Day 3: Missed Chances
Fandom: Free!
Pairing: Yamazaki Sousuke x Tachibana Makoto
Wordcount: 716
Day 1 | Day 2
Even as years passed, Makoto couldn’t help but think of how he met Sousuke on that rainy morning whenever September would roll around again. He’d also always thought about what could have been and wondered what happened to Sousuke—a lot of changes must have taken place with the time that went by. Maybe his injury healed. Maybe he completed his residency. Maybe he left the country. Maybe he forgot about the nursing student he met at Yowa Hospital.
Makoto, on the other hand, never forgot about him. He never could; Sousuke was one of the first people he had built rapport with as a nurse. As a result, he never thought of him as only a patient he’d helped. He wondered if he was just a nurse to Sousuke, though. He honestly wished he remembered him as something more and thought about him sometimes. Maybe even wondered about his current circumstances and whereabouts like Makoto would with him. But that was where Makoto was conflicted: he would hate for Sousuke to learn the reality of what had been going on in his life.
In reality, there was a lot of not much going on. Makoto had missed out on a lot since he dedicated himself to his job and committed to gaining recognition for his services in Tottori University Hospital’s nursing team. He had remained single all this while, and it was only partly due to his career ambitions to rise up the ranks to become a nursing manager. Maybe, somewhere in the back of his mind, he had held onto the hope of seeing Sousuke again—a man who happened to have everything he wanted in a partner.
Maturity. Dependability. Consideration. He sometimes wished he had never known more about Sousuke than what meets the eye; the height, the voice and those eyes would have been enough. Now he was stuck on the idea of having someone that just seemed too good to be true.
Too good for him, at least.
So when Sousuke finally appeared in his life again, Makoto was too stunned to properly react.
Why would Sousuke remember him, right? Makoto was only a student nurse back then, helping out in a couple of his many weeks of therapy sessions. Makoto didn’t even dare to probe about his injury at the time, for fear of being seen as a busybody. Strictly speaking, they were never friends.
So when Sousuke introduced himself in TUH as Dr Yamazaki, the new gastroenterology fellow, Makoto greeted him like it was their first time seeing each other. That would be the first of many missed chances he had to acknowledge his recognition of Sousuke from his time in nursing college.
It only became increasingly difficult to breach the topic from then on.
After all, what would he say?
Do you remember that student who was briefly there for your PT almost seven years ago? Hi, I’m that guy. Fancy seeing you here at TUH!
Yeah, no. He’d cringe just imagining that. While he was glad—over the moon, actually—to meet Sousuke again, he struggled to bring up their history, seeing as any attempts he had made to reach out were simply… not taken any notice of.
Katsumi-san had very kindly given him the phone number, saying that Sousuke had authorised him to do so. At that moment he had hope fired up in him: Sousuke had wanted to be friends! With time, could they develop into something… more?
At first, it was a courteous but casual text, saying hello and notifying Sousuke that he was stationed at the psychiatric ward now. No reply came.
Then, Makoto had let him know that he ended his time as a student nurse at the hospital and would be proceeding with his prep for the National Nursing Examination. That received no response either.
Finally, Makoto informed Sousuke that he had successfully gotten his nursing license. That was when his hope truly died, because he’d thought a congratulatory message would be anyone’s reply. Instead, all he got was radio silence.
Yeah, they were never friends. That thought would cause Makoto to have this constricting feeling in his chest, so he never liked to think about it for long.
It was a shame, but the past was supposed to be left in the past, anyway.
To be continued on Day 4

hi, curious question: what do you do for a living? Because how do you find the time to be posting and sourcing pictures? Cus I don't know how to balance that between work and this.
I work for a world renowned healthcare facility (top hospital in my state and among the very top in the US) in the Gastroenterology department as a scheduler. I get to work from home and I schedule colonoscopies and upper endoscopies (among other procedures) all day long. I just work a normal 8.5 hour shift, so since I have no travel time home (because I already am home), I can immediately transition into my personal life.
I rely heavily on Tumblr's queue feature, so I can dictate when and how often the site posts pictures. Right now, it posts 1 picture every 20 minutes. The queue currently has over 1,000 pictures, so I could step away for several days and the site would continue to run like normal, so it's not like I sit on the site for several hours every night. Usually, I upload one collection per day. A collection can be anywhere from 20-150 pictures.
Everybody's life is different, so what works for me might not work for everyone else. It takes awhile to build up a steady rhythm. In the beginning, I was adding dozens of collections a day, tagging them with dozens of different tags, trying to reach as many "audiences" as possible. Now, I just tag the pictures with the pertinent info because I'm lucky to have an amazing following who reblog to their audiences.
This isn't my first big blog though, I had a popular blog back in 2010-12 which I will not tell you the name of lol--not that I'm ashamed of it or anything, but just because I'm certain it's possible some of you might know me lol and I'd just like to remain anonymous this time. But, it gave me plenty of experience!

Hi , first off congratulations on your mrcp! Its massive ! 😍 Wishing you more happiness and success.
So i am to embark on this same journey. I did my mbbs and residency in Pakistan and then cuz of some life circumstances had to take a break. Because of which i lost my connections as well so basically dont have a friend who i could ask this stuff.
I would be very grateful if you please guide me about these things
1. Is it true that i only need attested copy of my primary medical qualification for the application of mrcp1 ? The website doesn’t mention any other documents but dont they need proof of internship?
2. I am doing passmedicine questions and plan on doing pastest as well. I turn to other sources when i wana read more on the topic. Do you think there are any facebook groups etc that can be beneficial? Or should i just keep going.
3.Any other tips you want to give would be much appreciated
Thank you in advance. Love and prayers for you.
Hi, thank you and good luck to you!
I'm not sure. I think I remember sending a copy of my MBBS cert for part 1? But it may be different requirements for you if you're not working in the UK. I'd suggest emailing them to make sure.
I basically just used passmedicine and pastest for part 1 (and part 2), I didn't use any facebook groups. A mix of the two is definitely good because they have slightly different styles of questions. Keep doing questions and make notes on the stuff you get wrong/don't know - that's what I did anyway. Definitely make sure you do the official mock on the MRCP website at some point as that's the closest you'll get to the real thing https://www.mrcpuk.org/mrcpuk-examinations/part-1/part-1-sample-questions
Another tip I have is look at the breakdown for the number of questions for each topic, on this page https://www.mrcpuk.org/mrcpuk-examinations/part-1/format. This is how I decided which topics to spend more time on - no point stressing too much about learning all of ophthal when there's only going to be 4 questions on it! Prioritise the big topics. I would also suggest that at least at first, filter the questions on the question banks by one specialty at a time. That way, you will learn key concepts quicker because of the repetition. If you start by just doing all the topics in one go it'll be hard to focus on learning the concepts because it'll be overwhelming. So e.g. I'd do one day haematology, next day gastroenterology. Then later on, once you're more slick, do all the topics together.
Good luck!

fucking hell. turns out ozempic/wegovy/etc is giving people gastroparesis. they are saying "extreme cases" are rare so far. who knows what the definition of "extreme" is. and at this point in my life I am automatically skeptical of declarations that medical conditions are rare. to wit:
In clinical trials, nearly half of people, 44%, who took Wegovy reported nausea, and almost 1 in 4 reported vomiting; both are common symptoms of gastroparesis. In the clinical trials for Ozempic, which is the same medication as Wegovy but given at a lower dose, 1 in 5 people reported nausea and 1 in 10 reported vomiting.
there would be a kind of folkloric poetic justice about this, it's very just-so story, especially regarding those who clear out stocks buying from shady "pharmacies" purely for weight loss. but in the end what we have here is mostly people who did what society and their doctors told them and are now suffering tremendously for it. people losing their jobs because they throw up too much every day to work. fucking horrific.
even better, diabetes is a risk factor for gastroparesis. diabetes. the thing other than weight loss for which these drugs are usually prescribed.
moreover. slowing the stomach is a known side effect for this medication class, but these particular, new drugs seem to have an exciting twist:
What seems to be unusual about cases like Wright’s and Knight’s, Nguyen said, is that they didn’t improve after they stopped taking the medication.
“In my experience, when you stop the GLP-1 agonist, the gastric emptying improves, and it gets better,” said Nguyen, who is also a spokesperson for the American Gastroenterological Association.
so, in at least some cases, the damage seems to be longer lasting if not permanent. who knows!!! this is medically induced dysautonomia, so of course nobody has a fucking clue.
speaking of which, here's the grimly familiar part:
Gastroparesis can have many causes, including diabetes, which is a reason many people are on these drugs in the first place. Women are known to be at higher risk for the condition, too. In more than half of cases of gastroparesis, doctors are unable to find a cause.
“They may just be really unlucky,” said Dr. Michael Camilleri, a gastroenterologist at the Mayo Clinic, said of the people who shared their cases with CNN.
On the other hand, this is how the drugs work, although not many doctors or patients understand this or the problems that may follow, he said.
same bat time, same bat channel.
there's a whole section on surgery risk, too. the reason you're supposed to fast before a surgery involving anaesthesia is to empty the stomach and minimize digestive acids, which can otherwise affect your lungs during surgery in really dangerous ways. if you're on these meds, you can follow your prep instructions perfectly and still have a full stomach when you show up. and patients often don't think to mention this med to the surgical team, because they're not aware of the connection - or even actively hide it, because being on a weight loss drug is embarrassing. anaesthesiologists are Worried.
anyway, since this sadly has to be said in the context of suffering for thinness qua beauty and patients being way too willing to accept awful side effects due to warped relationships with the medical system:
Nguyen, the Stanford doctor, said patients need to pay attention to the side effects. If you vomit once or twice, that might be normal, but persistent vomiting is not.
“They should be evaluated. Consider reducing the dose or stopping the medication,” she said.
“If your vomiting is affecting your hydration or you are having to take other medications to treat the side effects of this medication, then I think it’s time to reconsider.”

Nair’s treatment approach, outline the benefits of laser surgery, and explain why he is the ideal choice for your piles treatment needs.
What is Piles?
Piles, or hemorrhoids, are swollen blood vessels located in the anal canal or rectum. They can be internal, occurring inside the rectum, or external, developing under the skin around the anus. Piles can result from increased pressure in the lower rectum, causing the blood vessels to stretch and swell. Common contributing factors include chronic constipation, straining during bowel movements, prolonged sitting, obesity, pregnancy, and a sedentary lifestyle.
Grades of Piles:
Piles are classified into different grades based on their severity and level of prolapse. The grading system helps determine the appropriate treatment approach. The grades of piles include:
Grade 1: Internal hemorrhoids that do not prolapse and remain inside the rectum. They are usually not visible and may cause minimal symptoms.
Grade 2: Internal hemorrhoids that prolapse during bowel movements but retract on their own afterward. They may cause bleeding and discomfort.
Grade 3: Internal hemorrhoids that prolapse during bowel movements and require manual repositioning to return inside the rectum.
Grade 4: Internal hemorrhoids that remain prolapsed outside the rectum and cannot be manually repositioned. They may cause significant pain and discomfort.
Piles Symptoms & Causes:

The exact cause of piles is not always clear, but several factors can contribute to their development, including:
Chronic constipation or diarrhea.

Piles Treatment by Dr. Arun Nair:
Dr. Arun S. Nair provides comprehensive and individualized treatment for piles, tailored to each patient’s specific needs. As an experienced gastrointestinal and robotic surgeon, he offers the latest advancements in surgical techniques to effectively manage piles and he’s  treatment approach may include:
Conservative Management: For mild cases of piles, non-surgical treatments such as dietary modifications, increased fiber intake, adequate hydration, and the use of stool softeners may be recommended. These measures help alleviate symptoms and prevent further aggravation.
Medications: Dr. Arun S. Nair may prescribe medications such as topical creams or ointments to relieve pain, itching, and inflammation associated with piles. Oral medications, including pain relievers and stool softeners, may also be prescribed to provide relief.
Minimally Invasive Procedures: In cases where conservative management is insufficient, Dr. Nair offers minimally invasive procedures to treat piles.
These procedures include:
Rubber Band Ligation: This procedure involves placing a rubber band around the base of the hemorrhoid to cut off its blood supply. Over time, the hemorrhoid shrinks and falls off.
Sclerotherapy: Sclerotherapy involves injecting a chemical solution into the hemorrhoid, causing it to shrink and eventually disappear.
Infrared Coagulation (IRC): IRC uses infrared light to create scar tissue, which cuts off the blood supply to the hemorrhoid, causing it to shrink and resolve.
Laser Treatment: Dr. Arun S. Nair specializes in laser treatment for piles. Laser energy is used to precisely target and shrink the hemorrhoidal tissue, promoting healing and relief.
Surgical Intervention: In severe cases of piles or when other treatments have been unsuccessful, surgical intervention may be necessary. Dr. Arun S. Nair performs piles surgery in Thrissur with  advanced surgical procedures, including hemorrhoidectomy, which involves the removal of the hemorrhoids. He utilizes minimally invasive techniques to minimize discomfort and promote faster recovery.
Benefits of Laser Surgery:
Dr. Arun S. Nair’s expertise in laser surgery for piles offers several advantages, including:
Precision and Accuracy: Laser technology allows for precise targeting of the hemorrhoidal tissue, ensuring effective treatment while minimizing damage to surrounding healthy tissues.
Minimally Invasive: Laser surgery is a minimally invasive procedure that involves small incisions, resulting in less post-operative pain, minimal scarring, and faster recovery compared to traditional open surgery.
Reduced Bleeding: The laser’s coagulation properties help seal blood vessels during the procedure, leading to reduced bleeding and a lower risk of complications.
Minimal Discomfort: Laser treatment is associated with minimal discomfort during and after the procedure, enhancing patient comfort and satisfaction.
Faster Recovery: The minimally invasive nature of laser surgery promotes faster recovery, allowing patients to return to their daily activities sooner.
Overall, laser surgery provides patients with a safer, less invasive, and more efficient option for many medical procedures. At Elixir Clinic, patients suffering from this disease can benefit from laser piles treatment in Thrissur, provided by Dr. Arun S. Nair
