Questions about Medical Videos on YT and Consent

so context: i love watching RL medical videos on YT. fukken LOVE it. gimme them diabetic ulcers, wound care, strange cysts, whatever.

one of the series i've been watching a lot recently has been taking place in an emergency clinic, and it's a lot of fun because there's so much variety in what comes in. but there are some cases where the video's on the patient, they're clearly in pain or taking painkillers, and the doctor just says "you've consented to this going on Youtube, right?" and the patient nods and that's... not... great.

what might a patient be thinking then? are they in a state to consent? do any of them think, "if i say no, am i gonna get the same level of care?" there's no possible way that you can count on reasoned consent in that moment, am i wrong???
