WIP: Edgar Allen Poe fan art. I want to fill the negative space in top left. I have a few ideas. What would you put there?

Hey did you know there's a tell all book about the behind the scenes of Meta and the author is forbidden from promoting it?
The good news is however that it's already published and can't be stifled and whoever didn't sign the NDA can promote it as much as they want.

What kind of doctor?
by Rocco, 2025
I'm instantly unsettled by the doctor who enters the room and greets me. It is his socks. His black khakis raise up like lips as he settles into his chair revealing the toothy grin of his socks. On one foot, red and white horizontal stripes. On the other, white stars surrounded by cobalt blue. They are utterly clownish compared to the rest of his attire unremarkable medical professional. I say nothing. I'm here about my knee pain.
"Tell me what's going on with your knee." I tell him. I make movements as he suggests. Squats and such. We look at my x-ray images. Occasionally he speaks jargon into a recorder. "These are healthy knees." This is a relief. No surgery.
He prescribes ointment and exercises. He gives me papers explaining the exercises, with pictures too. People, all shapes and sizes demonstrating the exercises! One of them is an elderly woman. "I'm sorry about the old lady." "Please don't be offended." "I guess these images were part of some diversity program." What kind of doctor says "old lady"? What kind of doctors is offended by an "old lady"? "I sometimes feel like an old lady." is all I can think to say. He looks disappointed. I didn't take the bait. He grins and says quietly "Me too". An American doctor.
