Okay, hear me out. Unohana...but with freckles. Just...a little faint dusting of tiny freckles across her nose. Unnoticeable ones. You have to be really close to her face to see them - like 'antagonist leaning close into the protags face for intimidation' type close. Nose-to-nose type close. The thing is, Unohana hates her freckles. She's hated them since she went by the name Yachiru. She was a criminal! A murderer! What criminal/murderer has freckles?! Anyone who mentions them ends up dead in a ditch missing one or all their limbs. The real reason she threw hands with child!Zaraki? Little bastard noticed her freckles and called them out. She couldn't let him live after that and that totally backfired. The only one - the only one - who gets away with this is Isane. Because Isane gives her nose kisses when she calls them cute when they're alone, and Unohana is weak for nose kisses and the way Isane is smiling when she does it.

Random personal headcanons about Isane

— Due to her nightmares often keeping her up late Isane constantly has these faint circles under her eyes. — Can work on just a few hours of sleep. Pulls all nighters like a pro if her nightmares are gory enough. You wouldn't know she stayed up all night if you didn't notice the darker circles under her eyes. Her fellow healers are in awe. Kisuke is impressed. Kiyone, Hanataro, and Retsu are concerned. Please let this girl sleep — Has accidentally fallen asleep/napped on quite a few members of the SWA. No one has the heart to tell her about this, though. Not even Rangiku and Yachiru. She still has no idea. — Has the softest hair it's so addicting to the touch. Messy in the right way that makes you wanna run your hands through it which Retsu has done plenty of times.

— She and Kiyone are half-sisters; they have the same mother but different fathers. Isane's step-dad (Kiyone's dad) let Isane take the name Kotetsu when he married their mother. — Gets her messy hair, big hands and tall height from her mother; the rest comes from her father. — Warm hands with little nicks and scratches, and long slim fingers. Retsu loves tracing each one every night before bed. It actually helps calm her, ah, ever churning bloodlust. — Is the most crushed on female shinigami in the Fourth, okay? This gentle giant with her puppy dog eyes and her messy hair and soft-spoken voice has unknowingly caused a lot healers to lose a few sighs over her. — Saying that, she's been crushed on by a few shinigamis from different divisions. Rangiku and Momo still have crushes on her. — The most devastating puppy eyes ever, and it's so much worse because Isane has no idea she has them. — Is close with her zanpakuto Itegumo and respects them. It's the main reason Itegumo didn't rebel like the rest; they were just silent because they were focusing on fending off Muramasa's ability.

— It takes a lot to get Isane to snap. A lot. She's a healer/combat medic; she's used to being pressured and probably having bad tempered people swear and scream and snarl at her. A member of the Twelfth Division decided to find out how much it would take, including keeping Isane up at night. It is forever known as The Incident. Isane has no memory of what happened. Retsu has ordered no one - no one - talk about that day, and banned the Twelfth Division from the Fourth for a month and a half. She even threatened the Eleventh members that witnessed the event. Even Zaraki keeps his mouth shut.
