The idea is that pre-salvation (adoption into the Saverem household), Wolfwood was a part of a sort of hybrid dog-fighting setup with stimulants (the shot) that made them aggressive enough to fight each other; this is something that over time would cause damage to their health that the canon compliant serum would then be used as a long-term treatment to.
