Bear Claws was designed to celebrate the 10th birthday of the stacked stitch technique and the Fox Paws pattern. This engaging striped colorwork project features a large scale flame stitch motif. The pattern is relatively simple, using only a few rows of shaping in combination with color to create a distinct claw motif.
The wavy colorwork in Bear Claws is created using stacked increases and decreases. There are no loops on the wrong side and the wrap is very stretchy and drapey. The fabric is light and airy, not dense like stranded colorwork and mosaic knitting. If you're new to stacked stitches, check out my playlist on YouTube. You'll have an easier time with these stitches if you've tried increases, decreases and slipped stitches.
This pattern can be recolored in many ways, the coloring page available to anyone who wants to download it. You can use any number of colors you like, as long as they have good contrast. I recommend picking a Main Color that is not too bright or flashy in comparison to the others.
Written instructions are included in the pattern alongside a visual guide called a “stitch map.” It's not the same as a traditional chart and is meant to be used with the written pattern. The rapidly fluctuating stitch count of the stacked stitches doesn’t fit nicely into a square grid. Instead, the map uses symbols and lines that bend and flow as the rows do, as they would in a crochet chart. The primary use of the stitch map is to show how each row relates to the last and to help you find mistakes and get back on track.

How heavy are Corazon's and Doflamingo's coats?
Oooh, this is a great question! Thank you for this ask! This will be a long answer, since I need to tackle some body measurements for this, so... Yeh!
I'll go with the logic that the feathers are flamingo feathers, Doflamingo's from pink flamingos, and Cora's from black feather flamingos or myb they're dyed black, who knows. First, size!
The coats are all the way down to their feet, a few inches above ground. Let's give it 10 cm above ground. Let's put Cora's to be 2.83 meters in length. And Doflamingo's 2.95 meters in length.
Those coats are huge, and fully made of feathers both inside and outside it The only problem is are the feathers the small ones or the large ones? I'll say both, since some are pretty noticeable on the outline, especially at the collar, and even climb above Doffy's head, though that's because he puts the coat on his shoulders.
About Flamingo Feathers
Flamingo feathers range from 10-15cm in length and 2-4cm in width. They're larger than most bird feathers. Each feather can cover an area of 20-60cm² (0.002m²-0.006m²). This will be important later.
DQ Brothers Chest Width
Okay, that out of the way, we need width of the coats now, for that we need chest width and chest circumference (because the coats need to be WIDER than chest circumference) DQ bros... OFF WITH YOUR SHIRTS SO PHYSICS CAN MEASURE YOU!
Fuck yes, this is the best day ever. I should have became a stylist. If after this, nobody writes Doffy x Reader x Corazon where Reader has to take measurements of the DQ brothers bodies to make their clothes/coats, then I have failed in my endeavours.
The chest width is 1/3 or 1/4th of a height. So, with that in mind... We'll put 1/3rd:
Doffy:
3/3= 1, so 1 meter
3/4= 0.75 meter
So his chest is 0.75-1 meter wide. Mmmm. (Incoherent noises) I like taking measurements very much. Thank you, One Piece gods. Let's put Doffy's to be... The maximum. He's quite wide. 🫣
Doffy Chest Width = 1 meter
With that we measure chest circumference. Chest circumference is typically 1.5 times the chest width. Therefore!
Doffy's Chest Circumference: 1 × 1.5 = 1.5 meters
Corazon's Chest Width = 0.97 m
Corazon’s Chest Circumference = 1.46 m
Now, I want everyone to pull out their rulers. Spread those babies out to 1 m width. And lie on them. That's how big they are. That's how small you are when you lay atop their torso/chest. YOU WELCOME.
Corazon's Feather Coat
Cora is 2.93 m. His arms are half that length each. So that gives us... 1.46 meters for sleeves (which are not part of the circumference, but are important for area).
First, we need an area of the body of the coat.
The DQ brothers' bodies are wide even if they do look lean they look lean because they're 3 fckn meters tall. The 1 meter circumference usually used for loose coats is inaccurate. The coat is WIDER than the DQ brothers, it flares out, like wings.
Okay, we add FLARE. That's 20%.
Width (of Rosinante's coat)
0.97 + (0.20×0.97) = 0.97 + 0.194 = 1.16 m (I don't like this result bcs in the manga there is a lot of the coat on each side, so we'll round it to be 1.2 m instead, prob needed more flare, so 60cm of the coat on each side should be good for Rosi. Those coats are meant to be big, they're meant to INTIMIDATE. Can't intimidate SHIT with only 10 cm of the coat on each side.)
Width (of Rosinante's coat) = 1.2 m
Circumference (of Rosinante's Coat)
Chest Circumference + (0.20× Chest Circum)
1.46 + (0.20 × 1.46) = 1.75 m
Circumference of Rosi's Coat = 1.75 m
And now, with the circumference of his coat, we can now get the AREA of his coat. Remember how I mentioned the length of Rosi's arms? Well, we need that for the length of his sleeves + the area of his sleeves. First, coat body.
Rosi's Coat Body
Circumference of Rosi's Coat = 1.75 m
Length of Coat = 2.83 m
Surface Area of Body = length × circumference × 2 = 2.83 × 1.75 × 2 = 9.905 m²
Yaay, we got the coat body area. Now for sleeves
Rosi's Sleeves Area
Gotta be honest, I used my armchair's armrest width for this bcs I think it fits. The width of Rosi's arm is 20 cm up top. The circumference is then 0.6 m. We put that into the area formula:
Area of Sleeve = length of sleeve × circumference = 1.46 × 0.6 = 0.876 m²
For both sleeves
Both Sleeves = 2× 0.876m²= 1.75 m²
We add that to the body area...
Rosi's Coat Area = 9.90 + 1.75 = 11.65 m²
Number of Feathers on Rosi’s Coat
We'll use the big black flamingo feathers. Before anyone assaults these two for animal abuse, a flamingo has 10,000-50,000 feathers. Average feather coats have about 2,000 feathers used depending on the feather. Could I have just said Doffy & Rosi have that amount of feathers — 10,000? Yes. Would that be lazy of me? Yes. Also, flamingos have black flight feathers hidden on the underside of their wings, so that's where the feathers for Rosinante's coat came from, most likely! So, you can say Rosinante is the thing that keeps Doflamingo flying. (I'm fine, I'm fine. 😭) Remember how I said the feathers take up 0.002m²? Well... We did all these to get this simple calculation.
Surface Area/Area Feathers Cover = Feather Count
11.65/0.002 = 5,825 feathers
And now the simplest calculation of all. The underside black feathers are actually a bit heavier than the pink feathers. We'll do that they weigh 0.17 grams.
5,825 × 0.17 = 990 grams
Weight of Rosinante's Feathers = 0.99 kilograms
This (once you add more fabrics to carry the feathers) will be considered a heavyweight feather coat. So it is heavy. Fur is much more heavier. Feathers are light as FUCK. And theirs are full feather coats from both sides, it seems to me. They could have some insulation so myb that adds a kilogram. Otherwise, this. Now, onto Doflamingo.
Doflamingo's Feather Coat
Width of Doffy's Coat
1 + (0.20 × 1) = 1 + 0.20 = 1. 20 m (we're gonna put this one to be 1.60 m wide instead with more flare, so each side not covered by Doffy's body is 80 cm)
Width = 1.60 m
Circumference of Doffy's Coat
1.5 + (0.20 × 1.5) = 1.8 m
Circumference of Doffy's Coat = 1.8 m
Doflamingo's Coat Body
Surface Area of Body = 2.95 × 1.8 × 2 = 10.62 m²
Doflamingo's Sleeves Area
Area of Sleeve = 1.5 × 0.6 = 0.9m²
Both Sleeves = 2 × 0.9 = 1.8 m²
Add that:
Full Area of Doffy's Coat = 10.62 + 1.8 = 12.42 m²
Number of Feathers
12.42/0.002 = 6,210
Weight of Doflamingo's Feathers
Doflamingo has normal flamingo feathers, which weigh 0.1 grams, so his coat is actually LIGHTER than Corazon's even though he has more feathers. Sorta like the weight of the world, eh? Cora carried the weight of being a spy on his shoulders, while Doflamingo has a sorta carefree nature when after becoming a warlord.
6,210 × 0.10 = 621 grams
Weight of Doflamingo's Feathers = 0.62 kg
We have an update on the coats' weight! TURNS OUT, THEY ARE HEAVY!
Thank you to @lixiawinter (+their boyfriend who is physically fighting himself not to start working on Whitebeard's coat... Do it, do it.) for providing this information! We're adding this to the calculations! Thank you both so much! ❤️
Rosinante's Coat Full Weight
2.8kg (suiting wool for the shell) + 4.2kg (satin for the lining, omg this shit is so fancy holy fuck this is so cool I'm geeking out) +1 kg (canvas interlining) + 1kg (oil treatment for fabric being water repellant OMG I HAD NO IDEA THEY USE OIL FOR THAT THAT IS SO FCKN COOL OMG) + (feathers)
= 9.7kg (rough estimate)
Rosinante's Feather Coat Weight = 9.7 kg (~ 9.9kg)
We add the same numbers for Doffy.
Doflamingo's Feather Coat Weight = 9.5 kg (~ 9.62kg)
YESSS! NOW THIS MAKES MUCH MORE SENSE, AND NOW WE CAN PRESUME DOFFY DOES HAVE HIS CRANED NECK + SLOUCH FROM CARRYING THE COAT ALL THE TIME. He's wearing nearly 10 kg on his back!
And... (Panting) Done. Makes you wonder why the fuck Doffy's neck is that forward, then, when his coat isn't that heavy? Honestly, I always thought it was like 5kg weight myself which explains his terrible posture, so actually researching all this has been surprising. He just... He just walks like that. And has that stature. Bcs he... Just has that posture. You have to know, once becoming warlord, Doflamingo is very lax and acts quite carefree. In North Blue, he was full businessman even by posture - very straight-backed. Just compare his entrance on Minion Island to entrance on Punk Hazard. He's frowning on Minion Island, but is smiling in Punk Hazard - malicious as it is. He has the lax posture as a warlord because he's confident and genuine confidence makes you relax. That means shoulders, neck, back, they all slump. Sure, he's ready to attack anytime, his muscles definitely tense when he is ready for attacking. But still.
Oda probably wanted to go full symbolism with Doflamingo being as a flamingo so he gave him that craned forward neck like a flamingo's, and the back posture makes him seem like he is prowling toward you, like a lion. Since all the warlords are animal inspired and have the themes to match with their animal name, it's no wonder Oda chose to show it with Doflamingo's posture - even his legs are LONG as fuck! Doflamingo is all grandeur and is of the most rare bloodline and a flamingo is a very grandeurish bird considered royalty among bird species.
Doflamingo, why is your shirt still off?
Anyway, that's... (Panting) It. Nice calculations. Thank you for the great question, @dummyduck44! ❤️ And thank you to @lixiawinter for more information, providing the numbers of what else is needed for the coat and the weight! Unfortunately, as a scientist, I can only do so much, and much to my chagrin I'm not a badass like you for clothing. I have 0 knowledge about clothing. I could only do so much. As Sanji says "you do what you can do, and I'll do what you can't do" Thank you and your boyfriend for being the Sanji to my Usopp ❤️

Screentones Collection 1 - Halftones - Download
Apply these screentones to create a manga style or dot pattern on your illustrations, photos, designs and videos. This technique is mainly used in manga and comics to apply tones, shadows and patterns.
The number of lines corresponds to the number of dots in each row or column, the percentage is the amount of ink injected per inch equivalent to the grayscale. This method was implemented to reduce costs by printing only with black ink; when the dots are small enough, an optical illusion is created in which the eye interprets the patterned areas as a smooth grayscale.
This collection of screentones was made so that mangakas and digital illustrators could work freely in any software of their choice.

Shibusan was also selling Boueibu merch in August, including Ikaho collab merch until the end of September. Details here.
September 17th was Nanao's birthday. If you wanted this cake + cupcake set + macaron set in time for the day, then it should have been preordered until September 10th. This came with a bonus badge with the same design. The cake (which comes in 2 sizes - size 4: diameter: 12 cm, height: 5 cm and size 5: diameter: 15 cm, height: 7 cm) is in chocolate/chocolate cream and sponge/whipped cream varieties, with strawberries or yellow peaches inside for both (customer's choice), and the design is printed on to a sheet. The ink has a chance of running if you aren't careful. There are also Atsushi (preorder deadline: September 28th), Taishi (preorder deadline: September 29th), Kyotaro (preorder deadline: November 21st), Io (preorder deadline: December 10th) and Yumoto (preorder deadline: January 19th) versions. The cupcakes have a diameter of 8.4 cm, a height of 6.5 cm and come in a set of 4 - the cream on the cupcakes has matcha (green), chestnut (yellow), strawberry (pink) and chocolate (brown) and you can choose between black tea and orange peel/chocolate chip flavours for the cupcakes themselves. The macarons have a diameter of 4.3 cm, a height of 2.3 cm and come in a set of 6 - they come in strawberry (red), pistachio (green), orange (yellow) and chocolate (brown) flavours.
There are also Christmas cakes + macarons (preorder deadline: December 17th if you want it by December 24th) and Valentine's Day cakes + macarons (deadline: presumably February 7th) which come with badges (2 possible designs, 1 can badge that matches the design of the cake you chose). The Christmas cakes only have strawberries, as opposed to the strawberries and yellow peaches of the Valentine's and birthday cakes. The cakes only come in size 5. The badges are 54 mm in diameter. There was also a 5% discount applied to preordered Christmas cakes between November 6th and November 12th 2023.
The Shibukawa collab finished in September 2023. The crepes and drinks sold well. The collab page doesn't exist anymore, but there is a message from Shibukawa City: "To all the fans, thank you very much for coming to Ikaho time and time again. Look forward to the day we can hold another collab✰."
There was an online lottery on sale from October 20th at 12:00 pm - November 6th 2023 at 11:59 pm (all dates/times JST), with items estimated to be received in mid-January 2024. 1 attempt was 880 yen (tax incl.).
For said online lottery, if you won the A prize (probability of getting: 4.1%), you could pick which of the 2 fabric posters you wanted:
The B prize was a set of acrylic stands (probability: 13.8%, random out of 10 different kinds):
The C prize was acrylic keychains (probability: 20.5%, random out of 10 different kinds). The Anime Bako account (which is posting these tweets) recommends attaching them to a bag when you go out:
There are also D prizes (1 of 10 random square can badges, probability: 27.4%) and E prizes (1 of 20 random Polaroid-style cards, probability: 34.2%):
On January 1st 2024, the Boueibu official Twitter (X) account wished everyone a year overflowing with love and happiness. Please remember the Boueibu series this year too.
(Update: In terms of fruit, the Christmas cakes only have strawberries, while the Valentine's Day cakes have the choice of strawberries or yellow peaches.)
#boueibu#binan koukou chikyuu boueibu love!#Binan Koukou Chikyuu Boueibu Happy Kiss!#anime merch#boueibu hk#anime collab#(There is a slight difference in wording between LOVE! and HK cake tweets so I've put an example of both in the post.)#(“Yoroshiku onegaishimasu” is still a slippery phrase I see... /slightly annoyed)#(When I said I was going to be thorough this time I meant it...)

Pattern Notes
This pattern is worked in the round from the top down with increases for upper skirt shaping.
Dc rounds are started with a modified turning chain, a single crochet is worked without a ch-1 turning chain, followed by 2 chains to create the height needed for dc. When working the next round the top of the ch-2 is worked as the top of a regular dc st.
To make the skirt longer add rounds without increases between the increase rounds (for a shorter skirt, subtract rounds without increases between the increase rounds. The last round of the upper skirt needs to have a number of stitches divisible by 14 for the lace pattern set-up.
Drawstring for the skirt is a Crocheted Romanian Cord. If you need help there is a great video on Stitches n Scraps YouTube channel: https://www.youtube.com/watch?v=vniz9CAREVs Pattern Upper Skirt

So Facebook released their Meta AI, and yeah, I know, we all know these things are actually stupid as fuck. I’ve been having fun screwing around with it, and damn is this thing stupid as fuck, and know absolutely nothing about art.
Anyway, I asked it for a knitting pattern:
Here is a simple knitting pattern for a scarf:
Materials:
- Worsted weight yarn (approx. 300 yards)
- US size 8 knitting needles
- Scissors
- Yarn needle
- Tapestry needle
Gauge:
20 stitches x 30 rows = 4 inches (10 cm) in stockinette stitch
Abbreviations:
- kn = knit
- purl = purl
- k2tog = knit 2 together
- p2tog = purl 2 together
Pattern:
1. Cast on 40 stitches.
2. Knit 4 rows.
3. Row 5: k2tog until end of row.
4. Row 6: purl, working the purl stitches as they come.
5. Repeat Rows 5 and 6 until piece measures 60 inches (152 cm) from beginning.
6. Bind off in pattern.
Notes:
- You can adjust the length of the scarf by changing the number of stitches you cast on.
- You can also add a simple border, such as a garter stitch or ribbing, to the scarf if desired.
Please keep in mind that this is a simple pattern, and you may want to add more complex stitches or techniques to make it more interesting.
Let me know if you have any questions or need further clarification!

Truchet's Cowl is an interactive colorwork pattern based on the work of Designer and mathematician Sébastien Truchet. Each knitter can knit the pattern shown in the photo, or arrange and create their own unique pattern based on the provided colorwork tiles.
The possibilities are nearly infinite.
Finished Size: 22” (56 cm) around the top, 34” (86 cm) around the bottom and 13” (33 cm) tall.
Yarn: Baa Ram Ewe Titus, 350 yards (320 m) per 100g; 50% Wensleydale Wool, 30% Alpaca, 20% Bluefaced Leicester Wool.
▪️Color A: 30g, 105 yards (96 m) Parkin.
▪️Color B: 50g, 175 yards (160 m) Yorkstone.
▪️Color C: 50g, 175 yards (160 m) Eccup.
Needles: Size: 1, 2, 3 (2.25, 2.75, 3.25 mm) 16” (40 cm) circular needle, or size needed to obtain gauge.
Gauge: 20 sts x 40 rows = 4 x 4” (10 x 10 cm) square on size 2 (2.75mm) needles in stranded colorwork pattern.
Other Materials: Stitch markers in various colors, graph paper, scissors, tape, steamer, surface for blocking, tapestry needle.
History
Truchet Tiles are a symmetry game, first recorded in 1704 by Sébastien Truchet. His original tile game consisted of squares split along the diagonal into two triangles of contrasting colors. In quilting this shape is known as a “Half-Square Triangle”
Visit https://dmackinnon1.github.io/truchet/ to see an online version of his tiles.
More Yarn Info
This cowl works well with any fingering weight yarn. Thesample was knit from a set of leftovers from the Corner Point Scarf and the yarn is unfortunately discontinued.
The fuzzy, wooly qualities of this non-superwash yarn are similar to John Arbon Devonia 4-Ply.
The pattern is fully adjustable for any gauge. You can find it on Ravelry and on my website.

Cubao’s latest suit is here, just in time to celebrate Gong Jun’s 31st birthday!
The newest suit is inspired by latest collaboration between Jason Wu and Shu embroidery master Kang Ning. Gong Jun wore this suit to the recent Vogue Forces of Fashion event, where all three of them participated in a panel together.
The latest bao suit is fit for 20 cm Cubao, as opposed to the 15 cm Xiao Cubao I normally make for. Because I previously worked with a smaller size, the first step was to size up the patterns. I used the base pants and shirt pattern resized to 117% scale then made mock ups (I got lucky on the size selection and didn’t have to do too much trial and error). Then I sized up the previous pattern used for the Cannes suit (thank you, Jason, for using similar styles) and made another mock up to check the fit.
White sateen serves as the base fabric. This version of sateen is slightly thinner than the one used for the Elie Saab cape and what I am used to, but it was what I had on hand. After the pattern pieces were marked out, I used red pencil lead to trace out the patterns. Kang Ning said she used 10 different colors of silk thread to create the original embroidery; due to my limited real estate and short time frame, I used two colors of cotton embroidery floss. The stitches are mostly short stitches and rather haphazardly placed; the goal was to give a fur like appearance as everyone initially thought the pattern was fur so I wanted to try and replicate that (I hope I did).
Overall embroidery took about 12-14 hours over two days.
Details also include a signature embroidery on the back. I am not sure if this is both Kang Ning’s and Jason’s signature, or just one of them. It is a little detail, but one I really liked and did not want to leave out. The bao version is 1 inch x 1 inch.
Overall, this suit took 5 days to make. Gong Jun’s appearance was on Nov. 23 and the suit was completed on Nov. 27, just in time to post for his birthday.
And thus Cubao(s) now have had 5 custom suits made (in the last 6 months!). We are looking forward to seeing what will appear next.

Screentones Collection 2 - Dots - Download
Apply these screentones to create a manga style or dot pattern on your illustrations, photos, designs and videos. This technique is mainly used in manga and comics to apply patterns.
The number of lines corresponds to the number of dots in each row or column, this method was implemented to reduce costs by printing only with black ink; when the dots are small enough, an optical illusion is created in which the eye interprets the patterned areas as a smooth grayscale.
This collection of screentones was made so that mangakas and digital illustrators could work freely in any software of their choice.

Prolapsed Endometrioid Adenomyoma Associated with Post-Coital Bleeding by Krina Kathawadia, Sharmi Das, Ayman Aboda in Journal of Clinical Case Reports Medical Images and Health Sciences
Abstract
This case reports an atypical presentation of Uterine Adenomyosis. Adenomyosis is typically a diffuse, infiltrative disease of the endometrium and myometrium, which arises as a benign non-localized neoplasm of endometrial glands, stroma, and smooth muscle that almost always remains closely intimated to the uterine wall. Very rarely is it found to extend into the uterine cavity or, as in this case, develop into an Adenomyomatous polyp that passes through the cervix and into the vagina. More typically, these features are found with uterine leiomyomata. Both conditions are most often benign and are treated definitively by surgical excision. The distinction is important given the rarity of the finding reported by this case and because of the implications of differential effect given the specific progressive clinical nature associated with each condition.
Keywords: Adenomyoma, adenomyosis, vaginal bleeding, post-coital bleeding, endometrioid adenomyoma, uterine neoplasms, endometriosis.
Case Presentation
A 42-year-old woman presented to Gynaecology Outpatients with a history of vaginal bleeding for several months, associated with an awareness of an intermittent vaginal mass. The mass could be reduced digitally and was most apparent following coughing or accessory strain. She reported a long history of heavy and irregular menstrual bleeding, often with clotting. She also experienced urinary stress incontinence.
Past obstetric history included four children, three born vaginally and one by caesarean section. She had declined cervical screening for more than five years. Inspection of the external genitalia and vulva was unremarkable. Speculum examination revealed a 3 x 3 cm mass extending into the upper vagina from the external cervical os. The mass was smooth and non-ulcerated but bled with touch. Transvaginal pelvic ultrasound demonstrated a bulky uterus with an endometrial thickness of 5.3 mm and a uniformly enlarged cervix measuring 3.4 x 3.7 x 3.9 cm. An MRI was performed that confirmed a solid-cystic mass-like structure at the level of the cervix measuring 34 x 30 x 44 mm. Myometrial distortion suggested possible fibroid change, although no large fibroids were identified. The features did not suggest a malignant change.
Differential diagnoses included prolapsed uterine myoma, cervical fibroid, or possible cervical carcinoma. After discussing immediate treatment options, a hysteroscopic examination was performed following dilatation of the cervix and curettage of the uterine cavity. Multiple small submucosal fibroids distorted the cavity. A pedunculated, polypoidal lesion arising close to the internal os was excised and was found on histopathological examination to be an endometrioid-type adenomyoma measuring 55mm in size. Endometrial cuttings showed late proliferative changes with sparse fragments of Adenomyomatous change. The patient recovered well post-operatively and has since been booked for a hysterectomy with bilateral salpingectomy.
Discussion
Uterine leiomyoma, more commonly known as fibroids, and adenomyosis are two common but distinct gynaecological conditions that may significantly impact women's health (1-3), often presenting with pain, abnormal vaginal bleeding and, more rarely, polyps (4,5). These conditions share overlapping symptoms and clinical features but are inherently different in their pathophysiology, disease progression and clinical management (6, 7, 8, 9)
Uterine leiomyomas are benign smooth muscle tumours that typically originate from the myometrium of the uterine wall. They are the most common benign pelvic tumours in women, affecting approximately 20-50% of women of reproductive age and occurring in up to 70% of women by age 50 (10). Leiomyomas can range from tiny and virtually undetectable to large masses that distort and enlarge the uterus. Symptoms vary widely, with many women being asymptomatic. However, symptoms typically include heavy and prolonged menstrual periods, abnormal bleeding between periods, pelvic pain and pressure, urinary frequency and coital discomfort. (11) Fibroids can affect reproductive health, causing infertility and abnormal pregnancy outcomes such as recurrent miscarriage and obstetric haemorrhage. (11) They may occasionally promote the growth of endometrial polyps or may present as a polypoidal or pedunculated intrauterine mass. The aetiology of leiomyomas is not entirely understood but is believed to involve an interplay of genetic, environmental and hormonal factors, particularly estrogen and progesterone (11,12).
Adenomyosis, on the other hand, occurs when endometrial tissue, which typically lines the uterus, exists within and grows into the muscular wall of the uterus (myometrium) (13, 14, 15). This condition is commonly found in middle-aged women and those with children. Although it can be asymptomatic, adenomyosis often presents with heavy or prolonged menstrual bleeding, severe menstrual cramps, and chronic pelvic pain. The exact cause of adenomyosis is unclear, but it is believed to be associated with various hormonal changes, including those induced by childbirth and menopause (5) While adenomyosis can lead to significant discomfort, it is typically not considered a risk factor for other health complications, although it may be coexistent with pelvic endometriosis (16, 17). In rare cases, adenomyosis may contribute to the formation of polyps and lead to irregular bleeding (18). Typically, they contain haemorrhagic cystic spaces, which are unusual in other polyps such as leiomyomas or malignancies (19). The characteristic sonographic features of polypoid adenomyomas include heterogeneous or homogeneous echogenicity relative to the myometrium, a smooth surface, a poorly defined margin with the underlying myometrium, haemorrhagic foci, and usually, associated adenomyosis in the myometrium (20). When ultrasound evaluation demonstrates a soft tissue mass in the vagina prolapsed from the uterine cavity with a visible connecting stalk, it is termed the broccoli sign (21,22)
Uterine leiomyomas and adenomyosis can cause abnormal vaginal bleeding, typically heavier and longer than normal menstrual bleeding. However, the bleeding associated with leiomyomas is usually due to the increased surface area of the uterine lining, while in adenomyosis, bleeding results from the disruption of the muscular wall of the uterus (23, 24). In both cases, the bleeding may be severe enough to cause anaemia and disrupt a woman's perceived quality of life (25).
Regarding management, treatment for adenomyosis and leiomyomas can range from watchful waiting to medical therapy (e.g., hormonal treatments) and surgical interventions such as minimally invasive, local resection, or hysterectomy (8,11,14). In both cases though more rarely for the latter, medical management using hormonal treatments to control or mitigate the menstrual cycle may effectively reduce provocateurs for growth, leading to regression or diminution of size and consequent reduction of symptomatic effect (8,11,14).
Conclusion
This case report presents a unique manifestation of uterine adenomyosis, typically a diffuse disease, uniquely presenting as an Adenomyomatous polyp protruding into the vagina. This rare presentation mirrors features commonly associated with uterine leiomyomata. Both adenomyosis and leiomyoma are benign gynaecological conditions causing similar symptoms, such as abnormal vaginal bleeding, but each carries distinct clinical implications. Accurate diagnosis and treatment, typically surgical excision, and ongoing effective medical management, are crucial. Knowledge of the clinical and sonographic appearance of polypoid adenomyomas may facilitate diagnosis and help distinguish these tumours from other uterine polyps. This report advocates the need for heightened clinical awareness and comprehensive differential diagnostic criteria when managing abnormal vaginal bleeding, particularly when associated with evidence of an expansive polypoidal growth.
