Thank you for your previous post! I wanted to ask if is it possible for blood clots to form and obstruct cardiopulmonary bypass. And what actions are taken in the event of an air embolism or dislodgement of the cannula? Would they cause similar outcomes as in fics?

this is a great question!

while it is completely possible for clots and air emboli to form, bypass machines actually have very specific mechanisms that prevent these! also, the anesthetist and perfusionist would definitely be working hard to also prevent these from happening!

to prevent clots, the patient is usually injected with anticoagulants, and some parts of the tubes used are primed with anticoagulants as well for smooth blood flow! the anesthetist will also usually will top up the anticoagulants during the course of the surgery to ensure clots won’t form.

the bypass machine also has several monitoring mechanisms to check what’s going on in the patient’s blood!

as for air emboli, the bypass machine actually has an awesome suctioning feature that’s installed to ensure that no air bubbles go to the patient. also, if air bubbles enter the circulation during the surgery, the bypass machine has a way to flush and suction that out safely!

if the bypass machine malfunctions mid surgery it would be an extreme never event, and it should basically never happen in a proper hospital

everyone is usually very strict about the checks before these machines are used and I personally have never been in a situation where there was a malfunction!

I imagine if it does happen though the anesthetist would be working extra hard to ensure proper circulation for the sake of the patient

if it all goes south (we all pray it won’t) there are other options to save the patient’s circulation — such as mechanical circulation via an ECMO machine! (this is extreme though but it may happen)

while the surgeons do instruct what happens during bypass time, the machine malfunctioning would not be under their control as they are scrubbed in!

all of these are on the anaesthesist/medical side which is why zayne would actually not really have any part here if it was this kind of mechanical fault

i don't work in the cardiothoracic sphere but i have watched a lot of medical dramas (knowing that there are inaccurate and dramatized things there too), and i do also work in the healthcare sphere, so i understand your pain to an extent akrjakdl it's nice to know i can pick your brain if i ever want to write a more medically accurate fic for lads!!

there’s an update to this story…

basically my husband always wakes me up every morning at 6:30 am so I can prepare to go to work

I have an alarm of xavier’s voice at 6:15 am

ever since he found out about it he has started waking me up at at 6:00 am because ‘he should be first’ and it’s hilarious because he has recently been copying the xavier alarm ʅ（◞‿◟）ʃ

working in cardiothoracics has robbed me of being unable to enjoy zayne fics because it is dramatically medically inaccurate (that’s no one’s fault as even among healthcare workers not everyone would intimately know)

hearts are usually STOPPED with medications during cardiac surgery. we put patients on a bypass machine in order to mimic heart function. essentially the bypass machine functions as your heart while the surgeons operate on your non-beating heart. yes it is reversible and we make your heart beat again later on once it’s safe to do so.

yes there are heart surgeries where you don’t need a bypass machine but trust me the bypass machine is always on standby and so are many mechanical support options

so those fics where mc’s heart stops in the middle of the operation because zayne hesitates

or she goes lifeless on the table because she’s bleeding out

or there’s a one second difference between zayne’s decision and her life and death

this is unlikely to happen in the realm of modern medicine unless you are in a hospital without capacity for mechanical support

and firmly believe akso hospital is definitely capable of mechanical support as they do heart transplants

happy to answer questions about this but I just thought I would share as it is beginning to bother me :(

I have tons of comments about the translation of the Caleb intro story, and I know much discourse has been had about it, and I don’t really want to cause more disagreements so I will just stick with facts—

In the JP dub:

- Caleb gets called a siscon by another character (literally means he has a sister complex, and this is not a term used for non familial relationships)

- He literally says 「俺はお前のにいちゃんだ」 “I am your older brother” SEVERAL TIMES, TO REFER TO HIMSELF… but the EN version changes these lines to some version of “I’m always by your side” which was ??? to me but ok I get why they did it

- MC switches between calling him “older brother” and just “Mahiru” — which was a nice touch I feel as she was conflicted about her feelings regarding this returned version of him

- It’s pretty obvious he wants to protect her and is just playing cruel as a means to an end, the way his voice switches back and forth sounds very sincere (it was so good actually)

the older brother / found family trope is EXTREMELY COMMON in Japanese media, so I’m not surprised they didn’t change one inch of it. I’ve seen several romance manga with this trope and it’s quite acceptable here in Japan as long as they’re not blood related siblings

I know many people have thoughts on this but as someone who has been exposed to this media, I find nothing wrong with it as long as the portrayal is of two consenting adults trying to rediscover their relationship beyond what they were familiar with (which is exactly what is happening with Caleb and MC)

the emotional complexities of this trope is what makes it so alluring — and honestly I think Infold is doing quite well so far with the writing (especially Caleb’s lines oof they were so good!)

anyway just my two cents

too much Caleb now I need to go back to Xavier I’m having withdrawals
