparring partner, not to mention Ichigo was very easy on the eyes, and a few more years on him meant Yoruichi could appreciate the sight without feeling like she was preying on a child.
Ichigo kept coming back, and eventually Kisuke managed to wheedle Ichigo into getting a checkup and upgrades, especially when he started outgrowing a few of his joint ports. Yoruichi watched the two of them make moon eyes at each other, listened to Kisuke ramble about something Ichigo told him the day before, noted the way Ichigo’s eyes sometimes strayed to Kisuke when the man wandered outside without a shirt and his pants on backwards after too many hours in his lab, and she was almost tempted to lock them in a closet together.
(She doesn’t notice the way Kisuke smiles indulgently at her when she comes home from a trip into Seireitei with an icebox of fresh strawberries from the Kuchikis infamous gardens because they’re Ichigo’s favourite, nor does she see Ichigo blink and cock his head in new understanding sometimes when he observes the way she drapes herself over Kisuke, comfortable and relaxed, but never does it with anyone else.)
In the world they live in though, trust is more important than love. Yoruichi has trusted Kisuke since she met him, and Kisuke’s trusted her since she was down two legs and still flung herself between him and a thief with a knife who thought the shop easy pickings. And the day Ichigo brings his sisters over and lets them run around out of his sight is the day they know he trusts them. It’s only natural to offer him and his little family a room of their own at the shop.
5. The day Yoruichi comes back with news of the Silbern Kingdom’s royal family and Las Noches’ royal family both being overthrown by several of their own noble families - the Ishidas and the Kurosakis, and the Coyotes, the Tu Odelschwancks, the Cifers, and the Jaegerjaquezs respectively - is the same day Shinji comes to them and tells them about the revolution movement that’s been in the works for a while now, about the remains of the Shiba Clan currently helping the Ishidas and Kurosakis take over Silbern, and about Ichigo’s own past - memory wiped for his own good because rumours of a Shiba child successfully integrated with his clan’s still experimental but groundbreaking technology had leaked, and if they’d gotten their hands on him, they would’ve turned him into their weapon. Better to hide him in the Slums, along with his two sisters who wouldn’t be of any use in a war for several more years, until they need him again, which they do now, because as soon as Silbern and Las Noches are theirs, they’ll be moving on to Seireitei post haste, and a two-pronged attack while the Gotei is still scrambling to defend themselves would hit them hardest, because for all that the kingdoms have been at war with each other for years, it had never been so direct, nor had their goals ever moved beyond poaching each other’s technologies. But for the revolution movement to succeed, they need Ichigo on their side, and it wouldn’t hurt for Kisuke and Yoruichi to join them too, technically Kisuke’s been their weapons-backer for years, and Yoruichi’s been their ear to the ground in Seireitei for just as long, and they’ll need all hands on deck. The kids can be left with Tessai.
Ichigo storms out. Yoruichi demands to know why they were never told before. And Kisuke surveys a tense-looking Shinji (who explains that it was supposed to be for their safety too - because Yoruichi was their only successful spy in Seireitei, and very, very few could match Kisuke’s genius, and it was just better to keep them out of the way) from beneath his hat before smiling blandly and promptly catching the man with a flick of his hand and five threads, unceremoniously tossing him out the window before he and Yoruichi both go to find Ichigo. It doesn’t take a genius to guess he’d returned to the hole-in-the-wall home he’d made for his sisters, and they join him after Ichigo acknowledges them with a jerk of his shoulders. They don’t speak right away, Ichigo sitting in stony silence, Yoruichi curled on one side of him still seething, Kisuke on his other, absently flexing one red-tinted hand in that way he only does when he’s contemplating murder.
They’ll help, all three of them. They don’t even need to discuss that. It’s high time for the upper-class to get their lives shaken up, the Slums are a disgrace, and if they have the chance to change that, they’ll take it, even if it means working beside people who have been using them for their own ends without giving them so much as a heads-up. Or in Ichigo’s case, will be using him since it’s pretty apparent he might not be the Gotei’s weapon but he is still very much the Shibas’ weapon, reserved for emergencies.
“Regimes come and go every day,” Kisuke remarks first, right hand fanning open, then closing, then opening again, crimson circuits shimmering along the vein lines of his palm.
“What a shame,” Yoruichi agrees with a grin that’s two-parts teeth and all-parts spite.
“…Three of us against three kingdoms that’ve just taken a beating?” Ichigo muses, but his eyes flare gold, and he’s smiling too. “Sounds like fun.”

Phalloplasty Consultation with Dr. Chen & Dr. Watt
Here is what I remember from my experience going through my consultation with Dr. Chen from GU Recon (his private practice) and Dr. Watt from the Buncke clinic in San Francisco, CA, USA. My apologies for the length, I wanted this to feel kind of immersive for those of us still in the waiting process because stuff like this helped me. Also - small content warning I do use a couple anatomical terms.
For those of you unfamiliar, Dr. Chen is a urologist and Dr. Watt is a microsurgeon.
My partner and I traveled down to San Francisco to stay for two nights. We flew in on Thursday evening and flew out on Saturday evening, not wanting to be gone for too long and rack up even more expense on the hotel bill. If I had planned ahead several months ago, I would have tried to stay at the Quest House during this time but I didn’t realize a short stay was potentially possible.
We are fortunate to be able to use the public transit system offered in SF, which is pretty good in my opinion. They have buses, trolleys (cable cars?), and an underground/train system. We utilized this to make our way over to Castro street, where the medical office building and also the hospital are located.
Without any plans for the day we went ahead and headed over about 3 hours early because I had seen that there was a dog park right nearby. We sat and watched local dogs come and play and have a break in the middle of the day and we ended up meeting an older gentleman whose dog wouldn’t leave us alone asking for pets. It was pretty great, and nicely calming as I was pretty nervous before the consultation. We then got some food at the local cafe on the corner. It was actually pretty good.
We realized we still had time to kill so we decided to hike up the hill to the Buena Vista park where we looked out over the city and rested for a bit. There’s a path that has some disturbingly friendly squirrels on it.
About 30 minutes before my appointment headed over. Inside the medical office building, Dr. Chen’s suite is right across the hall from the Buncke clinic. I wasn’t sure where to go to check in, so we walked all the way down to the entrance of GU Recon and saw the door was open. Inside the waiting room was fairly spacious with comfy seating and plenty of random coffee table books to peruse. There was nobody else there at the time. At the receptionist window was a sign indicating to check in over at the Buncke clinic, so we quickly hopped across the hall.
The Buncke clinic waiting room was much smaller and was actually quite crowded for a Friday afternoon. I checked in and they asked for my ID and insurance card (even though I had sent in pictures), and they didn’t ask for any kind of copay or payment. I suspect I will receive a bill at some point for the specialist copay from my insurance which is $30. Hopefully.
They instructed us to head back over to Dr. Chen’s office to wait, so we went back over and started looking through a photography book. At this point I was still about 25 minutes early to the appointment so I was ready to wait however long it would take.
About 5 minutes later, Dr. Chen himself appeared behind the reception area with pizza and Starbucks in hand, apparently not expecting anyone to be in the waiting room. He noticed us right away and began apologizing for the wait. He explained the schedule didn’t indicate whether or not we were having a phone consultation, so he just assumed it was going to be over the phone based on my address.
This whole interaction solidified every good thing I had heard about Dr. Chen, and I immediately felt so… normal. That’s the best way I can describe what I felt. I felt like I had known Dr. Chen for years and that he was.. reachable. Human.
He told us it would be a few minutes, and sure enough a few minutes later Dr. Chen and Dr. Watt appeared at the door and we made introductions. My partner came with me because I wanted her to hear what the doctors had to say and I wanted another pair of ears listening, and also because I wanted the doctors to see that I had support.
We went down a narrow hallway and went into Dr. Chen’s office, which hosted another comfy couch which he had us sit on while he and Dr. Watt sat across on office chairs. They each had some papers (my medical information). The room was somewhat dimly lit, but calming and comfortable.
The consult started with Dr. Chen confirming my reason for the visit (seeking phalloplasty), and he asked me how important it was to me to stand to pee. I explained that my personal goals were 1 - Sensation, 2 - Stand to pee, 3 - Aesthetics, and 4 - Sexual function. Which, again, are personal goals and it is completely valid to have other priorities with lower surgery. This is my own journey.
We then went over my medical history, which is fairly short, but Dr. Chen was thorough and asked me about my minor eczema, asthma, and migraines. Dr. Watt was quietly taking notes and listening during this time. Next, they asked about any trauma to either arm and I basically explained how my right arm is essentially immediately disqualified from being a donor arm. In my specific case, I broke my right arm when I was 18 months old and had to have a surgical repair. This repair didn’t heal correctly and now my arm when extended is quite crooked.
This has put some strain on my ulnar nerve and gives me hypersensitivity in my palm. Further, I had a different surgery on my forearm which involved an incision and left me with a scar right in the middle of the graft area. This could compromise the blood supply, so we pretty much immediately dismissed my right arm as an option. To top it off, it is my dominant arm for most activities. I kind of would have preferred to keep my left arm nice and clear of any scars, but I think having 1.75 properly functioning arms is preferable to only 1 functioning arm in case my right side nerves ever gave out.
Next, Dr. Chen went on to explain his portion of the surgery - he starts with the vaginectomy and then relocates the end of the urethra to the natal phallus using labia minora tissue. He then mobilizes this and relocates it to the other side of the pubic bone to come out to the site of where the neophallus will be placed. At some point during this discussion, Dr. Chen explained the complication rate and he was both realistic and optimistic about it. He said the vast majority of complications that happen are fixable. Further, the most common complications often heal on their own. I can’t remember the exact numbers, but he said of the patients that do have fistulas, only around 20% of them end up needing a surgical fix. Strictures don’t show up right away, and usually occur within the first year.
While Dr. Chen does the work down below, Dr. Watt explained his team mobilizes the RFF and prepares it for the new location, using the tube-in-tube method to create the urethra and phallus. Dr. Chen places a foley catheter through the neophallus and into the relocated urethra to line everything up, and he sutures everything together once the microsurgeons connect the blood supply and nerves. Dr. Chen then places the suprapubic catheter and the RFF site is covered with the split-thickness graft from the leg. If requested, Dr. Watt would place integra on the RFF donor site before the split thickness graft (not staged like other teams).
They then explained what recovery typically looks like - 5 nights in the hospital, including 4 days of strict bed rest and then up and walking on day 5. If you’re able to walk well enough, you get to leave to recover elsewhere. They then check up every week for four weeks before sending you home. During your 1 week post-op visit, Dr. Chen removes the foley catheter from the neourethra complex. You start your peeing trials just before the 3 week checkup, and if you’re able to empty enough of your bladder the SP catheter can be removed. If you have significant fistula(s), an additional week for healing may be allotted and the SP catheter retained for that time.
Dr. Watt then did an exam of my arm, performing the Allen’s test to see if my hand receives enough blood if the artery they harvest for the RFF is removed. The test seemed really quick, but I guess with how fast my hand refilled with blood he was very confident I was a candidate for RFF. He indicated that no further testing of my forearm blood supply was needed.
He examined the hair on my forearm, which turned out to be really funny because while he was looking at it he guessed that I had undergone electrolysis up to about 6 inches down my forearm. I laughed a little and explained nope, I just haven’t grown hair there in my ~5 years on testosterone. He gently pinched/grabbed the skin to see the thickness and said they’ll likely delay my glansplasty, and when he looked at the underside of my arm where the urethra graft would be taken he said I was basically hairless there and that any electrolysis at this point would just to be to remove hair from what will be the outside of my phallus, which is optional and he said I can always shave or use something like Nair.
I then had a chance to quickly look over my questions to try to find any that hadn’t been answered. They were pretty thorough so the most I asked about was about Integra because I was most curious about it. Dr. Chen then explained that he needed to do a quick visual exam of the genital/mons region and we walked across the hall to an exam room.
He apologized for the discomfort and had me just quickly drop my shorts while standing. All in all I think it took about 5 seconds of exposure. When we got back into the other room he reported to Dr. Watt something along the lines of “minor prominence” of the mons. I checked my questions one more time and asked if they had any testicular implants that I could feel, but Dr. Chen explained that he had a patient waiting that was somewhat urgent and he promised that he would show me next time. He was very polite about it and I understood, and all in all I think the consultation took about 30 minutes.
We said our nice to meet yous and goodbyes and Dr. Chen showed us out the shortcut out of the clinic and boom it was over. Despite the quick ending, I still didn’t feel rushed out of there and felt like they really took the time to make sure I understood the surgery and that my possible concerns were heard.
All in all I left feeling really good, which for me was everything. I was actually excited about the future. Also, they said they would be forwarding my information to the phalloplasty team about our consultation, and that they should be reaching out to schedule with me. What ended up happening was I emailed Logan with a follow-up question and after we emailed back a forth a couple times, Logan asked me if I wanted to set my date. So now I am officially on the books for Left RFF Phalloplasty and words cannot describe how much joy/relief/excitement I feel about it.
Like, I still can’t believe I get to do this and I don’t know when reality will set in. But for the first time in months, I am hopeful and optimistic about the future.
Edit: I forgot to mention that Dr. Chen also will perform a scrotoplasty during his part of the procedure.

Currently being outfitted with lenses by a specialist - they're not helping nearly as much as they're supposed to. What should I realistically expect?
Hi guys and gals - I could use your opinion. I'll try to keep it short. I was officially diagnosed with keratoconus last March after years of undiagnosed progression and I had my left eye epi-off crosslinked in May, as it was much further along. Progress seems to have stopped (my next checkup is at the end of May, but so far the 3-monthly appointments have been encouraging), so that's good. Here's the thing. I finally managed to get an appointment with a well-known keratoconus contact fitter/ophthalmologist after not having much success with the first specialist I tried and... well, sclerals are not helping much. It changes the ghosting a lot and I'd probably be better off if I got used to it but it's nowhere near good vision My specialist seems to be somewhat confused - he says that my keratoconus progression isn't that bad and I should be getting 20/20 vision out of the eye with contacts. We've been having a rough time fitting them since my eyes are tiny anyway, but we've pretty much tried all the lenses he had (and he has a lot) and there's barely been any improvement in the ghosting (my main problem, sharpness isn't that much of an an issue). What should I expect from scleral fitting? Is he doing something wrong? I don't think so because he's pretty much THE specialist to see in my country and he seems very dedicated, but I'm wondering if this is normal. How much do contacts reduce your ghosting/improve your vision? He's going to arrange an eye CT for me this week because I haven't had one yet and he says he wants to check for anything the other doctors might have missed, so there's that, but I'm kind of freaking out. My right eye is mostly fine but some minor ghosting has already started... my main doctor said we should keep an eye on the progression for now because I'm 33 and the process might stop on its own, but I've noticed more ghosting in my good eye than a year and a half ago, so I don't think it's stopped. The starbursts are still minor, but I've gone through this with the other eye and I recognise it when I see it. If contacts don't help me at all... well, maybe it's time to crosslink ASAP? I've seen a few keratoconus specialists and opinions seem to be all over the place - some are cautious and tell me to wait in case it stops on its own because complications are always something to consider, some are telling me to go for it as right now this is literally the only way progression can be stopped before my vision gets too distorted, and that this is as good as it gets. I feel like I'm stuck between a rock and a hard place and I'd love to hear from anyone who's gone through this. Ugh, I'm rambling. I'm a little discouraged because I put a lot of hope into contacts and they've been a letdown (we'll keep trying, he hasn't given up yet). Any input is welcome. I went to Dr. Pounds for my regular checkup and to deal with a very minor issue on May 25, 2017. When I got the claim in the mail at the end of June, I found that I was responsible for $1946.27 in lab costs.
This seemed a bit high for a regular checkup and minor issue. So, I called Humana at the end of June, and Stacy told me that my PCP had not sent in the proper authorization form. She suggested I get in touch with my PCP's office and have them fax an authorization request form and that Humana would re-visit the claim. She gave me the fax number and said I had to wait 30 days for it to be reprocessed. Dr. Pounds' office faxed over the necessary forms, and I waited the 30 days.
I called Humana again on July 30th to follow up. The representative I spoke with told me that my PCP had not faxed in the authorization request form. So, I called Dr. Pounds' office, and they said that they had faxed the authorization request form, but that they would fax it again, which they did.
I called Humana again on August 31st and spoke with Tanisha. Tanisha revealed to me that there were actually two problems with this claim, one of which was the authorization forms. The other problem, Tanisha told me, was that Dr. Hannah Pounds had not been my PCP on May 25th (the date of service), and, therefore could not submit authorization forms on my behalf because she was not my doctor. That Dr. Butler had been my PCP on paper at the time, and she, at the time, was the only one who could submit authorization forms on my behalf.
To unpack this: When I changed PCPs online way back in April/May, Tanisha says the change did not take effect until June 1st. So when Dr. Pounds' office faxed Humana authorizations, Humana ignored them because they were not sent by the “right” doctor (my previously assigned PCP, Dr. Butler, whom I have never seen because she was not accepting new patients). Tanisha was the first of Human's staff to elucidate me on this fine point. I had been dutifully waiting and worrying and faxing for two months for the claims to be reprocessed when they were simply being ignored because the “wrong” doctor was sending in requests on my behalf.
To further complicate, a representative of Dr. Pounds' office re-faxed everything on September 5th and spent a total of two hours on the phone with Humana trying to help me. It was only when I spoke with Holly at Humana on September 12th that I learned that all of this had been a waste of time.
It seems like there was some kind of breakdown in communication.
On my part, I just assumed that the online change of PCPs was automatic from the moment I chose Dr. Pounds because: 1) she is In Network, 2) she is a primary care physician and 3) this is the age of the Internet.
Here is a heartbreaker: Holly told me that had I made the change of PCPs over the phone, it might have happened immediately (fine print: if I had called before the 15th of the month). But, because I changed PCPs online, the change did not take effect until the month following when the change was requested online. A fine detail of which I was not aware. I wonder how many other patients have been caught up in this bureaucratic shell game, and who is benefiting. The reality, however, is that I am not a human trying to see a doctor. I am a number (H72489189) who's family represents $708 a month until the end of this year.
Another little tidbit that happened today Sept. 25th: My husband, who is also a Humana member, needs to see his PCP fairly soon. His PCP is also Dr. Butler, whom is still not taking new patients. I called Humana four times today to ask what I should do. I have been hung up on by the automated system four times today, so I had to resort to Humana's twitter help to get an answer. Twitter help referred me to Humana's “on demand” service. The On Demand service suggested:
“On your plan you do not need to obtain a referral to see an in-network provider that is not your primary care physician. With that in mind, as long as you are able to obtain an appointment with someone listed in your network, please do. This includes specialists, urgent care, retail clinics and other primary care physicians. If you need lab work done, make certain the lab the specimens are sent a participating laboratory. Quest Diagnostics and LabCorp are the preferred laboratories.” If I unpack this and correlate it with my own experience, I can see another In Network doctor, but they cannot authorize out of network labs if they are not my assigned doctor. Actually, Dr. Pounds' staff were really appalled that the lab they sent my blood to was out of network. The fact that Dr. Pounds is In Network but the lab is out of network (which she did not know) and that I as a patient have to manage my care to make sure that my blood is going to an In Network lab, etc. is just a slap in the face. Even if I had asked about the lab, Dr. Pounds was not aware that the labs were out of network, and this is why her staff was on the phone to you for two hours.
This situation has caused me a great amount of stress, I've lost hours and hours of work on the phone with Humana and my doctor trying to figure this out, and I have not gone to the doctor for necessary followups because I'm afraid of getting a huge bill for something very, very minor. Also, my husband insists that we pay cash to the doctors we visit in future and not run anything through insurance because we will get slapped with costs. So why do we have insurance if we are not going to use it? Add in the additional work my doctor's staff has done in trying to help me, and this regular checkup has ballooned into a cost of over $5000. I estimate my own administrative costs to be around $1500 if I use my day rate.
I ask you to look to the perception: that Humana will do whatever paperwork shell game that needs to be done to deny claims until it can extract itself from the individual market and not have to deal with individuals anymore.
I respectfully ask you to re-submit this claim and to make this right. I am happy to pay what would have been the cost of the laboratory tests In Network through my PCP. Anything above that cost is simply not fair. Something is seriously wrong with healthcare when a healthy person would go to see a doctor for a very minor issue and come out owing nearly $2000 on top of the $708 she pays in premiums each month and the co-pays she made to her doctor just to even walk into the office. I don't have to point out to you that patients are already in a pickle when it comes to healthcare costs. We must submit to examination and tests BEFORE knowing how much it will cost us.
